1.Treatment of auricular keloids with surgery and intralesional injection of compound betamethasone.

OBJECTIVETo investigate the treatment of auricular keloid with dinuclear surgery and intralesional injection of compound Betamethasone.
METHODSFrom Jan. 2008 to Jan. 2012, a total of 186 cases of ear keloid were treated by surgery only (22 cases), or intralesional injection of compound Betamethasone (34 cases), or combined dinuclear surgery with compound Betamethasone (130 cases). All the patients were followed up for one year. SPSS 16.0 software was used for statistical processing and analysis, and GraphPad inspection method for inspection.
RESULTSThe effective rate was 54.55% (12/ 22) in surgery group and 55.88% (19/34)in injection group. The recurrence was obvious in injection group during the follow-up period. The effective rate was as high as 96.92% (126/130) in combined group with recurrence in 4 cases, which was significantly higher than that in other 2 groups (P < 0. 01).
CONCLUSIONCombined dinuclear surgery and compound Betamethasone injection has a good therapeutic effect on auricular keloids.

2.Curative effect of Quadrant channel transforaminal lumbar interbody fusion in the treatment of recurrent lumbar disc herniation

Objective To investigate the curative effect of Quadrant channel transforaminal lumbar interbody fusion(TLIF) in the treatment of recurrent lumbar disc herniation(RLDH).Methods Seventy-six cases patients with RLDH were divided into traditional TLIF group(36 cases) and Quadrant group(40 cases) by the different operation modes.The operation,VAS scores and ODI scores were compared between the two groups.Results Operation time((98.2±18.1) min vs.(118.5±27.2) min,t=3.79,P<0.05),incision length((2.7±0.6) cm vs.(5.5±1.0) cm,t=14.97,P<0.05),intraoperative blood loss((90.7±20.1) ml vs.(170.3±33.5) ml,t=12.71,P<0.05) and intraoperative drainage((43.8±12.4) ml vs.(117.1±28.9) ml,t=14.62,P<0.05) in Quadrant group were less than that in traditional TLIF group.The ambulation time((2.8±0.6) d vs.(6.8±1.1) d,t=19.95.P<0.05) and hospitalization time((7.1±2.0) d vs.(14.2±2.7) d,t=13.11,P<0.05) of Quadrant group were shorter than those of TLIF group.VAS scores in both groups were significantly decreased.VAS in Quadrant group was significantly lower than that in traditional TLIF group from 3 d to 6 months after operation((4.4±1.0,3.9±1.2,3.4±0.8,1.9±0.6,1.3±0.4) points vs.(5.0±1.4,4.5±1.1,4.1±0.9,3.6±0.7,2.5±0.6) points,P<0.05).ODI scores in both groups were significantly decreased.ODI in Quadrant group was significantly lower than that in traditional TLIF group from 1 d to 6 months after operation((36.7±7.1,33.4±6.2,30.7±4.1,27.8±5.3,24.5±5.4,18.4±4.7) points vs.(39.8±6.0,36.8±5.8,33.5±6.6,31.3±5.1,28.9±6.6,22.3±5.2) points,P<0.05).Conclusion The treatment of RLDH by Quadrant channel TLIF has obvious curative effect,short operation time,less trauma,faster recovery and shorter hospital stay.
3.Experimental study on cross-resistance of Culex pipiens pallens to 3 kinds of chemical pesticides

Objective To understand the cross-resistance of Culex pipiens pallens to common pesticides,so as to provide the evidence for improving the application of chemical pesticides. Methods The IV instar larvae of DDVP-resistant,propoxur-resistant and cypermethrin-resistant strains as well as the sensitive strain of Culex pipiens pallens were collected to detect the re-sistance to DDVP,propoxur and cypermethrin based on the WHO bioassay method. Results The resistance coefficients of DDVP-resistant strain to DDVP,propoxur and cypermethrin were 14.47,8.96 and 207.27 respectively. The resistance coeffi-cients of propoxur-resistant strain to DDVP,propoxur and cypermethrin were 3.27,6.93 and 8.65 respectively. The resistance coefficients of cypermethrin-resistant strain to DDVP,propoxur and cypermethrin were 2.93,1.61 and 501.11 respectively. Con-clusion The resistance and cross-resistance could be generated during the long-term application of a single kind of chemical pesticide,and we should pay more attention to the varieties and dosages of them.
4.Efficacy and safety of colistimethate sodium in critical patients: anin vitro study by using of Monte Carlo simulation

Objective To evaluate the efficacy and safety of colistimethate sodium (CMS) for the treatment of critical patients infected by pan-drug resistantAcinetobacter baumannii (PDR-AB) or pan-drug resistant Pseudomonas aeruginosa (PDR-PA).Methods 321 isolates of PDR-AB and 204 isolates of PDR-PA from critical patients admitted to 35 intensive care units (ICUs) of grade two or above were collected from the Anhui Antimicrobial Resistance Investigation Net (AHARIN) program from September 2012 to September 2015, while the minimal inhibitory concentrations (MIC) of colistin were determined by the E-test. A series of Monte Carlo simulations was performed for CMS regimens (1 MU q8h, 2 MU q8h, and 3 MU q8h, and MU meant a million of unit), and the probability of achieving a 24-hour area under the drug concentration time curve (AUC24)/MIC ratio > 60 and risk of nephrotoxicity for each dosing regimen was calculated. Each simulation was run over three CLCr ranges: < 60, ≥ 60-90, ≥ 90-120 mL/min. The probability of target attainment (PTA)for the AUC24/MIC ratio was calculated using the partial MIC value, while the cumulative fraction of response (CFR) was determined by integrating each PTA with the MIC distributions, the value greater than or equal to 90% or more than 80% was set as the optimal dosing regimen or suboptimal dosing regimen respectively. The probability of average 24-hour serum concentrations up to 4 mg/L for three dosage regimens was used to predict the risks of nephrotoxicity.Results All 321 isolates of PDR-AB and 204 isolates of PDR-PA were susceptible to colistin, the MIC50/90 against PDR-AB were 0.5mg/L and 1.0 mg/L, and those against PDR-PA were 0.5 mg/L and 1.5 mg/L, respectively. When recommended dose (1 MU q8h) was used for patients with CLCr of < 60 mL/min, high CFR value (89.78% for PDR-AB, 81.06% for PDR-PA) were obtained, but with a high risks of nephrotoxicity (> 32.51%). Moreover, low value of PTA (< 66.56%) was yielded for isolates with MIC of ≥ 1 mg/L. Recommended dose also yielded a low CFR value (56.97%-69.31% for PDR-AB, 44.76%-56.94% for PDR-PA) in patients with CLCr of ≥ 60-120 mL/min. When dose was increased to 2 MU q8h, CFR (77.45%-92.87%) and the risks of nephrotoxicity (< 0.15%) was optimal for patients with CLCr ≥ 60-120 mL/min, but low value of PTA (< 75.36%) was also yielded for isolates with MIC of ≥ 1 mg/L. The most aggressive dose of 3 MU q8h provided high CFR (> 89.24%) even in patients with CLCr ≥ 90-120 mL/min, and PTA was < 76.20% only for isolates with MIC of ≥ 1.5 mg/L, but this dosing scheme was associated with unacceptable risks of nephrotoxicity (> 33.68%).Conclusion Measurement of MIC, individualized CMS therapy and therapeutic drug-level monitoring should be considered to achieve the optimal drug exposure and ensure the safety of CMS.
5.Comparison of clinical efficacy between autologous fat injection and orbital septum fat reset for infraorbital margin depression in young people

Objective:To compare the clinical efficacy of autologous fat injection and conjunctival pedicled orbital septal fat grafting for infraorbital margin depression in young people.Methods:Fourty young patients with lower eyelid pouch and infraorbital margin depression with age of 20 to 40 years and average age of 30.6 yaers in the First Affiliated Hospital of Nanchang University from September 2017 to September 2019 were enrolled. According to the surgical method, all patients were divided into 2 groups with 20 cases each. Group 1 underwent transconjunctival orbital septum fat reset. Group 2 underwent various autologous fat injection after proper amount of orbital septal fat being removed, including structural fat, fine-particle fat and nano-fat, performed in different parts and layers. The tear trough rating scale (TTRS) scores and patients' satisfaction at 1 year after operation between 2 groups were compared.Results:Compared with the preoperative TTRS scores, the differences in scores at 6 months and 1 year after surgery were statistically significant ( P<0.01). The scores between two groups were compared at 6 months and 1 year respectively after the operation, and the difference were both statistically significant ( P<0.01). 6 months after the operation, the TTRS scores including four items were carefully compared between the two groups. It showed that the reduction of tear groove depth ( P<0.05), pigmentation ( P<0.01) and wrinkle ( P<0.01) in group 2 were more obvious than those in group 1. The percentage of satisfaction in group 2 (95%) was higher than that in the group 1 (70%), and the difference was statistically significant ( P<0.01). Conclusions:For infraorbital margin depression of young people, after proper amount of orbital septal fat being removed, various free fats graft injection into different parts and layers could obtain better treatment results, reducing not only the depth of the tear groove, but also periorbital pigmentation and fine wrinkles.
6.Simultaneous Determination of the Contents of 5 Indicator Components in Yanyan Tablets by QAMS

OBJECTIVE: To establish QAMS method for content determination of paeoniflorin, rutin, oroxin B, baicalin and cinnamates in Yanyan tablets. METHODS: HPLC method was adopted. The determination was performed on Hypersil GOLD-C18 column (250 mm×4.6 mm,5 μm) with mobile phase consisted of methanol-0.35% phosphoric acid solution (gradient elution) at flow rate of 1 mL/min. The detection wavelengths were set at 280 nm (rutin, oroxin B, baicalin, cinnamates) and 230 nm (paeoniflorin). The column temperature was 30 ℃, and sample size was 10 μL. Using paeoniflorin as internal reference, relative correction factors (RCF) of rutin, oroxin B, baicalin and cinnamates were established. Effects of different chromatogram system, chromatogram column, mobile phase proportion, flow rate and column temperature on relative correction factors were investigated; the chromatographic peaks of the components were located according to the relative retention time. The content of paeoniflorin as internal reference was determined by external standard method, and the other four components were determined by QAMS, and then compared with the results of external standard method. RESULTS: The separation degree of each component to be measured was greater than 1.5. The linear range was 3.97-119.22 μg/mL for paeoniflorin,1.96-58.68 μg/mL for rutin,2.39-71.64 μg/mL for oroxin B, 1.92-57.51 μg/mL for baicalin, 0.54-16.24 μg/mL for cinnamates(r≥0.999 7), respectively. RSDs of precision, reproducibility and stability tests were all lower than 2%. Average recoveries were 97.20%-98.07%(RSD<3%,n=6). RCFs of rutin, oroxin B, baicalin and cinnamates were 0.554 6,1.815 6,2.489 3 and 5.423 2, using paeoniflorin as internal reference. RSDs of RCF and relative retention time were all lower than 5% under different chromatogram conditions. Absolut relative error of four components (except for internal reference) in 10 batches of Yanyan tablets sampled by QAMS and external standard method were all less than 1%. The results of the two methods were identical. CONCLUSIONS: The established method is accurate, rapid, efficient and inexpensive, and it can be used for simultaneous determination of 5 indicator components in Yanyan tablet.
