1.The Modified Uvulopalatopharyngoplasty(UPPP)to Prevent the Complications and Destroy of UPPP.

Objective To improre typieal proeedure of UPPP,increase clinical effects and deerease complioations caused by the operation.Methods Twenty eight patients with obstructive sleep apnea syndrome who had been confirmed by PSG in our hospital from 2003 to 2005 were involved in this study.1)improving stripping method:the fat and tonsil were removed by electric knife.2)improving molding method:the muscle of uvula was maitained,but the fat was removed;mucous and the tissue under it were double-layer sutured.3) the data before and 6 months after UPPP were analyzed by questionnaire and polysomnography(PSG)detection.Result No fissuration occurred.The distance between soft palate and posterior wall was 3?0.6 before operation and it was 6?0.8 6 months after operation.The effect is 100% based on the questionnaire investigation and PSG detection.Conclusions 1)Hemostasis of electric knife was reliable;2)Double-layer suture of mucous and the tissue under it can avoid hemorrhage and insure molding of pharyngeal cavity;3)The results of the uvula preservation can improve the effect of the UPPP by increasing the nasopharyngeal and oropharyngeal cavity,and avoiding velopalatal insufficiency.

4.Preparation of candidate reference materials for potassium in frozen human mixed serum

Objective To prepare the candidate reference materials for frozen mixed serum potassium, in order to calibrate and evaluate the conventional methods to achieve mutual recognition of the results.Methods Fresh sera without hemolysis,lipemia and choloplania were collected.Serum pools were packed in the freezing tube in the 1 000 (1 ml/per).Use the Single Factor Analysis of Variance(ANOVA) to evaluate the homogeneity.The short-and long-term stability [(2 -8℃,room temperature,37 ℃) and long-term (-80 ℃ ) ] were investigatedby linear regression analysis.The value was assigned by transfer from NIST SRM-956c using reference method by ICP-MS and uncertain was calculated.We observed the commutability of 25 fresh patient serum samples and 3 levels candidate RMs between reference method and three analyzed systems.The candidate RMs( refernce materials) were then distributed to 33 laboratories in Beijing to apply in routine assay by using different detection systems.Results By statistical analysis of the SPSS 17 statistical software, the F value of homogeneity test of each level candidate RMs was 0.247, 0.117, 0.162.All of them were less than F0.05 (9,20) =2.39;Stability can be last at least 12 months,30 days, 12 days and 4 days at -80 ℃, 2-8℃, room temperature and 37℃respectively.The definited values of three levels candidate RMs for potassium were (2.349 ±0.028) mol/L,(3.845 ±0.024) mol/Land (5.831 ±0.042)mol/L;Coordinate dots of 3 levels candidate RMs are all within 95% confidence interval range of 25 serums regression line.They have the same speciality as serum.In correctness verification survey of 33 clinical laboratory of conventional methods,97%bias in results The biases of 97%RMs were within the range of ±2.5%( ±1/2 CLIA′88Tea).Conclusion The homogeneity, stability and commutability of 3 levels candidate RMs all meet the requirement and the target values are assigned accurately.
5.Risk factors analysis of blood type unexpected antibody in large sample of inpatients

【Objective】 To observe and analyze the detection results of blood type unexpected antibody and its risk factors in inpatients, so as to provide reference for promoting the safety of clinical blood transfusion. 【Methods】 Clinical data of 22 800 inpatients were collected retrospectively and unexpected antibody screening was performed by microcolumn gel method and saline method. According to the screening results, the inpatients were divided into positive group (n = 62) and negative group (n = 22 738). Clinical data of the two groups were collected and analized by univariate analysis, and specificity identification of unexpected antibody was performed in the positive group. The specific distribution characteristics of antibody were statistically analyzed, and the risk factors of unexpected antibody were analyzed by binary logistic regression. 【Results】 Among the 22 800 inpatients, the yield rate of unexpected antibody was 0.27% (62/22 800), with Rh, MNS, Lewis, Kidd blood group antibody, mixed antibody, autoantibodies and others accounted for 40.32% (25/62), 14.52% (9/62), 6.45%(4/62), 1.61%(1/62), 20.97%(13/62), 6.45%(4/62) and 9.68%(6/62), respectively. Univariate analysis showed that there were significant differences in gender, age, pregnancy history, blood transfusion history, blood transfusion frequency and disease type between the positive group and the negative group by chi-square test (χ2 = 11.142, 6.994, 12.453, 4.762, 5.493, 92.381, all P<0.05), while there was no significant difference in nationality (χ2 = 3.719, P>0.05 ) . Binary logistic regression analysis showed that female, age >60 years old, with history of blood transfusion, number of blood transfusion >3 times, history of pregnancy, solid tumor and blood diseases, severe internal medical diseases were independent risk factors for unexpected antibody(P<0.05) . 【Conclusion】 For patients who need blood transfusion, especially those with the above risk factors, conducting unexpected antibodies screening and identification before transfusion, avoiding corresponding antibodies and selecting antigen-negative blood for cross-matching were suggested to ensure the effectiveness and safety of clinical blood transfusion.
6.Application Value of Blunt Separation Method in the Modified Sytinger Technology PICC Catheterization in Hemopathic Patients.

OBJECTIVE:
To explore the application value of blunt separation method in the modified sytinger technology peripherally inserted central catheter (PICC) catheterization in the hemopathic patients.
METHODS:
One hundred and twenty cases of the hemopathic diseases used modified sytinger technology PICC catheterization were selected from January 2016 to July 2017 in our Hospital, and randomly divided into blunt and routine group, each with 60 patients. For the routine group, the routine longitudinal method was used to expand the skin, for the blunt group the blunt separation method was used to expand the skin.
RESULTS:
At the time point of 24 h after the blunt catheterization, the bleeding volume and exudation rate in the blunt group were significantly lower than those in the rouline group (P<0.05). At the 1, 3, 5 d after catheterization, pain visual analogue score (VAS) showed that the scores of blunt group were significantly lower than those of the routine group (P<0.05). in expanding skin, the successful rate of catheterization once in blunt group and routine group were not significantly different (P>0.05).
CONCLUSIONS
Compared with the longitudinal method, the blunt separation method has considerable skin expansion and sheath feeding effect on the modified sytinger technology PICC catheterization for the hemopathic patients. This method can effectively reduce the patient's catheter trauma and percolation, and is helpful to relieve the patients' pain symptoms, worthing for further clinical promotion.

7.Explore the bacterial spectrum and drug ressitance in the respiratory tract in schizophrene

Objective To explore the bacterial spectrum and drug resistance that separated from respiratory tract of schizophrene.Methods 311 samples taken from the respiratory tract of schizophrenes were investigated and analyzed.Results From 311 samples 58 pathogenic bacteria were separated,the gram negative was 55.93% and the gram positives was 44.07%;5 most pathogenic commoly bacteria were klesbsiella pneumoniae and S.aureus and escherichia coli and streptococcus and enterococcus;Except that all the staphylococcus were sensitive to vancomycin,enterobacteriaceae were sensitive to imipenem,all the other separated strains showed total resistance to the agents tested in different levels,also the multidrug resistance emerged in lower respiratory tract infections with mulitidrug resistance in high levels.Conclusion The most common pathogenic bacteria separated from these schizophrene are klesbaiella pneumoniae and S.aureus.In clinical therapy,sensitive antibiotics should be selected according to the drug sensitive tests,combine with other agents when necessary to accelerate the clearance of bacteria from infectious respiratory tract in schizophrene.

9.The determination of potassium in human serum by isotope dilution inductively coupled with plasma mass spectrometry

Objective To establish a method for the determination of serum potassium using isotope dilution inductively coupled plasma mass spectrometry.Methods Frozen human serum pools of different potassium concentrations were prepared from healthy donors of Chao-yang Hospital laboratory in 2010 Serum samples were diluted with 1% HNO3 supplemented with 41K spike and isotope ratio was measured after selection and optimization of ICP-MS analysis conditions.Serum 39K concentrations were calculated by interpolation method.3 different serum samples were used to evaluate the precision of the method.Standard Reference Material 909b Ⅰ and Ⅱ of National Institute of Standards and Technology were used to evaluate the accuracy.The recovery was evaluated through 2 serum samples added standard material for potassium,uncertainty was calculated.Results The analytical precision of serum potassium measurement was 0.14% to 0.11%.The results of SRM909b Ⅰ and Ⅱ gave the coefficients of variation (CVs) of 0.39% to 0.32%.Data was consistent with the certified value and the CV was + 0.12% and + 0.49% respectively.The recovery was 99.68% to 100.12% for 2 different serum samples.Conclusions ID-ICP-MS method was successfully established to measure the serum potassium concerntration precisely and accurately,it is an ideal method for the accurate determination of inorganic elements.
10.Clinical analysis of MAP in transabdominal laparoscopic radical nephrectomy of renal cell carcinoma

【Objective】 To evaluate the significance of Mayo adhesive probability (MAP) in predicting surgical difficulty and postoperative recovery in patients with renal cell carcinoma (RCC) undergoing laparoscopic radical nephrectomy (LRN). 【Methods】 The clinical data of 168 RCC patients who received transabdominal LRN during Jan.2017 and Dec.2020 were retrospectively analyzed. According to MAP, the patients were divided into low MAP group (n=100) and high MAP group (n=68). The differences in perioperative clinical data were compared between the two groups. 【Results】 Compared with low MAP group, the high MAP group had longer operation time (P<0.001), more intraoperative blood loss (P<0.001), higher Clavien-Dindo grade complications (P=0.008), longer hospital stay (P=0.003), higher levels of c-reactive protein (P=0.030) and IL-6 (P=0.009), lower levels of albumin (P<0.001) and prealbumin (P=0.020). 【Conclusion】 MAP can assess the risk of prolonged operation time, increased bleeding during transabdominal LRN, and postoperative recovery, thus guiding the preoperative planning.
