1.Inoculation of bacille calmette guerin combined with intraperitoneal adenosine injection enhances antibacterial effect of macrophage against Mycobacterium tuberculosis in mice.

OBJECTIVEThe protection rate of inoculation with BCG vaccine is only 50 percent, and most of patients with tuberculosis had a history of BCG vaccine inoculation. Adenosine (ADO) has an immunomodulating effect; it promotes immune reaction by increasing number of macrophage and enhancing phagocytosis. The present study was designed to investigate if combined use of adenosine with BCG enhances the anti-Mycobacterium tuberculosis effect of macrophage in mice.
METHODSFifty BALB/C mice were divided randomly into 3 groups: BCG group (n = 21), BCG plus ADO group (n = 21) and control group (n = 8). The mice in BCG and BCG plus ADO groups were inoculated with 0.1 ml BCG intradermally and the mice in BCG plus ADO group were injected intraperitoneally with ADO 30 mg/(kg.d) for 5 days. The mice in BCG group and control group were injected with NS 0.1 ml/d for 5 days. Six weeks after the last injection, all mice were challenged with intravenous 1 x 10(6) CFU human Mycobacterium tuberculosis virulent strain. After challenging, lung and spleen specimens were taken at the 10th, 20th and 30th days from the mice of BCG and BCG plus ADO groups and at the 30th day from mice in control group. The pathological examinations of lung and spleen sections were performed after HE staining and acid-fast staining, and detection of cell apoptosis was also performed.
RESULTSConsolidation with neutrophil infiltration was found in most of the lung tissue taken at the day 30; there were a lot of tuberculous granulomas and Mycobacterium tuberculosis in the lungs of control group. The alveolar septum in BCG gradually became wide and in interstitium lymphocyte infiltration dominated, and there were less tuberculous granulomas but there were large number of Mycobacterium tuberculosis in the lungs from 10th to 30th days after challenging. The widening of alveolar septum and consolidation of lung tissue in BCG plus ADO group became milder with monocytes infiltration, and there were few tuberculosis granulomas and Mycobacterium tuberculosis in the lungs from 10th to 30th days after challenging.
CONCLUSIONADO could increase the number of monocyte-macrophages and promoted anti-bacterial effects of these cells.

2.Clinical application of Wallis interspinous dynamic stabilization in treating adjacent segment degeneration (ASD) after lumbar spinal fusion.

OBJECTIVETo evaluate therapeutic effects of Wallis interspinous dynamic stabilization in treating ASD after lumbar spinal fusion.
METHODSTotally 40 patients (included 16 males and 24 females, aged 25 to 60 years old) with degenerative disc disease were treated with posterior interbody fusion. Among them, 20 cases (treatment group) were treated with posterior interbody fusion combined with Wallis interspinous dynamic stabilization, while other 20 cases (control group) only treated with posterior interbody fusion. JOA score and VAS score were compared after inserted Wallis interspinous dynamic stabilization at 1 month and 3 years, and changes of intervertebral disc height of adjacent segment and cross-sectional area of the canal were tested and compared.
RESULTSAll patients were followed up from 3 to 5 years with an average of 3.6 years. All injuries were healed at stage I and the pain were released after treatment. There were no significant meaning in JOA score and VAS score at 1 month after treatment between two groups (P>0.05), while had meaning at 3 years (P<0.05). There were no statistical significane in intervertebral disc height of adjacent segment and cross-sectional area of the canal at 1 month after treatment (P>0.05), while had statistical meaning at 3 years (P<0.05).
CONCLUSIONThere is no difference in immediate effects between two groups. Both of them can obtain good results for effective decompression. Medial-term effectiveness of treatment group is obviously better than control group, which depends on Wallis interspinous dynamic stabilization to plays good biology effects and effective accelerate adjacent degeneration caused by lumbar fusion.

3.The clinical comparison of double knee osteoarthritis patients undergoing unicompartmental knee arthroplasty and total knee arthroplasty.

OBJECTIVETo study the differences in clinical outcome of double knee osteoarthritis patients undergoing unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA).
METHODSFrom May 2009 to May 2012, 30 patients (60 knees) with isolated compartmental osteoarthritis of knees were enrolled. Each patient accepted UKA on one knee, TKA on the other. There were 9 male and 21 female patients, aged from 60 to 79 years, average 69 years. Patients evaluation focused on the hospital for special surgery(HSS) knee score, blood loss, hemoglobin 48 h after the operation, the time of knee being able to flex to 90° and patients' sensation after operation. Collection the UKA side and TKA side data and compare two groups of data.
RESULTSAll patients were followed up for 13 to 35 months, average 20.5 months. There were no component loosening and revision. HSS knee score improved significantly in both two groups: UKA group was promoted from 61 ± 3 to 87 ± 3 (t = 11.21, P < 0.001) and TKA group from 59 ± 5 to 86 ± 3 (t = 17.64, P < 0.001). Compared with the TKA group, the UKA group had less blood loss (t = 11.56, P < 0.001), and a decrease of hemoglobin 48 h after the operation (t = 12.38, P < 0.001). The dates of knees being able to flex ≥ 90° after operation were less (t = 4.03, P < 0.05) in the UKA group. As to therapeutic effects, 70% patients found that UKA was better than TKA; 16.7% patients had opposite opinion; and 13.3% patients found no differences between their two knees.
CONCLUSIONSUKA for the treatment of isolated compartmental osteoarthritis of knee shows as well as TKA, and it has less trauma, less blood loss, more rapid postoperative recovery than TKA.

4.Construction of the dioxin bioassay method based on the clonal expressed aryl hydrocarbon receptor system.

OBJECTIVETo study the specific binding of the artificial clonal aryl hydrocarbon receptor translocator (ARNT) with the natural aryl hydrocarbon receptor (AhR) and the recolonization by polyclonal antibody. The dose-response relationship with tetrachlo-rodibenzo-dioxin (TCDD) was also studied to develop TCDD detection method and the binding degree related to dose response.
METHODS(1) The target genes including AhR-PAS, AhR-C and ARNT-PAS were amplified by RT-PCR by using the total RNA purified from the liver cells of C57BL/6J mice as templates to construct pGEX-5X1 recombinants. The recombinant plasmids were expressed in E. coli. (2) The rabbits were immuned by the clonal fusion proteins: AhR-PAS, AhR-C to prepare the polyclonal antibody. (3) The natural AhR from the hepatic cytosol of C57BL/6J mice was extracted. The artificial cloning expressed fusion protein:GST-ARNT-PAS and the natural AhR were incubated in different dose of TCDD. The quantity of the heterodimer through affinity adsorption and Western blots were measured.
RESULTS(1) The target proteins including AhR-PAS, AhR-C and ARNT-PAS were successfully cloned and expressed in E. coli. (2) The detection limit of polyclonal antibody AhR-PAS and AhR-C were 5 ng and 1 ng, respectively. (3) The total protein concentration prepared from the liver cells was 60.5 mg/ml. The artificial clonal protein ARNT-PAS could specifically bind to the natural AhR complex with the existence of TCDD. The detection limit of TCDD was 0.25 pmol which was 80 pg approximately.
CONCLUSIONA TCDD detection method based on the aryl hydrocarbon receptor system was established and the detection limit might reach pg grade.

5.Traditional Chinese Medicine Intervention on Th17/Treg Balance in Chronic Obstructive Pulmonary Disease: A Review

Chronic obstructive pulmonary disease (COPD), a common clinical chronic respiratory disease, has a long course and is intractable. It is closely related to many factors, such as immune imbalance. Helper T cell 17 (Th17), an immune-promoting cell, and regulatory T cell (Treg), an immunosuppressive cell, maintain the balance of the immune microenvironment together. In the course of COPD, the proportion of Th17 cells usually increases, while the proportion of Tregs that inhibit Th17 activity decreases. Their coordination and balance are critical in the inflammatory and immune processes of COPD. At present, COPD is mainly treated with nasal inhalation preparations and oral drugs by western medicine. In spite of a certain therapeutic effect, side effects of drugs and heavy economic burden are becoming increasingly prominent. Modern research shows that traditional Chinese medicine (TCM) has the characteristics of few side effects, stable curative effect, and multi-target regulation, and it is advantageous and promising in the prevention and treatment of COPD. In recent years, a large number of TCM clinical and experimental trials on the intervention of Th17/Treg balance in COPD have been launched. Substantial pieces of evidence confirm that the intervention of Th17/Treg balance is an important potential target of TCM in the treatment of COPD. This study reviewed the previous research on the intervention effect of single Chinese medicine, effective components of Chinese medicine, and Chinese medicinal compound on Th17/Treg balance in COPD to comprehensively reveal the potential target of Th17/Treg balance in COPD for clinicians and scientific researchers, promote relevant research, and provide references for the rational application of TCM in the prevention and treatment of COPD.
