1.A expectation for breakthrough of glaucoma-related genetic study

Glaucoma,especially common primary open-angle glaucoma and primary angle-closure glaucoma,shows high genetic heterogeneity. The causing-disease genes known are difficult to explain some glaucomatous cases,and the study on the susceptibial gene of glaucoma do not achieve new breakthroughs. Combined with the newest progression in genetic study of glaucoma,some views were proposed in this paper in order to better understand the updating study of the pathogenesis mechanism.
2.Therapeutic window for the use of hyperbaric oxygen therapy in patients with spinal cord injury

Objective To compare the therapeutic effects of hyperbaric oxygen (HBO) therapy at different time points on patients with spinal cord injury. Methods A total of 284 cases of spinal cord injury (SCI) were divided into an HBO group and a control group. The control group was treated with routine interventions (including a dehydrator, medication for neurotrophy, rehabilitation management, acupuncture and supportive treatment). The HBO group was treated with HBO in addition to the routine treatments, initiated at different time points after onset of SCI ( from less than 8 hours to longer than 1 week). All of the patients were evaluated in terms of scores on the American Spinal Cord Injury Association (ASIA) assessment and Barthel's index (BI) before and after treatment.Results Both groups had excellent effects when the treatment was initiated within the first 8 hours after the onset of SCI. There was no significant difference between the two groups. Average ASIA and BI scores in both groups improved significantly within the first 24 hours, and over the 1 st week after the onset of SCI. The recovery of neurological function and ability in the activities of daily living (ADL) was significantly better when HBO treatment was initiated within 8 hours after SCI compared with that initiated after 24 hour and 1 week. The therapeutic effects in the HBO group patients were significantly better than in the control group when HBO was initiated at 8 and 24 hours after onset of SCI. Neither the HBO group nor the control group had significant further improvement in ASIA scores or ADL performance when treatments initiated after 1 week. Conclusions HBO can significantly improve neurological function and ADL performance after SCI if it is administered within 1-8 hours.
3.Techniques and feasibility of laparoscopic total gastrectomy combined with distal pancreatectomy and splenectomy

Laparoscopic curative gastrectomy has been widely adopted because it is minimal invasiveness.The efficacy of laparoscopic gastrectomy is comparable to that of open gastrectomy for patients with early or advanced gastric cancer,but few studies of laparoscopic total gastrectomy combined with distal pancreatectomy and splenectomy for patients with advanced gastric cancer have been reported.Seven patients with advanced gastric cancer received laparoscopic total gastrectomy combined with distal pancreatectomy and splenectomy at the First Hospital of Jilin University from September 2010 to September 2011.The mean operation time,blood loss,postoperative hospital stay and the nunber of lymph nodes resected were (218 ± 24) minutes,(366 ± 174) ml,( 14.6 ± 2.8 ) days and 32 ± 15,respectively.Two patients were complicated by pancreatic juice leakage and were cured with drainage. No anastomotic leakage,abdominal bleeding or mortality was observed. With the development of techniques and equipments,laparoscopic total gastrectony combined with distal pancreatectomy and splenectomy is safe and feasible in selected patients.
4.Analysis of prognostic factors in patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa

Objective To analyze the prognostic factors in patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa treated by surgery,and to investigate their guid roles in available post-operation adjuvant therapy. Methods The clinicopathologic records of 306 patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa who underwent radical hysterectomy and pelvic lymphadenectomy were retrospectively analyzed, and the prognostic factors were explored by univariate and multivariate methods. Independent prognostic factors were identified by COX proportional hazards regression model. Results The overall 5-year survival rate of these 306 patients was 78.1%. 6.Safety of umbilical cord versus bone marrow-derived mesenchymal stem cells transplantation for the treatment of nervous system lesions

BACKGROUND: The comparative study concerning the safety of umbilical cord and bone marrow-derived mesenchymal stem cells transplantation for the treatment of nervous system lesions is insufficient. OBJECTIVE: To assess the safety of umbilical cord and bone marrow-derived mesenchymal stem cells transplantation for treatment of nervous system lesions. METHODS: A total of 214 cases with neuropathy were randomly divided into A, B groups. Patients in the A group received umbilical cord derived stem cell transplantation, and those in the B group received bone marrow-derived mesenchymal stem cells transplantation. Totally (5-12)×108 stem cells were transplanted into each patient. RESULTS AND CONCLUSION: The count of lymphocytes, alanine aminotransferase, aspartate aminotransferase, IgA, and IgM were increased compared with those before treatment in both groups (P < 0.01); However there were no significant differences between two groups (P > 0.05). Moreover, white blood cell count and red blood cell count in cerebrospinal fluid of all patients were significantly greater than the normal level. There were no significant differences between two groups (P > 0.05). No significant differences of the positive rate of Pandy test and the incidence rate of adverse effect were found in both groups (P > 0.05). The safety of umbilical cord and bone marrow-derived mesenchymal stem cell transplantation for treatment of nervous system lesions showed no marked differences.
7.Analyzing the infectious state of coxsackievirus B in hospitalized children

Objective: To understand the relationship between coxsackievirus B and pediatric diseases. Methods: The infectious state of coxsackievirus B in hospitalized children were studied. Among 796 children studied, there were 218 upper respiratory tract infection cases, 179 pneumonia, 106 asthma, 155 myocarditis, 19 allergic purpura and 89 other diseases. The antigen (CVB-Ag) and IgM (CVB-IgM) were detected using ELISA method. Results: (1)There were 47% positive of CVB in upper respiratory tract infection and 48% positive of CVB in pneumonia(no difference between them, P>0.05). (2) There were 62% positive of CVB in asthma, 61% positive of CVB in myocarditis and 68% positive of CVB in allergic purpura(no difference among them, P>0.05); But the positive rate of CVB in asthma, myocarditis and purpura were higher than in upper respiratory tract infection and pneumonia, (P<0.05). (3) There were lower positive rate of CVB in other kinds of diseases (16%) and in healthy children (3%)(no difference between them, P>0.05). Conclusion: CVB infection was related to several kinds of diseases, the relationship between CVB infection and diseases such as asthma, myocarditis, and allergic purpura should be further studied.
8.Quantitation of albumin in urine by HPLC and initial clinical application

Objective To measure albumin in urine by HPLC and conduct primary clinical application Methods Solvent gradient and appropriate wave length was optimized and performance of the HPLC method was evaluated.Urine albumin of 46 patients with diabetes was measured.Results In standard and urine,retention time of Alb was 13.1 min.The linear measuring range extends to 1 820 mg/L.The lower limit of measurment for Alb was 4.2 mg/L.The intra-assay CV and the inter-assay CV were 3.36%,4.12% and 1.93%,1.97% at 24.5 mg/L and 546.9 mg/L of Alb respectively.Analytical recovery rate were 96.3%,98.2% and 97.5%.Microalbuminuria rate was 54.3% by HPLC,26.1% by immunoassay in 46 patients with diabetes.Conclusions Measurement of Alb in urine by HPLC is feasible as routine method until quantifying urinary total Alb conveniently.HPLC is the same to suit research for diabetic nephropathy and so on.
9.Effects of GSM 1800 MHz radiofrequency electromagnetic fields on protein expression profile of human breast cancer cell MCF-7.

OBJECTIVETo study the effects of GSM 1800 MHz radiofrequency electromagnetic fields (RF EMF) exposure on protein expression profile of human breast cancer cell line (MCF-7), as to exploring the possible effects on normal cell physiological function.
METHODSMCF-7 cells were continuously or intermittently (5 minutes field on followed by 10 minutes off) exposed to RF EMF for different duration (1 hour, 3 hours, 6 hours, 12 hours, or 24 hours) at an average specific absorption rate (SAR) of 3.5 W/kg. The extracted proteins were separated by 2-dimensional electrophoresis and the protein-spot distribution of the silver-stained gels was analyzed by using PDQuest software 7.1. Each experiment was repeated three times.
RESULTSOn the average, around 1100 proteins were detected using pH 4 - 7 IPG strip. There were no differential proteins found under continuous exposure at SAR of 3.5 W/kg for 6 hours. Under other exposure conditions, we found various differentially expressed proteins in exposure groups as compared with the sham-exposed controls. Especially in 3 hours intermittent exposure and 12 hours continuous exposure, eighteen and seven differential proteins were detected, respectively. The categories and functions of these differentially expressed proteins were analyzed by searching of SWISS-PROT protein database, which suggested that these proteins should be related to the functions of biosynthesization, signal transduction, and DNA damage and repair.
CONCLUSIONSData indicated that the protein expression changes induced by RF radiation might depend on exposure duration and mode. Many biological processes might be affected by RF exposure.

10.Establishment of a three-dimensional finite element model and stress analysis of the talus during normal gait.

OBJECTIVETo develop a three-dimensional (3D) finite element model of the human ankle with fine details and analyze the stress distribution on the talus during different gait phases.
METHODSMimics13.0 and Geomagic10.0 software were used for geometric reconstruction of the ankle based on the 3D CT data of the foot. The model was meshed and assigned with the material properties in Hypermesh10.0 software. The model was then imported to Abaqus6.9, and the stress condition of the talus during the 3 phases (heel-strike, midstance, push-off) of normal gait was simulated to calculate the stress distribution within the bone.
RESULTSThe three-dimensional finite element model of the ankle established consisted of 21 865 nodes and 73 440 elements. The stress distribution within the bone in 3 phases of normal gait differed significantly. The peak von Mises stress on the talus dome, from the heel-strike to push-off phases, was 3.0 MPa, 4.3 MPa and 4.8 MPa, as compared to 1.3 MPa, 1.9 MPa and 2.8 MPa on the talar neck, 2.8 MPa, 3.0 MPa, and 3.4 MPa on the talonavicular joint surface, and 2.2 MPa, 1.8 MPa and 1.5 MPa on the subtalar joint, respectively.
CONCLUSIONThe finite element model of the talus shows a good response against the experimental data and can be used to simulate the biomechanic experiment of the talus.
