3.A comparative study of the influence of off-pump and on-pump coronary artery bypass grafting operation on thyroid hormones

Objective To compare the influences of off-pump and on-pump coronary artery bypass grafting operation on plasma thyroid hormones.Methods Forty patients undergoing coronary surgery were randomly divided into off-pump group(n=20)and on- pump group(n=20).Plasma thyroid hormones(T_3,T_4,FT_3,FT_4,rT_3)and thyroid-stimulating hormone(TSH)were studied perio- peratively.Results Before operation,the values of aU the variables were within normal range in the two groups.In off-pump group, T_3 and FT_3 concentration dropped significantly after operation,reaching the lowest value at first day postoperatively[T_3:(39.7?6.2) ng/dl vs.(78.6?2.1)ng/dl,P0.05),which were within normal range all along,rT_3 concentration rose significantly after operation,reaching the highest value at third day postoperatively[(82.1?26.8)ng/dl vs.(42.3?15.6)ng/dl, P0.05),whereas baseline,operative and postoperative variables were similar.Con- clusion Off-pump operation has some influences on plasma thyroid hormones,showing"low T_3 syndrome"postoperatively,that is similar to what on-pump operation does.
4.Status and influencing factors of emotional and behavioral problems among migrant workers' children in primary school

Objective To investigate the cause of migrant workers' children's emotions and behaviors and to provide basis for intervening measures. Methods The parents of 546 migrant workers' children and 510 urban children in primary school in Hangzhou were assessed with the Survey on General Condition and the Strength and Difficulties Questionnaire (SDQ) to survey the difference of the rate of abnormal emotion and behavior between two groups. The factors were analyzed using multiple linear regressions. Results There was no difference between the two groups on the age, gender, ethnic group and grade (P>0.05) . The rate of registered rural residents and one-child family were higher during the general primary school students than the migrant workers' children. According to the SDQ, the abnormal rates of migrant workers' children were higher than the general primary school students in the behavioral problem, peer communication, social behavior, difficult factors, impact factors (P<0.05 for all) . Multiple linear regression showed that the factors that there were more emotional and behavioral problems with the male (β:0.063-0.117, P<0.05 for all), the one-child (β: 0.075-0.369, P<0.05 for all), lower scores (β: 0.239-0.715, P<0.05 for all) and the lower grade (β=-0.089, P<0.05) . Conclusion The emotional and behavioral problems of migrant workers' children were outstanding.
5.Granulocytic sarcoma: a clinical and pathologic analysis of ten cases.

OBJECTIVETo investigate the clinical and pathological features, differential diagnosis of granulocytic sarcoma.
METHODSThe clinical manifestations, histopathological features, immunohistochemistry, treatment and prognosis were analyzed retrospectively in 10 cases of granulocytic sarcoma.
RESULTSThe age of patients ranged from 10 to 56 years (means = 35.8 years). The male-to-female ratio was 1.5:1. Histologically, the malignant cells of granulocytic sarcoma grew in a diffuse pattern. The cytoplasm was scanty, with eosinophilic fine granularity in some cells. The nuclei were round or focally irregular, and had finely dispersed chromatin. The mitotic figures were visible. Immunohistochemical stains for MPO, CD43, CD117, CD34 and CD99 were positive.
CONCLUSIONSGranulocytic sarcoma can occur in patients of all ages with a male predominance. The diagnosis of granulocytic sarcoma is assisted by the cytochemical stain for naphthol-ASD-chloroacetate esterase and/or immunophenotypic analyses for MPO, CD43, CD117, CD34, CD99. These stains aid in the distinction of granulocytic sarcoma from: lymphoblastic lymphoma, Burkitt lymphoma, diffuse large B-cell lymphoma, small round cell tumours, particularly in children, and blastic plasmacytoid dendritic cell neoplasm.

6.Recombinant human brain natriuretic peptide in acute decompensated heart failure

Objective To investigate the clinical efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in older patients with acute decompensated heart failure. Methods A total of 86 aged patients with acute decompensated heart failure who admitted to our hospital were enrolled. First, they were given recombinant human brain natriuretic peptide (rhBNP) loading doses 1. 5 μg·kg-1,3-5 min, intravenous injection, then continuous intravenous pump 0. 015 kg-1 for 3-5 days. The brain natriuretic peptide(BNP) ,left ventricular ejection fraction(LVEF) and blood urea nitrogen(BUN) ,serum creatinine(SCr) ,urine output/ intake ratio and blood pressure were compared befor and after treatment one week. Result Before and after treatment,BNP was(2387. 50 ± 1030. 99) pg·mL-1 and (783. 57 ± 476. 71) pg·mL-1; LVEF was(40. 85 ± 8. 76) % and(44. 35 ± 0. 94) %; the systolic blood pressure was (135. 21 ± 15. 58) mmHg and(127. 51 ± 16. 24) mmHg; the urine output/ intake ratio was (0. 81 ± 0. 50) and (1. 26 ± 0. 24). Statistically significant differences before and after treatment were found(all P < 0. 05). Conclusion The early application of rhBNP in older patients with acute decompensated heart failure is effective and safe. It can improve cardiac function,and has no adverse effect on renal function,and increases urine output.
7.The Correlation between DDVP Resistance of Culex pipiens pattens and Esterase Activity

Objective To detect the resistance index and esterase activity of each generation of DDVP-resistant Culex mosquitoes and analyze the relationship between insecticide resistance and esterase. Methods WHO bioassay and micro-plate measurement were used for the detection. Results The resistance index increased to 12.17 after 43 generations' insecticide selection compared to 1.00 as sensitive isolate. The nonspecific esterase(NSE) activity of the mosquitoes became strengthened with the extension of the generations, and the individual frequency of those with OD values no less than 0.9 increased gradually, consistent basically to the bioassay. The AChE average inhibition rate decreased with the extended generation and increased resistance, and the individual frequency of those with inhibition rate less than 30% became strengthened with the extension of generations, showing a positive correlation. Conclusion The activity of NSE and AChE shows a correlation with DDVP resistance.
8.Fire-needle therapy for deglutition disorders in post-stroke pseudobulbar palsy:a randomized controlled trial

Objective:To observe the clinical efficacy of fire-needle therapy in treating deglutition disorders due to pseudobulbar palsy in the remission stage of stroke.Methods:Sixty-two eligible subjects were divided into a fire-needle group and a rehabilitation group by a simple randomization method at a ratio of 1:1.The two groups received same basic intervention;in addition,the fire-needle group received fire-needle treatment,while the rehabilitation group received rehabilitation training.The two groups of subjects all received a 3-week treatment and were evaluated by the dysphagia severity rating scale (DSRS),modified Mann assessment of swallow ability (MMASA) and Kubota Toshio swallow test (KTST) before and after the intervention.The complications and adverse events occurred during the trial were recorded.The data were statistically analyzed.Results:At the third week,the DSRS,MMASA and KTST scores changed significantly compared with the baseline in both groups (P<0.05),and the changes in the fire-needle group were more significant than those in the rehabilitation group (P<0.05).The between-group comparison at the third week showed that the therapeutic efficacy in the fire-needle group was superior to that in the rehabilitation group (P<0.05).Conclusion:Fire-needle therapy can obviously change the DSRS,MMASA and KTST scores in pseudobulbar palsy in the remission stage of stroke,and significantly enhance the therapeutic efficacy of the treatment of deglutition disorders in this stage.
9.Role of conditioned growth medium for ventricular fibroblasts in promoting fibroblast collagen synthesis and proliferation

Ventricular fibroblasts were cultured using conditioned growth medium for ventricular fibroblasts (FCGM). The rate of the total collagen synthesis of ventricular fibroblasts was measured by assaying the incorporation rate of [3H]-proline, whereas the proliferation of ventricular fibroblasts was assessed by determining the incorporation rate of [3H]-TdR and the expression of c-fos genes. FCGM significantly increased the [3H]-proline incorporation rate and [3H]-TdR incorporation rate of fibroblasts in a dose-dependent manner. Furthermore, FCGM promoted the c-fos gene expression of fibroblasts, which attained its maximum in 1 h. BQ123, an ETA receptor antagonist, partially blocked the above effects of FCGM, but AT1 receptor antagonist CV11974 and α-adrenergic receptor antagoist regitin did not. It is suggested that the ventricular fibroblast has an autorine function in promotion of collagen synthesis and proliferation of fibroblasts by secreting endothelin and other bioactive substances.
10.Effect of aldosterone on the secretion of endothelin by ventricular fibroblasts

Using cell culture, radioimmunoassay for endothelin and RT-PCR, the effect of aldosterone on the endothelin secretion of ventricular fibroblasts was studied. The results showed that aldosterone (1×10-7 mol/L) promoted the expression of ppET-1 mRNA, which began to increase in 2 hours and attained the highest level in 4 hours, thereafter decreased; aldosterone increased the endothelin level in ventricular fibroblasts and fibroblast conditioned growth medium (FCGM) as well, which was blocked by spironolactone (1×10-6 mol/L), an aldosterone receptor antagonist. The results suggest that aldosterone can increase endothelin secretion by ventricular fibroblasts, which can be inhibited by its receptor antagonist spironolactone.
