1.Assessment of choroidal thickness of central serous chorioretinopathy by optical coherence tomography with enhanced depth imaging

Abstract? AlM: To evaluate the meaning of using optical coherence tomography with enhanced depth imaging ( OCT-EDl ) to measure choroidal thickness of central serous retinopathy ( CSC) .?METHODS: With the retrospective case control study, 65 patients (65 eyes) with CSC and 50 healthy controls (50 eyes ) with age, gender and diopter - matched were recruited in this study. OCT-EDl were used to measure the subfoveal choroidal thickness ( SFCT ) in CSC eyes, the fellow eyes and also the control eyes. Of which 40 of the 65 CSC patients self-cured, 14 of them were treated with photodynamic therapy ( PDT ) , the left 11 accepted the laser photocoagulation ( LP ) . SFCT were measured 3mo after that. Compared with the previous data, the statistical analysis was carried out.?RESULTS: The SFCT value of 65 CSC eyes, 65 fellow eyes and 50 control eyes were 436. 23 ± 89. 50, 389. 45 ± 101. 03 and 329. 36 ± 95. 87μm, respectively. The SFCT of suffer and fellow eyes increased significantly compared to the control eyes ( P = 0. 008 and 0. 013, respectively). There was also significant difference in SFCT between the CSC eyes and the fellow eyes (P=0. 021). The SFCT were significantly decreased after PDT (P=0. 032), but with no significant changes after LP or self-cured ( P=0. 057 and 0. 076, respectively).?CONCLUSlON: OCT-EDl is a useful method to assess the choroidal topographic changes of CSC. The SFCT are significantly increased in the CSC eyes compared with that in the fellow eyes and the control eyes.
2.Effect of hyperoxia solution and hypertonic saline infusion on the arteries blood gas analysis of rabbits with traumatic hemorrhagic shock

Objective To study the effect of hyperoxia solution and hypertonic saline infusion on arteries blood gas analysis of traumatic hemorrhagic shock rabbits,and interaction of hyperoxia solution and hypertonic saline.Methods Forty rabbits established into traumatic hemorrhagic shock model by Lamson' s method.Post hoc analysis was adopted in trial according to hyperoxia solution and hypertonic saline and 40 rabbits were divided into four groups(n =10):saline(NS) group,normal hyperoxic solution (NSO) group,hypertonic saline(HS) group,hyperosmotic-hyperoxic solution(HSO) group.Then blood samples were collected at pre-shock,post-shock,the end of limited resuscitation and hemostatic resuscitation for blood gas analysis.Results At the pre-shock,post-shock stages,PaCO2 and PaO2 were no significant difference in each groups ( P >0.05 ).At the end of limited resuscitation and hemostatic resuscitation stage,PaCO2 in NSO group (33.50 ±5.93),(37.22 ±6.74) mm Hg,(31.70 ±7.39),(35.10 ±7.56 )mm Hg in HS group,HSO group(38.70 ±4.92),(41.80 ± 5.51 )mm Hg were higher than that in NS group ( 27.40 ± 3.60 ),( 30.83 ± 2.79 ) mm Hg (F =6.894,4.287,respectively ;P < 0.05 ),but there were no cross correlation between hypertonic saline and hyperoxia solution ( P > 0.05).There were no significant difference in PaO2 in each groups at the end of limited resuscitation and hemostatic resuscitation stage ( P > 0.05 ).Conclusions Both hyperoxia solution and hypertonic saline can effectively relieve hyperventilation,but there is no interaction between them.
3.Analysis of diagnosis and treatment of high-risk esophageal foreign body

[ABSTRACT]OBJECTIVETo investigate the diagnosis, treatment method, key points of operation, and postoperative complications of high-risk esophageal foreign body.METHODSA retrospective analysis of 41 cases of high-risk esophageal foreign body from January 1996 to December 2014. After adequate preparation, the foreign body was removed via esophageal endoscope under general anesthesia.RESULTSThe foreign bodies in 41 patients were removed via esophageal endoscope once or twice. Two cases suffered postoperative subcutaneous emphysema, that may be a result of a small perforation in esophagus. Emphysema was disappeared by fast, rehydration and anti-infection for 6 to 8 days, and other serious complications did not occur.CONCLUSIONMost high-risk esophageal foreign bodies can be removed through rigid esophagoscopy. Some of the foreign bodies of the patients were difficult to remove, some patients were presented with mediastinal emphysema and pneumothorax due to esophageal perforation, and some foreign body stuck in oesophagus so long to cause esophageal mucosa ulcer. In these conditions, foreign bodies should be removed by lateral neck incision or thoracotomy.
4.The clinical effect of iontophoresis of hyaluronidase in treating infants with congenital muscular torticollis

Objective To observe the clinical effects of hyaluronidase iontophoresis in the treatment of in- fants with congenital muscular torticollis(CMT).Methods Fifty infants with congenital muscular torticollis were divided randomly into a treatment group and a control group.Manual stretching was performed with both groups,while hyaluronidase iontophoresis was also administered to those in the treatment group.The range of side-flexion and rota- tion of the neck was examined at the beginning of and after 1 and 2 months of treatment.At the end of the treatment, the overall outcome was assessed according to a scoring system.Results There was no significant difference be- tween the two groups at admission.Compared with the control group,the range of side-flexion and rotation of the neck,and the overall treatment outcome were all significantly better in the treatment group after treatment.Conclu- sion Hyaluronidase iontophoresis can effectively improve function in infants with CMT and alleviate their symptoms.
5.Analysis of risk factors and prognosis on the post-resuscitation multiple organ dysfunction syndrome after successful cardiopulmonary resuscitation

Objective To investigate the risk factors and prognosis on the post-resuscitation multiple organ dysfunction syndrome(MODS) after successful cardiopulmonary resuscitation(CPR).Methods Clinical data of 53 patients who were suffered with cardiac arrest(CA) and undergone successful CPR with return of spontaneous circulation(ROSC) were analyzed. Results There were 48 patients accompanied with MODS(90.6%),and among them,35 patients died in hospital(66.0%).All the 10 patients with CPR interval ≥6min were accompanied with MODS and they died in hospital.There were 43 patients who underwent CA immediately and of them,38 patients were accompanied with MODS.The incidence and mortality of MODS in the patients with CA-ROSC interval 0.05).Conclusion The risk factors such as ROSC interval ≥6 min,AC-ROSC interval ≥10min and the SIRS after ROSC are significantly associated with the incidence of MODS.The organic function of the patients should be evaluated promptly.
6.Expression of recombinant human acetylcholinesterase and its application in screening its inhibitors.

This study is designed to obtain recombinant human acetylcholinesterase (rhAChE) and apply it in screening acetylcholinesterase inhibitors. The rhAChE was overexpressed in HEK293 cells transfected by plasmid of pCMV-AChE with the cationic liposome and rhAChE was found to be secreted into cell culture medium. AChE activity was assayed according to modified Ellman method to obtain kinetic parameters. IC so50 values for donepezil compounds of rhAChE were calculated to determine their activities of inhibition. The results showed that Km value was 151.9 micromol.L-1 donepezil inhibited rhAChE in a mixed competitive-noncompetitive way (Ki= 16.03 nmol.L-1, Ki = 18.36 nmol.L-1) and that most new compounds tested exhibited high activities of inhibition on rhAChE. The study suggests that rhAChE is available to be applied in screening AChE inhibitors in vitro.

8.Study of p16INK4/CDKN2 gene homozygous deletions and point mutation in squamous cell carcinoma of buccal mucosa.

OBJECTIVETo explore the correlation between homozygous deletions and mutation of p16 gene and the carcinogenesis and progression of squamous cell carcinoma of buccal mucosa.
METHODSThirty buccal cancers, 10 leukoplakias and 8 buccal mucosas were involved. DNA was extracted from the tissues. PCR was used to analyses homozygous deletion of p16 gene. PCR-SSCP-DNA sequencing was performed to detect the point mutation of p16 gene. Immunohistochemical techniques were used to detect the expression of P16 protein.
RESULTSGene deletions and point mutations were not found in leukoplakia and normal buccal mucosa. Gene deletions were found in 7 samples out of 30 cases of squamous cell carcinoma of buccal mucosa (23.3%), while point mutations were found in 5 samples out of 30 cases of squamous cell carcinoma of buccal mucosa (16.7%). Sequencing analysis showed that 5 cases point mutations were missense mutations, occurred on exon 2. Three cases occurred in the same point, codon 99 (GAT --> AAT). The result of immunohistochemical stains showed that 11 out of 12 cases gene inactivation did not expressed P16 protein.
CONCLUSIONHomozygous deletion and point mutation of p16 were the main pattern of gene inactivation in squamous cell carcinoma of buccal mucosa. There was a closely correlation between p16 gene inactivation and the carcinogenesis of squamous cell carcinoma of buccal mucosa.

9.Time course degeneration and expression of glial fibrillary acidic protein in mer-knockout mice.

BACKGROUNDMüller cells in the mammalian retina normally express low levels of glial fibrillary acidic protein (GFAP); however, its expression is upregulated in response to the loss of retinal neurons. The change in expression of GFAP is one of the earliest indicators of retinal damage and is correlated with the time course of disease. The aim of this study was to investigate the time course of degeneration and the expression of GFAP in the retina of mer knockout mice.
METHODSA total of 30 mer knockout mice, aged from 15 - 20 days to 1 year and 32 age-matched wild type mice as controls were tested. Immunohistochemistry was used to show the expression of GFAP in the central and peripheral retina of mer knockout and control mice at postnatal age of 15 days (P15d), 20 days (P20d), 4 weeks (P4w), 6 weeks (P6w), 8 weeks (P8w), 3 months (P3m), 6 months (P6m) and 1 years (P1y).
RESULTSThe expression of GFAP in the central and peripheral retina of wild type mice was limited to the retinal ganglion cell and nerve fiber layers. In the central retina of mer knockout mice, GFAP expression was upregulated at P4w and GFAP immunolabelling penetrates across the entire thickness of the retina at P8w; whereas in the peripheral retina, the GFAP expression was upregulated at P20d and GFAP immunolabelling penetrates the entire retina after P4w.
CONCLUSIONSIncreased expression of GFAP in Müller cells of mer knockout mice occur at P20d in the peripheral retina and P4w in the central retina. GFAP expression in Müller cells appears to be a secondary response to the loss of retinal neurons. Increased expression of GFAP may occur prior to any detectable morphological changes in the retina. This study suggests that the loss of retinal neurons may begin in the early stages of retinitis pigmentosa, prior to the discovery of any morphological changes in the retina.

10.Clinical application of Wallis interspinous dynamic stabilization in treating adjacent segment degeneration (ASD) after lumbar spinal fusion.

OBJECTIVETo evaluate therapeutic effects of Wallis interspinous dynamic stabilization in treating ASD after lumbar spinal fusion.
METHODSTotally 40 patients (included 16 males and 24 females, aged 25 to 60 years old) with degenerative disc disease were treated with posterior interbody fusion. Among them, 20 cases (treatment group) were treated with posterior interbody fusion combined with Wallis interspinous dynamic stabilization, while other 20 cases (control group) only treated with posterior interbody fusion. JOA score and VAS score were compared after inserted Wallis interspinous dynamic stabilization at 1 month and 3 years, and changes of intervertebral disc height of adjacent segment and cross-sectional area of the canal were tested and compared.
RESULTSAll patients were followed up from 3 to 5 years with an average of 3.6 years. All injuries were healed at stage I and the pain were released after treatment. There were no significant meaning in JOA score and VAS score at 1 month after treatment between two groups (P>0.05), while had meaning at 3 years (P<0.05). There were no statistical significane in intervertebral disc height of adjacent segment and cross-sectional area of the canal at 1 month after treatment (P>0.05), while had statistical meaning at 3 years (P<0.05).
CONCLUSIONThere is no difference in immediate effects between two groups. Both of them can obtain good results for effective decompression. Medial-term effectiveness of treatment group is obviously better than control group, which depends on Wallis interspinous dynamic stabilization to plays good biology effects and effective accelerate adjacent degeneration caused by lumbar fusion.
