1.Clinical observation of hypertonic saline used for the treatment of acute intracranial hypertension in patients with hemorrhagic shock

Objective To observe the effect of hypertonic saline complex solution (hypertonic saline plus hydroxyethyl starch,HSH) on patients with severe cerebral trauma,high intracranial pressure and shock by the measurement of the changes of the mean arterial pressure (MAP),central venous pressure (CVP) and intracranial pressure (ICP),as well as GOS score changes followed up for 6 months,in order to determine the value of HSH treatment in severe cerebral trauma,intracranial hypertension and shock.Methods Sixty patients with severe brain injury and uncorrected hemorrhagic shock were selected,while the degree of coma was assessed by using GCS score,and shock severity was estimated by using the shock index (SI) score.The patients were randomly divided into HSH group (n =30) and mannitol group (MT group,n =30).Thirty minutes,60 min and 120 min after administration either solution,The changes of MAP,CVP and ICP were observed in two groups,and all patients were followed up for 6 months to observe the outcomes of patients.Results There were no statistically significant differences in age,gender,GCS score,SI scores,and other medication between two groups (P > 0.05),and they were comparable between two groups.After resuscitation of patients in two groups,MAP and CVP were elevated,but the effect of HSH appeared sooner and higher within 30 minutes [MAP (63.1 ± 8.8) mmHg vs.(51.0-9.3) mmHg] (P < 0.05);At the same time,ICP dropped more than 10% lower [ICP (27.3 ± 5.9) mmHg vs.(32.8 ± 4.1) mmHg] (P <0.05),while the effect of MT appeared more slowly in hemodynamic improvement;at 120 min,the increase in MAP and reduction in ICP in HSH group were more significant than those in MT group [MAP (65.9 ± 13.2) mmHg vs.(60.4 ±7.2) mmHg] (P <0.01);the ICP [(22.2 ±4.7) mmHg vs.(28.1 ±6.1) mmHg] (P < 0.01).Followed up for 6 months,good recovery rate in HSH group was higher and poor recovery rate was lower than those in MT group.Conclusions In patients with acute intracranial hypertension and uncorrected hemorrhagic shock,the employment of hypertonic saline plus hydroxyethyl starch solution can produce faster and more effective therapy for shock and reduce intracranial pressure,improving the long-term neurological function of patients.
2.The survey of cataract surgery quality on blindness prevention in Jiangyan County

Objective To explore the cataract surgery quality on blindness prevention and postoperative problems in village in short period. Design Population-based survey. Participants 251 cases(254 eyes) received operation and 131 cases(134 eyes)were surveyed 6-month postoperatively. Methods Patients were examined 6-month after the small incision extracapsular cataract extraction and intra ocular lens(IOL) implantation. Examinations were conducted by a special oculist including far vision, near vision, external in- spection, anterior segment, posterior segment, intraocular pressure. Main Outcome Measures Visual acuity, intraocular pressure, diopter, eye complications of surgery. Results Naked far vision of the operated eye more than or equal to 0.3 was 41.8%, naked far vi- sion of the eye more than or equal to 0.05 was 82.8%; corrected far vision more than or equal to 0.3 was 64.2%, corrected far vision more than or equal to 0.05 was 92.3%. Naked near vision more than or equal to 0.1 was 79.9%, corrected near vision more than or e- qual to 0.1 was 85.8%. The main postoperative complications were ametropia, posterior capsule opacification(PCO), deformed pupil, pupil displacement, pigments of IOL, eccentric IOL and intraocular hypertension. The chief reasons of eyes that could not be recovered were vitreous, retina or optic nerve diseases, the key factors that caused living vision less than 0.3 were ametropia, PCO, the disease of vitreous, retina and optic nerve. Conclusions The serious complications affecting the surgery result are limited in a low range. The most important factors of the eye corrected far vision less than 0.05 are the vitreous, retina and optic nerve diseases. In order to improve the visual sight, we should add equipment to calculate the IOL diopter accurately.
3.Effects of methylprednisolone on cytokines in patients after successful cardiopulmonary resuscitation

Objective To investigate the effects of methylprednisolone on the release of cytokines in patients after successful cardiopulmonary resuscitation(CPR).Method Thirty patients after CPR with restoration of spontaneous circulation(ROSC)were randomly divided into two groups:group A(methylprednisolone group,n=14)and group B(control group,n=16)during the period from May 2005 through May 2007.The patients of group were treated with methylprednisolone 3 mg/kg by intravenously twice a day after ROSC.The levels of serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10 (IL-10)were measured by enzyme-linked immunosorbent assay(ELISA)before CPR,and 24,48,72 hours and 7 days after ROSC.The data were analyzed studentis t test and chi-gquare test.A P value less than 0.05 indicated significant difference.Results There was on significant difference in the mean time from cardiac arrest to return of spontaneous circulation,and the levels of serum cytokines between the two groups before CPR(P>0.05).In comparisorl with group B,the levels of serum TNF-α,IL-1β,IL-6,IL-8 decreased markedly at 24 and 48 hour after ROSC in group A(P<0.05-0.01),and the levels of serum IL-8 decreased markedly at 72 hours after ROSC in groupA(P<0.05).Thelevels of serum TNF-α,IL-1β,IL-6,IL-8 were not of significant differences between the two groups at 7 hys after ROSC(P>0.05).There was no significant difference in the levels of serum IL-10between the two groups at different time points after ROSC(P>0.05).Conclusions Methylprednisolone plays a role of preventive effects on patients with ROSC after PCR through decreasing the levels of serum TNF-α,IL-1β,IL-6,IL-8.
4.Analysis of risk factors and prognosis on the post-resuscitation multiple organ dysfunction syndrome after successful cardiopulmonary resuscitation

Objective To investigate the risk factors and prognosis on the post-resuscitation multiple organ dysfunction syndrome(MODS) after successful cardiopulmonary resuscitation(CPR).Methods Clinical data of 53 patients who were suffered with cardiac arrest(CA) and undergone successful CPR with return of spontaneous circulation(ROSC) were analyzed. Results There were 48 patients accompanied with MODS(90.6%),and among them,35 patients died in hospital(66.0%).All the 10 patients with CPR interval ≥6min were accompanied with MODS and they died in hospital.There were 43 patients who underwent CA immediately and of them,38 patients were accompanied with MODS.The incidence and mortality of MODS in the patients with CA-ROSC interval 0.05).Conclusion The risk factors such as ROSC interval ≥6 min,AC-ROSC interval ≥10min and the SIRS after ROSC are significantly associated with the incidence of MODS.The organic function of the patients should be evaluated promptly.

Human violent behavior is a complex behavior which is influenced by genetic and environmental factors. There is a trend in investigating the mechanism of violent behavior by using the genetic methods. This article reviews several candidate genes and advances in epigenetics which are associated with violent behavior. The prospects and significance of violent behavior research from the view of gene polymorphism and epigenetics are also discussed.

6.Study on application of the whole genome amplification in LCN.

OBJECTIVE:
To establish a method for whole genome amplification (WGA) based on (multiple displacement amplification MDA), and achieve DNA analysis of the human genome from low copy number (LCN).
METHODS:
DNA sample was used for WGA according to the REPLI-g kit protocol (QIAGEN, Germany). WGA product was used for DNA analysis according to the Applied Biosystems Profiler Plus kit protocol (ABI, USA).
RESULTS:
WGA product of DNA sample (10 pg) can be used for STR genotyping.
CONCLUSION
WGA technology could be helpful for LCN DNA analysis.

7.Establishment of the multiplex genotyping system for 16 SNP loci on mtDNA.

OBJECTIVE:
To establish a multiplex genotyping system of mtDNA SNP.
METHODS:
A multiplex analysis system of 16-plex mtDNA SNP loci was established with allele specific PCR and capillary electrophoresis genotyping technology. Fifty samples from unrelated Chinese Han individuals were typed with the multiplex system. The multiplex assay was validated by comparing with the direct sequencing method.
RESULTS:
The genotypes of all 50 samples were correctly determined by the multiplex system. The optimal genotypic graphs were obtained with an input DNA of 0.5-10 pg, and the typing results were completely consistent with those by direct sequencing method.
CONCLUSION
The established multiplex system by allele specific PCR has high sensitivity, operational simplicity and high accuracy. It provides an effective and high output method for mtDNA SNP typing.

Allele-specific polymerase chain reaction (AS-PCR) is a technique based on allele-specific primers, which can be used to analyze single nucleotide polymorphism (SNP) effectively including the transition, transversion and insertion/deletion polymorphism and has been exploited in the study of diseases research, molecular diagnosis, and forensic biological evidence. The article systematically reviews the principle, the detection methods, improvement of AS-PCR, and its research updates in the fields of autosome, Y chromosome and mitochondrial SNP, as well as its application in forensic science.

9.Application of Ion Torrent PGM™ System in Detection of Fetal DNA in Maternal Plasma.

OBJECTIVE:
To explore the feasibility of detecting of Y-STR of fetal DNA in maternal plasma using Ion Torrent PGM™ System.
METHODS:
A total of 16 fetal DNA samples from maternal plasmas (8 cases from 38 weeks gestational age and 8 ones from 12 weeks) were prepared and a multiplex assay with 7 STR loci (DYS390, DYS391, DYS393, DYS438, DYS437, DYS456, DYS635) was designed for multiplex-PCR amplification. Using Ion Torrent PGM™ System, the results of Y-STR sequences and capillary electrophoresis were obtained and compared.
RESULTS:
Y-STR specific alleles were detected in the maternal plasma of all the pregnant women having male babies of second and third trimester, which were higher than that detected by capillary electrophoresis. Consistent Y-STR genotypes were observed between fetal DNA from maternal plasma and genomic DNA from the newborn babies.
CONCLUSION
Based on Ion Torrent PGM™ System, the prenatal Y-STR detection method may provide a high-sensitive and high-throughput choice for prenatal STR detection in forensic testing.

10.Effect of PCR reaction volume on the accuracy of human identification tests.

OBJECTIVE:
To investigate the effect of different PCR amplification volume on the accuracy of human identification test.
METHODS:
Human genome DNA samples were amplified using ABI PRISM Profiler Plus kits in 50 microliters, 25 microliters, 12.5 microliters, and 6.25 microliters reaction volume, respectively. The thermocycle parameters were the same. All PCR products were then electrophoresized on ABI PRISM 310 Genetic Analyzer, 377 DNA Sequencer, and 3100 Genetic Analyzer. Data were processed by ABI PRISM GeneScan and Genotyper software.
RESULTS:
The less reaction volume, the more alleles losing or alleles adding observed.
CONCLUSION
Non-standard volume of PCR amplification reaction should be used carefully in human identification test, especially when the sample DNA quality is not so satisfied.
