1.Study of the application of scleral flap made by tunnel knife in trabeculectomy

Objective:To study the effect and complication of trabeculectomy with scleral flap made by tunnel knife in prospective randomized clinical trial.Methods:A total of 40 glaucoma eyes were divided into two groups by randomized method,Trabeculectomy was performed with fornix-based flaps,and scleral flaps made by tunnel knife(group A);or limbus-based flaps,and scleral flaps made by general method(group B)respectively.The samples were observed for 6 months after operation and the effect and complication of the two groups were compared.Results:There was no significant difference between group A and B in success rate,which were 75% and 70% respectively;and the conditiong of complication was the same.Conclusion:It can effectively simplify the procedure of fornix-based flaps trabeculectomy with scleral flaps made by tunnel knife.
2.Preparation of monoclonal antibody to human augmenter of liver regeneration:screening of hybridomas with unpurified antigen expressed by E.coli

Objective:To prepare monoclonal antibody(McAb) against human augmenter of liver regeneration(hALR) by hybridoma technique with unpurified recombinant protein expressed by E.coli as screening antigen.Methods:BALB/c mice were immunized with hALR thioredoxin(hALR Thi) fusion protein twice.The splenocytes were fused with myeloma cells by polyethylene glycol and the hybridomas were selected in HAT medium.The positive hybridomas secreting anti hALR McAb were screened by means of ELISA,with the lysates of E.coli containing the recombinant plasmid pQE30 hALR induced by IPTG as screening antigen and containing plasmid pQE30 as controlling antigen.The reactivity of McAb secreted by the positive hybridoma to hALR expressed in eukaryocyte and native hALR in human serum was confirmed by means of ELISA and Western blot methods.Results:A positive hybridoma was screened successfully;the specific antigen antibody reaction was confirmed between the McAb prepared and hALR expressed in eukaryocyte and native hALR in human serum.Conclusion:Without any purification procedure,recombinant protein expressed by E.coli can be used for screening bybridomas in preparing of McAb with the expressed product of empty plasmid as control;anti hALR McAb can be used to the further study of hALR.

5.Hyperfractionated radiotherapy combined with chemotherapy for advanced esophageal carcinoma

Purpose:To evaluate the effect of late accelerated hyperfractionated radiotherapy (LCAHFR) combined with chemotherapy on advanced esophageal cancer.Methods:176 cases of advanced esophageal cancer were treated from March 1994 to March 1997.Patient were divided into 2 groups, including radiotherapy alone group (80 cases) receiving conventional irradiation to a total of 64~70 Gy and combined treatment group (96 cases) which received LCAHFR groups plus chemotherapy with PF regimen. Combined treatment group patients were treated with conventional fractionated radiotherapy during the firs two thirds of the treatment to a dose about 40 Gy/20 F/4 weeks. This was followed by accelerated hyper fractioned radiotherapy using reduced fields, twice daily at 1.5 Gy per fraction to a dose about 24~30 Gy.All cases of the combined treatment group were treated with PF(DDP 20 mg iv drip on days 1 to 5,and 5 FU 750 mg iv drip on days 1 to 5,Every 28 days as one cycle).All patients were followed for ≥ 5 years.Results:The short term results of LCAHFR combined with chemotherapy was better than that of radiotherapy alone. The difference between these two groups was statistically significant( P 0.05).The one , three and five year survival rate were 69.8%,51.1%,29.2% in combined group and 46.3%,21.3%,11.3% in simple irradiation alone group. 6.Osteoblast differentiation and mature gene regulation inside femoral head with steroid-induced osteonecrosis in response to mechanical stress stimulation

Objective To study dynamic changes of gene expressions and protein synthesis of Runx2 (runt-related transcription factor-2), Osterix, osteocalcin and AJ18 inside the femoral head with steroid-induced osteonecrosis after mechanical stress stimulation in rats. Methods A total of 50 Wistar rats (half male in sex) weighing 250-270 g (mean 260 g) were involved in this study and randomly divided into experimental group (40 rats) and normal group (10 rats). The rats in experimental group were injected with dexamethasone (20 mg/kg) via bilateral gluteus maximus alternatively once a week and then trained on laboratory animal treadmill twice weekly to make rat model of femoral head necrosis. After identifying the successfully induced model by Hematoxylin and eosin stain, glucocorticoid injection was ceased and the experimental group was randomly divided into model control group, 4 weeks, 6 weeks, 8 weeks groups after hormone training stopped. Then, total RNA and total protein were extracted from femoral head for detecting dynamic changes of genes expressions and proteins synthesis of Runx2, Osterix, osteocalcin and A J18 after mechanical stress stimulation inside the femoral head with steroid-induced osteonecrosis by means of real-time quantitative PCR and Western blot assay. Results In 4 weeks, 6 weeks, 8 weeks groups after hormone training stopped, the gene expressions and proteins synthesis of Runx2, Osterix and osteocalein were reduced more significantly compared with model control group, mBNA expression values of Runx2, Osterix and osteoealcin were 0. 1809, 0. 1639, 0. 1374 and 0. 4219, 0. 3026, 0.2652 and 0. 2857, 0.2027, 0. 1583 times of those in model control group. The expressions of Runx2, Osterix and osteocalcin showed a downward trend with time. The mBNA expression and protein synthesis of AJ18 at 4th, 6th and 8th weeks after hormone training stopped were 2.6391,4. 2718 and 5. 3165 times of model control group. Conclusions In addition to hormonal factors, inappropriate mechanical stress inhibits expressions and proteins synthesis of Runx2, Osterix and osteocalcin, while the expression and protein synthesis of AJ18 are upgraded in early steroid-induced femoral head necrosis in rats.

ObjectiveTo investigate the proliferation of rat's osteoblasts promoted by mechanical strain via the protein kinases C ξ(PKCξ) protein path-way.MethodsOsteoblasts were retrieved from SD rats' skulls in the sterilized environment.BioDynamic testing instrument was used to exert 2% and 0% mechanical strain on rats' osteoblasts for 180 min on the each group(2% mechanical strain was experimental group and 0% mechanical strain was control group); and the same method was applied on the rats' osteoblasts which pretreated with the classical PKC ξ inhibitor with 2% mechanical strain (inhibitor group).The cell cycles of rats' osteoblasts were measured by flow cytometry; and Western blotting and immunofluorescence were used to detect expression of protein PKC ξ and phosphorylationed PKC ξ in the rats' osteoblasts.ResultsThe mechanical strain obviously increased the ratio of S period in the cell cycles.Compared to the control group and inhibitor group,a significant increase of the expression of protein PKC ξ and phosphorylationed PKC ξ in the osteoblasts was detected in the mechanical strain experimental group.ConclusionPKCξ can respond to the stimulus of the mechanical strain,which promotes the proliferation of osteoblasts through PKCξ pathway.And it plays an important role in the process of signal conduction.
8.Breast-conserving surgery for early-stage breast cancer guided by intra-operative ultrasound

Objective To evaluate the feasibility of intra-operative ultrasound guide in breast conservative-surgery and its effect on positive margin rate and breast tissue volume.Methods Fifty-five cases of invasive breast cancer staged T1-2N0-1 were randomly assigned to palpation-guided group(control group) 26 cases and ultrasound-guided group(experiment group) 29 cases.Before and during operation,high-frequency ultrasound was used to guide the resection of breast cancer with 1 cm margin in experiment group.The resection margin positive rate deteced by tissue section frozen biopsy and resected breast tissue volume were compared between experiment and control group.Results The clinicopathologic features were accordant between two groups.The margin positive rate and near-margin positive rate in experiment group were 0 and 3.45% respectively,slightly lower than that of control group 3.85% and 15.38%,but with no statistical significance (P > 0.05).The same situation occurred at the re-exision rate.The resected breast tissue volume in experiment group (32.40 ± 10.93) cm3 was lower than that of control group (55.11 ± 12.88) cm3,and with statistical significance (P < 0.01).Conclusions Intra-operative ultrasound guide could reduce the margin positive rate and unnecessary resected brcast tissue volume in breast conservative-surgery,and improve cosmetic effect.

Objective To investigate the nutritional components and aging-delaying action of some wild berries such as vaccimum vitia-idaea, vitis amurensis, and actinidia arguta in Changbai Mountainous area. Methods Contents of vitamin B 1, B 2, C, niacin, ?-carotene, and minerals such as Fe, Ca and Zn were analyzed. Wild berry juice was administered to aged mice for 30 days respectively at the doses of 3 g?kg -1 and 6 g?kg -1. Then activities of SOD and GSH-Px, and contents of MDA and lipofuscin were measured. Results Contents of vitamin B 1, B 2, C, ?-carotene, and Fe, Ca, Zn in the berries were significantly higher than those in common cultivated fruits. 10.Meta-analysis of use of highly cross-linked and conventional polyethylene liners in total hip arthroplasty

Objective To assess whether highly cross-linked polyethylene liners are superior to conventional polyethylene liners in total hip arthroplasty (THA).Methods Randomized controlled trials (RCTs) of highly cross-linked and conventional polyethylene liners in THA were retrieved from MEDLINE,PubMed,EMBASE,Cochrane Central,CBMdisc,CBM,and VIP.Until April 2012,reference lists of the included articles and related reviews were manually searched as supplements.Meta analysis was performed with RevMan 5.1.Mean difference (MD) was used to assess the linear wear rate in highly cross-linked group versus conventional group.Odds ratio (OR) was used to estimate the osteolysis and revision incidence in the two groups.Results Meta analysis indicated that the two-dimensional linear wear rate [MD =-0.08,95% CI (-0.1,0.06),P < 0.05],three-dimensional linear wear rate [MD=-0.15,95%CI (-0.22,0.08),P<0.05],volumetric wear rate [MD=-53.03,95% CI (-84.39,-21.67,P<0.05],total linear wear [MD=-0.19,95%CI (-0.23,-0.15),P<0.05],and osteolysis incidence [OR =0.23,95% C1 (0.13,0.39),P < 0.05] were significantly reduced in highly cross-linked group than in conventional group.There were no statistical differences between the two groups concerning the creep value of the polyethylene liners and revision incidence.Conclusions Highly cross-linked polyethylene liners for primary THA present a low incidence of wear and osteolysis.As the number and quality of the included studies are limited,a long-term follow-up of more large-sample and high-quality RCTs are needed to evidence the outcome.
