1.Operation Practice of Outpatient Prescriptions Audit before Payment in Our Hospital

OBJECTIVE:To provide reference for prescriptions audit before payment in hospital pharmacy. METHODS:The management measures of prescriptions audit and intervention before payment in our hospital were introduced. 1 200 outpatient pre-scriptions within 1 year before and after management were subjected to a statistical analysis. The effects of prescriptions audit be-fore payment were evaluated through analyzing and comparing qualification rate of prescription and unqualified prescriptions. RE-SULTS:Our hospital adopted a series of measure to realize prescriptions audit and intervention before payment by clinical pharma-cists,such as using information audit system,formulating audit standard,educating pharmacists,establishing audit job system, prescriptions review and feedback system,setting up communication system,etc. Of the total audited 189 665 prescriptions, 14 581(7.69%)failed to audit and 606 prescriptions hadn’t been revised after 1 year management,with effective intervention rate of 95.84%. Compared with before management,the prescription qualification rate increased from 88.83% to 98.67% after manage-ment(P<0.01). The quantity of overdose,inconformity with medical insurance,overload and other conditions reduced to 0. CON-CLUSIONS:It is feasible for pharmacists to audit and intervent prescriptions before payment in hospital pharmacy to improve pre-scription qualification and promote rational drug use in the clinic.
2.Effects of puerarin combined with saxagliptin on renal fibrosis in type 2 diabetic rats

[ABSTRACT]AIM:Toobservetheeffectsofpuerarincombinedwithsaxagliptinonrenalfibrosisintype2dia-betic rats.METHODS:Fifty male Wistar rats were used, of which 8 rats were randomly chosen as normal control group , and the remaining rats were used to establish the type 2 diabetic model.The rats that met the criterion for the diabetic mo-del were randomly divided into model group , puerarin treatment group , saxagliptin treatment group , puerarin combined with saxagliptin treatment group and metformin combined with saxagliptin treatment group .The above-mentioned drugs were ad-ministered for 8 weeks.After that period, all rats were sacrificed.The kidney index (kidney weight/body weight),and blood glucose and HbA1c were examined in all the rats.The morphological changes were observed by HE and Masson stai-ning.The levels of TNF-αand macrophage migration inhibitory factor (MIF) in the serum were measured by ELISA .The mRNA expression of TNF-α, MIF and CD68 was examined by RT-PCR.RESULTS: Compared with normal group , the kidney index, blood glucose and HbA1c, the levels of TNF-αand MIF in the serum and the mRNA expression of TNF-α, MIF and CD68 were increased (P<0.05) in the kidney tissues of model group .Compared with model group , the kidney index, blood glucose and HbA1c, the levels of MIF and TNF-αin the serum and the mRNA expression of TNF-α, MIF and CD68 were decreased ( P<0.05) in puerarin combined with saxagliptin treatment group .CONCLUSION:Puerarin com-bined with saxagliptin reduces blood glucose , decreases MIF and TNF-α, and down-regulates the mRNA expression of TNF-α, MIF and CD68 in the kidney tissues of type 2 diabetic rats, which may contribute to the inhibition of renal fibrosis .
4.Clinical analysis of complications in children with cerebral palsy

Objective To investigate the prevalence rate of complications in children with cerebral palsy (CP).Methods 80 CP children were recruited.Their parents were interviewed for high risk factors and feeding his- tory.Each case was assessed neurologically for type of CP and diagnosed for complications.And all cases were detect- ed by magnetic resonance imaging(MRI)of the brain.Results Among 80 patients,60 cases(75%)suffered from complications,the majority patients had difficulty in taking in food,intelligence retardation,hearing impediment and vision impediment.The prevalence rate of complications with mixed CP was 85 %.MRI abnormalities of the brain were accounted for 74.5 % and periventrieular leakomalacia(PVL)was more common(45.1%).Conclusion This study suggested timely rehabiliatation and interventions must be started to significantly improve the levels of intelli- gence.And the epilepsy and nonepileptic events must be identified to avoid mistakes in diagnosis.
5.Exploration on the relationship between positive antisperm antibody and nitric oxide level of infertile patients and on integrative Chinese and Western medicine treatment.

OBJECTIVETo study the relationship between the positive antisperm antibody (AsAb) and nitric oxide (NO) content in blood of infertile patients and the therapeutic effect of integrative Chinese and western medicine (ICWM) treatment on the disease.
METHODSInfertile patients, 75 of AsAb positive and 68 of AsAb negative were selected to observe the mature partuient rate and abortion rate. Serum NO level were measured before treatment to observe the relationship between NO and positive AsAb. Patients were treated with ICWM and followed-up for five years.
RESULTSMean value of serum NO in patients with positive AsAb was significantly higher than that in fertile women, and there was no significant difference between patients with negative AsAb and fertile women (P < 0.01). In the 5 years after treatment, the mature partuient rate between the two groups showed insignificant difference (P > 0.05).
CONCLUSIONAsAb positive and NO level abnormal may interfere mutually, to intervent the reproductive process. ICWM therapy could effectively regulate auto-immunity and endocrine function, and make the infertile patients obtain satisfactory efficacy.

8.A prospective observational study on nutritional status of patients with pancreatic tumor

Objective To investigate the rates of undernutrition and nutritional risks of surgical pa-tients with pancreatic tumors .Methods Totally 121 surgical patients with pancreatic tumors from Depart-ment of Surgery Beijing Hospital were enrolled in a prospective study during January 2014 to December 2015 . Patients were divided into two groups:the pancreatic cancer group ( n=90 ) and other pancreatic tumor group ( n=31 ) .Nutritional Risk Screening 2002 was used to assess the nutritional status .Other data including an-thropometric measure, body composition, blood biochemistry and clinical outcome were collected and ana-lyzed.Results Among 121 patients, the mean age was ( 61.9 ±13.6 ) years, the mean body mass index was ( 23.20 ±2.95) kg/m2 , the mean mid-upper circumference was ( 28.8 ±3.5 ) cm, the mean muscle weight was (44.6 ±7.4) kg, and the mean fat mass was (16.8 ±7.6) kg .There was no significantly differ-ence in anthropometric measurement results and body compositions between two groups ( all P>0.05 ) .In the pancreatic cancer group, albumin [ (39.0 ±4.7) g/L vs. (42.3 ±2.9) g/L, P<0.001], total protein [ (62.8 ±6.2) g/L vs.(66.3 ±2.9) g/L, P<0.001], and prealbumin [ (136.1 ±85.4) mg/L vs. (197.8 ±112.6 ) mg/L, P=0.011 ] were significantly lower than those in the other pancreatic tumor group and a higher fasting blood-glucose [ (6.45 ±2.47) mmol/L vs.(4.95 ±0.79) mmol/L, P<0.011] was found.Among all patients , the rates of undernutrition and nutritional risk were 4.1% and 78.5%, and the pancreatic cancer group had a higher rate of nutritional risk (91.1% vs.38.7%,χ2 =36.525, P<0.001). Conclusion In this prospective study , surgical patients with pancreatic cancer have a high incidence of nutri -tional risk, with low protein level and abnormal glucose metabolism .
9.Effect of rhynchophylline on TGF-β1/Smad pathway for processing ven-tricular remodeling in spontaneously hypertensive rats

AIM:Toinvestigatetheeffectofrhynchophylline(Rhy)onbloodpressure,cardiachypertrophy and myocardial fibrosis in spontaneously hypertensive rats ( SHR) .METHODS:Spontaneously hypertensive rats were ran-domly divided into model group , high dose (10 mg? kg-1? d-1 ) and low dose (2.5 mg? kg-1? d-1 ) group of rhyncho-phylline, captopril group (17.5 mg? kg -1? d-1).Wistar-Kyoto rats were used as normal control.Respectively, systolic blood pressure was measured by tail cuff every 2 weeks.After 10 weeks, heart weight index and left ventricular weight in-dex were calculated .The myocardial hydroxyproline and plasma angiotensin Ⅱwere detected .Moreover , basic myocardial histopathological changes and myocardial collagen fibres were observed by HE staining and Masson staining , respectively . The protein expression of TGF-β1 and Smad3 in the myocardium was measured by the methods of immunohistochemistry and Western blot .RESULTS:Compared with SHR model group , Rhy significantly reduced blood pressure ( P<0.05 ) , the levels of HYP in the myocardium (P<0.05) and the levels of AngⅡin the plasma (P<0.01).The pathological dama-ges of the myocardial tissues and collagen deposition were attenuated .The protein expression of TGF-β1 and Smad3 was sig-nificantly reduced by the treatment with Rhy (P<0.01).CONCLUSION:Rhynchophylline reduces blood pressure and adjusts to improve ventricular remodeling of SHR .The mechanism may be involved in the TGF-β1/Smad pathway and re-ducing AngⅡcontent.
10.Protective Effect of Extract from Morindae Officinalis on the Oxidative Injury of Human Sperm Motility

ObjectiveTo observe the protective effect of extract from Morindae officinalis on the oxidative injury of human sperm motility.MethodsThe sperm specimens were cultivated in vitro, the reactive oxygen species (ROS) was made from hypoxanthine-xanzine xanzine (HX-XO), and sperm specimens were co-incubated with ROS and different dosages of extract and oxygen. The motor parameters were evaluated, the motility were analyzed by the test of capillary tube penetrating, and compared with the control group.ResultsThe sperm function were injured, the motility and penetrating ability decreased evidently under the action of ROS, and with a significant difference compared with the normal group ( P<0.001). The sperm motility and penetrating ability could be improved by all of the small, medium and massive extract compared with vitamin C, especially 0.25 and 0.5 g/ml Morindae officinalis had more evident action in the rate of progressive motility and vitality exponent.ConclusionThe ROS can injury the sperm motility and penetrating ability, the appropriate content of extract from Morindae officinalis can significantly intervene peroxidation in sperm by the ROS, and protect the sperm motility.
