AIM: To report the use of modified four ports pars plana vitrectomy in managing a dislocated posterior chamber intraocular lens.METHODS: A young man with bilateral pseudophakia and dislocated PC-IOLs had undergone modified four ports pars plana vitrectomy in relocating a dislocated posterior chamber intraocular lens. This procedure was surgically less tedious and demanding compared with previous methods of IOL retrieval.RESULTS: No significant intra- or post-operative complication was noted. The visual recovery of the operated eye was rapid with the best corrected visual acuity at 14 months post-operation being 0.5. In contrast, the right eye without operation due to patient refusal developed total retinal detachment with hand movement visual acuity.CONCLUSION: Modified four ports pars plana vitrectomy may be a more ophthalmic surgeon friendly alternative in the retrieval of dislocated IOL.

ObjectiveTo aralyze The prevalence of premature children, mortality and complications to related factors, discussed measures to reduce the premature children mortality. MethodsFrom January 2003 to December 2006 and January 2007 January 2010 12 after 8 years in our hospital obstetric preterm children born in 1110 into the first 4 years of group and group 4 years after the two groups. Each group of children on the incidence of premature causes of death, complications were retrospectively analyzed. Results Preterm birth rate of first 4 years 2.85％ to 3.67％ after 4 years,the difference was statistically significant( P ＜0.05 ). Between the two groups the proportion of different age groups and the main types of diseases,no significant difference(P＞ 0.05). By comparison,the mortality of premature children dropped from the 12.06％ to 2.36％, a significant difference before and after ( P ＜ 0.01 ).ConclusionThe prevention and treatment of premature children was still very trduous task, particularly in preventing complications of preterm children was very important to prevent the occurrence of lung disease.

BACKGROUND:Acellular dermal matrix possesses good flexibility and simple trimming.The intracutaneous or subcutaneous injection of acellular dermal matrix powder has fibroblast migration and collagen deposition.It has been widely used in plastic and reconstructive surgery.OBJECTIVE:To explore the feasibility of reconstructed acellular dermal matrix as a scaffold for chondrocyte implantation.DESIGN,TIME AND SETTING:Comparative observation.The study was performed at the Peking University Medical Department and Beijing Jishuitan Hospital between August 2003 and February 2007.MATERIALS:Neonatal calf dermis was provided by Beijing Yuanheng Shengma Biology Technology Research Institute.A total of 24 healthy adult SD rats,weighing 250 g,regardless of gender,and 36 New Zealand rabbits,aged 3 months,were selected.METHODS:①Calf full-thickness back skin was incubated with cell free buffer or ABS/AES for decellularization,followed by surface modification using growth factors.②Three rectangle skin flaps at two sides of the spinal cord of rats were made,and implanted with acellular dermal matrix.The implants were harvested at 2,6,and 12 weeks postoperatively.③The rabbits were divided into experimental and control groups.The cartilage was obtained from the left articular facet to isolate chondrocytes.The chondrocytes were seeded on the acellular dermal matrix.The cartilage defect was made on the right hind limb of experimentalrabbits,and implanted with acellular dermal matrix containing autoiogous chondrocytes.Biogel wass dropped on the surface of carrier.In the control group,the cartilage defect was made on the right hind limb of rabbits and the wound was sutured.Two rabbits from control group and 5 from experimental group were selected respectively at 4,12 and 24 weeks postoperatively.MAIN OUTCOME MEASURES:Cross-linking effect comparison;repair effect of rabbit bone defects.RESULTS:①The acellular dermal matrix cross-linked by glutaraldehyde demonstrated an obvious inflammatory reaction with tissue bleeding and necrosis.Conversely,ADM treated with water-soluble cross-linking agent caused displayed good histocompatibility.②The cartilage defects were repaired completely;the attached cells survived and proliferated and the acellular dermal matrix was degraded after 24 weeks of surgery.CONCLUSION:The acellular dermal matrix decellularized with cell free buffer,digested with digestive buffer,Cross-linked by water-soluble cross-linking agent,and further decorated with growth factor exhibited good histocompatibility,and was suitable forcell attachment and growth.The acellular dermal matrix scaffold almost degrades in the rabbits,with no rejection,and the bone defects were repaired after 24 weeks.

To analyse the relationship between tumor necrosis factor (TNF)-α and interleukin-6 with hyperuricemia in ethnic Uygur,Kazak,and Han in Xinjiang province.1 200 cases of Uygur,Han,and Kazak populations were randomly enrolled.Serum uric acid,triglyceride,total cholesterol,high density lipoprotein-cholesterol,low density lipoprotein-cholesterol,fasting plasma glucose,body weigh and height,waist circumference,and hip circumference were measured.Waist-to-hip ratio and body mass index were calculated,and TNF-α,interleukin-6 were determined.The differences between uric acid and inflammatory factors were statistically significant; The differences in TNF-α was statistically significant between high uric acid group and control group in Uygur and Han,and the differences in interleukin-6 was statistically significant between high uric acid group and control group in Uygur,Kazak,and Han(P＜0.01).TNF-α and interleukin-6 were positively correlated with serum uric acid in Uygur,but in Kazak only interleukin-6 was negatively correlated with serum uric acid,in Han only interleukin-6 was positively correlated with serum uric acid(P ＜ 0.01).Hyperuricemia was associated with inflammatory factors,TNF-α,interleukin-6,but there were ethnic differences among Uygur,Kazak,and Han,thus it may provide people of new ideas for prevention and treatment of hyperuricemia and other related metabolic disorders.

Objective: To study a new method of alveolar cleft bone grafting. Methods: Group A: 62 cases (74 sides) alveolar cleft patients were transplanted with self-ilium spongy bone. Group B: 26 cases (30 sides) alveolar process patients used autologous ilium combined with DDM. Anterior occlusal radiographs and panoramic oral radiogram were taken before and after the operation to observe if there was new bone formation in the bone grafting area. According to Bergland grade criterion, analyzed the X-ray results after 3 months of the operation. Results: Group A: 17cases(17 sides) in class 1 group(23%), 17 cases(20 sides) in class 2 group(27%), 13 cases(14 sides) in class 3 group (19%), 15 case(23 sides) in class 4 group(31%). The overall survival rate of ABGR was 68.9%, and the clinical success rate was 50 %;Group B:16 cases(16 sides) in class 1 group(53.3%), 7 cases(8 sides) in class 2 group(26.7%), 2 cases(4 sides) in class 3 group (13.3%), 1 case(2 sides) in class 4 group(6.7%). The overall survival rate of ABGR was 93.3%, and the clinical success rate was 80%. Conclusion: Ilium spongy bone combined with DDM is good for alveolar process cleft bone grafting. It is better to use decalcified dentinal matrix of human than autologous ilium spongy bone.

The heterogeneity of high-density lipoprotein-cholesterol (HDL-C) in hyperuricemia was analyzed in Uygur,Han,and Kazak ethnic groups in Xinjiang.A total of 2 396 subjects were surveyed.The HDL-C levels were significantly decreased in hyperuricemia group compared with control group in Han and Uygur,while the reverse occurred in Kazak.The serum uric acid (SUA) level in Kazak was the lowest.The SUA was negatively correlated with HDL C in Kazak (P ＜ 0.05).HDL-C was a recognized protective factor for cardiovascular disease.With the lowered SUA level in the Kazak group,HDL-C level was increased.When SUA level was higher than a certain level (such as the SUA-4),there existed a negative impact on cardiovascular disease

An HPLC method to determine six alkaloids of the Coptidis for Chinese Pharmacopoeia of 2015 Edition was established through C18 column. The mobile phase was CH3CN-0.25 mol·L-1 NH4Ac (36:64) (containing 8 mmol·L-1 SDS and adjusting pH 9.3 with ammonia) at the flow rate of 1 mL·min-1, the detective wavelength was 270 nm and the column temperature was 35 oC. The linear ranges of jatrorrhizine hydrochloride, columbamine hydrochloride, epiberberine hydrochloride, coptisine hydrochloride, palmatine hydrochloride and berberine hydrochloride were 0.006 96-0.233, 0.004 75-0.152, 0.003 30-0.528, 0.006 31-1.010, 0.004 71-0.753, 0.017 8-2.884 μg·mL-1, respectively. The average recoveries were 99.65%, 98.59%, 98.49%, 98.66%, 98.64%, 98.63% and RSD were 0.03%、0.15%、0.21%、0.12%、0.28%、0.23%, respectively. The method is simple and accurate, and can be used to determine the contents of jatrorrhizine, columbamine, epiberberine, coptisine, palmatine and berberine in the Coptidis.

Objective To summarize the image features of primary retrograde type A aortic dissection by CTA examination, aiming to improve the precise diagnosis of this type of specific aortic dissection, and to provide imaging evidence for subsequent therapies.Methods The CTA data of 17 patients with primary retrograde type A aortic dissection treated in Cardiothoracic Surgery Department of Nanjing General Hospital of Nanjing Military Command from January 2010 to December 2014 were retrospectively reviewed.The image features including the dissection type, the location and quantity of intimal tear, the aorta involving area, and the origin of branch vessels were analyzed.Results Nine cases of the total 17 patients had pericardial/pleural effusion.Fourteen patients showed intramural hematoma in ascending aorta, while 3 patients showed active blood flow and thrombosis formation in false lumen of ascending aorta.Five patients showed aortic ulcers and intramural hematoma in descending aorta, and the other 12 patients showed typical dissection image with active blood flow in both true and false lumen,among the 12 patients, there were obvious intimal separations observed in 11 cases, while multiple aortic ulcers in 1 case.Abdominal aorta was involved in l0 cases of the above 11 patients.All of the abdominal branch vessels were originated from true lumen in 2 of the 10 patients,and the other 8 patients showed 1-2 vessels originated from false lumen.Conclusion Primary retrograde type A aortic dissection is relatively rare.However,specific imaging features different from typical aortic dissection can be observed in this type of aortic lesion.Careful viewing of CTA image is extremely necessary for accurate diagnosis.

Objective To analyze and summarize the features of children's facial wound and to share the emergency treatment experiences.Methods An retrospective study was conducted on the pathogenesis,age distribution,emergency treatment methods and complications in 1407 children (under 15 years of age) during recent 8 years (from January 2004 to December 2011).Results In this study the major facial wound type was simply lacerations (1196/1407,85.0 ％),and the most common wound cause was trumbling injury (1154/1407,82.0 ％).The T area was the easiest injured,successively,forehead (562/1407,39.9％),chin (211/1407,15.0％),lips (196/1407,13.9％) and nose (95/1407,6.8％).Here were 2 peaks in age distribution which were 2 and 5 years of age,respectively.Pleased outcome would be found while normalized plastic means were taken.After 3 to 6 months following up,there were 5 infection cases,27 cases healed with hypertrophic scars,and 7 cases with traumatic tattoos in wound location.Conclusions The most common wound type is simply laceration.2 and 5 years of age are two of the highest risk stages.It is necessary to act up to plastic surgery principles and take children's particularity into account when emergency treatment,and to do our best to minimize the scar in the first operation time.

Objective:To investigate the effects of xuesaitong on the retinal microcirculation of patients with diabetic mtinopathy (DR).Methods:Ninety-five patients with DR admitted in our hospital were randomly divided into the control group and the observation group.Forty-eight patients in the control group were treated with conventional hypoglycemic agent,and those in the observation group were treated with xuesaitong.The retinal microcirculation indexes including EDV,PSV,RI,Vmax,Vmin,MV,hemorheology indexes including NBL,NBH,DE,Hct,AE,ESR,and clinical efficacy in both groups were observed and compared.Results:After treatment,the clinical efficacy of observation group was 87.8％,which was much higher than that of the control group (61.4％,P＜0.05).The EDV,Vmax,and Vmin of control group were significantly improved than those before treatment (P＜0.05),and EDV and PSV were much higher than those of control group,the RI,Vmax,Vmin,and MV were much lower (P＜0.05).Additionally,the NBL and NBH in control group were much lower compared with those before treatment,while NBL,NBH,DE,Hct,AE,and ESR in control group were improved than those in control group,which were much improved that those of control group (P＜0.05).Conclusion:Xuesaitong combined with conventional hypoglycemic therapy was effective in treating patients with diabetic retinopathy,which could significantly improved the retinal microcirculation and hemorheology.
