1.Potential role of bFGF in airway remodeling of chronic obstructive pulmonary disease rat model

Objective To observe the expression and distribution of the basic fibroblast growth factor(bFGF)in lung through chronic obstructive pulmonary disease(COPD)rat models and to investigate the potential role of bFGF in the mechanism for airway remodeling in COPD.Methods Forty male,8 weeks old Wistar rats were divided into groups randomly:COPD group;bFGF interfere in COPD group;anti-bFGF interfere in COPD group,and control group.COPD models were established by intratracheal instillation of lipopolysacchairde(LPS)twice in a month and exposure to cigarette smoking daily and in order to contrast respectively instillate particular quantity bFGF and anti-bFGF into different COPD model group from caudal vein in every week.After these models became true,observe the pathologic alteration of small airways,arterioles and pulmonary alveolus by hematoxylin and eosin stain Van-Gieson +elastic fibers stain.At the same time,the thickness of the smooth muscle and collagen in bronchi and pulmonary arterioles were measured by computer image analyzer;also the protein and gene relative content of bFGF as well as the effects of antibody on them were observed by Histostaining and ISH(in situ hybridization)detection.Results There was a significant increase in the thickness of the smooth muscle and collagen in bronchi of bFGF interfere in COPD group compared with that of control group and COPD group(P
2.Analysis of different time variations on blood pressure after the surgeries of fiducial implantation

Objective To propose a new observation procedure of blood pressure after the surgery of fiducial implantation for cyber knife, using the statistical analysis of the measurements of blood pressure at different time after the surgery. Methods By taking 200 patients after the surgery of fiducial implantation, the usual blood pressures and the blood pressures at different time after the surgery were compared statistically. Results Based on the statistical analysis, the usual blood pressures were statistically different from the blood pressure immediately after surgery, as well as the blood pressure at 0.5 hours after surgery [ systolic pressure (131.60 ±16.86), (129.08 ±14.77) mmHg (1 mmHg=0.133 kPa) vs. (123.00 ±11.80) mmHg, t=2.897 and 2.308, P<0.01 or 0.05;diastolic pressure (83.49±10.01), (81.75±8.99) mmHg vs. (78.22±7.59) mmHg, t=2.901 and 2.066, P<0.01 or 0.05]. The blood pressures measured at the other time after surgeries were not statistically different from the usual blood pressures, P>0.05. Conclusions Measuring the blood pressures at the time of 0, 0.5, 3.5 hours after the surgery can not only assure the complete recovery of the patients after the surgery, but also greatly reduce the work of nurses.
3.Clinical characteristics of nineteen vasculitis patients associated with propylthiouracil-induced antineutrophil cytoplasmic antibody

Clinical data were analysed in 19 patients with propylthiouracil (PTU)-induced antineutrophil cytoplasmic antibody associated vasculitis. Among them, 17 patients (89.5%) were diagnosed as Graves′ disease originally. The duration of PTU therapy in 18 patients was more than two years. Multi-organ involvement was common, with 15 patients (78.9%) involved in kidneys and 8 patients (42.1%) involved in lungs. The prevalences of arthralgia, skin rash and fever were also high. All patients withdrew PTU, and some with severe organ involvements received prednisone and immunosuppressant.
4.Clinical and laboratory characteristics of 112 rare cases misdiagnosed as spondyloarthritis

Objective To reduce the misdiagnosis rate of spondyloarthritis (SPA) by reviewing the rare cases misdiagnosed as SpA.Methods Cases misdiaguosed as SpA were collected from our hospital from January 2004 to April 2014.Reported cases among Chinese journals from January 1998 to October,2014 were also collected.According to the Assessment of Spondylo Arthritis international Society (ASAS) axial SpA criteria (2009) and peripheral SpA criteria (2011),the diagnostic accordance rate was studied.Results There were 112 cases within the objective scope,out of which,27 cases (24.1%) were infectious diseases,47 cases (42.0%) were heredity and metabolic diseases,25 cases (22.3%) were hematonosis or tumor,13 cases (11.6%) were osteoarthropathies.Also,only 10 cases (8.9%) out of 112 had the symptoms of inflammatory back pain (IBP),23 cases (20.5%) exhibited fever.Human leukocyte antigen (HLA)-B27 was positive in 20.4% (21/103) of the cases.Eleven cases out of those 29 cases performed X-ray in the sacroiliac joint and showed blurred articular surface,narrowing of joint space or bone destruction.Four cases were diagnosed based on magnetic resonance imaging (MRI).18/91 (19.8%) cases met the criteria of ASAS axial SpA criteria (2009),2/6(33.3%) cases were in accordance to the ASAS peripheral SpA criteria (2011).Conclusion For patients with atypical back pain,if accompanied with fever,other diseases such as tumor,infection,heredity and metabolic diseases should be considered.The diagnosis should not only based on HLA-B27 for SpA diagnosis.Due to the ambiguity of X-ray in sacroiliac joint,CT or MRI may be recommended to assist the diagnosis.Careful clinical history taken is also with great significance.
5.Analysis of 5-year survival rate and prognostic indicators of systemic lupus erythematosus

Objective To analyze the 5-year survival rate,causes of death and prognostic indicators of systemic lupus erythematosus (SLE).Methods A retrospective analysis was performed on 243 newly diagnosed SLE patients who Were admitted into our hospital from 1998 to 2005.The clinical features and serologic data were studied.Survival rate of SLE patients over time was studied by the Kaplan-Meier method,and prognostic indicators of mortality were studied by Cox proportional hazards models.Results The 1-,3- and 5-yr survival rate was 96%,94% and 91%,respectively.Renal failure and infection were the main causes of death,followed by lupus encephalotmthy and pulmonary hypertension.Cox regression analysis revealed that lupus nephritis and lupus encephalopathy at the diagnosis were independent risk determinants for mortality.However,age,sex,low C3 level,positive anti-dsDNA antibody,hematological abnormalities,lupus lung involvement and heart damages at diagnosis and immunosuppressant treatment had no strong association with survival.Conclusion Early diagnosis,control of SLE organ damage and infection prevention are critical to improve survival of SLE patients.
6.Clinical study of cervical anterior and posterior decompression and internal fixation in the revision of anterior cervical fixation failure.

OBJECTIVETo investigate the clinical effects and prevent the complications of posterior and anterior decompression and internal fixation in the revision of cervical anterior internal fixation failure.
METHODSFrom 2008 January to 2011 December, 17 patients with cervical anterior internal fixation failure were treated with posterior and anterior decompression and internal fixation. There were 12 males and 5 females, aged from 26 to 68 years old with an average of 44.1 years. The lower screw loosening was found in 6 cases, the upper screw loosening in 5 cases, titanium mesh caving in 3 cases, the upper screw breakage in 2 cases, the lower screw breakage in 1 case. Informations of bone fusion were observed by X-ray, CT, MRI. Clinical effects were evaluated by modified JOA score.
RESULTSAll the revision operations were successfully completed. One case with poor blood coagulation function before operation resulted in postoperative hematoma and occurred neurological symptoms; after hematoma removal and fresh frozen plasma infusion later, neurological symptoms of the patient disappeared. All patients were followed up from 6 to 38 months with an average of (22.4±10.0) months. Postoperative at 2 weeks, 3 months, and final follow-up, JOA score had obviously improved and respectively was 13.1±1.6, 13.4±1.6, 14.2±1.5. All internal fixation locations were good after revision,and obtained bone fusion at 10 months after operation, with an average fusion time of 6 months.
CONCLUSIONThe combined posterior and anterior decompression and internal fixation in the revision of cervical anterior internal fixation failure is safe, can achieve thoroughly decompression, maintain the cervical curvature, reconstruct the three column stability, and it may be used for the patients of cervical anterior fixation failure.

8.The value of procalcitonin for diagnosing infection in critically ill patients receiving long-term immunosuppressive therapy

Objective To assess the value of procalcitonin(PCT)measurement to differentiate infection from non-infection in critically ill patients requiring long-term immunosuppressive therapy.Methods A prospective study was conducted in patients with underlying diseases requiring corticosteroids or chemotherapy in ICU from January 2008 to December 2009.Patients were divided into the infection group and the non-infection group and their PCT levels were compared.Results A total of 103 patients (65 women)were enrolled in this prospective study[aged(47.9 ± 21.9)years old]with 84 in the infection group and 19 in the non-infection group.The baseline level of PCT was significantly higher in infection than in non-infection patients[2.58(0.08-44.65)pg/L vs 0.62(0.15-6.00)pg/L,P =0.002].Different levels of PCT were manifested in different pathogen groups with 3.41(0.45-44.65)pg/L in bacteria infection,0.99(0.28-6.67)pg/L in fungus infection,0.11(0.08-0.20)pg/L in virus infection group(P =0.018).The AUCROC of PCT was 0.867 for diagnostic bacterial infection.By multivariate analysis,the factors associated with the level of PCT were bacteria infection(OR 5.1,P =0.031)and septic shock(OR 7.5,P =0.027),while the factors not associated with the level of PCT were age,renal function,infection site and prognosis(P > 0.05).Conclusions The level of PCT is increased in the critically ill patients requiring immunosuppressive therapy with infection and it can be used for diagnosis for bacterial infection.
9.Correlation analysis of NF-κB signaling pathway activated by IL-18 in CD4+ T cells and the pathogenesis of PBC

Objective To explore the correlation between NF-κB signaling pathways activated by IL-18 in CD4+ T cells and the pathogenesis of PBC.Methods We detected the expression of IL-18 mRNA in PBMCs,IL-18 level in plasma,receptor IL-18R on surface of CD4+ T cell,proliferation rate of CD4+T cell and its NF-κB signaling pathway protein IκBα and NF-κB p65 by qRT-PCR,ELISA,flow cytometry,MACS and Western blot on 32 cases of patients with PBC (PBC group) and 32 healthy people (control group) in Guizhou provincial people′s hospital.Results The level of IL-18 in PBC group was significantly higher than that in control group (P<0.05).The relative expression of IL-18 mRNA in PBC group was significantly higher than that in control group (P<0.05).The percentage of CD4+T cells expressing IL-18Rα in PBC group was higher than that in control group (P<0.05).The proliferation rate of CD4+T cells stimulated by IL-18 in PBC group was significantly higher than that in healthy control group (P<0.01).The relative expression levels of NF-κB p65 protein were up-regulated in IL-18,and the expression of IκBα protein in each group was significantly increased,especially in PBC group (P<0.01).Conclusion IL-18 can activate NF-κB signal pathway in CD4+ T cells and participate in the pathogenesis of primary biliary cirrhosis.
10.Effect of estrogen deficiency on the expression levels of nuclear factor-kappa B and interleukin-17 in periodontal tissues

BACKGROUND:There is a very close relationship between osteoporosis and periodontal disease in postmenopausal women, but the mechanism remains unclear. OBJECTIVE:To investigate the expression of nuclear factor-κB in the alveolar bone andinterleukin-17 in the serum and gingiva in the mouse model of osteoporosis caused by ovariectomy. METHODS:Female mice aged 3 months were randomly divided into ovariectomy and sham operation groups. At 6 months after surgery, the mouse models were evaluated histologically on the submandibular bone and thigh bone stained with hematoxylin and eosin. In the submandibular bone, the expression levels of OCN and Runx2 were detected by RT-PCR, and the expression level of nuclear factor-κB was detected by immunohistochemical staining and western blot assay. Besides, the expression level of interleukin-17 in the serum and gingival homogenate was evaluated using Cytometric Beads Array. RESULTS AND CONCLUSION:The thigh bone in the ovariectomy group revealed the thin cortical bone, enlarged marrow cavity, and increased resorption lacunae, as well as fewer, thinner trabeculae with lower density and irregular structure. Compared with the sham operation group, the expression levels of OCN and Runx2 in the alveolar bone were decreased in the ovariectomy group. The activation of nuclear factor-κB (P65)appeared with P65 positive expression in the submandibular bone in the ovariectomy group, and the relative expression level was higher than that in the sham operation group. The serum level of interleukin-17 in the ovariectomy group was higher than that in the sham operation group, but the level in the gingival tissue showed no significant difference between the two groups. These results indicate that estrogen deficiency after ovariectomy can activate nuclear factor-κB signal pathway to play a role in periodontal osteolysis. However interleukin-17 in the local periodontal tissue may not be a key cytokine to damage the periodontal tissue.
