1.Clinical effect of breast augmentation by transplantation of autologous fat granules

Objective To investigate the method and clinical effect for transplanting auto-fat granules to augment breast. Methods The autologous fat granules were obtained by liposuetion using syringe from the sites with cumulate fat. After repeated wash and purification, they were transplanted into the interspace behind the breast by injection. The quantity of auto-fat granules injected was 50 - 100 ml in each side of breast per transplantation at 3 - 6 months intervals, and the whole course of treatment needed 2 -6 transplantations. Results 91 of 107 cases were followed up from 6 months to 3 years after final transplantation. All of them were satisfied with the postoperative effect, showing per-fect and plump appearance of the breasts. However, small indurations were found sporadically in 13 cases (21 breasts) within 2 -9 months, and calcifications in 8 cases (11 breasts) within 8 - 17 months after the first transplantation. Conclusion Both patients and surgeons are almost satisfied with the re-sults. The transplantation of autologous fat granules for breast augmentation is an effective and practi-cal method but the postoperative effect is influenced by multiple factors. A few of cases present com-plications after transplantation, so more clinical observation should be taken.
2.Effect of fluvastatin on activation of nuclear factor kappa B induced by angiotensin Ⅱ in rat kidney tubular epithelial cells

Objective To investigate the effect of fluvastatin on activation of nuclear factor kappa B (NF-κB)induced by angiotensin Ⅱ (Ang Ⅱ ) in rat kidney tubular epithelial cells (NRK-52E). Methods NRK-52E cells were divided into (1)control group ; (2)Ang Ⅱ groups with different concentration and time; (3)Ang Ⅱ (10-6 mol/L)+SB203580 ( 10 μmol/L)group; (4) Ang Ⅱ (10-6mol/L) +different fluvastatin concentration (10-7, 10-6, 10-5 mol/L)groups;(5)Ang Ⅱ (10-6mol/L) +fluvastatin (10-5 mol/L) +mevalonate (10-4 mol/L)groap. Electrophoretic mobility shift assays (EMSA) was used to detect NF-κB activation. Phosphorylation of cellular p38 mitogen-activated protein kinase (p38MAPK) was determined by Western blot. Monocyte chemoattractant protein (MCP)-1 mRNA was determined by RT-PCR. Results Ang Ⅱ stimulated the DNA-binding activity of NF-κB,phosphorylation of p38MAPK and up-regulated the expression of MCP-1mRNA in cultured NRK-52E cells in a dose-dependent manner (P<0.01). Ang Ⅱ (10-6 mol/L) induced a rapid (5 minutes) elevation of the p38MAPK phosphorylation. NF-κB DNA binding activity was increased at as early as 30 minutes(P<0.01), peaked at 2 hours after AngⅡ treatment (P<0.01). This stimulatory effect of Ang Ⅱ on NF-κB was blocked by SB203580 (a specific inhibitor of p38MAPK) (P<0.01). Incubation of cells with fluvastatin significantly inhibited the Ang Ⅱ-induced NF-κB activation and expression of MCP-1 mRNA in dose-dependent manner (P< 0.05). Exogenous mevalonate (10-4 mol/L) prevented the effect of fluvastatin on NF-κB activation (P <0.05). Conclusions Fluvastatin reduces Ang Ⅱ-induced NF-κB activation via the p38MAPK pathway in NRK-52E cells. Such effect of flurastatin is partly through blocked by mevalonate.
3.The serum homocysteine levels and dynamic changes in patients with different etiologic macrocytic anemia

Objective To examine the serum homocysteine,folic acid and Vitamin B12 levels in macrocytic anemia patients and observe their dynamic changes following therapy. Methods Homocysteine,folic acid and Vita-min B12 were analyzed by electro-chemiluminescence immunoassay. Complete blood cell count was analyzed by opti-cal method and resistance method. Results The homocysteine is significantly higher in nutritional megaloblastic a-nemia[ (71.26±27.84)μmoL/L ] than in drug-induced megaloblastic anemia[(11.44±5.06)μmol/L],in myel-odysplastic syndrome[ (9.51±4.13)μmol/L] and in the normal control group[(8.74±5.42)μmoL/L] (P<0.01). After treated with low-dosage folic acid and Vitamin B12,the patients with megaloblastic anemia presented slow declining but eventually normal homocysteine levels,compared with those received high-dosages. Conclusion Homocysteine can be used for differential diagnosis of macrocytic anemia. The duration of remaining of abnormal ho-mecysteine levels is related to the dosage of folic acid and Vitamin B12.
5.High-frequency ultrasonographic study of finger joints synovial lesions in rheumatoid arthritis

Objective To study the synovial lesions of finger joint of rheumatoid arthritis (RA) with high frequency ultrasound ( HFUS). Methods HFUS examination of finger joints of both hands were performed in 42 patients with RA and 30 healthy volunteers. The ultrasound findings of the finger joints in RA patients included intra-articular fluid,the thickness of synovial membrane,vascular proliferation. and the blood flow characteristics. Results The intra-articular fluid was found in 256 joints and the detection rate was 42. 38% (256/840). Synovial membrane hyperplasia was found in 223 joints with the detection rate of 26.55% (223/840). The detection rate of HFUS for vascular proliferation was 6.07% (51/840). Conclusions HFUS is an easy, safe and effective method for the diagnosis of RA thus providing useful evidences for clinical diagnosis.
6.Clinical observation and prevention of side reaction of high dose methotrexate in the treatment of leukemia

Objective To analyze the clinical efficacy,adverse events and corresponding preventive measures of high dose methotrexate in the treatment of acute lymphoblastic leukemia(ALL).Methods Ninety-two patients with ALL who hospitalized in Heamotology Department of Jinshan Hospital Affiliated to Fudan University were randomly divided into observation group and control group,and each group had 46 cases.Patients in observation group were given high dose melhotrexate + vincristine + cytoxan + pirarubicin chemotherapy treatment,and in control group were given soft red enzyme + vincristine + metacortan dracin + L-asparaginase treatment.The clinical efficacy,adverse events and the required course of remission of two groups were compared.Results The total effective rate in the observation group was 84.78%,while it was 80.43% in the control group,and there was no statistically significant difference (x2 =0.45,P > 0.05).The average remission was (1.26 ± 0.28) in the observation group,while (1.31 ± 0.31) in the control group,and no significant difference was observed (t =2.13,P > 0.05).The complete remission rate of initial treatment,retreatment and refractory were 75.00% (24/32),50.00% (4/8) and 33.33% (2/6) respectively in observation group,while 76.67% (23/30),50.00% (4/8) and 25.00% (2/8) respectively in control group,and there was no statislically significant difference between two groups (x2 =0.98,P >0.05).The rate of bone marrow suppression in the observation group(23.91%) was lower than that in the control group(43.48%),while the rate of liver and kidney injury(54.35%) was higher than that inthe control group(17.39%),which showed significant difference (P < 0.05).The survival rate of 5 years in observation group was 67.39%,while 45.65% in control group,and there was statistically significant difference between two groups (P < 0.05).Conclusion The high dose methotrexate in the treatment of acute lymphoblastic leukemia was proved to be effective and the adverse reactions could be tolerated.
7.Infection after cardiac-lung transplantation in ICU

Objective To explore the characteristics of infection after cardiac-lung transplantation.Methods March 23th 2006,one patient received orthotopic heart-lung transplantation,the clinical data were observed and analyzed.Results Depend on etiology,antibioti and antifungul were selected,the patient recoved.Conclusion It is very important to early etiology diagnosis and reasonable selection of antibiotic.
8.Study on preparation and fluoride-relesae properties of fluorinated nanosilica filler

Objective The aim of the study was to develop a kind of new fluoride composite resin .We explored the practica-bility of preparing fluorinated filler by using hollow silica microsphere as a carrier of sodium fluoride (NaF) as well as its fluoride-re-lease properties in composite resin . Methods Hollow silica microspheres with mesoporous shells were prepared by the sacrificial template method.NaF was loaded into hollow silica microspheres by the impregnation method .Scanning electron microscopy ( SEM) was used to determine the morphologies of hollow silica microspheres before and after NaF loading .Hollow nanosilica microsphere con-taining NaF and NaF powder were respectively mixed with resin at the same intial NaF concentration of 13.9, 27.8 and 55.6 mg/mL for comparison, which were taken as the experiment group and the control group .The samples were put into 100 mL artificial saliva at 37℃to caculate the cumulative fluoride release quantities from 1 d to 47 d.Results The morphology of hollow microsphere did not chang significantly before and after NaF loading .In the experiment group ,the cumulative quantities of fluoride release were 0.026, 0.105, 0.21 mg/cm 2on 1 d and increased to 0.088, 0.239, 0.484 mg/cm 2on 47 d according to respective initial NaF concentration . While in the control group, the corresponding figures were 0.006, 0.025, 0.04 mg/cm2on 1 d and 0.022, 0.045, 0.056 mg/cm2on 47 d.The difference between these two groups was of great statistical significance . Conclusion Hollow silica microsphere could be used as a carrier of NaF .Fluorinated nanosilica filler using hollow microsphere promotes the fluoride release in composite resin .
9.Current Situation of TCM Human Resources Distribution and Equity Research in China

Objective To discuss existing problems in talent team construction; To provide evidence for the government to enact proper strategies based on the analysis of the current situation of TCM practitioners (assistants) distribution in China from quantity, quality and distribution equity. Methods The research is studied through conducting a descriptive analysis of TCM practitioners (assistants) from quantity, education, job titles, ages, and other basic conditions and changes. Gini coefficient was used to evaluate the distribution of TCM practitioners (assistants). Results The quantity of TCM practitioners (assistants) increased by 53.7% from 2010 to 2015. Concerning the quality of personnel, the ratio of the personnel with bachelors degree or above increased from 38.6% to 44.2%, and a slight increase was observed with the personnel with senior title. In terms of age structure, the proportion of the age of TCM practitioners (assistants) under 25 years old, 35 to 59 years old decreased, and the proportion of the age of TCM practitioners (assistants) above 60 years old, 25 to 34 years old increased. From the perspective of equity, there is a difference between the provincial-level administration of human resources distribution. The Gini coefficient as per population distribution was around 0.1, which indicated good equity. From the geographical area configuration, the Gini coefficient was approximately 0.58, which showed high level of disparity. Conclusion There are problems of insufficient quantity, low quality and distribution disparity among regions in TCM practitioners (assistant) in China.
10.Analysis of iodine nutritional status of pregnant women and level of neonatal heel blood thyroid stimulating hormone in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai

ObjectiveTo investigate the iodine nutritional status of pregnant women,newborn heel blood thyroid stimulating hormone(TSH) level and their relationship with urinary iodine(UI) level during pregnancy in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai.Methods A total of 993 urinary samples(the first,second and third trimesters of pregnancy were 200 people,respectively),breast feeding(193 people) and non-pregnant women (200 people) in Zhoupu and Kangqiao districts of Pudong New area were collected from Apr 2009 to Dec 2010.Two hundred copies of neonatal heel blood samples were collected.Median of UI was measured by arsenic-cerium catalysis.TSH in neonatal heel blood was analyzed 72 h after birth by time resolved fluoroisnmunoassay(TRFIA).ResultsMedian UI of all pregnant women was 161.35 μg/L,and that in third trimesters of pregnancy( 126.35 μg/L) was lower than that of the first,the second,the breast feeding and non-pregnant women (178.80,180.50,167.90,163.40 μg/L,all P< 0.05).The percentage of UI level less than 150 μg/L in the third trimester[57.5%(115/200) ] was higher than that of the first[39.0%(78/200) ],the second[39.5%(79/200) ],the breast feeding [ 16.6% (32/193) ] and non-pregnant women [ 23.0% (46/200) ],respectively (all P < 0.05).The percentage of UI level higher than 300 μg/L in the first [9.0%(18/200)],the second[8.0%(16/200) ] and the third trimester [ 5.0% ( 10/200 ) ] of pregnancy was lower than that of the breast feeding [ 20.2% (39/193) ] and nonpregnant [20.5%(41/200) ] women,respectively(all P < 0.05).The level of neonatal heel blood TSH was(2.92 ± 1.83)mU/L,the range was 0.01 - 9.76 mU/L,11.0%(22/200) of the neonates heel blood TSH level(5 mU/L)exceeded the ratio of World Health Organization (WHO) standard ( < 3% ) suitable for iodine nutrition.Conclusions The overall level of iodine nutrition among pregnant women in Zhoupu and Kangqiao districts of Pudong New Area of Shanghai is in the appropriate range,but the pregnant women in the third trimester is in mild iodine deficiencies,and the neonates in these districts may be prone to iodine deficiency.Monitoring of iodine nutrition of pregnant women should be strengthened and iodine supplementation should be done scientifically.
