ObjectiveTo investigate the clinical features and risk factors of patient with Wegener's granulomatosis complicated with pulmonary infection.Methods Patients with Wegener's granulomatosis admitted to our hospital in the past 11 years were retrospectively analyzed.Comparisons between groups were performed by t tests or Fisher test.ResultsPulmonary infection occurred in 27 cases with an incidence rate of 29％.Twenty-six percent of pulmonary infections occurred at the initial diagnosis,and 44％ occurred within 6 months,while 30％ occurred later than 6 months.The clinical manifestations of pulmonary infection were productive cough (89％),hemoptysis (63％),fever and fatigue (56％),chest pain and pactoralgia (33％).The most common causative pathogen were bacteria(59％ ),fungi(37％ ),and tubercle bacillus(37％ ).Sinus infection(P=0.01),hypoproteinemia(P=0.03),hypoimmunoglobulinemia (P=0.007),and methylprednisolone pulse therapy(P=0.002) were the risk factors for pulmonary infection.ConclusionThe occurrence of Wegener's granulomatosis complicated with pulmonary infection is high within 6 months.The most common clinical manifestation is productive cough.The most common causative pathogens are bacteria,tubercle bacillus and fungi.Sinus infection,hypoproteinemia,hypoimmunoglobulinemia,and methylprednisolone pulse therapy are risk factors of pulmonary infection.

Objective To explore the dynamic spiral CT findings of adrenal ganglioneuromas and to evaluate its clinical value.Methods All 7 cases patients with adrenal ganglioneuromas confirmed by surgical pathology underwent spiral CT plain scanning and two phases dynamic enhancement scanning before operation.The CT images were reviewed and analysed retrospectively in comparison with surgical and pathological results.Results Six of 7 lesions were located in the right adrenal gland,1 in the left.The diameter of the tumors was 3.7—7.5cm,with a mean diameter of 4.6cm.The CT value of the plain scans, AP phases and PVP phases was 16.2—31.7 HU(with a mean value of 24.3 HU),20.9—36.6 HU(with a mean value of 30.2 HU),and 27.4—45.5 HU(with a mean value of 36.0 HU),respectively.The masses were well defined and showed a tendency of wedging into the space between the adjacent organs and structures and encasing the large vessels such as IV,but didn't compress or occlude them.Conclusion The dynamic SCT features of adrenal ganglioneuromas were obvious,dynamic SCT scans could suggest the diagnose.

Objective:To explore the relationships between the endothelial microparticles (EMPs)in peripheral blood and CD4+ CD25 + Foxp3+ regulatory T cells and their cytokine levels in the patients with acute coronary syndrome (ACS),and to clarify the mechanism of EMPs in the pathogenesis of ACS by affecting the Treg cell differentiation and function.Methods:Twenty-three patients with stable angina (SAP)were allocated to SAP group,and 52 patients with ACS were allocated to ACS group.Twenty individuals with normal conventional coronary angiography results were recruited as control group.The levels of EMPs and the percents of CD4+ CD25 +Foxp3+ regulatory T cells in peripheral blood of the patients in various groups were measured by flow cytometry. The expressions of Foxp3 mRNA and the plasma levels of TGF-β1 were detected with Real-Time PCR and ELISA, respectively.Correlation analysis was performed between EMPs and regulatory T cells,Foxp3 mRNA expression level and TGF-β1 level.Results:Compared with control group and SAP group,the level of EMPs in peripheral blood of the patients in ACS group was increased significantly,but the percentage of regulatory T cells,the Foxp3 mRNA expression level and the plasma TGF-β1 level were decreased markedly (P <0.01).The correlation analysis showed that there were significantly negative correlation between EMPs and regulatory T cells,Foxp3 mRNA expression level and TGF-β1 level (r = - 0.452,P = 0.001;r = - 0.466,P = 0.001;r = - 0.555,P = 0.000 ). Conclusion:EMPs may involve in the process of occurrence and development of ACS and plaque instability by regulating the differentiation and function of CD4+ CD25 + Foxp3 + regulatory T cells.

A total of 12 cases with Achilles tendon rupture were repaired by modified Kessler's methods.The sutured tendons were then strengthened with Mitek anchor system with 2 anchors inserted into the calcaneus lateral and medial to insertion of Achilles tendon and the sutures passed through the repaired tendon using Bunnel methods.The healing of skin incision,time to weight bearing,range of motion of the ankles,calcaneal pain and re-ruptures were recorded.The results were evaluated according to Amer-Lindholm criteria.All the patients were followed up with mean 17 months (range from 14 to 25 months).There were no calcaneeal pain or re-rupture cases,neither deep nor superficial wound infection or skin necrosis cases.According to Arner-Lindholm criteria for curative result,the result was excellent in 4 cases and good in 8 cases.The Mitek anchor system can be used successfully to treat Achilles tendon rupture and decrease postoperative complications.Thus,the technique is an ideal option for Achilles tendon rupture repair.

Objective To observe the effect of total flavonoids of ampelopsis grossedentata on blood lipid and hemorheology in atherosclerosis rats .Methods The atherosclerosis model in rats was made by freeing high grease food and intraperitoneal injection of vitamin D3 ,total flavonoids of ampelopsis grossedentata was gave by gavage ,and the level of serum lipid and blood rheology were tested 24 weeks later .Results Total flavonoids of ampelopsis grossedentata could obviously decreased the level of TG ,TC ,LDL , and increased HDL(P<0 .05) .It could significantly decrease whole blood viscosity ,plasma viscosity and hematocrit (P<0 .05) . Conclusion Total flavonoids of ampelopsis grossedentata can adjust fatty substance metabolism ,improve hemorheology of athero-sclerosis rats ,and has therapeutic effect for atherosclerosis .

Objective To observe the effect of dihydromyricetin of ampelopsis grossedentata on lipid metabolism and antioxidation in atherosclerosis (AS) rats. Methods Sixty Wistar rats were randomly divided into five groups (n = 12 each):normal control group, model control group, positive control group, high dose dihydromyricetin group (40 mg·kg-1 ) and low dose dihydromyricetin group (10 mg·kg-1 ). Except normal control group, rats in the other groups were injected with a single dose of vitamin D3(600 000 U·kg-1 ) and loaded with high fat diet to establish AS model. Simvastatin (5 mg·kg-1 ) was intragastrically administered to positive control group. High and low dose dihydromyricetin groups received intragastric administration of 40 and 10 mg·kg-1 dihydromyricetin, respectively. Equal volume of purified water was given to normal and model group. After 24 weeks of administration, serum levels of lipids, activities of superoxide-dismutase (SOD) and malonicdialdehyde (MAD) were determined. Objective To explore the imaging features and dynamic CT findings of primary adrenocortical carcinoma and evaluating its clinical value.Methods All 11 cases patients with primary adrenocortical carcinoma confirmed by surgical pathology underwent spiral CT plain scanning and two phases dynamic enhancement scanning before operation.Contrast injection was done by using nonionic Omnipaque at a flow rate of 3 ml/s with a power injector.The CT images were reviewed and analysed retrospectively in comparison with surgical and pathological results.Results Five of 11 lesions were located in the right adrenal gland,6 in the left.The diameter of the tumors was 4.1—16.0cm with 2 cases10.0cm.The attenuation of the plain scans,two phases contrast scans was 21.9—46.7 HU(with a mean value of 35.3 HU),30.5—65.8 HU(mean value 47.1 HU),and 52.6— 97.0 HU(mean value 74.2 HU),respectively.Four were in the shape of capsule,6 reticular,1 slightly inhomogeneous masses.The irregular tumor vessels of 6 reticular masses were seen in the artery phase. Conclusion The dynamic CT features of primary adrenocortical carcinoma were obvious,dynamic SCT scans could suggest the diagnose.
