Objective To evaluate diagnostic value of diffusion tensor imaging(DTI)in ringlike- enhanced lesions.Methods Nine abscesses,12 glioblastomas,10 metastases confirmed clinically or pathologically underwent conventional MRI and DTI.Average diffusion coefficient(ADC)value,fractional anisotropy(FA)value and maps were calculated in the central portion and peripheral edema of the lesions. Results On DTI,the abscesses displayed as hyperintense signal with hypointense or isointense signal of edema;but glioblastomas and metastases all showed as hypointense signal with isointense or hypointense signal of edema.On ADC map,the abscesses showed as hypointense signal,the mean ADC value was (0.66?0.07)x10~(-3)mm~2/s,The mean ADC value were(2.50?0.11)x10~(-3)mm~2/s and(2.37?0.52)x10~(-3)mm~2/s for the glioblastomas and metastases,respectively,all demonstrated as hyperintense signal with slightly hyperintense signal of edema.The difference between abscess and necrotic tumors was statistically significant(F=108.80,P

Objective To improve our understanding concerning radiographic manifestations of cryptogenic organizing pneumonia(COP).Methods The diagnosis of cryptogenie organizing pneumonia was made based on clinical and radiological features,and was verified with lung biopsy and pathological examination in 23 eases.All data were analyzed and relevant literatures were reviewed.Results CT scans revealed multi-patch shadows,patchy air-space consolidations in 15 cases,often located in predominantly subpleural and(or)both inferior lungs,with or ground-glass opacities,bronehieetasis,and cords.Lesion sites changed over time in some patients.Cortieosteroid treatment led to significant improvement in most cases.Conclusions The diagnosis of cryptogenic organizing pneumonia required the converging evidence from clinical and radilogieal manifestations as well as pathologies.It is important to appreciate CT manifestations of COP.
4.Analysis on neuropsychological characteristics of subtypes of attention deficit hyperactivity disorder.

OBJECTIVETo explore the neuropsychological characteristics of children with attention deficit hyperactivity disorder (ADHD).
METHODSNeuropsychological tests, including visual working memory, Stroop test, digits inverse reciting, vocabulary fluency, Wisconsin card sort test (WCST), and Temporal discounting were used to evaluate the capacity of response inhibition, phonological working memory, visual working memory executive function and delayed satisfying capacity of subjects.
RESULTS1. The ADHD children spent longer time [ADHD-I (84(20), ADHD-C: 98 (31), normal: 70 (28)] to accomplish color naming and made more errors [ADHD-I: 3 (3), ADHD-C: 6 (19), normal: 2 (5)] than the normal control when the color was inconsistent with the word meaning in Stroop test (P < 0.01). 2. The scores of digits reciting [ADHD-I: 3 (3), ADHD-C: 3 (4), normal 4 (4)] inverse was lower in ADHD than in normal control (P < 0.01). 3. The representation of ADHD was poorer than normal control in visual working memory [ADHD-I: 21 (3), ADHD-C: 20 (5), Normal: 20 (3)], and in delayed visual memory [ADHD-I: 19 (5), ADHD-C: 19 (5), Normal: 20 (5)] (P < 0.01). 4. The scores of vocabulary fluency [ADHD-I: 1 (1), ADHD-C: 2 (1), normal: 0 (0)] was lower in ADHD than in normal control (P < 0.01). 5. In WCST, the ADHD children made more errors [ADHD-I :15 (17), ADHD-C: 15 (15), normal: 13 (13)] and less classification [ADHD-I: 5 (4), ADHD-C: 5 (4), normal: 5 (3)] than normal control (P < 0.01). 6. In Temporal discounting, the ADHD children showed significantly more impairments than normal control did (P < 0.01). 7. There was significant difference between the two subtype groups on some tests (P < 0.01).
CONCLUSIONSObvious cognitive impairments were found in children with ADHD, involving poor response inhibition, impaired working memory, dysfunction of planning and set-shifting, and there was no significant difference between the two subtype groups.

5.Structure of secondary saikosaponins in saiko decoction.

OBJECTIVETo study the structures of secondary saikosaponins in saiko decoction.
METHODThe structures of saikosaponins were separated from saiko decoction and identified by various separation and spectral techniques. HPLC method was adopted for identifying all of separated saikosaponins.
RESULTEleven saikosaponins were separated from saiko decoction. Four of the seven new constitutes contained in saiko decoction were identified.
CONCLUSIONThis study lays a foundation for improving the identification of secondary saikosaponins.

6.The role of endogenous opioid peptides in paraventricular nucleus of hypothalamus in the antagonism to myocardial ischemia by electroacupuncture at acupoint "Neiguan" (PC 6).

OBJECTIVETo explore the mechanism of electroacupuncture at "Neiguan" (PC 6) antagonizing myocardial ischemia injury.
METHODSForty rabbits were randomly divided into 4 groups, model control group, naloxone control group, electroacupuncture plus cerebral spinal fluid group, and electroacupuncture plus naloxone group, 10 rabbits in each group. The effects of electroacupuncture at "Neiguan" (PC 6) after injection of naloxone into the paraventricular nucleus (PVN) of the hypothalamus on myocardial ultrastructure, II lead ECG and transmembrane potentials of the ischemic myocardial cell in the rabbit of myocardial ischemia were investigated.
RESULTSThe injection of naloxone into PVN has no effect on myocardial functions in the normal rabbit and a little effect on ischemic myocardial functions, but at 60 min after myocardial ischemia, this has a certain protective action on myocardial functions (P < 0.05); electroacupuncture at "Neiguan" has obvious protective action on ischemic myocardium, which can be reduced by injection of naloxone into PVN (P < 0.05), but can not be blocked.
CONCLUSION(1) PVN plays an important role in the process of antagonizing myocardial ischemia by electroacupuncture at "Neiguan"; (2) endogenous opioid pepetides in PVN are involved in the acupuncture effect of antagonizing myocardial ischemia; (3) other transmitters in PVN and other nucleus groups in the center also can produce obvious effects in the antagonism to myocardial ischemia by electroacupuncture at "Neiguan".

7.Molecular epidemiologic survey of rotaviruses from infants and children with diarrhea in Shanghai.

OBJECTIVETo investigate molecular epidemiologic features of rotaviruses circulating in Shanghai, China.
METHODSStool samples were collected from 1230 hospitalized children with community-acquired and nosocomially acquired diarrhea in Children's Hospital Affiliated to Fudan University between November 1, 1999 and December 31, 2001. Polyacrylamide gel electrophoresis (PAGE) was used to detect rotavirus genomic RNA and identify electropherotypes of group A rotavirus RNAs. Reverse transcription polymerase chain reaction (RT-PCR) was performed to amplify full length VP7 gene and dot blot hybridization was performed to identify rotavirus G serotypes using digoxigenin-labelled variable regions from VP7 genes as probes. These probes were amplified by PCR from recombinant plasmids containing full length G1, G2, G3 and G4 VP7 genes from rotavirus field strains detected in Beijing and digoxigenin labelled dUTP was integrated into the PCR products. The Kruskal-Wallis analysis of variance was employed to analyze whether there were significant differences in variables.
RESULTSOut of 1230 samples investigated, 493 (40.1%) were group A rotavirus gene positive by PAGE, among which 397 (80.5%) showed long electropherotypes, 55 (11.2%) showed short electropherotypes, 18 (3.7%) showed mixed electropherotypes which suggested that the children were co-infected by rotaviruses with different electropherotypes, 23 (4.7%) were non-typable because of degradation of some of the genomic RNA fragments. No group B or group C rotavirus was found. RT-PCRs were performed for 328 fecal specimens containing sufficient rotavirus RNAs and VP7 gene products were obtained from 254 (77.4%) samples. Dot blot hybridization showed serotype G1 accounted for 55.5% (141) of these samples, serotype G3 accounted for 27.6% (70), serotype G2 accounted for 9.4% (24), co-infection by 2 rotaviruses with different G types accounted for 6.3% (16), only 1 G4 was detected and 2 were non-typable. The genomic RNA patterns of all G2 strains were short and those of G1, G3 and G4 strains were long. There were no statistically significant differences for age distribution and clinical manifestations among those infants and children infected by rotaviruses with different G serotypes.
CONCLUSIONGroup A rotavirus is the major pathogen for diarrhea in infants and children in Shanghai during the period of Nov. 1999 to Dec. 2001. Rotaviruses with long electropherotype were dominant during these years. Serotypes G1 to G3 constituted 98.8% of all 254 strains tested, and G1 was the most common serotype followed by G3 and G2, whereas serotype G4 was seldom found. Some of the children were co-infected by rotaviruses with different G serotypes. Clinical manifestations were not related to the infecting rotavirus with different G serotypes.

8.Studies on quality control of Xueyakang capsule.

OBJECTIVETo establish the quality control standard for Xueyakang capsule.
METHODRadix Rehmanniae, Cacumen Platycladi, Folium Artemisiae Argyi, Folium Nelumbinis were identified by TLC, and the content of quercitrin was determined by HPLC.
RESULTThe TLC sports developed was fairly clear, the HPLC method showed good repeatability, and the average recovery of quercitrin was 100.7% with RSD 2.0%.
CONCLUSIONThe method is simple, accurate and can effectively control quality of Xueyakang capsule.

9.Experimental study on p53 gene mutation in keloid fibroblasts.

OBJECTIVETo explore the significance and the role of the p53 gene mutation in the exon 4 to 8 in keloid fibroblasts.
METHODSTissue samples from twelve patients with keloid and twelve hyperplastic scar respectively were harvested for in vitro culture of fibroblasts, and normal skin samples from the same patients were employed as the control. Polymerase chain reaction-based single-strained conformational polymorphism (PCR-SSCP) and DNA sequencing were employed to detect p53 gene mutations of the fibroblasts.
RESULTSThe points and frameshift mutations in the exon 4, 5, 6, 7 of p53 gene were identified in 9 of the 12 keloid tissue samples. No p53 gene mutation was detected in all hyperplastic scar and normal skin samples.
CONCLUSIONp53 gene mutation might play an important role in the formation and development of keloids.

10.Reducing the incidence of healthcare-associated infection in department of critical care medicine by risk assessment method

Objective To explore the effect of risk assessment method on the incidence of healthcare-associated infection (HAI) in high-risk departments.Methods A hospital was selected as the research object, risk assessment of HAI management at the hospital and department level was carried out, high-risk departments and high-risk links were screened out, targeted intervention was performed. Patients hospitalized in April-June 2017 were as control group and those hospitalized in July-September 2017 were as intervention group, incidence of HAI between two groups was compared.Results Through risk assessment at the hospital level, department of critical care medicine was the department with the highest risk, risk assessment at the department level showed that without wearing isolation clothes when contacting isolated patients during diagnosis and treatment, without using sterile sheeting when catheterization, and low correct rate of hand hygiene were high-risk links in department of critical care medicine. Targeted intervention was performed, isolation clothing allocation rate for contacting isolated patients increased from 0 to 100%, compliance rate to wearing isolation clothing among medical staff increased from 0 to 97.62%, implementation rate of using sterile sheet for deep vein catheterization increased from 72.50% to 100%; hand hygiene correct rate increased from 85.00% to 96.59%. Incidence of HAI decreased from 5.90% to 2.64%, difference was statistically significant (P<0.05).Conclusion Implementing risk assessment management of HAI in medical institutions can effectively guide the prevention and control of HAI in high-risk departments, and improve the level of HAI management.
