ObjectiveTo investigate the clinical features of intramedullary spinal cord tumors(IMSCT) in children.MethodsThe clinical data including sex,age,pathohistology,clinical manifestations and medical check-up collected from 16 children diagnosed as IMSCT in Shengjing Hospital Affiliated to China Medical University,were retrospectively reviewed and analyzed.ResultsThe study subjects inclu-ded 11 male and 5 female,with an average age of(8.55?4.28) years old,ranged from 15 months to 14 years old.The interval lasted 1 day to 9 years from onset of the syndrome to confirmed diagnosis of IMSCT.The first clinical manifestation was mostly pain.The main clinical mani-festations included limb dyskinesia in 14 cases,pain in 12 cases,urine and stool disturbance in 9 cases,gait irregularity in 8 cases,extremity atrophy in 3 cases.Medical check-up included abnormal tendon reflex in 14 cases,abnormal superficial reflex in 9 cases,sphincter dilatation in 6 cases,positive Babinski's signs in 5 cases,spinal deformity in 4 cases,region masses in 3 cases,paraesthesia in 2 cases,inequality of lower limb in 1 case,dermai sinus in 1 case,pigmentation in 1 case.The cases were evaluated by magnetic resonance imaging,which presented thickened spinal cord and intramedullary abnormal signals in all the cases.Pathological types: teratoma in 5 cases,dermoid cysts in 2 cases,epidermoid cyst in 2 cases.Misdiagnoses included urinary tract infections in 2 cases,rheumatoid arthritis in 1 case,and lumbar disc protrusion in 1 case.ConclusionsThe main pathohistology of pediatric IMSCT is congenital tumor.First clinical manifestation is pain.MRI can show a reliable evidence for the diagnosis.It is important to improve the outcomes of pediatric IMSCT by establishing the diagnosis and administering proper management as early as possible.

GDF-15,a distant member of the TGF-superfamily,is identified as an apoptotic accelerat- ing,anti-tumorigenesis and nerve2nutritional factor in varied injures and tumors and has cardioprotective ac- tivity.The characteristics and roles of GDF-15 gene/protein and antibodies are expounded besides the rela- tionship between GDF-15 serum level/genetypes and CRC.It is also discussed here that some antitumorigenic substances inducing GDF-15 in CRC tissues and CRC cells.

Objective To study the expression of N-myc downstream regulated gene 1 (NDRG1) in renal cell carcinoma and its relationship with microvessel density (MVD) and clinicopathologic parameters.Methods Immunohistochemical study for NDRG1 and CD34 was performed on paraffin sections of cases of renal cell carcinoma and adjacent non-neoplastic renal parenchymal tissue. MVD was analyzed by CD34immunostaining.Results Immunohistochemical study showed that non-neoplastic proximal convoluted tubule, distal convoluted tubule and collecting ducts was positive for NDRG1 (membranous and cytoplasmic). The expression rate of NDRG1 in renal cell cancer was 51%, which was significantly lower than that in normal renal tissues of 100 % (P ＜0.05).Clinicpathologically, the result also showed a close relation between lower NDRG1 expression and higher pathologic grade (χ2 =9.968, P =0.007), later clinic stage (χ2= 6.437,P =0.011), lymph node metastases (χ2=5.800, P =0.016) and higher MVD of cancer (t =2.235, P =0.030)whereas no relation with other factors. Conclusion NDRG1 might play as a cancer suppressor gene in renal cell carcinoma in that it could be correlated with tumor invasion and metastasis.It might also suppress tumor growth and metastasis by regulation of tumor angiogenesis.

Objective To study the value of prealbumin(PAB) ,C reactive protein(CRP) and apolipoprotein A1(Apo A1) in e-valuating the prognosis of patients with severe pneumonia .Methods 63 cases of patients with severe pneumonia were selected ,and the fasting serum level of prealbumin ,C-reactive protein and apolipoprotein A1 were detected in 24 hours at the admission day ,and APACHE Ⅱ score was calcumulated .The patients were divided into group A (APACHEⅡscore <20) and group B(APACHEⅡscore≥20) ,with the incidence of MODS and mortality rate compared .The patients were divided into non-MODS group and MODS group .In addition ,the patients were divided into survival group and death group according to the prognosis .PAB ,CRP and Apo A1 in each group were compared .Results The incidence of MODS and mortality rate in group B (57 .9% ,47 .4% ) were higher than that in group A(24 .0% ,16 .0% )(P<0 .01 ,P<0 .05) .PAB and Apo A1 of patients in MODS group[(134 .13 ± 36 .20)mg/L , (0 .62 ± 0 .21)g/L] and death group[(129 .05 ± 52 .24)mg/L ,(0 .76 ± 0 .29)g/L] were respectively lower than that in non-MODS group[(215 .03 ± 72 .08)mg/L ,(1 .06 ± 0 .39)g/L] and survival group[(185 .52 ± 57 .63)mg/L ,(1 .15 ± 0 .36)g/L](P<0 .05) , while the CRP in MODS group(102 .37 ± 35 .65)mg/L and death group(96 .37 ± 34 .72)mg/L were higher than that in non-MODS group(69 .68 ± 32 .92)mg/L and survival group(62 .94 ± 38 .36)mg/L (P<0 .05) .Conclusion The measurement of PAB ,CRP and Apo A1 are valuable to evaluate the prognosis of the patients with severe pneumonia .

Objective To investigate the effect of “Shuangguyitong acupuncture therapy on serum interleukin-6 of streptozotocin (STZ) induced diabetic rats. Methods Among seventy-three 180～220 g Wistar rats, thirteen were assigned as normal control group (group A) and sixty were induced diabetes by injecting STZ into celiac cavity. The diabetic rats were randomly divided into model group(group B), “Yitong” treatment group (group C), “Shuangguyitong” acupuncture group (group D) and common acupuncture group (group E). After two courses of treatment, serum IL-6 was tested by radioimmunoassay (RIA) . The blood glucose was measured by sugar equipment. Objective To investigate the value of emergency interventional embolization in the treatment of traumatic splenic rupture.Methods Clinical data of 45 patients with traumatic splenic rupture in our hospital were analyzed retrospectively.Selective splenic artery embolization (embolization group) was performed in 29 patients guided by DSA,and surgical resection or repair operation (operation group) was performed in 16 patients.Results Data of two groups were compared:①For operation time and length of hospital stay,the intervention group was significantly better than the operation group (P<0.05);②For incidence of postoperative fever and pleural effusion,the operation group was lower than the intervention group (P<0.05);③For the postoperative abdominal pain index,there was no significant difference between the two groups (P>0.05);④Compared with the operation group,patients in the intervention group had less long-term complications,and did not require long-term anticoagulation therapy.Conclusion Interventional embolization in the treatment of traumatic splenic rupture is safe and effective, and has less postoperative complications.The interventional embolization improves spleen preservation rate and improved the life quality.

Objective:To investigate surgical indications and operative techniques of intracranial arachnoid cysts(IAC). Methods:Thirty patients with IAC treated in Nanjing General Hospital of Nanjing Command were analyzed retrospectively.Cyst wall removal and communicated cyst with adjacent cistern or subarachnoid space was performed in 18 patients,cyst-peritoneal shunt in 8 patients,simple resection in 4 patients. Results: Follow-up CT scans after surgery showed that all cysts disappear or shrinked.Conclusion: Cyst-peritoneal shunt was indicated in recurrent cysts after resection,senile patients and infants.Microsurgery resection of the cyst is preferred in majority of the patients,removal of the cyst wall and communicating the cyst with arachnoid cistern were key procedures for preventing relapse.

To compare the effects of laparoscopic total or subtotal hysterectomy and myomectomy on ovarian reserve by measuring the serum level of anti-Mullerian hormone (AMH).A total of 96 patients with uterine fibroids underwent total laparoscopic hysterectomy (TLH group,n =32),laparoscopic subtotal hysterectomy (LSH group,n =31) and laparoscopic myomectomy (LM group,n =33).Changes in ovarian reserve were examined by measuring the level of AMH pre-operation and at 1 and 4 months post-operation.No significant differences existed in AMH in LM group[(1.42 ±0.65),(1.31 ±0.53) & (1.33 ±0.61) μg/L,P ＞ 0.05].The levels of AMH in both groups were significantly lower than those at pre-operation [(1.17 ± 0.11),(1.01 ±0.10),(0.48±0.54) & (1.18±0.93),(0.45 ±0.39),(0.14±0.0) μg/L,P＜0.001].The mean percentage decrease in AMH was lower in TLH group than that in LSH group at 4-month follow-up [(-37.4 ± 37.6) ％ vs.(-27.0 ± 23.0) ％,P =0.017].The results showed that LM had no effect on ovarian reserve.Both LSH and TLH had adverse effects on ovarian reserve and TLH was more marked.
