1.Construction and application of the sustainable development reproductive medicine nursing model under the internet environment

Objective To explore a variety of sustainable development under the network environment of doctor-patient communication service platform in the role of reproductive medicine nursing,to improve the reproductive efficiency of medical care services in the new environment,build new doctor-patient communication platform.Methods In reproductive medicine nursing practice,based on the internet to build diversification of doctor-patient communication information service platform,such as reproductive center website,QQ group of doctor-patient communication,Wechat public service platform.Through the questionnaire survey form,understand reproductive center patients through various network platform in the query,the information,make an appointment for reproductive assisted reproduction related information and interact with staff and other aspects of the usage and satisfaction.Results Recycling effective questionnaire was 517 papers.A total of the use of network platform for the overall coverage was more than 80% of patients,among them the most widely Wechat public platform,utilization rate was 78.1% (400/512);60.8% (312/513) patients think Wechat platform using the most convenient.The overall satisfaction rate for center web services platform was 89.5% (460/514).Conclusions Online platforms provide better nursing efficiency and quality,as well as improving doctor-patient communication based on the internet environment.It provides a new way for sustainable development for nursing system in the field of reproductive medicine.
2.Study on technique of phase detection in magnetic induction tomography

In the magnetic induction tomography(MIT) system,the electrical conductivity of biological tissue is direct proportion to the phase difference between the excitation signal and the detection signal.To obtain the image of the contribution of tissue's electrical conductivity,the system must have the function of phase detection with high accuracy.The paper focuses on the means of digital phase detection,including FFT method,the correlation method and the classic method,which are ultimately compared with analogue phase detection method.The experimental results show that FFT method and the correlation method,with low error level and high linearity,can better detect the phase difference with the level of 0.1?.The digital phase difference detection provides a kind of effective method for MIT system.
3.A case report of primary hepatocellular carcinoma with gallbladder cancer

ABSTRACT:Objective To a rare case of double primary cancer (hepatocellular carcinoma with gallbladder)to guide clinical application.Methods and Results We reported one case of primary hepatocellular carcinoma and gallbladder.A male patient,54 years old,had main complaints of intermittent right upper quadrant pain for 4 days. The abdominal CT of the local county hospital showed gallstones,gallbladder with liver infiltration.And then he went to the First Affiliated Hospital of Xi’an Jiaotong University for further treatment.Laboratory examination revealed:HBsAg(+),HBcAb(+),alpha-fetoprotein (AFP)> 60 500 ng/mL,carcinoembryonic antigen (CEA) 5.25 ng/mL.Abdominal CT showed hilar slightly stronger light echo groups:liver cancer or gallbladder cancer? Hepatic artery and portal vein CT imaging (CTA+CTV)examination showed the malignant tumor shadow in the inside of the left hepatic lobe huge,uneven thickening of the gallbladder wall,suspected liver disease with gallbladder infringement or gallbladder disease with liver infringement. With the preoperative preliminary consideration of primary liver cancer with infiltration of the gallbladder,we chose the operation as the resection of segment Ⅳ b and Ⅴ of the liver,cholecystectomy and T tube drainage.Pathological examination postoperative showed the bulky liver carcinoma grade Ⅲ and the poorly differentiated adenocarcinoma of gallbladder.A month later,abdominal CT showed the tumor spread intrahepatic,prompting the poor prognosis.Conclusion The two which are not continuous,which is the standard of double primary cancer,are not suitable for all double primary cancers.This case provides useful experience for future similar diagnosis and treatment of disease,and also helps us with timely and accurate identification of “metastatic”or “primary”,which is the key point for clinicians to give patients an effective treatment.
4. Quality control research of Yuye Qinghuo Tablets by UPLC MS/MS multicomponent determination coupled with chemical pattern recognition

Objective: To establish the UPLC-MS/MS method for the simultaneous determination of 10 active components (geniposidic acid, chlorogenic acid, shanzhiside methyl ester, 3,5-O-dicaffeoylquinic acid, geniposide, mussaenoside, β-ecdysone, andrographolide, wedelolacton, and dehydroandrographolide) in Yuye Qinghuo Tablets (YQT), in order to make a systemic, comprehensive, and scientific quality evaluation of the drug combined with the chemical pattern recognition method. Methods: The chromatographic separation was achieved on Agilent Zorbax SB C18 (50 mm × 3.0 mm, 1.8 μm); The mobile phase was consisted of methanol-0.1% acetic acid (containing 20 mmol/L ammonium acetate) at a flow rate of 0.3 mL/min with gradient elution, the mass spectrum was scanned by ESI+ and ESI- multiple reaction monitoring (MRM) mode. And the content analysis was carried out by cluster analysis (CA), principal component analysis (PCA), and orthogonal partial least squares discriminant analysis (OPLS-DA) to comprehensively evaluate the difference in quality of YQT. Results: Under the optimized conditions, 10 components showed good linear relationships in the ranges of 0.352 5—14.100 0, 5.402—270.100, 0.205 8—8.232 0, 1.050—42.020, 4.020—160.800, 4.328—173.100, 2.044—204.400, 2.251—225.000, 0.232 8—9.312 0, and 4.708—188.300 μg/mL, respectively (r > 0.999 1), whose average recoveries were 95.02%—99.66% (RSD < 3.0%). The analysis results showed that the quality of the most batches was stable. However, it was found that there were still slight differences in the quality of different batches of drugs by CA and PCA. Finally, six symbolic components causing the quality differences among batches were screened out by OPLS-DA, they were andrographolide, dehydroandrographolide, mussaenoside, shanzhiside methyl ester, 3,5-O-dicaffeoylquinic acid, and chlorogenic acid, respectively. Conclusion: This simple, sensitive, and efficient method can be used for the rapid determination of main active components in YQT. The analysis of results combined with the chemical pattern recognition technology can comprehensively evaluate the drug quality as a whole, which provide new scientific basis and data processing methods for the quality control research of YQT.
5. HPLC fingerprints of Feijiehe Pills

OBJECTIVE: To establish the HPLC fingerprints of Feijiehe pills and provide a foundation for its quality control and evaluation. METHODS: A gradient elution method was applied. Ten batches of samples were analyzed by HPLC on a Diamonsil C18 (4.6 mm × 250 mm, 5 μm) column with mobile phase composed of acetonitrile-0.01% phosphoric acid solution at a flow rate of 0.8 mL·min-1, the detection was carried out at 270 nm, and the column temperature was maintained at 25℃. RESULTS: There were 10 common peaks in the HPLC fingerprints. The similarity between peaks was more than 0.90. CONCLUSION: The method is simple, accurate, reliable, and suitable to be applied to the quality control of Feijiehe pills.
6.Content Determination of Phosphatidylcholine in Eustachian Tube Lavage Fluid of Patients with Secretory Otitis Media by HPLC

OBJECTIVE:To establish a method for content determination of phosphatidylcholine in eustachian tube lavage fluid of patients with secretory otitis media. METHODS:HPLC was used. The samples were pretreated by liquid-liquid extraction. It was performed on the column of Hypersil CN with mobile phase of acetoneitril-methanol-phosphoric acid (100∶10∶0.6,V/V/V)at the flow rate of 1.8 ml/min,the detection wavelength was 205 nm,temperature was 30 ℃ and volume was 20 μl. RESULTS:The lin-ear range of phosphatidylcholine was 11.99-119.9 μg/ml(r=0.999 6);RSDs of precision tests of intra-day and inter-day were no more than 15%;average recovery was 97.54%(RSD=9.36%,n=9);the average content of phosphatidylcholine in eustachian tube lavage fluid of patients was(24.43±3.61)μg/ml. CONCLUSIONS:The method is simple and accurate,and can be used for the content determination of phosphatidylcholine in eustachian tube lavage fluid of patients with secretory otitis media.
7.Effects of Early Rehabilitation and Hyperbaric Oxygen Treatments on Stroke Patients

ObjectiveTo observe effects of early rehabilitation and hyperbaric oxygen treatments on stroke patients.Methods80 patients were randomly divided into rehabilitation group (40 cases) and control group (40 cases). Patients in rehabilitation group were treated with rehabilitation and hyperbaric oxygen treatments; patients in control group were treated only with hyperbaric oxygen treatment. The activities of daily living (ADL) were assessed by modified Barthel index (MBI), and the cognition function was measured by mimi-mental state examination (MMSE).ResultsAfter treatment the scores of MMSE and MBI of two groups increased (P<0.05) and scores of rehabilitation group were higher than that of control group (P<0.05); the process of scores in rehabibilitation group were obviously superior to that in control group (P<0.001).ConclusionEarly rehabilitation and hyperbaric oxygen treatments can obviously improve cognition function and ADL of stroke patients.
8.Exploration of the theory of "Fei and Dachang being interior-exteriorly related" from observing changes of inflammatory cytokines and oxygen free radicals in the lung tissue of ulcerative colitis rats.

OBJECTIVETo explore the theory of "Fei and Dachang being interior-exteriorly related" and the pathogenesis of lung injury by observing changes of inflammatory cytokines and oxygen free radicals in ulcerative colitis (UC) rats.
METHODSTotally 50 healthy male Wistar rats were randomly divided into two groups, the normal control group and the model group, 25 rats in each group. The UC model was established by allergizing colon mucosa combined with TNBS-alcohol (50%) enema. Another 25 rats were recruited as the normal control group. At week 2 and 4 after modeling, the pathomorphological changes of the lung were observed. Furthermore, the contents of tumor necrosis factor alpha (TNF-alpha) and IL-1beta were determined by ELISA. The activities of superoxide dismutase (SOD) and malondialdehyde (MDA) were evaluated with colorimetry.
RESULTSCompared with the normal control group, the pathomorphology of the lung tissue in the model group appeared abnormal at week 2 and 4. Compared with the normal control group, levels of TNF-alpha, IL-1beta, and MDA in the lung tissue significantly increased in the model group (P < 0. 01) and the activities of SOD significantly decreased (P < 0.05, P < 0.01).
CONCLUSIONTNF-alpha, IL-1beta, SOD, and MDA might be common material bases for the large intestine involved in lung disease of UC patients, thus providing a modern scientific basis for the theory of Fei and Dachang being interior-exteriorly related.

10.Diagnosis realization of knee joint synovial chondromatosis:clinical data of 28 cases.

OBJECTIVETo investigate diagnosis and therapeutic effects of knee joint synovial chondromatosis with arthroscopic.
METHODSFrom March 1995 to July 2011, 28 patients with knee joint synovial chondromatosis were treated. Among them, 18 males and 10 females ranging age from 25 to 81 (mean 55.2) years,the course of disease ranged from 0.5 to 15 (mean 5.6) years. Clinical manifestation mainly included pain, swell and functional limitation of knee joint. Knee open surgery (17 cases) and laparoscopic surgery (10 cases) were respectively used. Clinical symptom,image data,pathological manifestation and effects under arthroscopy were observed, Lysholm scoring was used to evaluate effects.
RESULTSAll patients were followed up except one lost, the duration ranging from 6 to 24 months. Lysholm score in knee open surgery was increased from (41.89 +/- 6.81) preoperatively to (67.73 +/- 7.62) postoperatively;while in laparoscopic surgery it was increased from (40.78 +/- 7.54) preoperatively to (77.46 +/- 8.43) postoperatively.
CONCLUSIONArthroscopic surgery, which has no risk of rupture of incision, nonunion, earlier to exercise, is a good method to diagnosis and treat knee joint synovial chondromatosis.
