1.Effect of community care and family members′participation on rehabilitation of patients with cerebral infarction

Objective To study the effect of community nursing and family members′participation on rehabilitation of patients with cerebral infarction. Methods Forty-two patients with cerebral infarction discharged from our hospital was chosen to take community rehabilitation training. The scores by modified Barthel index (MBI) and functional comprehensive assessment (FCA) before intervention were compared to those after intervention at the time points of pre-intervention and 1 month , three months after intervention. Results The scores by modified Barthel index (MBI) and functional comprehensive assessment (FCA) before intervention were significantly higher than those before intervention (P<0.01). The scores by MBI 1 month after intervention were significantly higher than those three months after intervention (P<0.01), indicating the longer the rehabilitation training, the better the effect. Conclusion The community rehabilitation care to patients with cerebral infarction sequelae can improve the quality of rehabilitation training and finally their quality of life.

3.Susceptibility genes for diabetic retinopathy

Diabetic retinopathy (DR) is a sight-threatening chronic complication of diabetes mellitus and is the leading cause of acquired blindness in adults. Long-term exposure to the hyperglycemia of diabetes patients leads to the development of DR. Several studies have provided evidence that good diabetes control is important to prevent DR. However, emerging evidence suggests that genes are a significant contributor to an individual's risk of retinopathy. This evidence is from evaluations of familial aggregation and different incidence of DR in racial and ethnic groups. Some groups of patients develop DR despite good control and some escape retinopathy despite poor control. This suggests that the genes are involved in the susceptibility to DR. Genes suggested as having a role include those encoding aldose reductase, nitric oxide synthase, receptor for advanced glycation end products, angiotensin converting enzyme, vascular endothelial growth factors and pigment epithelium-derived factor. An understanding of the role of susceptibility genes will ultimately allow the development of novel therapeutic strategies. This article reviews the role of genetic factors in the etiology and progression of DR.
4.Screen of crystallin protein mutation genes in a hereditary congenital cataract family

With the development of biological techniques,the study on the pathogenesis of disease-causing genes of congenital cataracts has substantial progress.Some positive results of screen of mutation gene in congenital cataract family has been reported,but the report of negative result is rate.ObjectiveThe present study attempts to screen the mutation of CRYAA,CRYAB,CRYA1/A3,CRYBB2,CRYGC and CRYGD gene in a Chinese family with autosomal dominant congenital cataract. MethodsThe periphery blood samples were exacted from 8 patients of 4 generations of with congenital cataract in this family.The complete coding region and intron spliced sites of CRYAA,CRYAB,CRYA1/A3,CRYBB2,CRYGC and CRYGD were amplified with polymerase chain reaction (PCR),and the products of PCR were directly sequenced.The control blood samples were from 10 normal subjects.This study followed the Declaration of Helsinki.Written informed consent was obtained from all of the patients.ResultsThe patients were found in each generation in this family and the mode of inheritance was in accordance with the characteristic of autosomal dominant inheritance.The sequence of amplified genetic fragments of CRYAA,CRYAB,CRYA1/A3,CRYBB2,CRYGC and CRYGD genes were inaccordance with those of normal subjects and GeneBank.No any mutation loci was found in all of the patients of this family.ConclusionCRYAA,CRYAB,CRYA1/A3,CRYBB2,CRYGC and CRYGD genes is not the causing-disease genes in this family.
5.Antibacterial Activity of Alkaloids Extract from Oxytropis falcata Bunge Against 9 Pathogens

Objective To investigate the antibacterial activity of the alkaloids extract from Oxytropis falcata Bunge against 9 common pathogens in vitro. Methods Alkaloids was extracted from Oxytropis falcata Bunge. The agar plate dilution method was adopted to evaluate the activity against bacteria of 4 Gram-negative test strains and 5 Gram-positive test strains, while the MIC and MBC were determined by tube double dilution method. Results The alkaloids extract from Oxytropis falcata Bunge had the antibacterial activity against 9 pathogens and there was a good dose-response relationship between sensitivity and concentration. Antibacterial activities against bacillus subtilis and stentrophmomas maltophilia showed stronger than other tested bacteria, the MIC and MBC were 0.95 mg/mL and 1.91 mg/mL, respectively. In other tested bacteria, MIC and MBC were ranged in 1.91-7.63 mg/mL. Conclusion The alkaloids extract showed a broad spectrum of a stronger antibacterial activity against all tested pathogens. Gram-positive strains were more sensitive than Gram-negative strains to the inhibitory effects of the alkaloids extract.
6.Application of prism alternate cover test and perimeter strabismometry in strabismic operation

AlM:To discuss the application of prism alternate cover test ( PACT) and perimeter strabismometry in strabismic operation.METHODS: A total of 145 patients with concomitant esotropia or exotropia, aging 2 ~ 50 years old, were included. They were examined by PACT and perimeter strabismometry before operation. The strabismic operations were performed, the orthotropia rate of postoperation was assessed.RESULTS:Using PACT and perimeter strabismometry, there was a significantly difference of deviating angle ( P<0. 05) in mild and severe strabismic group. But there was no significantly difference of deviating angle (P>0. 05) in moderate strabismic group. The functional cure rate of postoperation was respectively 70. 83%, 67. 01%; the clinical cure rate was 20. 84%, 24. 74%; invalid rate was 8. 33%, 8. 25% .CONCLUSlON: The application of PACT and perimeter strabismometry in strabismic operation design, referring to the eye axial length, this can be helpful of improving the cure rate postoperation and decreasing the danger of reopreation.
7.The Isolation and Identification of Strain Streptomyces xanthocidicus IMB-14

The strain Streptomyces sp.,nominated IMB-14,was isolated from the soil sample of WuDang Mountain by the method of cellulose ester membrane filter.The studies on antibiotic activities,morphological characteristics,cultural characteristics,physiological characteristics,16S rDNA sequence analysis and the metabolite of strain IMB-14 showed that the strain IMB-14 was accordance with Streptomyces xanthoci-dicus.The study on the isolation and identification of strain Streptomyces xanthocidicus establishes a foundation on screening of novel antibacterial and antitumor agents.
8.Fresh and old tibial intercondylar eminence fracture by arthroscopic wire fixation

BACKGROUND:Arthroscopic treatment of tibial eminence fracture requires tiny wire,induces less epiphyseal plate damage during operations,obtains strong fixations,alows early movement postoperatively,obviously attenuate the injuries,facilitate wound healing,and reduce complications.OBJECTIVE:To investigate the biocompatibility of arthroscopic reduction and wire fixation in treatment of fresh and old tibial eminence fractures.METHODS:Twenty-five patients with fresh and old tibial eminence fractures were treated with arthroscopic reduction and wire fixation from January 2010 to April 2012 in the Second People's Hospital of Yichang City.The postoperative fracture healing time and knee function improvement in the two groups were observed.RESULTS AND CONCLUSION: Compared with the fresh fracture patients,the fracture healing time was longer,Lachman test positive rate was higher in the old fracture patients than (P<0.05),knee flexion and extension activity and Lysholm score were lower (P<0.05).Experimental findings indicate that,arthroscopic reduction and wire fixation is a feasible clinical treatment of fresh and old tibial intercondylar eminence fractures.
9.Influence of age on prevalence rates of chronic complications of hospitalized aged patients with hypertension——analysis of 17,682 cases

Objective To understand the relationship between age and chronic complications in hospitalized aged patients with hypertension, to provide evidence for hypertension prevention and control. Methods To retrospectively analyze the clinical and laboratory data on 17,682 patients with essential hypertension during Jan 1st,1993-Dee 12th, 2008 in PLA general hospital. Results 1)Among all of the inrolled cases, those aged 60-64 account for 27.87%, 65-69 years group account for 26.55%, 70-74 years group accounted for 23.96%, 75-79 years group accounted for 14.14%, 80-84 years group accounted for 5.26%, 85-89 years group accounted for 1.69%, > 90 years accounted for 0.41%. 2) The prevalence rate of chronic complications in 60-69 years group were 31.3-31.2% for diabetes and,22.6-27.0% for cerebrovascular disease, 9.5-11.1% for myocardial infarction, 6.7-9.1% for heart failure, 5.8-6.0% for renal dysfanction 4.9-6.8% for atrial fibrillation, 0.1-0.3% for multiple organ dysfunction syndrome (MODS) in the elderly(P <0.05 ). 3) The first four complications of hypertension were diabetes(33.5%), cerebrovascular disease (31.9%), myocardial infarction(13.2%) and heart failure(12.3%) in 70-74 years group (P<0.05), cerebrovascular disease (42.8%), diabetes (32.8%), heart failure (16.5%) and myocardial infarction(15.9%) in 75-79 years group (P<0.05), cerebrovascular disease (45.4%), diabetes (35.0%), heart failure (21.1%) and myocardial infarction(15.9%) in 80-84 years group (P<0.05), cerebrovascular disease(42.5%), diabetes (35.8%), heart failure (23.1%) and renal dysfanction (17.7%) in 85-89 years group(P<0.05 ),and cerebrovascular disease (45. 2%), heart failure(31.5%), diabetes (26.0%) and renal dysfanction (20.5%) in patients more than 90 years group (P<0.05). Conclusions The prevalence rate and kinds of chronic complications in hospitalized aged patients with hypertension were changed with the increasing age, and the first kind of complication is cerebrovascular disease. It is of more importance to prevent the occurrence of renal dysfanction and heart failure in those hypertension patients who were more than 80 years old.
10.Development of an evaluation system for excellent clinical nurses based on competency theory

Objective To construct a scientific,valid evaluation system for excellent clinical nurses.Methods Based on the framework of competency theory,systematic literature reviewing,nursing experts discussion and the Delphi method were used to determine the primary indexes for the evaluation system.The evaluation system was determined from three perspectives,doctors',nurses' and patients'.Analytic Hierarchy Process (AHP) was then used to determine the weight of each index.Results The weight of doctors',nurses' and patients' perspective was 0.2,0.4,0.4 respectively.Doctors' and nurses' perspective(hereinafter referred to as staff's perspective) had the same evaluating indexes,which both consisted of 5first-dimensions,22 second-dimensions.The patients' perspective contained 3 first-dimensions,14 seconddimensions.In two-round Delphi technique,the rates of questionnaire retrieve were 91.2% (31/34),100.0%(31/31),respectively;the colleagues' coordination coefficients were 0.784,0.858,respectively,and the patients' coordination coefficients were 0.05,0.216,respectively.Conclusions A evaluation system of high reliability and validity for excellent clinical nurseshas been successfully constructed.It may be utilized as a tool to for nursing administrator selection,training,assessment of excellent clinical nurses.
