Objective To investigate the effect of continuous subcutaneous insulin injection(CSII)and multiple subcutaneous insulin injection(MSII)on islet b cell function at the onset of type 2 diabetes.Method 64 patients were randomly divided into CSII(f/m=18/20)and MSII groups(f/m=14/12).There was no significant difference in age, fasting C peptide level, fasting blood glucose level,BMI and HbA1c.The patients of both groups were given intensive insulin therapy.Once the total dosage of daily insulin was kept less than 30U and the fast blood glucose was 3.6-6.0 mmol/L,intensive insulin therapy was changed to oral administration of hypoglycin A(OHA),otherwise the intensive insulin therapy was continued. The number of patients who were switched over to oral drug was compared after one month after the intensive therapy.Result 31 patients in CSII group and 8 patients in MSII group were changed to OHA one month after intensive therapy.A significant difference existed between the two groups(?2=3.37,P

5. Research Progress in Synaptic Plasticity Impairment of Diabetic Encephalopathy
Chinese Pharmaceutical Journal 2018;53(21):1805-1809
Previous studies demonstrate that diabetes mellitus induces cognitive impairment, leading to diabetic encephalopathy(DE), which is closely related with hippocampal synaptic plasticity impairment, including synaptic structural and functional damage. Structural damage mainly embodied in the synapse degeneration.Functional damage mainly reflects in the LTP damage, including the composition variation and functional lesions of N-methyl-D-aspartate receptors(NMDARs), α-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid receptors (AMPARs) and patassium channels.Abnormal synaptic plasticity may be critical in the pathogenesis of diabetic encephalopathy. In this review, we summarized the relationship between DE and synaptic plasticity impairment.

A biosurfactant-producing strain(S_6)was isolated from oil-containing wastewater in oxidation ditch and identified as Pseudomonas aeruginosa based on physiological and biochemical experiments and 16S rDNA sequence analysis.Infrared spectrum analysis revealed that S_6 produced glucolipid in the process of metabolism. It was observed that S_6 decreased the surface tension of water from 72 mN/m to 33.9 mN/m with the critical micelle concentration(CMC)of 50mg/L.The measurement of oil displacement and surface tension demonstrated that the fermented liquid had stable surface activity at varying range of salinity,pH,amount of dissolved oxygen. The optimal culture condition was obtained through orthogonal experiment:glucose 10g/L,urea 5g/L,KH_2PO_4 1g/L,liquor of microelement 2mL,pH 8.0,water 1000mL;and the biosurfactant production under optimal culture condition was 0.173g/L.

Objective To investigate the influence of probiotics on gastrointestinal motility and hormone secretion of rats with severe brain injury. Methods Severe brain injury rats were prepared. 54 Sprague-Dawley rats were randomized into three groups: group A ( sham-operation), group B ( standard enteral nutrition) and group C (standard enteral nutrition plus probiotics). Each group was equally divided into three subgroups on 1 d, 3d, 7 d, respectively. Gastric emptying rate, intestinal transmission rate, motilin ( MTL) and calcitonin gene related protein (CGRP) level in plasma were measured 1 d, 3 d, 7 d post trauma. Results Compared with group A, gastric emptying rate and intestinal transmission rate decreased significantly on 1 d post trauma(P <0. 01). They were still much lower in group B on 3 d post trauma(P < 0. 01), while they increased significantly in group C on 3 d post trauma(P <0.05). MTL and CGRP level in plasma increased significantly after severe brain injury; however, the levels in group B were much higher than those in group C on 1 d and 3d(P<0.01). There was no significant difference among the three groups regarding gastric emptying rate, intestinal transmission rate, and MTL and CGRP level. Conclusions Severe brain injury could induce disorder of gastrointestinal motility and dysfunction of hormone level. Early enteral nutrition supplemented with probiotics could improve gastrointestinal motility and transmission and regulate highly increased gastrointestinal hormone after severe brain injury.

Objective To investigate the epidemic situation of coal-burning-fluorosis,and to explore key areas for implementing prevention and control measures in Guizhou Province. Methods Survey data of dental fluorosis of children aged 8-10,anti-fluoride knowledge and health-related behaviors of target students and their families before implementing the comprehensive disease control measures in 2010,were analyzed and evaluated using the method of factor analysis in 23 diseased districts. Results ①The three main factors (F) related to the endemic of coal-burning-fluorosis were economic factor (F 1),knowledge factor (F2) and behavior factor (F3):F1 included the detection rate of 8 to 12 years old children's dental fluorosis, iron stove ownership, corn correct drying rate and chili correct drying rate; F2 included stove correct utilization rate, students awareness and head of the household awareness; F3 included corn food washing rate before cooking and chili food washing rate.②In the economic factor score arrangement, the first three were Zunyi,Xiuwen and Qingzhen. In the F2 score arrangement, the first three were Zunyi,Xishui and Xixiu. In the F3 score arrangement, the first three was Liuzhi,Bijie and Xixiu.③After comprehensive score,Zunyi,Qingzhen,Xishui,Xixiu,Puding and Liuzhi Counties were found to be relatively stronger in control of the epidemic of coal-burning-fluorosis; Xiuwen,Renhuai,Guanling and so on 13 counties were found to be relatively moderate, and Tongzi,Hezhang,Shuicheng and Nayong Counties were found to be relatively weak. Conclusions ①The endemic of coal-burning-fluorosis is related with many factors, and the indexes we selected in this study have a strong correlation and sensitivity, which may be used in evaluating the prevention effect of control measures to coal-buming-fluorosis.②The endemic of coal-burning-fluorosis in Guizhou Province is quite different, and key prevention and control measures can be applied according to a comprehensive factor analysis.
