1.Compliance rate of clinic appointments among residents contracting family doctor service and its influencing factors

Objective To investigate compliance rate of clinic appointments among residents contracting family doctor service and its influencing factors.Methods Total 1 442 clinic appointments among residents contracting family doctor service in Yuetan community were investigated by face to face questionnaire.Results Among 1 442 clinic appointments in Yuetan community health service center in June 2015,563 were kept with a compliance rate of 37.2%.There were significant difference in compliance rate among the residents with difference degree of education (OR =1.461,95 % CI:1.296-1.646,P =0.000),between those with and without chronic diseases(OR =2.009,95% CI:1.136-3.550,P =0.016),with and without receiving first contact care at community (OR =1.360,95% CI:1.036-1.785,P =0.027),with and without satisfying with family doctor service (OR =1.315,95 % CI:1.060-1.630,P =0.013),with and without recognizing clinic appointment system (OR =2.765,95% CI:2.142-3.569,P =0.000).Among the 906 failed appointments,the reasons were forgetting the appointment in 525 (57.9%),missing the date in 124 (13.7%),time conflict in 92 (10.2%),not bring reservation form in 85 (9.4%),being hospitalized or engaging in other medical service in 41 (4.5%) andneglecting appointment system in 39 (4.3%).Conclusion Compliance rate of clinic appointments among residents contracting family doctor service is lower,and necessary measures should be taken to increase the compliance rate.

4.Apoptosis of Neural Cell and Changes of Expressions of Bax,Bcl-2 Proteins after Frontal Lobe Sharp Injury in Infant Rats

Objective To investigate the alterations and molecular mechanism of neural cell apoptosis after frontal lobe sharp injury in infant rats.Methods Brain damage was induced by frontal lobe sharp injury.dUTP nick end labeling(TUNEL) method was used to detect cell apoptosis after brain injury,and immunohistochemistry was used to detect the expression of Bax,Bcl-2 proteins in and around the area of injury,respectively.Results The apoptosis cells were found as early as 1 hour and reached a peak at 24 hours post-injury,then gradually diminished until 120 hours when only few apoptosis cells presented.The expression of Bax and Bcl-2 proteins increased significantly 1 hour after frontal lobe injury.Bax protein reached its peak at 12 hours while Bcl-2 protein reached its peak at 1 hour post-injury.The ratio of Bax to Bcl-2 increased after lesion with peak at 24 hours,and then decreased thereafter.At 120 hours,very weak expression of Bax and Bcl-2 protein detected.There were few Bax and Bcl-2 positive cells in frontal lobe cortex in blank control and sham operation group.Conclusions Cell apoptosis exists after frontal lobe sharp injury in infant rats and the change of the number of apoptotic cells is correlated with time course of post-injury.The ratio of Bax to Bcl-2 may play a vital role in cell apoptosis.
5.Experimental and theoretical studies of the freezing processing in cryosurgery

The thermal characters such as temperature field, temperature grads and cooling power are very important for sur geons to estimate and improve curative effect. The freezing experiment around a 3.4mm cryoprobe was conducted and a multi dimensional numerical model based on enthalpy method and solved by finite element scheme for the freezing process of simu lative biological tissues during cryosurgery is presented. The results of the numerical solution were validated against experimen taal data .The utility of the numerical solution for the design and application of c ryodevices is demonstrated.
8.Gait disorders research in Alzheimer's disease

Objective To investigate the gait differences among patients with Alzheimer's disease (AD) and mild cognitive impairment(AD-MCI) and the person with normal cognitive function,and analyze the reasons for the abnormal gait.Methods Eighty patients were included and divided into 3 groups according to the cognitive status:noncognitive impairment (NCI group,n =30),patients with mild cognitive impairment (AD-MCI group,n =29),Alzheimer's disease (AD group,n =21).The mini-mental state examination (MMSE),the Montreal Cognitive Assessment (MoCA),Addenbrooke's Cognitive Examination (ACE-R),the connection test (TMT-A),Digit Span Test(DST),activities of daily living questionnaire(ADL) and associated gait and balance scale (5 times sit to stand test (FTSTT),3 m walking test (TUG),Berg balance scale) were used to evaluate all research objects in each group.Results There were significant differences in NCI group compared with AD-MCI group and AD group in terms of FTSTT,TUG,Berg balance Scale and leg speed((9.59±2.39) s vs.(13.71±4.65) s vs.(14.15±4.20) s,(7.70±1.58) s vs.(11.13±3.02) s vs.(11.35±4.43) s,(54.20±1.56) points vs.(48.17±4.93) points vs.(48.10±5.46) points,(82.12±22.79) cm/s vs.(57.49± 14.89) cm/s vs.(57.70±14.68) cm/s;P<0.05).There were significant differences in NCI group compared with AD-MCI group and AD group in terms of MMSE,MoCA,ACE-R,TMT-A,DST((28.67±1.27) points vs.(26.76±2.59) points vs.(21.86±5.29) points,(26.03±2.58) points vs.(22.39±5.05) points vs.(16.90±5.05) points,(85.80±5.90) points vs.(78.03±7.58) points vs.(60.95± 13.99) points,(12.66±5.36) s vs.(18.99 ± 11.46) s vs.(30.49±26.98)s,(18.60±4.64) points vs.(15.94±4.76) points vs.(12.86±5.23) points;P <0.05).Conclusion Gait disorder might be an early sign for cognitive impairment.
9.A retrospective analysis of 374 casualties from the Nepal earthquake

Objective To analyze the feature of victims transported from Nepal Pokhara's earthquake,and summarized the rescuing experiences got by the Chinese government medical team.Methods A total of 374 casualties from the 2015 Nepal's earthquake magnitude 8.1 transported to the Field Hospital of the Chinese government medical team were classified into four groups according to different causes of injury,and then retrospectively analyzed the types of injury,vital signs,severity of injury and causes of injury.Results Of 374 casualties,the average age was (30.1 ± 12.4) years,and were divided into group A (n =154) in which casualties trapped under ruined buildings,group B (n =123) in which the victims had crushed injury during escaping with their lives,group C (n =58) in which the wounded had injury resulted from fall on the ground during fleeing for their lives and group D (n =39) in which the injury of casualties was caused by fall from high level.Of them,273 patients (296 parts of body injured) suffered from fracture including 34 patients with severe multiple trauma.The average age in the group D (22.3 ±7.4 years old) was significant younger compared with the others (all P < 0.05).The average age in the group C (48.4 ± 13.3 years old) was significant older compared to the others (all P < 0.05).The injury probability was higher for male in the group D (14.2% vs.7.1%,x2 =5.077,P =0.024) and the injury probability was higher for female in the group C (19.7% vs 10.8%,x2 =5.635,P =0.018).The incidence of limbs fracture was highest (66.6%) in all 296 parts of body injured.The incidence of skull fracture in the group B (31.6%) was higher compared to the others (all P<0.05),and the incidence of spinal fracture in the group D (40.0%) was higher compared to the others (all P < 0.05).There was no significant difference in the severe multiple trauma incidence between the group A and the group D (16.9% vs.12.8%,x2 =0.381,P =0.537),but both group A and group D had significant higher incidence of severe multiple trauma compared to the rest two groups (all P < 0.05).The wound debridement was the leading rate of treatment among variety of treatments (57.5%) and the plaster fixation was the second high rate of treatment (30.9%),and the wound infection was found in 25 patients (17.4%) at the first dressing among 144 patients with soft tissue laceration.After active and regular debridement,72% patients' wound smear tests were negative 3 days later,and 100% were negative 7 days later.Conclusions The casualties treated by Chinese government medical team had relatively young average age,not very severe critical illness,and quite high rate of wound infection.The treatment of all kinds of fractures and open wound was a matter of critical importance.Although facing the challenge of incomplete set of medical equipment,the lacking of medicine and many other unexpected problems,the Chinese government medical team successfully fulfilled the mission with the prior experience accumulated from several domestic earthquake disaster rescue actions.
10.Brain protection of isoflurane preconditioning on the rat liver against ischemia-reperfusion

Objective To investigate the brain protective effect of isoflurane preconditioning on the rat liver against ischemia-reperfusion through determining the content of S-100β protein in peripheral blood in combination with mitochondrial ultrastructure in rat brain.Methods A total of 45 SD rats were randomly divided into three groups,sham group (group S):the only separation of the hepatoduodenal ligament,but did not block the hepatic portal blood supply; ischemia-reperfusion group (group I/R):liver ischemia 60min,reperfusion 120 min; isoflurane preconditioning group (group ISO):60 min before liver I/R,ISO pretreatment for 30 min,elution in the air after 30 min; 24 h after recirculation the forebrain tissues were rapidly removed.The changes of mitochondrial ultrastructure were observed by electron microscopy.The content of S-100β protein in serum was measured before ischemia and reperfusion 120 min through the application of Elisa kit.Results Marked swelling of mitochondria with disrupted cristae and damaged matrix were observed in group I/R,while relative intact mitochondria were seen in sham and ISO groups.The content of S-100β protein in serum was significantly higher in I/R group [(1.52 ±0.26) μg/ml] than in sham [(0.31 ±0.05)μg/ml] and ISO [(0.79 ± 0.21) μg/ml] groups (P <0.05).Conclusions The liver ischemia-reperfusion may injure the brain of the rat and isoflurane preconditioning can protect the rat brain from injury.
