1.Application of He-Ne laser combined with TDP irradiation in the treatment of herpes zoster ophthalmicus

AIM: To evaluate the effect of He-Ne laser combined with TDP irradiation in the treatment of herpes zoster ophthalmicus.METHODS: Totally 22 patients (22 eyes) with herpes zoster ophthalmicus were treated by He-Ne laser irradiation combined with TDP (Group A).Another 22 patients (22 eyes) with herpes zoster ophthalmicus (Group B) were treated by medication.The duration of the disease, the incidence of complications, and the time of pain relief were compared between the two groups.RESULTS: The cure rate of Group A was 91%, that of Group B was 73%, there was no significant difference between the two groups (x2=1.375, P>0.05).The average cure time of Group A was 8±2.22d, Group B was 12±1.88d, the difference between the two groups was statistically significant (t=5.22, P<0.05).The complication rate of Group A was 18%, Group B was 59%, the difference between the two groups was statistically significant (x2=6.86, P<0.05).The average pain relief time of Group A was 5±1.23d, Group B was 10±1.34d, the difference between the two groups was statistically significant (t=11.17, P<0.05).CONCLUSION: He-Ne laser combined with TDP irradiation can shorten the treatment time of herpes zoster ophthalmicus, shorten the course of disease, reduce the incidence of complications, relieve pain, with no significant effect on the cure rate of herpes zoster ophthalmicus.
2.Comparison of 2 . 2 mm micro incision and 3. 0mm incision coaxial phacoemulsification

AIM:To compare the operation effect of 2. 2mm micro incision and 3. 0mm incision coaxial phacoemulsification.
METHODS: Patients with age - related cataract ( 90 cases, 90 eyes) were enrolled in the study from January 2012 to June 2013 in our hospital, and they were randomly divided into 2 groups:2. 2mm incision group (45 eyes of 45 cases), 3.0mm incision group (45 eyes of 45 cases). Corneal tunnel incision coaxial phacoemulsification was performed. At 1d, 1wk, 1, 3mo after operation, the visual acuity, corneal endothelial cell count, central corneal thickness, operation astigmatism were recorded.
RESULTS:One day after operation, the visual acuity of 2. 2mm incision group were significantly improved, the difference was statistically significant ( P<0. 05 ), there were no differences in two groups at 1wk, 1, 3mo after operation. No significant differences between the two groups of corneal endothelial cell counting, central corneal thickness at 1wk, 1, 3mo after operation were observed(P>0. 05). Operation source astigmatism had statistically significant difference at 1d, 1wk, 1, 3mo after operation ( P< 0. 05 ), it decreased significantly in the 2.2mm incision group.
CONCLUSION: The 2. 2mm small incision coaxial phacoemulsification with postoperative visual acuity improved more significantly, can significantly reduce the operation astigmatism.
3.Endovascular embolization treatment of intracranial aneurysms with Gugliemi detachable coils

Objective To report the experiences in the endovascular embolization treatment of intracranial aneurysm. Methods A total of 29 patients with intracranial aneurism were treated with Gugliemi detachable coils(GDC). Results Complete occlusion was found in 15 cases, 90% occlusion in 12 and 80% in 2. Three patients had different degrees of headache. The coil escaping from the sac of the aneurysm and suspending in the parent artery of the aneurysm was seen in 1 patient. Two patients died: one was due to disruption of aneurysm and another severe bleeding in digestive tube. Conclusion Endovascular embolization of intracranial aneurysm is a minimally traumatic, safe and effective method for the treatment of intracranial aneurysm. GDC system is of easy operation, high safety and low incidence of complications.
4.Study on food intolerance in patients with ulcerative colitis

Objective To investigate the status of food intolerance of patients with ulcerative colitis,(UC),to provide the basis for directing UC patients diet related nursing.Methods Forty-seven UC patients were selected from September 2013 to August 2014 in General Hospital of Tianjin University.The Southerland Disease Activity Index (DAI) was used to divide the UC patients into remission,mild activity and moderate to severe activity groups.The food-specific IgG antibodies status were tested by enzyme-linked immunosorbent assay in UC patients and healthy people.Relevant clinical parameters were analyzed statistically.Results Food intolerance rate in UC patients was as high as 68.08%(32/47);the food species:egg,wheat and milk ranked the most frequently intolerance food.The food intolerance rates in remission,mild activity and moderate to severe activity groups were 9/16,11/18,12/13,there were significantly different,x2=7.418,P<0.05.Extraintestinal manifestation group had significantly higher food intolerance rate compareed to non extraintestinal manifestation group:1 1/13 vs.55.88%(19/34),x2=2.234,P<0.05.No significant difference of food intolerance rate were showed among groups categorized according to sex,age,body mass index,site of lesion,P>0.05.Conclusions UC patients show higher food intolerance rate than healthy people.UC disease activity and extraintestinal manifestations are correlated to food intolerance,which is of great significance to guide diet related nursing of UC patient.
5.Analysis of CT features of pulmonary sarcoidosis

Objective To investigate the CT features of pulmonary sarcoidosis.Methods Ninety patients with histologically proved pulmonary sarcoidosis were retrospectively studied by using CT scans and clinical recording.Results The main CT findings of pulmonary sarcoidosis were nodules which were seen in 69 cases(76.7%),and the nodules mostly distributed around the bronchovascular bundle(n=37, 41.1%).Other abnormalities included consolidation(n=31,34.4%),ground-grass(n=39,43.3 %), thickening of bronchovascular bundle(n=30,33.3%),interlobular septal lines(n=58,64.4%), fibrosis(n=17,18.9%)including bronchial distortion(n=8,8.9%),linear shadow(n=5,5.6%), and honeycombing shadow(n=4,4.4%),air-trapping(n=3,5.3%),bronchial straitness(n=8, 8.9%),pleural thickening(n=42,46.7%),and hilar and mediastinal adenopathy(n=76,84.4%). Two or more abnormal findings co-existed in 83 cases.The pulmonary lesions co-existed with hilar and mediastinal adenopathy in 76 cases.The nodules(n=25),consolidation(n=9),ground-grass(n=11), thickening of bronehovascular bundle(n=10)were improved after therapy.Ten cases of the interlobular septal(10/22),0 of bronchial distortion(0/4),1 case of diffuse linear(1/3),and 0 case of honeycombing(0/2)were improved.Conclusion CT manifestations of pulmonary sarcoidosis are varied, but has some specific radiographic features.A correct diagnosis can be made.combined with hilar and mediastinal adenopathy.
6.Terpenoids and sterols from Ricinus communis and their activities against diabetes.

Seven terpenoids and three sterols were isolated from the methanol extracts of the aerial parts of Ricinus communis by chromatography methods and their structures were identified by spectra analysis as ficusic acid( 1), phytol(2), callyspinol(3) , lupeol(4), 30-norlupan-3beta-ol-20-one(5) , lup-20(29)-en-3beta,15alpha-diol(6) , acetylaleuritolic acid( 7), stigmast4-en-3-one(8) , stig-mast-4-en-6beta-ol-3-one(9) , and stigmast4-en-3,6-dione(10). Compounds 1-3 and 5-10 were obtained from this species for the first time and 5 and 6 showed significant inhibitive activity and good selectivity against 11beta-HSD of mouse and human in vitro. 8.Prevalence and related risk factors of posttraumatic stress disorder in rescue paratroopers 1.5 year after Wenchuan earthquake

Objective To investigate the prevalence rate and the related risk factors of posttraumatic stress disorder ( PTSD) in the rescue paratroopers 1.5 year after Wenchuan earthquake. Methods A total of 183 rescue paratroopers who participated in disaster rescue after Wenchuan earthquake were investigated with the questionnaires composed by General state of health questionnaire, PTSD Checklist-Civilian Version ( PCL-C), Symptom Checklist-90 ( SCL-90), Self-Rating Depression Scale ( SDS) and Eysenck Personality Questionnaire (EPQ). Results Six rescue paratroopers had total PCL-C score ≥ 38 points, with the prevalence rate of PTSD for 3.28%. Total PCL-C score was positively correlated with total SCL-90 score, N score of EPQ and total SDS score (P < 0. 01), but negatively correlated with E score of EPQ. Total PCL-C score showed statistical difference between injury group and uninjury group, family loss group and family integrity group, contacting corpses group and none contacting corpses group (P<0.05 or <0.01). Regression analysis showed the length of military service, nervousness, family loss by earthquake, suffering injuries or contacting corpses during relief operations were the risk factors for PTSD. Conclusion The prevalence rate of PTSD in the rescue paratroopers is higher than that in normal troops (0.49% ) 1.5 year after Wenchuan earthquake, indicating that further attention should be paid to the rescue paratroopers in later period after earthquake.
10.Effect of microglia/macrophage pre-activation on TLR2/NF-κb signaling pathway early after ischemic brain injury in rats

Objective To investigate the effect and significance of microglia/macrophage activation prior to cerebral ischemic preconditioning (CIP) in regulating toll-like receptors 2 (TLR2)/nuclear factor-kappa B (NF-κB) inflammatory signaling pathway in early stage after ischemic brain injury in rats.Methods Thirty healthy male SD rats were selected and divided into normal control group,sham operation group,ischemia group,intervention group and treatment group according to random number table,with six rats per group.A rat model of focal permanent cerebral infarct was established by occlusion of middle cerebral artery (MCAO).CIP was performed by local ischemia-reperfusion.Minocycline was used to inhibit microglia/macrophage activation after CIP.Features of microglia/macrophage activation after CIP were detected by immunofluorescence; mRNA expressions of predominant factors (NF-κB inhibitor α,IκB-α;tumor necrosis factor α,TNF-α) of TLR2/NF-κB inflammatory signaling pathway in parietal cerebral cortex by in situ hybridization method; death rate by Kaplan-meier survival curves; neurological deficits by a 5-point neurological scale; brain infarct size by triphenyl tetrazolium chloride (TTC) staining.Results Microglia/macrophage started activation at one hour after cerebral ischemic injury in preconditioning group and presented a significant increase at 12 hours.Speed and range of activation were higher in preconditioning group than in ischemic group.IκB-α mRNA in preconditioning group started expression at one hour.TNF-α mRNA in preconditioning group remained a low expression in 12 hours and had a significantly lower peak value as compared with that in ischemic group (P < 0.05).CIP increased rat survival rate significantly,improved nerve function and reduced infarction size when compared with the ischemia group (P < 0.05).Minocycline inhibited nerve protection by CIP significantly (P <0.05).Conclusion CIP induces rapid activation of microglia/macrophage in early period of rat cerebral ischemic injury and provides brain protection probably via inhibition of TLR2/NF-κB activity and inflammatory overreaction to cerebral ischemia.
