1.The therapeutic effect of PNF on shoulder subluxation after stroke

Objective To investigate the curative effects of selected proprioceptive neuromuscular facilitation (PNF) patterns and techniques on shoulder subluxation soon after strokd. Methods Forty-one stroke patients with shoulder subluxation were randomly divided into a treatment group(n=22) and a control group (n=19). The patients in the treatment group receiveed the selected PNF techniques daily for 4 weeks, while the patients in the control group were treated with their previous therapeutic protocol. The shortest distance from the lower border of the acromion to the nearest point of the humeral head(AHH) was measured. Brunnstron grading was used to evaluate motor function in the affected upper linb, and a visual analogue scale (VAS) to assess subLuxation. Shoulder pain before and after treatment was also assessd. Results There was no significant difference in general condition between the patients in the two groups before treatment. In the treatment group, a significant improvement in AHH distance of the affected shoulder was observed after 4 weeks. Shoulder subluxation waw cured in 13 patients, with AHH distance <0.75 cm after 4 weeks of treatment. Conclusion The PNF technique applied was effective for the early treatment of subluxation after stroke. Motor function in the affected upper limb could be improved significantly and pain alleviated.
2.Effects of estrogen on the expression of stromal cell-drived factor- 1 in human matrix fibroblasts and breast cancer cell lines

ObjectiveStromal cell-derived factor -1 (SDF-1 ) is closely related to the biological characteristics of breast cancer. We aimed to explore whether estrogen affected breast cancer by SDF-1. MethodsThe breast cancer cell line MCF-7 and MRC5 were chosen, and divided into three groups: the control group, the estrogen group and the estrogen + estrogen receptor blocker group. Each group was cultured with different physiological concentrations of 17-β estrogen at certain time, and the same alcohol concentration of 17-β estradiol at different time points, and then the enzyme-linked immunosorbent assay (ELISA) was used to measure the concentration of SDF-1 in culture medium, and the semi-quantitative reverse transcriptionpolymerase chain reaction (RT -PCR) was used to detect the expression of SDF-1 mRNA in each group.ResultsSDF-1 can be detected in the culture medium of both MCF-7 and MRC5 cell lines. All different concentrations of 17-β estradiol may increase the secretion of SDF-1 in MCF-7 cells. When adding 17-β estradiol to the concentration of 107mol/L, the secretion of SDF-1 reached the peak in 2 hours, which was 6 times and 2.7 times that of control group ( P < 0.01 ). The effect could be ehminated by pure estrogen receptor ICI182,780. In addition, the mRNA expression of SDF-1 was consistent with the SDF-1 protein levels-l07 mol/L group. The expression of SDF-1 mRNA was higher than both that of the control group and the blocking group in 2 hours (P < 0.05 ). ConclusionsIn some breast cancer cell lines, physiological concentrations of estrogen can increase the secretion of SDF- 1, and this effect is mainly achieved through the estrogen receptor. Estrogen can influence the biological characteristics of breast cancer by SDF-1.
3.The impact of obstructive sleep apnea-hypopnea syndrome on early renal damage among patients with type 2 diabetes mellitus

[Summary] The aim of this study was to examine the association between obstructive sleep apnea-hypopnea syndrome ( OSAHS ) and microalbuminuria in type 2 diabetes mellitus patients. We found that severe OSAHS significantly increases the risk of early renal damage in type 2 diabetes mellitus patients with HbA1C<7% ( lowest oxygen saturation:OR=2. 41, 95% CI 1. 19-8. 08; apnea hyponea index: OR=2. 91, 95% CI 1. 50-9. 11), suggesting that OSAHS may increase the risk for early renal damage in type 2 diabetes mellitus, especially in those with successful control of glucose.
4.Effects of National Medicine Policy on the Use of Essential Medicines in A Public Hospital at County-level

OBJECTIVE:To provide reference for promoting the further implementation of national essential medicine system and related policies. METHODS:Hospital information system(HIS),Excel 2007 and SPSS 13.0 software were adopted to statisti-cally compare the changes in equipping the specific varieties of national essential medicines and the differences in the overall use of national essential medicines before and after implementing the National Essential Medicine System (2012 edition)(referred to NEMS,2012 edition) and cancelling drug addition policy in a public hospital in Chengdu. RESULTS:After implementing the NEMS(2012 edition),60 kinds of equipping national essential medicines increased in the hospital,including 38 kinds of chemical medicines and biological products,22 kinds of Chinese patent medicines;compared with before,the percentage of essential medi-cines to prescription drugs,prescription rate of essential medicines,average essential medicine in a single prescription and prescrip-tion rate of all essential medicines were significantly improved(P<0.05). After cancelling drug addition policy,5 kinds of equip-ping national essential medicines increased in the hospital,including 3 kinds of chemical medicines and biological products,2 kinds of Chinese patent medicines;the above-mentioned 4 indexes showed no obvious changes,with no statistical significances (P>0.05). CONCLUSIONS:After implementing the NEMS(2012 edition)and cancelling drug addition policy,the equipping na-tional essential medicines are still insufficient,some indexes do not meet the requirements of Sichuan Province. It is suggested that the public medical and health institution at county-level should to choose suitable national essential medicines based on its specific situation;strengthen publicity and education,improve the popularity of physicians to national essential medicine system;strengthen the assessment and management of the use of essential medicines for physicians. Meanwhile,the government and the relevant de-partments are suggested to explore more effective and reasonable compensation mechanism to improve the enthusiasm of physicians to use the national essential medicines.
5.Diagnosis of X-Linked creatine transporter deficiency in a patient from Northeast China

Background: Creatine transporter (CRTR) deficiency is the most common creatine deficiency syndrome,
of which the final diagnosis relies on mutation in the X-linked CRTR gene. To date, more than 90
mutations in the SLC6A8 gene have been reported. This paper discusses a novel mutation detected via
the thorough sequencing of all the X-chromosome-specific exons investigated in a four and a half year
old boy with an intellectual disability, speech and language delay and motor disturbance. Methods:
A brain magnetic resonance imaging (MRI) and a proton magnetic resonance spectroscopy (MRS)
were carried out, the creatine and creatinine concentrations in the urine were checked and all exons
were sequenced. Results: A detailed clinical investigation revealed a reduction in the cerebral creatine
levels in the brain by the MRS, elevated creatine and creatinine concentrations in the urine and signal
abnormalities in the left frontal cortex of the brain by the MRI. A novel change was identified in the
heterozygosity of the exon 10: c.1395-c.1401 deletion.
Conclusion: The use of a combination of powerful new technologies, such as thorough exome-nextgeneration
sequencing and a brain MRS, should be considered, in order to determine any neurometabolic
diseases, especially when the signal abnormalities in the brain MRI cannot be explained by any other
factors. This mutation results most likely in a dysfunction of the creatine transport and synthesis,
hence causing central nervous system symptoms.

In order to demonstrate PTB bind to HPRE,reverse transcription,PCR-mediated detection,were used.HepG2.2.15 cell line and HBs-HPRE transient expression cells were adopted to identify PTB function in HBV life cycle.The results showed that PTB could directly bind to HPRE RNA.Functional analysis indicated that PTB could inhibit the expression of HBs antigen and this inhibition was in a dose-dependent manner in HepG2.2.15 cells.Higher expression of HBs in cells transfected pcDNA3-HBs-HPRE comparing with pcDNA3-HBs,and this high expression could also be inhibited by PTB.The data demonstrated that PTB inhibits HBs expression by interacting with HPRE.
7.High-bone-turnover Osteoporosis and Aortic Calcification in Opg Knockout Mice

Bone turnover is regulated by local concentrations of cytokines such as osteoprotegerin (OPG) and receptor activator of nuclear factor kappaB ligand (RANKL). To explore the in vivo biological function of Opg and the mechanism of osteoporosis due to deficiency of Opg, Opg knockout mice have been generated through homologous recombination. Opg-/- mice exhibit a sharply decrease in bone density and strength as expected. The number of osteoclasts in Opg-/- mice significantly increases. Morphologically, osteoclasts appear more cuboidal in shape in Opg-/- mice than those of wt mice, suggesting that active osteoclastogenesis occurs in the absence of Opg. In consistent with this finding, an increase of osteoblast activity was also observed with accelerated mineral accumulation rate by histomorphometric measurement and elevated serum alkaline phosphatase activity (ALP) in Opg-/- mice. Interestingly, more than 50% of 2-month-old Opg-/- mice manifest medial calcification of aorta with comparable serum concentrations of calcium and phosphorus to wt mice. In conclusion, Opg-/- mice have a high-bone-rurnover type osteoporosis. The aortic calcification in Opg-/- mice is not due to abnormality of calcium and phosphorus metabolism. The mechanism underlying aortic calcification in Opg-/- mice needs to be further investigated.
8.Investigation on the running status and influencing factors of the water-improving projects in Gansu Province

Objective To evaluate the running status of the water-improving projects in Gansu Province in order to further improve the supervising mechanism and raise the efficiency of defluoridation in drinking water.Methods The water-improving projects,either being normal,or intermittent,or discarded and the cause of abnormal running in 33 counties in Gansu Province were surveyed with unified standard method.Fluoride content in water was determined by F-ion selective electrode.Results 993 water improving projects covering 3389 fluorosis villages were surveyed,682 projects worked well[68.68%(682/993)]and had supplied water with normal fluoride contents to 2174 villages[64.15%(2174/3389)]and benefited 116.56 hundred thousand populations.And the rest 311[31.32%(311/993)]projects worked abnormally or supplied unqualified fluoride water.The main causes were the exceeded lasting life,insufficiency of water resources,high fluoride content in water and unqualified administration.Conclusions Most of the water-improving projects in Gansu Province are basically running normally,but about 1/3 of the projects do not function well,so the management of the projects must be improved and consummated.
10.Chemoembolization of bilateral internal iliac arteries in Miles operation.

OBJECTIVE:
To prospectively investigate the efficacy of chemoembolization of bilateral internal iliac arteries in patients underwent Miles operation.
METHODS:
One hundred and thirteen patients with rectal cancer were randomly classified into 2 groups. The chemoembolization of bilateral internal iliac arteries with anticancer agents and gelatin sponge particles was applied during Miles operation in the treatment group (68 cases), while Miles operation was performed in the control group (45 cases). The amount of intraoperative bleeding, the operation time, the incidence of local recurrence, and the 5 year survival rate were analyzed and compared between the 2 groups.
RESULTS:
The complications and the operation time had not significantly different in both groups (P > 0. 05); the amount of bleeding and the incidence of local pelvic recurrence were less, and the 5 year survival rate was higher in the treatment group than that in the control group (P <0.05).
CONCLUSION
In Miles operation of rectal cancer, the chemoembolization of bilateral internal iliac arteries is a safe and effective therapy, and it can improve the efficacy of rectal cancer both in the short and long run.
