1.Brief instruction of the prevention of endemic fluorosis

OBJECTIVE: To summarize the data about endemic fluorosis, and investigate its prevalence and the effects of prevention and cure, so as to suggest more effective measures for the prevention and cure in the future. DATA SOURCES: A computer-based online search of China National Knowledge Infrastructure (CNKI) and VIP Chinese database was undertaken to identify articles about the distribution, monitoring and effects of prevention and cure of endemic fluorosis published in Chinese from January 1995 to March 2006 by using the keywords of "endemic fluorosis, monitor". STUDY SELECTION: Literatures about endemic fluorosis, including mainly brief introduction, practice in prevention and cure, summary of meeting, etc., the repetitive studies were excluded, and only one of them published the latest was involved. DATA EXTRACTION: Totally 25 relevant literatures were collected, and 18 of them were involved, the others were excluded. DATA SYNTHESIS: The monitoring results showed that until the end of the 2004, 44.51% of the villages, in the drinking water type disease areas with endemic fluorosis, had executed the low-fluorine treatment of water, and 194 villages had been controlled basically; In the coal pollution type disease areas, 24 villages had been controlled basically,25.42% of the families had reformed the stoves to prevent the coal pollution. The fluorine in water and air had been obviously declined after the improvement, some place had reached the standard of government.The morbidity rate of dental fluorosis in children of 8 to 12 years old was lower than before; The improving condition was good in most disease areas, and part of them had reached the control line; Meanwhile,the problems also existed that the measures of prevention could not befulfilled completely for the short of money, and the role of decreasing fluorine could not be acted completely because of the ill-management after the changed project.CONCLUSION: After the continuous efforts for tens of years, great progress has been obtained in the prevention and cure of endemic fluorosis,and the prevalence rates of dental fluorosis and skeletal fluorosis have reduced obviously, but many problems also exist at the same time. Therefore,the prevention and cure should be reinforced to restrain its prevalence more effectively.
2.Clinical observation on every 3-week oxaliplatin plus 5-FU/CF regimen as postoperative adjuvant chemotherapy for colorectal cancer

Objective:To investigate the clinical efficacy of every 3-week regimen of oxaliplatin (OXA) plus 5-fluorouracil (5-FU)/leucovorin(CF)as postoperative adjuvant chemotherapy for colorectal cancer.Methods:Ninety eight patients with stage Ⅱ to stage Ⅲ colorectal cancer received every 3-week OXA plus 5-FU/CF adjuvant chemotherapy for six cycles after resection surgery. All the patients received a complete reexamination every 3 months after the end of adjuvant chemotherapy. The disease-free survival period and 1- and 2-year disease-free survival rates were observed.Results:The total 2-year disease-free survival rate was 74.5%. It was 87.0% for stageⅡpatients and 63.5% for stage Ⅲ patients. The main adverse reaction included peripheral neurotoxicity, neutropenia and diarrhea at grades Ⅰ to Ⅱ. Grades Ⅲ to Ⅳ adverse reactions occurred rarely.Conclusion:The every 3-week OXA combined with 5-FU/CF adjuvant chemotherapy regimen had definite efficacy and good tolerance, which was a rational choice of postoperative adjuvant chemotherapy for colorectal cancer.
3.Association of the Number of Negative Lymph Nodes with Prognosis of Post-operative Gastric Cancer Patients

Objective: To evaluate the impact of the number of negative lymph nodes on disease free survival (DFS) of patients with locally advanced gastric cancer. Methods: A total of 485 patients who underwent surgery for gastric cancer were enrolled in this study. We analyzed the data of these patients including complete information of follow-up and definite postoperative pathological results. The disease free survival of all patients was more than 6 months and was analyzed according to the pathological stage respectively. COX regression analysis was performed to screen the independent prognostic factors. Kaplan-Meier method was used to draw the disease free survival curve and the disease free survival rate was compared among subgroups. Results: Cox regression analysis showed that the disease free survival rate of gastric cancer patients with pathological stage T_3N_(0~2)M_0 was significantly associated with age, degree of tumor differentiation, tumor location as well as the number of negative lymph nodes. In the T_3N_0M_0 subgroup, when the number of negative lymph nodes was 1 to 4 and 5 or more, the 2-year disease free survival rate was 8.3% and 55.6%, respectively, and the 3-year disease free survival rate was 0 and 24.9%, respectively (P=0.025).In the T_3N_1M_0 subgroup, when the number of negative lymph nodes was 3 or less, 4 to 9, and 10 or more, the 2-year disease free survival rate was 17.3%, 39.1%, and 52.6%, respectively, and the 3-year disease free survival rate was 4.2%, 6.0%, and 17.1%, respectively (P<0.001). In the T_3N_1M_0 subgroup, when the number of negative lymph nodes was 7 or less and 8 or more, the 2-year disease free survival rate was 11.5% and 35%, respectively, and the 3-year disease free survival rate was 0.8% and 5%, respectively (P=0.015).Conclusion: For gastric cancer patients with pathological stage T_3N_(0~2)M_0, the number of negative nodes could reflect the level of regional lymph node dissection. The number of negative nodes can be an index for evaluation of the accuracy of pathological staging and is considered as an independent prognostic factor for disease free survival of gastric cancer patients.

5.Budd-Chiari syndrome in youth: clinical features and interventional therapy

Objective To present the clinical features and to evaluate interventional therapy for Budd-Chiari syndrome in Chinese youth.Methods From January 1990 to April 2012,227 cases who hospitalized at the age < 29 underwent color Doppler ultrasound scan and digital subtraction angiography (DSA).87 cases were with occlusive inferior vena cava (IVC type),105 cases with occlusive hepatic veins (HV type) and 35 cases with occlusive inferior vena cava and hepatic veins (MIX type).The occlusive veins were opened by percutaneous transluminal angioplasty (PTA),endovascular stent placement,intravenous catheter thrombolysis or combination.Postoperative anticoagulation was given to all patients.Results The symptoms and signs of portal hypertension disappeared or were alleviated in successful cases.Technical success was achieved in 210 patients.The success rate was 100% in IVC type,85.7% in HV type and 94.3% in MIX type.IVC pressure decreased from (26.52 ± 8.16) cm H2O to (14.28 ±4.08) cmH2O(P < 0.05) and HV pressure dropped from(35.70 ± 13.26) cm H2O to(18.36 ±8.16) cm H2O (P <0.05).Restenosis or occlusion was found in 21.4% (45/210) patients after a follow-up of 1 month to 15 years.The rate was 13.8% (12/87) in IVC type,31.1% (28/90) in HV type and 15.2% (5/33) in MIX type.These patients were managed by interventional procedures.Technical successwas achieved in 44 cases with restenosis.Conclusions Hepatic vein occlusion was the most common type of BCS in Chinese youth.The symptoms and signs of portal hypertension were the initial clinical manifestations.Postoperative recurrence rate in HV type was higher than that in the other two types.
6.Anti-tumor activities of CIK cells derived from lymph nodes of lung cancer patients

Background and purpose:Cytokine-induced killer (CIK) has both the advantages of T lympho-cytes’ powerful anti-tumor activity and NK cells’ tumor killing capacity without MHC restriction. It could directly kill tumor cells, regulate and enhance immune function, without damaging the structure and functions of the immune sys-tem. Its effects on the treatment of malignant solid tumors has been widely recognized. This study aimed to evaluate the anti-proliferation effects of CIK cells obtained and cultivated from lung cancer patients’ lymph nodes. Meanwhile, the safety of clinical transfusion was observed.Methods:The peripheral blood and lymph nodes of 6 surgery patients with lung cancer from Shanghai Chest Hospital were used to cultivate CIK cells for 14 days. The phenotypes of CIK cells were detected by lfow cytometry. The anti-proliferation activities of CIK cells on A549 lung cancer cells were detected by CCK8 assay. The morphological changes of CIK cells were observed by invert microscope. The expression of CEA level and adverse events were evaluated after CIK transfusion.Results:The proportion of CD3+CD56+T lymphocyte in two groups were both more than 30%. The CCK8 assay showed that the suppression rate of lymph nodes group was higher than that of peripheral blood group at each effect/target ratio(P<0.05). The adverse effect of CIK transfusion was mild and tolerable. The expression of CEA level decreased in patients.Conclusion:Lymph nodes of surgery patients with lung cancer can be used for cultivation of CIK cells. The anti-tumor activity of CIK isolated from lymph nodes is better than that of CIK cells cultivated from peripheral blood. Preclinical experiments showed high safety.
7.Clinical observation of pingyangmycin,lidocaine,iodipin emulsion and absorbable gelatine sponge granules embolization treatment for hysteromyoma

Objective To study the feasibility and availability of uterine arterial embolization for hysteromyoma with pingyangmycin,lidocaine,iodipin emulsion and absorbable gelatine sponge granules immersed with lidocaine.Methods The observation of pain sensation change of clinical symptoms,size of hysteromyoma,change of uterus volume together with the specimens and pathological features of 68 cases of hysteromyoma during and after the embolization by using pingyangmycin,lidocaine,iodipin emulsion and absorbable gelatine sponge granules immersed with lidocaine were carried out and compared.Results Intramuscular analgesic injection couldn't control the pain in 13 cases,covering 19.1% symptom remission rate was 92.3%,including 89.2% menstruation recovery,90.8% disappearance of pelvic pain,abdominal distension and compression symptoms,78.8% average shrinkage of tumor size and 47.5% reduction of uterus volumes.Conclusion Pingyangmycin,lidocaine,iodipin emulsion and absorbable gelatine sponge granules immersed with lidocaine are the ideal embolization material for treating hysteromyoma.
8.Relationship between plasma ghrelin levels and insulin resistance and blood pressure in octogenarians.

The association between fasting plasma ghrelin levels and insulin resistance and blood pressure (BP) in octogenarians was investigated in this study. A total of 487 unrelated octogenarians (including 203 men and 284 women) were enrolled in this cross-sectional study at the Healthy Care Center of Shanghai East Hospital, Tongji University, China, from October 2008 to April 2009. Plasma ghrelin was determined by using the enzyme linked immunosorbent assay (ELISA). Insulin sensitivity was assessed using the homeostasis model of assessment-insulin resistance (HOMA-IR). The age of the participants ranged from 80 to 89 years (mean=83.9+/-4.8 years) with a body mass index (BMI) of 25.3+/-4.9 kg/m(2). Plasma ghrelin levels were 20.94+/-5.34 mug/L, being 20.89+/-5.53 mug/L in men and 21.38+/-3.73 mug/L in women respectively. Plasma ghrelin was not associated with systolic (P=0.981) or diastolic (P=0.724) BP, waist circumference (P=0.278), fasting insulin (P=0.246), fasting blood glucose (FBG) (P=0.693) and HOMA-IR (P=0.232). In the control cohort, no significant differences in plasma ghrelin were found between genders (P=0.489), and among subjects with hypertension (BP>140/90 mmHg) (P=0.284) and type 2 diabetes (P=0.776). In conclusion, fasting plasma ghrelin levels are not directly correlated with insulin resistance and BP among octogenarians.
9. Research progress of three-dimensional speckle tracking imaging in evaluation of left atrial structure and function

Three-dimensional speckle tracking imaging (3D-STI) is a new ultrasonic technology developed in recent years, which is based on gray-scale imaging. 3D-STI can accurately quantify the global and regional myocardial function by tracking trajectory of the spot signal myocardial echo in three-dimensional space. Recently, 3D-STI has been used to assess the structure and function of left atrium. The relative advancements were reviewed in this article.
