1.Primary Report on Gasless Laparoscopy for Totally Extraperitoneal Hernia Repair

Objective To evaluate the efficacy of gasless laparoscopy for totally extraperitoneal(TEP) hernia repair.Methods Between April and August 2007,totally 82 patients with inguinal hernia received TEP in our hospital,in 30 of them the surgery was completed with gasless laparoscopy,while in the other 52,pneumoperitoneum was used for the operation.The operation time,medical cost,hospital stay,complications,and follow-up data of the two groups were compared.Results The gasless group had a longer operation time [(65.3?19.4) min vs(43.6?21.9) min,t=4.501,P=0.000],but less medical cost [(6186.7?1283.7) RMB vs(8391.8?850.8) RMB,t=-9.347,P=0.000].No significantly difference was observed in the hospital stay and rate of complications between the two groups [(2.1?0.8) d vs(1.8?0.9) d,t=1.513,P=0.134;and 6.7%(2/30) vs 5.8%(3/52),?2=0.027,P=0.870].Conclusion Gasless laparoscopy is effective and safe for totally extraperitoneal(TEP) hernia repair.

5.Research progress of polyamidoamine dendrimer in targeting drug delivery system.

Targeting drug delivery system (TDDS) is one of the most concerned research fields in cancer treatment because it can bind selectively and react with the target diseased sites at the cellular or sub-cellular level, making distribution and release of drugs in a controlled manner, thus enhance therapeutic effects and reduce toxic and side-effects on normal cells. Polyamidoamine dendrimer (PAMAMD) is a kind of newly developed polymer in nanometer degree. Hyper-branched, monodispersity, three-dimensional structure and host-guest entrapment ability make it used as drug carrier, gene delivery system and imaging agent. Various targeting ligands, which have high affinity to specific organs, tissues or cells in human body, can be linked to surface functional groups of PAMAMD. And drugs and theoretical gene are carried by encapsulation or chemical conjugation. Finally, PAMAMD targeting drug delivery system can carry drugs and theoretical gene to diseased sites and then release them for targeted therapy. The PAMAMD-based conjugates have small size, ligh permeability and retention effect (EPR), low toxicity and so on. The research progress of PAMAMD modified by different ligands in targeting drug delivery system is reviewed, and research direction of the PAMAMD targeting delivery system in the future is also suggested.

6.The Research of Pulse Wave Velocity and Heart Rate Variability in patients with Isolated Systolic Hypertension

Objective To investigate the characteristics of pulse wave velocity(PWV)and heart rate variability (HRV)and evaluate their feasibility in grading the cardiovascular risk in patients with isolated systolic hypertension (ISH).Methods Eighty-nine ISH patients and systolic-diastolic hypertension patients(DH,n=98)admitted in our hospital were submitted carotid-radial PWV(crPWV),carotid femoral PWV(cfPWV)and HRV,ISH patients were categorized depending on their risk grade as:low risk group(n=3),moderate risk group(n=17),high risk group(n=35)and very high risk group(n=34).Results The cfPWV in ISH patients is significantly higher than that of sys-diastolic hypertension group(ISH: 399.6?48.2 vs sys diastolic hypertension:374.3?39.7 cm/s,P0.05).The LF in ISH group are markedly higher than those in DH(ISH:4.35?1.07 log ms~2 vs 3.78?0.82 log ms~2,P 0.05).LF are markedly positive correlated with the degree of the total cardiovascular risk(rs=0.438,P
7.Serotonin transporter gene polymorphism in slow transit constipation.

OBJECTIVETo investigate the association between the polymorphism of serotonin transporter gene-linked polymorphic region(5-HTTLPR) and slow transit constipation(STC).
METHODSPolymerase chain reaction was used to assess 5-HTTLPR polymorphism of SERT gene in 54 patients with STC and 100 healthy controls.
RESULTSThe frequencies of serotonin transporter short/short(S/S) and allele S genotypes were significantly higher in STC patients than those in controls(72.2% vs 50.0%; 83.3% vs 72.5%; both P< 0.05). There were no significant differences in 5-HTTLPR polymorphism respectively between the two groups according to gender and age(less than 45 and more than 45 years old). The frequency of S/S genotype was higher in the patients with less than 40% of the ingested markers evacuated within 72 h than those with more than 40% evacuated(71.7% vs 42.6%, P< 0.05).
CONCLUSIONThe presence of 5-HTTLPR allele S may contribute to the pathogenesis of STC.

8.Investigations and studies on medicinal plants from Selaginellaceae in Hubei Province.

OBJECTIVETo investigate the species, the distribution and the utilization of the medicinal plants from Selaginellaceae in Hubei Province.
METHODThrough field investigations and comparing the collected specimens and literatures, the classification and identification of the species in Hubei Province were studied.
RESULTThe results indicated that 15 species of plants from Selaginellaceae in Hubei, including 14 medicinal and 1 newly recorded species existed. The distribution and use in folk medicine were investigated. And the morphological description of several species was appended.
CONCLUSIONThe results provided a basis for the exploitation and utilization of the medicinal plant resources of Selagingellaceae.

9.Effect of APRIL on growth and apoptosis in transplanted tumor with human colorectal cancer cell line SW480 in nude mice.

OBJECTIVETo study the effect of pGCsi-H1-APRIL on the growth of human colorectal cancer cells in transplated tumor in nude mice and to improve the effect of APRIL on proliferation and apoptosis of colorectal cancer (CRC).
METHODSHuman CRC model was established in nude mice, and the nude mice were treated with APRIL siRNA twice per week for 2 weeks. APRIL mRNA expression was surveyed by PCR and APRIL protein expression was detected by immunohistochemistry. The expression of PCNA protein was detected by ELISA. The expression of bcl-2 and bcl-xl was assessed by immunohistochemical staining, and TUNEL staining was used to detect apoptosis.
RESULTSThe expression of APRIL mRNA in the APRIL siRNA group was (0.13 ± 0.05) × 10(-3), significantly lower than that in the vector group (0.95 ± 0.04) × 10(-3) and the PBS group (0.96 ± 0.05) × 10(-3). The expression of APRIL protein in the APRIL siRNA group was (87.5 ± 5.0)% lower than that in the vector and PBS groups (P < 0.05). APRIL siRNA significantly suppressed the growth of SW480 tumor: the IR (inhibitory rate) of APRIL siRNA group was (60.7 ± 1.5)% (P < 0.05). The expression of PCNA in APRIL siRNA group was (176.8 ± 18.1) ng/ml, was (56.5 ± 2.0)% lower than that of PBS group (328.4 ± 22.8) ng/ml. Furthermore, the expressions of anti-apoptosis proteins bcl-2 and bcl-xl of APRIL siRNA group were (82.6 ± 4.5)% and (79.2 ± 3.5)% lower than those of the PBS group. The apoptotic rate of the APRIL siRNA group was 40.1% ± 2.5%, significantly higher than that in the vector group (2.5 ± 0.1)% and PBS group (2.5 ± 0.2)% (P < 0.05).
CONCLUSIONAPRIL siRNA may significantly suppress the growth and promote apoptosis in transplanted tumor of human colorectal cancer in nude mice. APRIL may become a candidate gene of gene therapy of human colorectal cancer.

10.An evaluation on dietary pattern and quality among residents in Zhejiang province

Objective To assess the dietary pattern and quality of residents in Zhejiang province.Methods 21 52 residents aged 1 8 -79 years old were selected using multi-stage stratified cluster random sampling method and the data from Chinese nutrition status monitoring project during 201 0 -201 2 was used.The results were evaluated using Chinese diet balance index.Results 41.08% of the residents in Zhejiang province was mild diet imbalance and 50.79% was middle diet imbalance while insufficient and excessive intakes were coexist.Mild and middle insufficient intakes were accounted for 66.1 7%and 27.88%respectively,while mild excessive intake was accounted for 35.55%.The diet balance indexes including DBI-LBS,DBI-HBS and DBI-DQD were 46.00,26.00 and 57.00 respectively.The dietary quality of males,rural areas was lower while residents aged 40 -49 years had the lowest dietary quality.There was positive correlation between dietary quality and degree of education (P<0.05 ).The main dietary pattern of residents in Zhejiang was pattern B (62.41%)following by pattern A (26.44%).Conclusion Insufficient and excessive intakes coexist among residents in Zhejiang province.Male,living in rural areas,people aged 40 -49 years or with lower degree of education should be taken as target intervention subjects.
