1.Reconstruction of upper lip defect.

OBJECTIVETo summarize the clinical experience for the reconstruction of upper lip defects in different degrees.
METHODSDifferent treatment methods were selected to reconstruct the upper lip defects according to the subunit, size or location of defects on the upper lip. The therapeutic results were analyzed and compared.
RESULTSFrom Jan. 1998 to Apr. 2012, 243 cases with upper lip defects were treated, including 85 cases of traumatic defects, 110 cases of defects secondary to lip cleft and 48 cases of defects after tumor resection. 48 cases were treated with direct closure, 69 cases with cross-lip flaps, 5 Cases with orbicularis oris muscle flaps, 53 cases with unilateral labial flaps, 42 cases with bilateral labial flaps, 22 cases with lower lip flaps, and 1 case with forehead flap, 1 case with forearm flap and 2 with nasolabial fold flaps. The patients were followed up for 3-18 months with satisfactory results.
CONCLUSIONSSatisfactory results could be achieved for upper lip defects with appropriate treatment methods according to the defects degree and characters.

2.Clinical significance of differential expression of inflammatory factors in chronic non-bacterial prostati-tis/chronic pelvic pain syndrome

Objective To investigate the role of inflammatory cytokines in the pathogenesis of chronic non-bacterial prostatitis/chronic pelvic pain syndrome (CAP/CPPS) patients. Methods The 38 cases with CAP/CPPS patients (18 cases of CAP and 20 cases of CPPS) and 20 cases of healthy controls were selected. The differential expressions of 40 kinds of inflammatory cytokines were detec-ted by antibody arrays in prostate fluid. Results The inflammatory cytokines which increased more than 1.5 times expression have been found. There were seven kinds in CAP including monocyte che-moattractant protein (MCP)-1, solution tumor necrosis factor receptor Ⅱ(s TNF R Ⅱ), platelet-de-rived growth faetor-BB (PDGF-BB), interleukin (IL)-β, IL-11、IL-6、MCP-2 and five kinds in CPPS groups including MCP-1、PDGF-BB、MCP-2、s TNF R Ⅱ、It-11 respectively, compared with healthy control group. The cluster analysis results showed that protein expression of Monocyte chemoattrac-tant protein 1 (MCP-1)and platelet-derived growth factor BB (PDGF-BB) were significantly increased in CAP (3.47 and 2.07 times) and CPPS (2.25 and 2.19 times) compared with healthy control group and were the final polymerization of inflammatory cytokines. The protein expression of interleukin 1 β (IL-1 β), MCP-1 and soluble tumor necrosis factor Ⅱ (s TNF R Ⅱ) in CAP group was increased more than 1.85,1.55,1.67 times compared with CPPS group. Conclusions Elevated expression of inflammatory cytokines may play an important role in the course of CAP/CPPS disease. The extent of the inflammatory response of CAP was higher than CPPS. The inflammatory factors of MCP-1 and PDGF-BB could serve as a novel diagnostic marker.
3.Distribution features of Chinese medicine syndrome types in IgA nephropathy patients complicated with hypertension and analysis of its correlated factors.

OBJECTIVETo explore the distribution features of Chinese medicine syndrome types in immunoglobin A (IgA) nephropathy patients complicated with hypertension and its correlation with main prognostic indicators of hypertension classification and chronic kidney disease (CKD) staging, thus providing the diagnostic standards of Chinese medicine syndrome types and reliance for accurate syndrome differentiated medication.
METHODSBy on-the-spot survey, the Chinese medicine syndrome and laboratory testing data of 154 IgA nephropathy patients complicated with hypertension confirmed by the pathology of kidneys were collected to analyze the distribution of Chinese medicine syndrome types, its correlation with hypertension classification and CKD staging.
RESULTSAsthenia was the most common syndrome in the 154 patients (146 cases, 94.81%), covering Pi-Shen deficiency syndrome (58, 37.66%), Shen qi-yin deficiency syndrome (48, 31.17%), and Gan-Shen yin deficiency syndrome (40, 25.97%). Of them, asthenia accompanied by asthenia was seen in 80 cases (54.79%) and pure asthenia in 8 cases (5.2%). Shen qi-yin deficiency syndrome and Gan-Shen yin deficiency syndrome were mostly seen in hypertension III, while Pi-Shen deficiency syndrome was mostly seen in hypertension I. Pi-Shen deficiency syndrome was mostly seen in CKD stage 4, Shen qi-yin deficiency syndrome mostly seen in CKD stage 1-2, and Gan-Shen yin deficiency syndrome mainly distributed in CKD stage 1-3. No obvious correlation was seen between Chinese medicine syndrome types and 24-h urine protein quantitation.
CONCLUSIONSPi-Shen deficiency syndrome, Shen qi-yin deficiency syndrome, and Gan-Shen yin deficiency syndrome were main Chinese medicine syndrome types in IgA nephropathy patients complicated with hypertension. Asthenia accompanied by asthenia was mostly seen. Pi-Shen deficiency syndrome was mostly seen in hypertension I. Shen qi-yin deficiency syndrome and Gan-Shen yin deficiency syndrome were mostly seen in hypertension III, Shen qi-yin deficiency syndrome mostly seen in CKD stage 1-2, and Gan-Shen yin deficiency syndrome mainly distributed in CKD stage 1-3. Pi-Shen deficiency syndrome was mostly seen in CKD stage 4 patients.

4.Retrospective analysis on pathological features of 1561 patients with colorectal cancers in fifteen years

Objective To retrospectively study pathological characteristics of 1561 patients with colorectal cancer in Nanchang area.Methods Analysis was made regarding the results of colonoscopy and pathological examination in patients from 1990 to 2004.Results Totally 21 853 patients received colonoscopy during the period,and 1561 patients were diagnosed as colorectal cancer.The overall diag- nostic yield was 7.1%.The average age diagnosed was 52 years old.Ratio of male to female was 1.4: 1.The lesion located in rectum accounted for 43.4%.The percentage of cancer in left colon was higher than that in right side (30.5% vs 26.1%,P<0.05).The proportion (57.5%) of patients with rectal can-cer under 30 years old was the highest in all patients with rectal cancer.Cancer located in sigmoid colon and left side of the colon increased with age.Well and moderately differentiated adenocarcinoma was observed in 83.6% of the patients.Comparing the clinical characteristics of every five years,increasing diagnostic yield was noted from 1990 to 1999,hut the decreased trend was found in last five years.The propor- tions of rectal cancer was also increased in the last fifteen years,and overall proportions of cancer located in left side of colon was decreased.Conclusions The overall diagnostic yield of colorectal cancer in Nanchang area was lower than other province,especially rectal cancer.The proportion of cancer of left side of colon was increased with age.The well and moderately differentiated adenocarcinoma was found increasing in the last fifteen years, while the poorly differentiated adenocarcinoma was decreased.
5.Biological effects of phenytoin on cultured human periodontal ligament fibroblasts in vitro.

OBJECTIVETo study the biological effects of phenytoin (PHT) on cultured human periodontal ligament fibroblasts (hPDLF), and explore the possibility of its accelerating periodontal regeneration.
METHODSIncreasing concentrations of PHT (1, 5, 20, 100, 500, 2 500 mg/L) were added into the medium of the fourth passage of cultured hPDLF, respectively. After co-incubated for 3 days, cell proliferation activity, the total amount of protein and alkaline phosphatase (ALP) activity were detected. Mineralized sodium and PHT (20, 100, 500 mg/L) were added into the medium of the fourth passage hPDLF. After co-incubated, the mineralized nodules formation were detected by Von Kossa staining. The third passage hPDLF were stimulated by PHT (20, 100 mg/L), bone morphogenetic protein-2 (BMP-2) concentration was analyzed by enzyme linked immunosorbent sandwich assay (ELISA).
RESULTSAt the concentration of 20 or 100 mg/L, PHT significantly enhanced the proliferating activity and ALP activity of hPDLF (P<0.01). PHT at 100 mg/L could increase protein synthesis of hPDLF (P<0.05). The capability of mineralization and BMP-2 expression of hPDLF were increased significantly (P<0.01) in 100 mg/L group when compared with that in the control group. However, higher concentration (2 500 mg/L) not only changed cell morphology, but also significantly inhibited cell activity.
CONCLUSIONThe results suggested that proper doses of PHT could promote proliferation and biosynthesis and also enhance osteogenesis by increasing the differentiation, mineralization and BMP-2 expression of hPDLF while higher concentrations of PHT had cytotoxic effect.

6.Development of new methods for the diagnosis of nasopharyngeal carcinoma.

OBJECTIVEDevelopment of new methods, ELISA and immunostrip test, for the diagnosis of nasopharyngeal carcinoma.
METHODSThe engineering purified antigens coat plate or absorb on nitrocellulose filter. The plate and diagnostic strips carrying antigens were used for detection of IgG antibody in the sera from nasopharyngeal carcinoma patients and outpatients patients.
RESULTS127 cases sera from nasopharyngeal carcinoma patients were parallel detected TK/IgG antibody by ELISA and immunostrips. The TK/IgG antibody are all positive in the 127 cases of nasopharyngeal carcinoma patients. 55 cases show positive by ELISA, 58 cases positive by immunostrips in 247cases sera from outpatient. The antibody positive rate to early antigen p54 lower then to TK. Conclusion ELISA and imuunostrips are sensitive and specific means for detection of the IgG antibody to TK of EBV and the diagnosis of nasopharyngeal carcinoma.

7.Culture and phenotype characteristics of mouse dental follicle cells.

OBJECTIVETo establish a method for isolating and culturing mouse dental follicle cells and to study the phenotype characteristics of dental follicle cells.
METHODSMandibular first molars from 9 day old Balb/c mice were digested with 1% trypsin, subsequently, and the dental follicle was enucleated from the tooth germ and cultured. The shape and ultrastructural appearance of dental follicle cells were observed under phase-contrast microscope and scanning electron microscope (SEM). Immunocytochemistry was used to detect the expression of alkaline phosphatase (ALP), bone sialoprotein (BSP) and osteopontin (OPN).
RESULTSThree types of cells were isolated: some were cuboidal/polygonal; some were elongated, spindle-shaped, fibroblast-like cells; and a minor, third cell type was very thin and elongated. The cytoplasm of the first two cell types was filled with abundant granules. The cells were pleomorphism under SEM and had many filipodia and microvilli. According to whether there were filaments overlying the surface, the cells could be divided into two subtypes. Some but not all follicle cells expressed ALP, BSP and OPN.
CONCLUSIONThe cultured dental follicle cells consisted of several cell phenotypes and had the potential of differentiating into cementoblasts, periodontal ligament fibroblasts and osteoblasts.

Objective To investigate the diagnosis and the relationship between its clinicopathological characteristics,treatment and the prognosis of primary colorectal lymphoma.Methods The clinical data of 20 primary colorectal lymphoma patients who received operative treatment in the past 34 years in our hospital were retrospectively summarized,and the influence of treatment and each clinicopathological factor on prognosis was assessed.Results Tumor size,lymph node metastasis,invasion of neighboring organs and distant metastasis,and type of operative treatment were significantly related to the survival(P0.05).Conclusions Diagnosis of primary colorectal lymphoma before operation is difficult,and misdiagnosis is common.Operative treatment is the main therapeutic means for colorectal lymphoma.Radical operation combined with chemotherapy can gain a good prognosis.
9.Calcium-dependent chloride channels in plasma membrane of oocytes from toad, Bufo bufo gargarizans.

In this paper, membrane current properties of the fully-grown oocytes from toad, Bufo bufo gargarizans, were studied by using two-microelectrode voltage clamp technique. Axion of adult female toad was destroyed, and then ovarian lobes containing oocytes in stage I to VI were removed and incubated in Ca(2+)-free ND96 solution with collagenase (1.5 mg/ml) for 1 h. Subsequently, the oocytes were washed in Ca(2+)-free ND96 solution for 10 min to completely remove the follicular layer. For the experiments only the oocytes in stage V and VI were selected and used during 1 to 5 d. The membrane was depolarized from a holding potential of -80 mV to +60 mV in 10 mV step. It was found that a sustained outward current was elicited by depolarization. Potassium channel blockers (tetraethylammonium chloride, TEA, 10 mmol/L and 4-aminopyridine, 4-AP, 10 mmol/L) reduced the outward current to (23.4+/-0.72)% of the maximum. However, further addition of chloride channel blocker (5-nitro-2, 3-phenypropylamino benzoate, NPPB, 30 micromol/L) could almost completely block the outward current to (2.1+/-0.08)% of the maximum. In the presence of TEA and 4-AP, removal of extracellular Ca(2+) or adding verapamil (40 micromol/L), could also reduce the outward current to (2.2+/-0.04) % and (3.1+/-0.15) % of the maximum, respectively. It is concluded that calcium-dependent chloride channels exist in plasma membrane of Bufo bufo gargarizans oocytes, besides potassium channels.

10.Distribution laws of Chinese medical syndrome types and analyses of risk factors in senile hypertension patients: a clinical study.

OBJECTIVETo explore the distribution laws of TCM syndrome types and to analyze the distribution of dynamic blood pressure curve, atherosclerosis, and age in senile hypertension patients.
METHODSTotally 1 131 senile hypertension patients were recruited from 7 provinces and municipal cities. Features of TCM syndromes, classification and distribution curves, and syndrome distribution laws were observed. The distribution curves of dynamic blood pressure, carotid atherosclerosis, and age were compared in each TCM syndrome types.
RESULTSThere were four main syndrome types in 736 cases (56.15%), i.e., excessive accumulation of phlegm-dampness syndrome (210 cases, 16.02%), yin deficiency and hyperactivity of yang syndrome (177 cases, 13.50%), Gan-Shen yin deficiency syndrome (79 cases, 6.03%), and deficiency of qi and yin syndrome (252 cases, 19.22%). Besides, there were two more sub-types, i.e., collateral obstruction by blood stasis syndrome and collateral obstruction by phlegm and stasis. Circadian blood pressure monitor was completed in 211 cases. Of them, abnormal circadian blood pressure occurred in 152 cases (accounting for 72. 38%); yin deficiency and hyperactivity of yang syndrome, excessive accumulation of phlegm-dampness syndrome, deficiency of qi and yin syndrome plus collateral obstruction by blood stasis syndrome were most often seen. Color ultrasound of carotid artery was performed in 660 patients of main syndromes. The incidence was quite higher in those of excessive accumulation of phlegm-dampness syndrome (182 cases, 27. 58%), deficiency of qi and yin syndrome plus collateral obstruction by blood stasis syndrome or collateral obstruction by phlegm and stasis (322 cases, 48.79%). Yin deficiency and hyperactivity of yang syndrome was dominant in patients 60 -79 years old, while deficiency of qi and yin syndrome and Gan-Shen yin deficiency syndrome were dominant in patients older than 80 years.
CONCLUSIONSExcessive accumulation of phlegm-dampness syndrome, yin deficiency and hyperactivity of yang syndrome, Gan-Shen yin deficiency syndrome, and deficiency of qi and yin syndrome were main syndrome types in senile hypertension patients. There was statistical difference in the distribution curves of blood pressure, atherosclerosis, and age of various TCM syndrome types.
