2.Effect of Atorvastatin on Inflammation Markers and Intima-media Thickness of the Common Carotid Artery in Patient with Hyperlipidemic and Atherosclerosis

Objective To investigate the effects of atorvastutin on the levels of plasm imflammation markers (hs - CRP, MMP - 3, TNF - α) and intima - media thickness(ITM) of the common carotid artery in patient with hyperlipidemic and atherosclerosis. Methods Fifty patient with hyperlipidemic and atherosclerosis were treated with atorvastatin (10mg qd) for 12 weeks. The levels of TC, TG, LDL -C, HDL -C, hs -CRP, TNF -α and MMP -3 were detected with radioimmmuneassay and eolorimetric methods. Color Doppler echo-cardiography was used to measure intima -media thickness(ITM) of the common carotid artery after 6 weeks and 12 weeks. Results TC, TG, LDL -C, hs -CRP, TNF -α and MMP -3 were decreased significantly(P < 0.01)after 12 weeks. IMT was decreased signifi-cantly(P < 0.05) and HDL - C was increased significantly (P < 0.05) after 12 weeks. Conclusion Atorvastatin could be effective in re-ducing blood - fat and anti - inflammatory action , regulating blood vessel endothelium (BVE) function and reversing structural chan-ges in IMT of the common carotid artery.
3.Follow-up and retreatment strategies in the anti-vascular endothelial growth factor-A therapy for neovascularization of age-related macular degeneration

Wet age-related macular degeneration (AMD) is one of major causes of blindness in elder people.Intraocular injection of anti-vascular endothelial growth factor (VEGF) -A regimen has made big breakthrough for the treatment on choroidal neovascularization of wet AM D,while long-term follow-up and necessary retreatments are the key issues to remain obtained visual acuity.Multiple strategies of wet AMD have been used in following-up and retreating based on the visual acuity,optical coherence tomography (OCT),ophthalmoscope and fluorescein fundus angiography (FFA) in abroad.However,there also are major differences in the patient' s composition,treatment habits and distribution of medical sources in China from Western.So we suggest to standardize the follow-up and retreatment strategies about intravitreal injection of VEGF-A for wet AMD as to achieve a better effectiveness.OCT-guided individual follow-up and retreatment strategies should be very helpful for maintaining a long-term efficacy,minimizing the treatment time and reducing medical cost.
4.A prospective, randomized, controlled study on anterior cruciate ligament reconstruction with or without remnant preservation

Objective To compare the clinical outcomes of patients who underwent remnant-preserved anterior cruciate ligament (ACL) reconstruction with those who underwent conventional ACL reconstruction (ACLR).Methods From January 2010 to October 2012,93 eligible patients suffered from ACL injuries were randomly allocated into remnant-preserved group (ACLR with remnant preservation;n=48;Male/Female=34/14;Left/Right=25/23) and conventional group (ACLR without remnant preservation;n=45;Male/Female=33/12;Left/Right=22/23).The mean age of patients,time from injury to surgery,rates of combined meniscal injuries,and medial collateral ligament injury all showed no significant difference between the two groups preoperatively.All the patients underwent ACL reconstruction using 4-strand hamstring autografts.The postoperative assessments included knee functional scales (IKDC grade classification and Lysholm score),stability examinations (Lachman,pivot-shift,and KT-1000 arthometer),proprioceptive evaluations,and second-look arthroscopy.Results The average follow-up time was 25.4 months for the remnant-preserved group and 25.2 months for the conventional group.For IKDC grade classification,there were 32 patients with grade A,9 with grade B,1 with grade C in remnant-preserved group,whereas 30 with grade A,8 with grade B,2 with grade C in conventional group,which showed no significant difference between the two groups.Moreover,the Lysholm score (95.9±5.2 vs.95.4±1.7),Lachman test,pivot-shift test,KT-1000 arthometer (1.1±1.2 mm vs.1.2±0.9 mm),proprioceptive evaluations (joint position sense:3.6°± 1.8° vs.3.9°±2.2°) all showed no significant differences between the two groups.Additionally,the synovial coverage of grafted tendon under second-look arthroscopy was categorized as grade A in 11,grade B in 6,grade C in 2,grade D in 2 in remnant-preserved group,whereas grade A in 10,grade B in 5,grade C in 2,and grade D in 2 in conventional group,which still showed no significant difference.Conclusion In terms of the knee functional scales,stability examinations,joint position sense and graft synovial coverage,remnant-preserved ACLR group showed no superiority to the conventional ACLR group.
5.Eight Cases of Caroli′s Disease in Children

Objective To explore the clinical characteristics and treatment of Caroli′s disease in children.Methods The clinical data,laboratory examination and radiological feature of 8 children with Caroli′s disease between Feb.1998 and Dec.2007 were analyzed retrospectively.All children underwent CT and abdominal ultrasonogram.Results Five cases of the 8 children were male and 3 cases were female.The mean age was 6.3 years old.The cases′ history were from 5 days to 4 months.The clinical symptoms showed that 3 cases had hematemesis,5 cases had hepatosplenomegaly,and 1 case had fever and turbid urine.Of the total 8 cases,5 cases were hepatic fibrosis and liver cirrhosis and hepatosplenomegaly,3 cases were portal hypertension,and 1 case had cholangitis.The other 3 cases were simple types.One case had infantile polycystic kidney disease.Laboratory analysis revealed 2 cases had dysfunction of liver and 1 dysfunction of renal.The imaging characteristics showed multiplied irregular dilatation of the intrahepatic bile ducts in enlarged liver,with central dot sign on CT scan.One case presented enlarged gastroesophageal vein.The 8 cases undertook conservative treatment,with no surgery.Conclusions The symptoms of Caroli′s disease are highly variable.Caroli′s disease should be focused especially on children with abdominal pain and hepatomegaly.CT is important for diagnosis of Caroli′s disease at earlier stage.The disease can be conservatively treated,and(or) surgically operated.
6.Pharmacokinetics of salvianolic acid A after single intravenous administration in Rhesus monkey.

Salvianolic acid A (Sal A) is one of the most effective compounds isolated from the root of Salvia miltiorrhiza. Up to now, several studies regarding the pharmacokinetic profiles of Sal A have been reported, however there is no such study reported in monkeys, the species which is more similar to human. The aim of this study is to develop a LC-MS method for the determination of Sal A in monkey plasma and apply it to the pharmacokinetic studies of monkeys. After single intravenous administration of Sal A, the plasma concentration-time curves were observed and the main pharmacokinetic parameters were calculated. The plasma concentration at 2 min (C2 (min)) values were (28.343 ± 6.426), (45.679 ± 12.301) and (113.293 ± 24.360) mg x L(-1) for Rhesus monkeys treated with Sal A at 2.5, 5 and 10 mg x kg(-1). The area under the concentration-time curve (AUC(0-∞)) values were (3.316 ± 0.871), (5.754 ± 2.150) and (13.761 ± 2.825) μg x L(-1) x h, respectively. Furthermore, this method was improved and applied to the simultaneous determination of Sal A, Sal B and Sal C, which provided useful information for preclinical studies and clinical trials of Sal A, Sal B and Sal C.

8.Relationship between skin prick test reactivity to dermatophagoides farinae and autologous serum reactivity in patients with chronic urticaria

Objective To investigate the role of and relationship between reactivity to autologous serum and dermatophagoides farinae (Df) in the pathogenesis of chronic urticaria (CU). Methods Autologous serum skin test (ASST) and skin prick test (SPT) to Df were carried out in 831 patients with CU. The correlation between reactivity to autologous serum and Df was statistically analyzed. Results The positivity rate of ASST and SPT to Df was 51.74% and 64.62%, respectively in the 831 patients. SPT was positive in 56.52% of patients with positive ASST and in 73.86% of those with negative ASST (P < 0.05). In patients with positive ASST, the degree of autologous serum reactivity was negatively correlated with that of reactivity to Df (P < 0.05). Conclusions The skin reactivity to Df and autologous serum plays an important role in the pathogenesis of CU, and the degree of the reactivity to Df and autologous serum is negatively correlated. To conduct ASST and SPT simultaneously in patients with CU may favor the clinical classification and therapy of CU.
9.Optimum harvest study of Gentiana crassicaulis in Ludian.

The paper is aimed to study the difference in yield and quality at different harvest time and determine the optimum harvest of planting Gentiana in Ludian traditional harvest period. The authors analyzed the variation in fresh weight, dry weight, dry discount rate, length, diameter, volume and the content of gentiopicroside, loganin acid, alcohol-soluble extract and total ash and made a comprehensive appraisal of yield, appearance quality and intrinsic quality by gray relational distance ideal Comprehensive Evaluation method. The results showed that there is a big difference in yield and quality both 2-year-old and 3-year-old Gentiana harvested in traditional harvest period and the comprehensive evaluation more better when harvested more later. It can be seen, Gentiana harvested the later had a better yield and quality in Ludian traditional harvest period. The harvest of Gentiana can be appropriate delayed depending on the particular circumstances of production.

10.Study on factors affecting the gastric emptying in patients with type 2 diabetes mellitus

Objective · To investigate the possible factors affecting the gastric emptying rate in patients with type 2 diabetes mellitus. Methods · 94 type 2 diabetic patients treated at the Department of Endocrinology of Renji Hospital affiliated toShanghai Jiao Tong University School of Medicine from September 2013 to March 2014 were enrolled. The half gastric emptying time (T1/2) and retention at 120 min (R120 min) were measured.Results · Female patients had longer T1/2 (P=0.000) and higher R120 min (P=0.000) than male patients. The differences in age, duration of iabetes,BMI, blood glucose level, chronic diabetic complications, and gastrointestinal symptoms between normal and delayed gastric emptying groups were not statistically significant. Acarbose had no effect on the gastric emptying rate in type 2 diabeticpatients. Conclusion · The gastric emptying rate in type 2 diabetic patients is associated with the gender and has no significant correlation with blood glucose level, chronic diabetic complications, gastrointestinal symptoms, and acarbose.
