2.Differential diagnostic value of the detection of calcitonin for bacterial and viral infections

Objective To observe the clinical effect of calcitonin(PCT) in the identification of bacterial infection and viral infection.Methods 150 cases were randomly selected,including 50 cases of viral infection,100 cases of bacterial infection.The people with physical examination at the same period in the hospital whose peripheral blood culture were negative were selected as control group.PCT concentrations of various bacterial infections were observed,as well as the concentrations of PCT in various viral infections,and compared with the control group.Results The PCT level in the bacterial infection group was (8.023±3.542)ng/mL, which was significantly higher than (0.031±0.011)ng/mL in the control group(t=15.960,P=0.000).There was no significant difference in the level of PCT between the viral infection group [(0.032±0.008)ng/mL] and the control group [(0.031±0.011) ng/mL] (t=1.903,P=0.059).In different bacterial infections,the PCT level in sepsis was the highest [(17.544±4.511)ng/mL],followed by bacterial peritonitis and infectious pancreatitis[(5.131±1.231)ng/mL,(4.941±1.665)ng/mL].The PCT level of cellulitis was the lowest [(27.544±4.511)ng/mL].Of a variety of viral infections,the patients with EB virus infection had slightly higher PCT level[(0.027±0.002)ng/mL],which of measles virus infection was slightly lower[(0.021±0.002)ng/mL],the overall PCT level had no significant difference.Conclusion It is important to identify the bacterial infection and viral infection, which can provide a reliable basis for clinical treatment.

4.Antioxidative and antinitrosative effects of Copperleaf on ulcerative colitis rats

AIM: To investigate the antioxidative and antinitrosative effects of copperleaf(Acalypha australis L) on TNBS-induced ulcerative colitis rats. METHODS: Using biochemical method,the activity of antioxidative enzyme SOD,GSH-Px and MDA were determined in normal,model and three rat's groups with low,moderate and high copperleaf decoction respectively.At the same time,the production of NO and the activity of iNOS were also measured.The expression of iNOS mRNA was also determined in normal,model and moderate-copperleaf decoction rats groups through RT-PCR method. RESULTS: The index SOD and GSH-Px increased and MDA reduced significantly in high-and moderate-copperleaf decoction groups compared with the model group.The production of NO the activity of iNOS reduced significantly in high-and moderate-copperleaf decoction groups compared with the model group.RT-PCR results demonstrated that the expression of iNOS mRNA was significantly inhibited in TNBS-induced ulcerative colitis rats after being treated with moderate-dosage copperleaf. CONCLUSION: Acalypha australis L.has antioxidative and antinitrosative effects which is probably one of the mechanism of copperleaf for treating UC.
5.Analysis of Pathogens and Drug Resistance in 380 Children with Vulvovaginitis

OBJECTIVE:To explore the characteristics of pathogens distribution of pediatric vulvovaginitis and drug resis-tance,and to provide reference for clinical drug use. METHODS:409 strains of positive routine bacteria and fungus culture were collected from 380 children with vulvovaginitis in 2014. The pathogens culture and drug sensitivity were recorded. RESULTS:The top 5 bacteria were Escherichia coli [36.67%(150/409)],Haemophilus influenzae [20.29%(83/409)],Streptococcus hemolyticus group A [13.20%(54/409)],Klebsiella pneumonia [8.80%(36/409)] and Staphylococcus aureus [5.37%(22/409)]. CONCLU-SIONS:The main pathogenic bacteria of pediatric vulvovaginitis is Gram negative bacilli,so that drug resistance should be paid at-tention to improve therapeutic efficacy when antibiotics are used in the clinic. The result of sensitivity test should be taken as basis for rational application of antibiotics in the treatment.
6.Diagnostic value of HbA1 c combined with u -ALB in early type 2 diabetes

Objective To investigate the value of glycated hemoglobin(HbA1c)and urine trace albumin (u -ALB)content in early diagnosis of type 2 diabetic nephropathy.Methods 200 patients with type 2 diabetes were selected as diabetes group,and 30 cases of healthy people as control group.According to the content of HbA1c, the diabetes patients were re -divided into low group (HbA1c <7%),the median group(7.1%≤HbA1c≤10%) and high value group (HbA1c >10.1%).The levels of HbA1c and u -ALB were detected and the correlation between them was calculated.Results The values of HbA1c (8.85 ±1.22)% and u -ALB (88.3 ±12.4)mg/L in the diabetes group were significantly higher than those of the control group (t =10.88,54.25,all P <0.05);The level of HbA1c in the high,medium and low group[(11.02 ±1.37)%,(8.45 ±2.01)%,(6.88 ±1.23)%]were consistent with the levels of the respective levels of fasting glucose[(13.22 ±2.05)mmol/L,(9.25 ±1.28)mmol/L, (6.27 ±0.63)mmol/L].HbA1c and constituting the u -ALB levels were positively correlated in the high,medium and low group(r =0.452,0.512,0.452,all P <0.05).Conclusion The combined detection of HbA1c and u -ALB levels has important value for early diagnosis of type 2 diabetic nephropathy.
7.Clinical Observation of Butylphthalide in the Treatment of Alzheimer’s Disease

OBJECTIVE:To observe clinical efficacy and safety of butylphthalide in the treatment of Alzheimer’s disease (AD). METHODS:94 AD patients selected from our hospital during Jan. 2013-Jun. 2014 were divided into control group(45 cas-es)and observation group(49 cases). Control group was given routine treatment as nourishing nerve,lowering blood lipid,taking Memantine hydrochloride tablet orally 5 mg,bid. Observation group was additionally given Butylphthalide soft capsule orally 0.2 g,tid,on the basis of control group. Both groups were treated for consecutive 1 year. Clinical efficacy of 2 groups were observed, and MMSE,ADL,the levels of TNF-α,IL-1,CRP and BDNF were observed before and after treatment;the incidence of ADR was compared. RESULTS:The total effective rate of observation group was 85.71%,which was significantly higher than that of control group(66.67%),with statistical significance(P<0.05). There was no statistical significance in MMSE and ADL between 2 groups before treatment (P>0.05);those of 2 groups increased significantly after treatment,the observation group was higher than the control group,with statistical significance (P<0.05). There was no statistical significance in the levels of TNF-α,IL-1, CRP and BDNF between 2 groups before treatment(P>0.05). Above indexes of 2 groups were improved significantly after treat-ment,the observation group was better than the control group,with statistical significance(P<0.05). There was no statistical dif-ference in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Butylphthalide is effective in the treatment of AD,can improve cognitive and life skills and reduce inflammatory level with good safety.
8.Neoglycoprotein preparation of YCP, a polysaccharide from Phoma herbarum YS4108 and immunogenicity evaluation in mice

Aim:To characterize the immune response to YCP, a polysaccharide (PS) isolated from marine filamentous fungus Phoma herbarum YS4108. Methods:YCP was coupled to bovine serum albumin (BSA) to construct three neoglycoproteins which are different in their degrees of substitution (DSs; ≈ 3, ≈ 6, or ≈ 10 mol of BSA carrier/mol of YCP hapten, respectively), and their immunogenicities were evaluated in mice following subcutaneous immunization,respectively. IgG subclasses against the PS and the carrier protein were measured in addition to the total IgG and IgM antibodies for YCP induced by the neoglycoproteins. Results: While unconjugated PS was weakly immunogenic, theneoglycoprotein induced vigorous primary IgM and booster IgG responses to PS and the carrier protein. Interestingly,the IgG subclass distribution was different between PS and the carrier protein; for PS, the IgG response was predominant of IgG2a subclass with approximately low levels of IgG2b and IgG1, while the response to the carrier protein was mainly of the IgG1 subclass with relatively low levels of IgG2a and IgG2b, and the lowest of IgG3. Conclusion:YCP has the potential to elicit preferentially IgG2a as the dominant isotype antibody in mice.
9.Comparison of set-up errors detected by EPID and CBCT using two different immobilization ;techniques for patients with nasopharygeal carcinoma

Background and purpose: With the development of therapy equipments and technology, the treatment for nasopharyngeal carcinoma(NPC) has entered into the era of precision radiotherapy, and setup errors have become a very important factor affecting treatment effects. The purpose of this study was to analyze the set-up errors detected by the kilovoltage cone beam CT(EPID) and the megavoltage electronic portal imaging device(CBCT) using 2 kinds of different immobilization techniques (pillow+head neck shoulder mask and vacuum bag+head neck shoulder mask) for NPC patients. Methods:A total number of 40 NPC patients were randomly assigned into 2 groups (pillow+head neck shoulder mask group and vacuum bag+neck shoulder mask group). Then each group was further divided into CBCT scan group and EPID group for veriifcation before treatment delivery. We matched the EPID images with the DRRs and acquired the set-up errors in x, y, z axis. Setup errors of CBCT were calculated according to its matched and planned CT images in left-right (x), superior-inferior (y) and anterior-posterior (z) directions. Paired t-test was used to evaluate the differences. Results:In the pillow+head neck shoulder mask group, the set-up errors of CBCT in the x, y, z axis were x (0.67±2.01)mm, y (0.51±1.71)mm and z (0.57±2.04)mm, respectively. The errors of EPID were x (0.69±2.19)mm, y (0.54±2.03)mm and z (0.61±2.11)mm. In the vacuum bag+head neck shoulder mask group, the set-up errors of CBCT in the x, y, z axis were x (0.42±1.81)mm, y (0.33±1.55)mm and z (0.50±1.75)mm, respectively. The errors of EPID were x (0.44±1.87)mm, y (0.43±1.70)mm and z (0.54±1.77)mm. The vacuum bag+head neck shoulder mask ifxed technique was more accurate when compared to the pillow + head neck shoulder mask ifxation method (P<0.05). Conclusion:CBCT and EPID were similar in detecting set-up errors for the NPC patients. However, the vacuum bag+neck shoulder mask ifxed technique was more accurate when compared to the pillow+head neck shoulder mask ifxation method.
10.Silencing itch by small speciifc interfering RNA enhance immune activity of mouse T lymphocyte to kill MFC stomach neoplasms cells in vitro

Background and purpose: Itch protein is an established regulator of T cell immune response thresholds, belong to a class of E3 ubiquitin-transferring enzymes, widely involve in the ubiquitination of several key signaling molecules, such as ZAP70, P85, VAV, PLC-γ, PKC-θ, etc, plays a critical role in tumor induced immu-nosuppression. Itch ligase activity regulate T-cell anergy and development of regulatory T cells in the periphery by modulating key components of T-cell receptor and transforming growth factor-βsignaling. Therefore, manipulation of Itch activities may provide the opportunities to develop future therapies for immune disorders such as autoimmunity and cancer. speciifc small interfering RNA(siRNA) was utilized to silence the expression of Itch gene of T-lymphocytes and investigate the cytotoxicity activity of transfected T lymphocytes against MFC stomach neoplasms cells in vitro. Methods:T lymphocytes were isolated from the spleen of 615 mice and transfected by speciifc siRNA to silence the expression of Itch gene, The expression of Itch protein were examined by Western bolt in each group;72 hours after transfection, The secretion level of IL-2, INF-γwere measured by enzyme-linked immunosorbent assay (ELISA). At the end, the cytotoxicity activity changes against MFC stomach neoplasms cells was compared between transfected T lym-phocytes, negative control and blank control in vitro. Results:Compared with control group, the expression rate of Itch protein of transfected T-lymphocytes was decreased to 16%after transfection 48 hours;72 hours after transfection, the secretion level of IL-2 in transfection group, negative control and blank control respectively were (1 891.96±141.91)pg/mL, (1 241.69±91.67)pg/mL and (1 175.03±89.14)pg/mL (P<0.001), the secretion level of INF-γin transfection group, negative control and blank control respectively were (958.33±75.46)pg/mL, (683.33±66.67)pg/mL and (691.72±68.72) pg/mL (P<0.05). Transfected T lymphocyte also showed more efifcient killing ability against MFC stomach neoplasms cells than negative control and blank control in vitro, the highest killing rate has reached (54.18±2.96)%. Conclusion:Silencing Itch gene can signiifcantly promoted the secretion level of IL-2, INF-γof mice T lymphocyte, enhanced the cytotoxicity activity of T lymphocyte against MFC stomach neoplasms cells in vitro.
