1.The prevalence and metabolic risk factors of nonalcoholic fatty liver disease among middle-aged and aged people

Objective To investigate the prevalence of nonalcoholic fatty liver disease in a population of routine check-up,and analyze the correlation between non-alcoholic fatty liver disease and metabolic risk factors.Methods Physical examination data of 2 064 routine check-up (aged 45 or more) in the Third People's Hospital of Chengdu from January 2014 to December 2015 were randomly selected.HbA1c,fasting blood glucose,blood lipids and other indicators were measured to calculate the prevalence of NAFLD,and to analyze the correlation between NAFLD and metabolic risk factors.Results Among 2 064 cases underwent physical examination,the overall prevalence of NAFLD was 16.09% (332/2 064).The male prevalence rate of NAFLD was significantly higher than that in female (18.51% vs.12.87%;x2 =11.913,P =0.001).In four age groups (< 50 years old,50 to 59 years old,60 to 69 years old,≥ 70 years old),the NAFLD prevalence rate was 16.26%,15.63%,16.97% and 14.72% respectively.No statistical significance (x2 =0.900,P =0.826) was found in NAFLD prevalence rate difference in all age groups of has.In 50 ~ 59 years old and ≥70 years old group,the NAFLD prevalence rates in male were higher than that in female,the difference was statistically significant (both P < 0.05).BMI,waist circumference,systolic blood pressure,diastolic blood pressure (DBP),HbA1c,fasting blood glucose,TC,TG,LDL-C,uric acid levels in NAFLD patients were higher than the normal population.HDL-C levels in patients with NAFLD is lower than that of the normal,and the difference is statistically significant (both P < 0.05).Logistic regression analysis showed that BMI,hypertension,diabetes,and high TG were the independent risk factors of NAFLD;HDL-C and NAFLD were the protective factors.Conclusions In the Chengdu area of Sichuan Province,the prevalence rate of NAFLD was high,the incidence rate of NAFLD in male was higher than that in female.BMI,hypertension,diabetes mellitus and high TG were the independent risk factors of NAFLD,and HDL-C was the protective factor.
2.Effects of halofuginone on maturation of dendritic cells and immune tolerance during homograft heart transplantation of rats

Objective To explore the role of halofuginone in immune regulation,observe the effects of halofuginone on maturation of dendritic cells and immune tolerance during homograft heart transplantation of rats,in order to provide new ways and means to treat transplantation tolerance and to explore its possible mechanism.Methods Rat bone marrow derived dendritic cells were isolated and the effects of halofuginone in LPS-treated DCs were investigated by flow cytometry;heart transplantation model of rats were established and the effect of halofuginone stimulated dendritic cells in the immune tolerance by pathological analysis and mixed lymphocyte reaction were observed.Results Flow cytometry results show that the expression of cell surface CD80 CD86,OX62 antigens were not high in control group,their expression increased significantly when stimulated by LPS for 24h,and decreased when stimulated by both Changshan ketone and LPS for 24h,and even lower than their expression levels in blank group,which showed that Changshan ketone has the ability to inhibit the maturation of dendritic cells induced by LPS.In rat heart transplantation model,dendritic cells stimulated with Changshan were injected into recipient rats in vivo,and receptor of rat transplantation rejection decreased significantly,which showed that dendritic cells stimulated with Changshan ketone may be used as a new method for cell therapy in treatment of transplant rejection.Conclusion This study systematically observes that halofuginone can inhibit the maturation of rat bone marrow-derived dendritic cells and lymphocyte proliferation for the first time in vivo and in vitro.
3.Dental maturation assessment of Nanjing children aged 3 -16 years

[Abstract ]Objective:To estimate dental maturation norms of permanent dentition in a group of Nanjing children using Nolla's tech-nique.Methods:549 cases of panoramic radiographs (279 males and 270 females)of Nanjing children at the age range of 3 to 16 years were collected for the study.All permanent mandibular teeth on the left side (except for the third molars)were scored according to Nolla's method,and data were presented in tables and used to plot dental maturation curves for the Nanjing boys and girls.Results:Females were found to be more advanced in the degrees of dental development.Dental maturation is significantly associated with chron-ological age (for males and females:r =0.959,P <0.001 and r =0.953,P <0.001,respectively).The mathematical model of them was established,and then the fitting equation was obtained.Conclusion:The dental maturation curve and chronology of permanent dentition can provide the growth norms of permanent teeth for Nanjing children and facilitate the way for dentists to assess the growing children during diagnosis and treatment planning.
4.Three-dimensional conformal radiotherapy (3DCRT) in treating patients with recurrent nasopharyngeal carcinoma:planning optimization and dosimetry analysis

Purpose:To investigate the possibilities of three-dimensional conformal radiotherapy (3DCRT) in treating the patients with recurrent nasopharyngeal carcinoma;and planning optimization,dosimetry analysis for recurrent disease in different sites.Methods:Three patients with recurrent disease in posterior area of styloid process、cavernous sinus and with maximum volume were analysed. Comparison was made for optimized 3DCRT treatment planning with different number of beams and angles of incidences.Results:It is possible to significantly improve the dose coverage to target volume and the dose to critical organ was under the tolerance dose.Conclusions:3DCRT may be a suitable alternative to conventional therapy in patients with recurrent nasopharyngeal carcinoma.
5.Application of problem-based learning in the experiment teaching of forensic toxicology analysis

Students(64)of two years in our department were divided into two groups in experimental teaching of forensic toxicology analysis:in one group,the methods of LBL was performed,while the methods of PBL was performed in the other group. By comparing effects between these two methods,we analyzed the function methods of PBL to improve students'experimental skills and capacity of analyzing and solving problems. It was concluded that the PBL methods was more suitable for the experiment teaching of forensic toxicology analysis than the LBL methods.

OBJECTIVE:To study the therapeutic effect and safety of domestic tolterodine tartrate in treating patients with urinary bladder overactivity.METHODS:56 cases of bladder overactivity were divided into two groups randomly:tolterodine and control(oxybutynin)group.The course of treatment was 6 weeks.RESULTS:The effect of tolterodine in treatment group was comparable to that of oxybutynin in control group,however,the adverse reactions in oxybutynin group were more common than those in tolterodine group.CONCLUSION:Tolterodine is a suitable drug to treat bladder overactivity.
7.The therapeutic effects of intermittent hypoxia therapy in isolated systolic hypertension patients

Objective To study the therapeutic effects of intermittent hypoxia therapy(IHT)in isolated systolic hypertension(ISH)patients with elevated cerebral blood flow velocity(Vp),and to explore the mechanisms involved.Methods Seventy-six ISH patients with increasing Vp and normal pulsatility index(PI)of the middle ce- rebral artery(MCA)were randomly divided into a therapy group and a control group.IHT was administrated in the therapy group,and air in the control group.The Vp and PI of the MCA and blood pressure(BP)were observed be- fore and after treatment.Results Vp and systolic blood pressure(SBP)were significantly reduced after IHT(P<0.01)compared with the therapy group's scores betore treatment,but PI and diastolic blood pressure showed no sig- nificant difference.There was no significant change in BP,Vp or PI in the control group before or after treatment. Conclusion IHT has therapeutic effects on ISH by reducing Vp and moderating SBP.
8.Influence of Inhaled Ambroxol Hydrochloride and Budesonide Suspension on Partial Pressure of Oxygen and Partial Pressure of Carbon Dioxide of Premature

Objective To explore the effect of inhaled ambroxol hydrochloride(mucosolvan) and budesonide suspension(pulmicort respules) on the prevention and treatment of neonatal respiratory distress syndrome(NRDS).Methods Ninty-two preterm infants without asphyxia and mother suffered from illness,were selected from neonatal intensive care unit(NICU) in Jiangyin hospital from Oct.2003 to Mar.2006.Their gestational age ranged from 29 to 33 weeks and they were all cured within 2 hours after birth.They were divided randomly into treatment group(group A) and control group(group B) at equal number.All of them were given oxygen treatment,aminophylline,VitK1,nutrition support treatment and kept in infant incubator.Every 8 hours,inhaled ambroxol hydrochloride(10 mg/kg) and budesonide suspension(0.25 mg) driven by oxygen were given to group A.Meanwhile,only ambroxol hydrochloride(10 mg/kg) were given to group B.After 3 days,the data of p(O2) and p(CO2) of premature were detected at d1,2,3 in 2 groups.Result At d2,d3,the arterial blood P(O2) in group A were(8.01?0.62),(9.25?0.76)kPa respectively;At d2,d3,arterial blood P(O2) value in group B were respectively(7.63?0.59)kPa,(8.75?0.63)kPa.By that statistics handles two set of preemie d2,d3 arterial blood P(O2)value,difference had notable.Group A preemie d2,d3 arterial blood P(CO2) value were respectively:(4.55?0.58),(4.20?0.51)kPa;group B preemie d2,d3 arterial blood P(CO2) value were(4.87?0.67)kPa,(4.44?0.59)kPa respectively.By that statistics handles two set of preemie d2,d3 arterial blood P(CO2) value,difference had notable.Conclusion Inhaled ambroxol hydrochloride and budesonide suspension treatment has be well effect in NRDS.
9.Hemodynamic changes and oxygen metabolism during off-pump coronary artery bypass grafting

Objective To examine the hemodynamic changes and oxygen metabolism during off-pump coronary artery bypass grafting (OPCABG) .Methods Twenty patients (18 male, 2 female) aged (60?8)yr, weighing (71?9)kg undergoing OPCABG were studied. The patients were premedicated with diazepam 10 mg PO 2h before operation and morphine 10 mg and scopolamine 0.3 mg im 30 min before operation. Anesthesia was induced with midazolam 0.1-0.2 mg?kg-1, fentanyl 5-10 ug?kg-1 and vecuronium 0.12-0.15 mg?kg-1 and maintained with 0.5%-2% isoflurane inhalation, fentanyl 10-20 ug?kg-1 and intermittent iv boluses of vecuronium and in some patients propofol infusion. Radial artery was cannulated. Swan-Ganz catheter was placed via internal jugular vein into pulmonary artery. Hemodynamic parameters including MAP, HR, cardiac index (CI), stroke volume (SV), MPAP, PCWP, SVR, left ventricle stroke work index (LVSWI) and RVSWI; oxygen metabolism including oxygen delivery (DO2) , oxygen consumption (VO2) and oxygen extraction ratio (ERO2); regional cerebral oxygen saturation (rSCO2) and arterial blood lactate level (ABL) were measured and recorded before skin incision (A), when pericardium was incised before operation on the heart (B), during operation on the heart (C), 10 min after operation on the heart (D) and at the end of surgery (E) . Results During surgical manipulation on the heart (C) MAP, SV and CI decreased while CVP and HR increased, but the decrease in CI and SV was not significant. During manipulation on the heart(C) DO2 decreased whereas ERO2 increased, but there was no significant change in VO2. After manipulation on the heart (D) DO2 and ERO2 returned to the pre-manipulation level. Arterial blood lactate concentration increased at E. Conclusion OPCABG can be performed uneventfully under our general anesthetic regimen. During operation while the beating heart is being manipulated MAP tends to decrease while CVP and HR increase. The changes are of short duration and do not result in any serious consequeaces.
10.The analysis of progression of ischemic leukoaraiosis and the risk factors in elderly patients

Objective To study the features and risk factors of ischemic leukoaraiosis (LA) progression in elderly patients aged 80 years and over. Methods The 56 elderly patients with ischemic LA confirmed by MRI were enrolled in this study. The characteristics and risk factors of ischemic LA were comparatively analyzed between baseline and 3 years later by single and multi-variable logistic regression analysis. Results Ischemic LA progressed mostly in the subcortical white matter in elderly patients, especially the frontal lobes (progression of white matter lesion was present in 40% in the frontal lobes at baseline and 52% after 3 years), followed by the parietal lobes (35% at baseline and 36% after 3 years); Furthermore, previous high homocysteine (Hcy) and chronic bronchitis were risk factors of ischemic LA progression. Conclusions Ischemic LA progresses mostly in the subcortical white matter in the elderly, especially the frontal lobes; High Hcy and chronic bronchitis are risk factors of ischemic LA progression in elderly patients.
