1.Rationality Analysis of Hydroxyethyl Starch Clinical Application in A Hospital

OBJECTIVE:To provide reference for the clinical rational application and preventing the adverse reactions of hy-droxyethyl starch(HES). METHODS:The HES application of inpatients in a hospital in Jul. 2014 were retrospectively analyzed, the usage and dosage,indications,and the changes of renal functions and blood coagulation function before and after HES treat-ment were detected,and the application rationality was evaluated. RESULTS:The usage of HES in the hospital was hydroxyethyl starch 200/0.5 and Sodium chloride injection,which was the second generation of HES. In the 347 patients,295 patients had moni-tored the renal functions before the HES treatment,the result was normal;and 21 patients had monitored the renal functions after HES treatment,the result was normal. Meanwhile,326 patients had monitored the blood coagulation functions before and after HES treatment,the results were normal. HES was not used for the patient of severe sepsis,renal dysfunctions and coagulopathy in this hospital. CONCLUSIONS:The application of HES can induce renal damage and coagulation disorder,so relevant indicators should be closely monitored before and after treatment to prevent the incidence of adverse reactions;further more,clinicians should strictly handle indications,usage and dosage and precautions during the clinical application;in addition,it is suggested to replace the second generation of HES by the third generation,which is more safer.
2.Ischemia-reperfusion insult induced apoptosis of rats' retinal cells

Objective To investigate the damage to the retinal cells and apoptosis of retinal cells of rats after ischemia-reperfusion insult. Methods The retinal ischemia-reperfusion model was developed by increasing intraocular pressure to 109 725 mm Hg in rat eyes. Morphological changes of the rat eyes were observed by means of routine histopathology with HE staining. Apoptosis of the retina was assayed by both DNA fragmentation gel-electrophoresis and terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin nick end labelling (TUNEL). Results Compared with the normal control, no histopathological changes were revealed in the rat retinas 30 min after the ischemia and then reperfued for 24 h or 48 h. Retinal ganglion cell layer (RGL) and inner plaxiform layer (IPL) of the retina were observed, however, to become significantly thinner 60 min after the ischemia and then reperfued for 24 h or 48 h. Together with the pathological changes DNA ladder pattern was detected in the same group of the rats. Further, immunochemical stain of the eye demonstrated that TUNEL positive cells were localized in RGL and IPL of the retina. Conclusion Ischemia-reperfusion insult of the eye may remarkably damage the retina of the rat eye. The damage to the retinal cells is mainly localized within RGL and IPL and apoptosis is the important mechanism of the retinal disorder.
4.Value of HBP in diagnosis of catheter-related infections in maintenance hemodialysis patients

Objective To investigate the diagnostic value of Heparin-binding protein (HBP) in maintenance hemodialysis patients in catheter-related infections.Methods 75 patients with maintenance hemodialysis were enrolled in the study and divided into observation group(n=45,with catheter-related infections) and control group(n=45,without catheter-related infections).The serum HBP,procalcitonin (PCT) and C-reactive protein (CRP) levels were detected.The values in diagnostic of catheter-related infections and Gram-negative bacterial infections were analyzed by receiver operating characteristic(ROC) curve.Results There were 34 cases of patients infected by Gram-positive bacterial with the rate of 75.6% and 11 cases of patients infected by Gram-negative bacterial with rate of 24.4%.The serum HBP,PCT and CRP levels in observation group were significantly higher than those in control group and the serum HBP,PCT levels of patients infected by Gram-negative bacterial were higher than those of patients infected by Gram-positive bacterial (P<0.05).The area under curves of HBP,PCT,CRP in diagnosis of catheter-related infections were 0.955,0.890,0.871 with sensitivity of 86.7%,88.9%,62.2% and specificity of 93.3%,73.3%,93.3%.The value of HBP in diagnosis of Gram-negative bacteria infections was better than PCT and CRP.The area under ROC was 0.818 and the cut off value was HBP>27.52 μg/L with sensitivity of 100.0% and specificity of 61.8%.Conclusion Serum HBP,PCT and CRP may be the effective indicators for diagnosis of catheter-related infections,and the HBP has important reference value for diagnosis of Gram-negative bacterial infections.
5.Down-regulation of beta-catenin nuclear localization by aspirin correlates with growth inhibition of Jurkat cell line.

In this study, we examined the effects of aspirin on the growth rates, subcellar distribution of beta-catenin protein, the expression of beta-catenin/TCF signaling pathway target gene cyclinD1 mRNA, and cell cycle of Jurkat cell line (Human T-acute lymphoblastic leukemia). Our results showed that the treatment with aspirin inhibited the growth of Jurkat cell line. Jurkat cells treated with 3 mmol/L of aspirin could significantly decrease nuclear localization of beta-catenin, and at 5 mmol/L of aspirin, the nuclear localization of beta-catenin was undetectable. QRT-PCR showed that the target gene cyclinD1 mRNA expression was gradually decreased with the dosage of aspirin. Aspirin induced G0/G1 cell cycle arrest in Jurkat cells. We are led to conclude that aspirin acts through beta-catenin-independent mechanisms. The effects of aspirin include down-regulation of beta-catenin nuclear localization and G0/G1 cell cycle arrest, which might serve as a means of growth inhibition in aspirin-treated human Jurkat cell line.
6.Effect of self -intervention on the self -efficacy and the level of blood lipid in patients with cerebral infarction

Objective To analyze the clinical significance of patients self -intervention on self -efficacy and lipid levels in patients with cerebral infarction.Methods 300 patients with cerebral infarction in our hospital were selected from May 2014 to May 2015,and they were randomly divided into two groups by digital table method:the observation group(n =1 50)and the control group(n =1 50),the two groups of patients had no significant differences in the general data.The observation group treated according to the conventional rehabilitation treatments for rehabilitation nursing,in view of the self -management group.After three months,the two groups of patients'self -efficacy and lipid levels were recorded before and after the intervention and the patients'self management abilities were measured. Comparisons between before and after the intervention of the indicators and between the different groups were analyzed,and the relationship of self -management ability and lipid levels of patients was analyzed by Pearson correlation analysis.Results Before the intervention,there was no difference between the two groups in the self -efficacy scale(P >0.05).After the treatment,the self -efficacy scale of the observation group was (29.85 ±5.34), which of the control group was (22.74 ±4.73),the difference between the two groups was statistically significant(t =1 2.207,P <0.001 ).For the lipid levels after treatment,the TC,TG,LDL -C,HDL -C in the observation group were (4.06 ±0.58)mmol/L,(1 .26 ±0.56)mmol/L,(2.27 ±0.49)mmol/L,(1 .42 ±0.54)mmol/L,which in the control group were (4.86 ±1 .05)mmol/L,(1 .54 ±0.48)mmol/L,(3.1 5 ±0.29)mmol/L,(1 .23 ±0.37)mmol/L, the differences between the two groups were statistically significant (t =8.1 68,4.649,1 8.929,3.555,all P <0.001 ).The correlation analysis of self -management ability and the blood lipid improvement of the two groups showed that the observation group presented positive correlation.The patients'self -management ability was stronger, its lipid improve ability was better.Conclusion From the self -efficacy and the blood lipid level analysis,cerebral infarction patients'self -intervention can promote physical and mental health and improve the symptoms of cerebral infarction.In summary,it is helpful for recovery and has positive clinical effect on cerebral infarction.
7.Repair of segmental bone defects with intramedullary nailing and bone cement

Objective:To investigate the clinical efficacy of intramedullary nailing combined with bone cement in repair of segmental bone defects after tumor resection.Methods:A retrospective analysis was conducted of the 5 patients with malignant bone tumor who had been treated at Department of Orthopaedics, Qiannan People's Hospital from April 2018 to September 2019 for remaining segmental bone defects following limb salvage surgery. They were 4 males and one female, aged from 11 to 55 years (average, 35.4 years). Their defects ranged from 6 to 21 cm (average, 12.3 cm) after tumor resection. By the Karnofsky performance score (KPS) for long-term quality of life, all of them scored less than 50 points. Of them, 3 were treated by interlocking intramedullary nails and bone cement filling, and 2 by elastic intramedullary nails and bone cement filling. In the 2 cases with defects of 21 cm and 13 cm, the fixation was assisted by a plate and an external fixator. Defect length after resection, operation time and intraoperative bleeding were recorded; the efficacy was evaluated by the Enneking functional evaluation of reconstructive procedures after surgical treatment of tumors of the musculoskeletal system, visual analogue scale (VAS), and KPS.Results:All the 5 patients had uneventful surgery, with operation time ranging from 112 to 225 min (average, 154.2 min), intraoperative bleeding from 300 to 500 mL (average, 382 mL), and defect length after resection from 6 to 21 cm. The 5 patients were followed up for 6 to 28 months. Of them, 2 died of disease progression 6 and 7 months after surgery, respectively. According to the Enneking's evaluation, one patient scored 28 points, 2 patients 23 points and 2 patients 21 points, giving a high degree of satisfaction. Their VAS scores 6 months after surgery ranged from 1 to 6, averaging 3.6; their postoperative KPS scores ranged from 60 to 80, averaging 72.Conclusion:In repair of segmental bone defects after tumor resection, intramedullary nailing combined with bone cement filling can relieve pain of patients and lead to satisfactory short-term curative efficacy.
8.Effect of Ornidazol on acute pericoronitis of wisdom tooth

ObjectiveTo observe the effect of Ornidazol on acute pericoronitis of wisdom tooth (PWT).Methods125 patients with PWT were randomly divided into the experiment group (63 cases, treated with Ornidazol) and control group (62 cases, treated with Metronidazole). The curative effect was observed when teeth extracted.ResultsThe effective rate of the experiment group was 95.2%, that of control group was 82.2%. There was a significant difference between two groups (P<0.05).ConclusionOrnidazol has an obvious effect on acute PWT.
9.A survey of dietary iodine intake among residents in Fujian Province

Objective To survey the dietary iodine intake among residents in the coastal province of Fujian after universal salt iodization. Methods Using multi-stage stratified random sampling method, four different geographic areas of coastal cities, inland cities, coastal rural and inland rural areas were selected; 3 counties (cities, districts) were selected from each area; 30 families were selected from each of those counties (cities, districts). Total dietary study method was applied to investigate the basic information, all food and water intake during the investigation, the spice variety, quantity and cooking methods, and recipes. All foods consumed were divided into 13 classes:grains and their products, beans(including nuts) and their products, potato and its products, meat and its products, eggs and their products, aquatic products, milk and dairy products, vegetables, fruits and their products, sugar and sugar products, beverage and water, alcohol and seaweeds. Food samples were collected and iodine contents of different types of food were measured, and daily food consumption of the reference man(adult male) in the four areas was calculated. Then the male adult dietary iodine intake in the four areas and Fujian Province was estimated. Results A total of 69 families(accounting for 19.2% of the 359 families surveyed) ate seaweed foods during the three days survey period. The average salt intakes per reference man were 4.5, 5.8, 7.5, 12.3 g, respectively, in coastal cities, inland cities, coastal rural and inland rural. The adult male dietary iodine intake in Fujian Province was 343.8μg/d, and the values were 234.4, 347.1, 328.4, 465.3μg/d, respectively, in coastal cities, coastal rural, inland cities and inland rural. If people did not eat seaweed foods, the results would be 239.7, 137.5, 253.6, 200.3, 367.4μg/d. The main sources of dietary iodine intake were vegetables (33.2%), seaweeds(30.3%), meat(13.4%) and aquatic products(13.0%). If people did not eat seaweed foods, the results would be vegetables(47.6%), meat(19.1%) and aquatic products(18.7%), and iodine contribution rate of the 3 types of food was 85.4%. Conclusions Under the conditions of eating iodized salt, male adult dietary iodine intake is appropriate in different areas of Fujian Province. There is no such problem as iodine excessive. Iodized salt is the main source of dietary iodine. Residents in coastal city, if not eating seaweed foods, will be at risk of iodine deficiency, even if they eat iodized salt.
10.Effects of BMI-1 over-expression on HOX family expression and cell cycle in HeLa cells

AIM: To observe whether transfection of mammalian expression vector pEGFP containing the gene of B-cell specific moloney leukemia virus insertion site 1(BMI-1) could express in human cervix cancer cell line HeLa, and to detect its effect on HOX family expression and cell cycle.METHODS: pEGFP-BMI-1 was transfected into HeLa cells with Lipofectamine 2000. The expression of pEGFP-BMI-1 was determined by EGFP fluorescence and Western blotting. SYBR green I real-time RT-PCR was used to quantitate mRNA expression of P16~(INK4a), hTERT, HOXA9, HOXB4 and HOXC13. FACS analysis was used to detect the change of cell cycle.RESULTS: In HeLa cells transfected with pEGFP-BMI-1, the results of real-time RT-PCR showed that the mRNA expressions of P16~(INK4a), HOXA9 and HOXC13 were reduced to 9.2%, 10.9% and 69.7%, respectively, as compared to control HeLa cells (P<0.01). However, hTERT and HOXB4 mRNA expressions did not change significantly (P>0.05). FACS analysis showed a decrease from 65.68 % to 50.53% in G_1 population and a significant increase from 27.17% to 39.59 % in S population after transfection (P<0.01).CONCLUSION: BMI-1 over-expression in HeLa cells down-regulates mRNA expressions of P16~(INK4a), HOXA9 and HOXC13, decreases G_1 population and increases S population. Therefore, BMI-1 may be involved in carcinogenesis and cancer development.
