Advanced hospitals and high-level hospitals are different from grass-root hospitals,since they not only treat difficult diseases but also have the obligation of scientific studies and teaching.Grass-root hospitals mainly focus on treating common diseases,and only conduct a small amount of teaching,thus resulting in the difference between grass-root hospitals and large-scale hospitals in the aspect of purchasing medical equipment.Because the money for grass-root hospitals may only be used as designated and some even comes from loan or collecting,we have to pinch pennies and make the best use of everything so as to recover the cost rapidly and bring the equipment into full play.

Growth arrest special 5 (GAS5) is a common long non-coding RNA (lncRNA).Recent studies find that lncRNA GAS5 is down-regulated in various tumors,such as breast cancer,cervical cancer,lung cancer,gastrointestinal cancers and urologic cancers.GAS5 is closely related to the development,metastasis and prognosis of cancers,which becomes a new hotspot in cancer research.
3.Establishment and application evaluation of rapid detection method of Mycoplasma pneuomoniae

Objective To prepare the specific monoclonal antibodies(mAb) of Mycoplasma pneumonia(MP) and to establish a colloidal gold rapid detection method of MP by using mAb .Methods The nasopharyngeal swab samples were collected from chil‐dren patients with acute respiratory tract infection ,separated and cultured and performed the MP identification ,MP antigen was pu‐rified ,mAb of MP was prepared ,then the MP colloidal gold test strip was established by using MP mAb .The throat swab sample was collected from the patients with suspected MP infection and detected by using the fluorescent quantitative PCR and colloidal gold test strip .The detection results were observed and statistically analyzed .Results MP strain was successfully isolated and inac‐tivated ,the MP antigen was purified .The mouse was immunized by using this antigen ,19 strians of mAb against MP were prepared by using the hybridoma technique .Among them ,2 strains of mAb with high titer and good specificity(MP‐5 and MP‐19) were se‐lected as the raw materials for preparing the MP colloidal gold rapid test strip .The lowest detection limit was 20 ng .The clinical samples were detected by using the MP colloidal gold rapid test strip and fluorescent quantitative PCR .The results showed that the sensitivity of MP colloidal gold rapid test strip by using this established method was 88 .2% and its specificity was 82 .6% .Conclu‐sion specific mAbs against MP is prepared and the colloidal gold rapid detection method is preliminarily established which provides the help for rapid diagnosis in the patients with MP infection .

Recently,the relationship between psoriasis and coronary heart disease(CHD)have been casting close attention.It has been demonstrated that the prevalence of CHD in patients with psoriasis is more than one time higher than that of healthy people. Meanwhile,the prevalences of hypertension,diabetes mellitus, hyperlipoidemia and smoking in patients with psoriasis are higher than those of normal people,and some drugs for treatment of psoriasis may induce CHD.Therefore,the relationship between psoriasis and CHD is still unclear.In this paper,two cases of psoriasis complicated with CHD are reported,and the relationship between psoriasis and CHD is explored.
5.Survey of Latrine Improvement in Rural Areas in Qingdao City

Objective To understand the situation of rebuilding sanitary latrine in the rural areas in Qingdao and to evaluate the effect of sanitary latrine improvement, explore the new measure of sanitary latrine improvement in the rural areas. Methods In 2005, the typical method of investigation was used to choose 365 farmers from 8 rebuilding sanitary latrine villages among three counties in Qingdao, and to conduct on-site survey and health questionnaires on the construction of household latrines and excreta treatment, sanitation and related knowledge. Hygienic evaluation was made in accordance with hygienic standard for rural household latrine. Results 100% of the houses had the household latrines, 80.0% were the sanitary latrines, dejecta treatment rate was 78.36%. Conclusion All evaluation indexes reach the high level, the effect of household latrine improvement in the investigated rural areas is satisfactory.
6.Relationship between Survivin protein and esophageal cancer: a systematic review

Objective To investigate Survivin protein and esophageal cancer with a systematic review.Methods The published studies were searched in the Cochrane Library (Issuel,2011),Pubmed and CNKI databases, and other relevant journals were also hand searched to identify all the relevant case-control trials.Then the quality of the included trials was assessed and meta-regression analysis was performed,by STATA 11.0 software.Results A total of 15 studies were recruited.As for the positive rate of Survivin expression, significant differences were tested between esophageal cancer vs.normal esophageal tissues (OR =39.98, 95 % CI =15.08-106.02, P < 0.05).No significant difference was tested between cell differentiation G3 vs.cell differentiation G1-G2(OR =1.47,95 % CI =0.59-3.68,P > 0.05).There were significant differences between T stagesⅢ-Ⅳ vs.T stages Ⅰ-Ⅱ (OR =2.82,95 % CI =1.35-5.90,P < 0.05),clinic stages Ⅲ-Ⅳ vs.clinic stages Ⅰ-Ⅱ (OR =2.99, 95 % CI =2.03-4.41, P < 0.05), lymph node metastasis vs.non-lymph node metastasis (OR =4.11,95 % CI =2.79-6.05,P < 0.05).Moreover,Survivin expression was positively correlated with the expression of hypoxia inducible factor-1α (HIF-1α) (r =0.542,P < 0.005).Conclusion Survivin expression increased malignant behaviors of esophageal cancer.There might be substantial correlations between Survivin and HIF-1α expression and esophageal cancer.
7.Efficacy of I-gel laryngeal mask airway for airway management in patients undergoing radical mastectomy

Objective To evaluate the efficacy of I-gel laryngeal mask airway (LMA) for airway management in patients undergoing radical mastectomy.Methods One hundred and twenty ASA Ⅰ or Ⅱ patients (Mallampati Ⅰ or Ⅱ),aged 25-64 yr,weighing 45-90 kg,with body mass index < 30 kg/m2,scheduled for elective radical mastectomy under general anesthesia,were randomized into 2 groups (n=60 each): I-gel LMA group (group Ⅰ) and Classic LMA (group C).Anesthesia was induced with iv midazolam 0.04 mg/kg and vecuronium 0.10 mg/kg.I-gel and classic LMAs were inserted in I and C groups,respectively,after induction of anesthesia.The success rate of LMA placement at first attempt,LMA placement time,airway sealing pressure,peak airway pressure,leaks and hypoxemia,and complications (nauseas and vomiting,bucking,aspiration and blood stain on the LMAs,sore throat and hoarseness within 24 h after surgery) were recorded.Correct position of the LMAs was verified by fiberoptic bronchoscopy.Results The success rate of LMA placement were 100 % in both groups.The success rate of LMA placement at first attempt and fiberoptic bronchoscope scores were higher,the LMA placement time was significantly shorter,and the incidence of blood stain on the LMAs and sore throat were significantly lower in group Ⅰ than in group C (P < 0.05).A transient increase in airway pressure and leaks occurred in 5 patients in group C,and the airway pressure returned to normal and no leaks developed after treatment.No transient increase in airway pressure and leaks occurred in group Ⅰ.Hypoxemia was not found in both groups.Conclusion The placement of I-gel LMA is easier than that of Classic LMA and the success rate of placement at first attempt is high,with fewer complications.I-gel LMA can be safely and effectively used for airway management in patients undergoing radical mastectomy.
8.Long term outcomes of percutaneous renal interventional therapy in patients with severe renal artery stenosis

Objective To determine the long-term outcomes of renal interventional therapy in patients with renal artery stenosis. Methods One hundred thirty-four consecutive patients reveived renal stenting in our hospital. The changes of BP and serum creatitine were measured 24 hours after the procedure and during the first and second year of follow-up. Results All procedures were performed successfully. The mean serum creatitine level increased [(109.8?24.6) ?mol/L vs (99.4?27.8 baseline level) ?mol/L] and the glomerular filtration rate decreased [(57.6?19.3)mL/min vs (68.5?18.9) mL/min] significantly after the procedure. However during the first and second year of follow-up, the serum creatitine level returned to the pre-procedure level. Sixty-four patients attained benefit of BP improvement at about 6 months after renal artery stenting. The mean systolic pressure 6 months after the procedure was (148.6?22.6) mm?Hg (1 mm?Hg=0.133 kPa), which was significantly lower than the baseline level. During the first and second year of follow-up, there were 56 patients (50.6%) and 50 patients (49.6%) achieved BP improvement from renal stenting respectively. Conclusion Renal stenting is efficient and has satisfactory long-term outcomes in treating renal artery stenosis.
9.Postoperative quality of life of internal fixation versus hemiarthroplasty for femoral neck fractures in the elderly

BACKGROUND: The treatment strategy for femoral neck fractures has not been confirmed, and choosing internal fixation or arthroplasty remains controversial. After internal fixation, the occurrence of bone nonunion and osteonecrosis of femoral head makes its long-term treatment outcomes far from satisfactory. Can we improve the functional recovery through high-quality reduction, accurate fixation and active postoperative exercise? If so, it may be favorable for the elderly with femoral neck fractures, especially for those combined with other serious diseases.OBJECTIVE: To compare the quality of life in the elderly with femoral neck fractures after internal fixation and hemiarthroplasty.METHODS: 148 patients with femoral neck fractures aged more than 60 years old in the First Affiliated Hospital of Dalian Medical University from January 2010 to December 2013 were analyzed retrospectively. The operation time,intraoperative blood loss, hospitalization time, mortality rate, reoperation rate and postoperative quality of life were compared between internal fixation and hemiarthroplasty in the patients with undisplaced femoral neck fractures (Garden type I or II) displaced femoral neck fractures (Garden type III or IV), respectively.RESULTS AND CONCLUSION: (1) Among 148 patients, 137 cases completed follow-up, and the follow-up rate was 92.6%. (2) The operation time, intraoperative blood loss and hospitalization time in the internal fixation group were significantly superior to those in the hemiarthroplasty group in patients with displaced or undisplaced femoral neck fractures (P < 0.05). (3) There was no significant difference in the mortality rate between two groups, and the reoperation rate in the internal fixation groups was slightly higher than that in the hemiarthroplasty group. (4) The Harris and SF-36 scores in both groups were increased with time, and the difference between two groups was decreased with time, which became equal at 2.5 years postoperatively (P > 0.05). Besides, the time of the patients with undisplaced femoral neck fractures in the internal fixation group achieving the same quality of life with the hemiarthroplasty group was earlier than that of displaced ones. (5) These results suggest that internal fixation holds short operation time, simple operation skills and less trauma in the treatment of femoral neck fractures, and with the development of technology, it can achieve the early functional recovery. Thereafter, it is an effective method for senile femoral neck fractures.
10.Effects of transverse arch reconstruction by modified Youngswick osteotomy on young and middle-aged hallux valgus

[Objective]To evaluate the effect of transverse arch reconstruction by modified Youngswick osteotomy on young and middle-aged hallux valgus. [Methods]From November 2005 to November 2008,25 female(36 feet) hallux valgus patients with the average age of 37.7years old(range 22 to 49 years old) were treated surgically.All feet had hypemobility of the first tarsometatarsal joint and collapse of the transverse arch of the foot.In the operation,the first metatarsal head was pushed laterally,downward and rotated downward.Modified McBride procedure was used on the correction for hallux valgus deformity.The X-ray films of all patients were obtained before operation,and 12 months after operation.The hallus valgus angle and intertatarsal angle were measured and analyzed on radiographs.The surgical outcome was evaluated combined with the Gu Xiang-jie's score.All pamameters were statistically analyzed.All cases were followed-up regularly.[Results]Twenty-one cases(28 feet) were followed-up for averaged 15.9 month.Hallax valgus angle reduced from 26.8??1.07? to 11.1??0.29?,intertatarsal angle reduced from 14.9??1.36? preoperatively to 8.3??1.19? postoperatively.The painful callus beneath the forefoot was alleviated or diminished obviously in all cases.Based on Gu Xiangjie'score,the subjective assessments were given as excellent in 16 feet,good in 7 feet,and fair in 5 feet.The good to excellent result was 82.1%.[Conclusion]For the hallux valgus with hypermobility of the first tarsometatarsal joint and transversal arch collapse,modified Youngswick surgery can recover the transverse arch,correct first metatarsal adduct abnormality,and regain the weight-bearing function of the first metatarsal head.Modified Youngswick surgery is an ideal choice for young and middle-aged hallux valgus because of less complications and reliable surgery outcomes.
