1.Recent development of the medical laboratory accreditation in China and the related international standards

Medical laboratory accreditation with ISO 15189 has brought up concerns from various interest parties in China. It is a common concern for both Accreditation Body (AB) and Medical Laboratory (ML) on how to ensure and improve the accreditation work scientifically and effectively. It will help us to find a way to solve the problem and to find the right way for the development of the medical laboratory accreditation if we study the drafting, implementation and apphcation of the international standards which found the basis of the accreditation scheme. This article will give the reader a brief introduction of the ISO/ TC212, which is the technical committee of international standard organization responsible for drafting and revising ISO 15189, medical laboratory accreditation related international standard documents, and the recent status and the future development of the medical laboratory accreditation in China. We hope this article can give some information for the medical laboratory stuffs and the professionals in laboratory medicine on their research work of the technique and the trend of development in medical laboratory management and medical laboratory accreditation.
2.Pharmaceutical Care for Hypertensive Patients after Renal Transplantation

OBJECTIVE:To provide a pharmaceutical care model for the practice of patient-centered clinical pharmacy so as to ensure safe,effective and economical clinical medication.METHODS:The concept,desired therapeutic outcomes and the treatment method of hypertension after renal transplantation were expounded;the issue of the key points in pharmaceutical care were decided,and immunosuppressive agents and antihypertensive drugs were administered and the pharmaceutical care plan was made based on patients’ conditions.The therapeutic outcomes were evaluated;the patients were educated to elevate their compliances.RESULTS & CONCLUSION:It is feasible to provide clinical pharmacists with an effective pharmaceutical care mode and method so as to provide better services for patients,doctors and nurses.

4.Clinical study of postoperative individualized chemotherapy based on genetic testing results for non-small cell lung cancer

Objective To explore the efficiency of postoperative individualized chemotherapy based on genetic testing results for non-small cell lung cancer (NSCLC). Methods Fifty-six NSCLC patients at stageⅡorⅢA who accepted video-assisted thoracic operation were divided into two groups:the individualized chemotherapy group (n=26) and non individual?ized chemotherapy group (n=30). The fresh lung tumor tissue of individualized chemotherapy group was tested target gene,in?cluding excision repair cross complementing 1 (ERCC1),ribonucleotide reductase subunit M1 (RRM1),β-tubulinⅢ,thymi?dylate synthase(TS),epidermal growth factor receptor (EGFR) and breast cancer gene 1 (BRCA1). The theraputic plan was based on genetic testing results in individualized chemotherapy group, and the non individualized chemotherapy group re?ceiving gemcitabine plus cisplatin. The 1-year disease free survival (DFS), 2-year disease free survival (DFS), the progres?sion-free survival (PFS) and the overall survival (OS) were compared between two groups. Results The 2-year DFS (57.69%), PFS (22.1 ± 5.0 months) and OS (24.1 ± 3.2 months) were significantly higher in the individualized chemotherapy group than those of non individualized chemotherapy group (respectively 30.00%, 18.9 ± 6.2 months, 21.9 ± 4.3 months, P<0.05). There was no significant difference in 1-year DFS between two groups (88.46%vs 83.33%, P<0.05). Conclusion The individualized chemotherapy based on genetic testing results can enhance the 2-year DFS, PFS, OS and the efficiency of postoperative adjuvant chemotherapy for NSCLC.
5.Effect of CYP3A5~*3 and CYP3A4~*18B genetic polymorphisms on cyclosporine pharmacokinetics in renal transplant patients

Aim To evaluate retrospectively the effect of genetic polymorphisms in CYP3A4 and CYP3A5 on cyclosporine pharmacokinetics during the early period in renal transplant patients.Methods Polymerase chain reaction-restriction fragment length polymorphism was performed to genotype for CYP3A4*18B and CYP3A5*3.Cyclosporine whole blood levels were measured by fluorescence polarization immunoassay.Results The frequencies of CYP3A4*18B and CYP3A5*3 variant alleles were 0.235(95 CI:0.235~ 0.241)and 0.770(95 CI:0.767~0.773),respectively in 63 of renal transplantation patients, and these alleles exhibited incomplete linkage disequilibrium. The median (range) value of cyclosporine dose adjusted blood trough concentrations (C0/D) and C0 in CYP3A4*1/*1 genotype patients was higher than that in CYP3A4*1/*18B and CYP3A*18B/*18B patients, the same result was found in CYP3A5 expression (CYP3A5*3/*1 and CYP3A5*1/*3) (P
6.Structural Characteristic of the Outer Tunnel of Corti's Organ in the Guinea Pig Cochlea

Objective To understand the structural characteristics and significance of the outer tunnel of corti's organ.Methods The structural characteristics of the outer tunnel were observed and analyzed with celloidin section and scanning electron microscope(SEM).Results The inner wall of outer tunnel consisted of the third row of outer hair cells(OHC) and the stalk of Deiter's cells in the basal and the first turn.Hensen's cells made up the outer wall of the outer tunnel.The stalks of Deiter's cells gradually moved to outer side to constitute the outer and top walls of outer tunnel from the basal turn to the top turn.The inner wall of outer tunnel consisted of the third OHC.The stalks of Deiter's cells moved to Hensen's cells to make the outer wall of the outer tunnel.The space of outer tunnel was gradually decreased.The outer tunnel was full of Deiter's cells in the third and fourth turn.The stalks of Deiter's cells tightly contacted with the Hensen's cells.Conclusion The structural characteristics of the outer tunnel play an important role in maintaining the stability of Corti's organ.
7.Interpretation of ISO 15189:2012“Medical laboratories-Requirements for quality and competence” -Requirements for IVD manufacturers

In vitro diagnostic ( IVD) manufacturers were important suppliers of external servicesto medical laboratories .There equipment , reagents , calibration and maintenauce services played an important role in ensuring the quality of medical laboratory tests .It is also helpful for IVD manufacturers to establish and maintain a good reputation and credibility .ISO 15189:2012“Medical laboratories -Requirements for quality and competence” was issued onNovember 1, 2012, which replaced the second edition (ISO 15189:2007).Requirements related to IVD manufacturers are involved in the new edition standard .The article aimed to help medical laboratories and IVD manufacturers understand the requirements ; therefore , laboratories would know how to select suppliers based on their ability to supply external services , IVD manufacturers would know how to meet the needs of laboratories , and the cooperation between IVD manufacturers and laboratories would be facilitated .

9.Study of the expression of matrix metalloproteinase and its tissue inhibitor in hepatocellular carcinoma

Objective To study the expression of matrix metalloproteinase (MMP) and the tissue inhibitor of matrix metalloproteinase (TIMP) in hepatocellular carcinoma (HCC) and its relation with the prognosis of patient. Methods MMP 1,MMP 2,MMP 9 and TIMP 2 in human HCC tissue and HCC cell line were measured and quantified with immunohistochemistry, Northern blot hybridization and image analysis. Results MMP 1,MMP 2 and MMP 9 were localized in the cytoplasm of tumor cells. The 5 years survival rate were markedly higher in the cases of MMP 1 negative, MMP 9 negative, both MMP 1 and MMP 2 negative or both MMP 1 and MMP 9 negative staining than that of the positive cases ( P
10.Locating and protecting recurrent laryngeal nerve in minimally invasive video-assisted thyroidectomy

Objective To study the anatomic landmarks of recurrent laryngeal nerve (RLN) in minimally invasive video-assisted thyroidectomy (MIVAT), and to evaluate the operative skills to avoid nerve injury. Methods 106 patients were enrolled in the study dated between August 2008 and August 2010, in which 8 patients were converted to the conventional thyroidectomy. Intraoperative anatomic landmarks for location of RLN were the gap between trachea and carotid artery (GTC), as well as the middle and posterior portion of tracheal wall. Results 98 RLN were at risk, and 97 (98.98 %) nerves were recognized by means of two landmarks. 1 nerve failed to locate which was non-recurrent laryngeal nerve. Temporary RLN paralysis happened to 1 nerve(1.02 %), and no permanent RLN paralysis appeared. Conclusion GTC combined with middle and posterior portion of tracheal wall are safe and effective anatomic landmarks to locate RLN in MIVAT.
