4.Optimization of beam orientations in three-dimensional conformal radiotherapy with the genetic algorithm

Objective To develop a beam orientation optimization algorithm for the gantry orientation in three-dimensional conformal radiotherapy(3 DCRT).Methods Patients' data were imported from the Pinnacle v 7.2 treatment planning system.including the DICOMRT and dose distribution files.These imported files were merged using a uniforiB coordinate system.The algorithm determined the optimized beam weight for each beam group and optimized the beam orientation with genetic algorithm.The optimized parameters,including the optimized beam orientations and weights,were exposed back to the Pinnacle v 7.2 to compare with the conventional 3DCRT plan.The optimized algorithm was implemented with our in-house program. The dose distributions, the DVH diagram and the conformity index of two lung cancer patients were compared. Results for the two lung cancer patients,the conformity index of the optimized plan(0.59 and 0.7)was higher than the conwentional 3DCRT plan(0.36 and 0.58).The maximum dose in spinal cord was reduced by 17.8%and 22.4%,the lung V20 reduced by 3.12%and 4.35%,and V30 reduced by 4.47%and 1.49%.For the brain tumor patient. the dose of lens and eyes was also decreased significantly. Conclusion This beam orientation optimization can be used as an assistant planning tool.
5.The application of self-made blenderized diet for nutritional support treatment in the critically ill patients with diabetes

Objective To investigate the effect and safety of self-made blenderized diet used for nutritional support treatment in the ICU critically ill patients with diabetes. Methods 74 ICU critically ill patients with diabetes were randomly divided into two groups:study group and control group. The self-made blenderized diet were given to the patients in study group and enteral nutrition to the patients in control group for nutritional support treatment for four weeks. Then glucose metabolism, various nutrition indicators and gastrointestinal dysfunction were observed in these two groups. Results The fasting blood glucose and fructosamine levels were decreased significantly,while the nutritional indicators was significantly higher after treatment than at baseline in all patients. There was no significant difference in the levels of fasting blood glucose, postprandial glucose, fructosamine and nutritional indicators between two groups after treatment,so were the incidence of the hypoglycemia and gastrointestinal complications. Conclusion The self-made blenderized diet was a good enteral nutrition support therapy for ICU critically ill patients with diabetes which had a considerable effect, but more cheaper,and practical compared to the enteral nutrition emulsions.
6.Monte Carlo calculation of the impact on dose distribution by width of energy spectrum and angular distribution of electron beam

ObjectiveTo study of the impact on dose distribution by electron width of energy spectrum and angular distribution using Monte Carlo simulation.MethodsThe simulated electron energy spectrum and angular distributions was as the input parameters and percentage depth dose (PDD) and offaxis curves were simulated by a modified PENELOPE code package. Results PDD and off-axis dose distribution curves are almost the same and are not sensitive to energy spectrum width and angular distribution with the exception of energy spectrum width of 2.5 MeV with obviously different curves.ConclusionsIn the situation of clinical treatment,spectrum and angular distributions can be ignored when their width are not very large.It is helpful to save about 9% time by using monoenergy beams in treatment planning system development.
7.Analysis of complication in anterior decompression and fixation technique for thoracolumbar fractures in cases with concomitant paraplegia

Objective To discuss causation and clinical treatment of complication in anterior decompression and fixation technique (ADFT) for thoracolumbar fractures with associated paraplegia. Methods A total of 204 cases that had thoracolumbar fractures with paraplegia were operated with ADFT from 1998 to 2004. Of all, 51 times (34 cases) of complication took place. Change of Cob angle was measured according to X-film of thoracolumbar spine before and after operation. Meanwhile, sensation and motion of patients was evaluated. Results There was incision infection leading to septic cerebo-meningtis in one case, CSF leakage in seven, pneumothorax in eight, rib nerve-root injure in five, lumbar nerve-root injure in two, genitofemoral nerve injure in three, lateral femoral cutaneous nerve injure in two, kyphosis in three, deep venous thrombosis in three and eight times of skin-temperature change. X-film that was taken after operation for all patients showed scolisis in five cases. A follow up for 3-6 years (average 2.5 years) was performed in 84 cases, of which seven cases had chronic low-back pain, six scolisis and five kyphosis. No patient showed complications such as screw breakage, plate breakage, fixation device loosening and pseudo-articulation in thoracolumbar spine. Conclusions Many complications can be avoided if we master well anterior decompression and fixation technique of thoracolumbar fracture, reduce surgical trauma and give appropriate and rational guide for rehabilitation.

Current occupational health and occupational medicine teaching content focused on the detection of harmful factors,and more belonged to validation experiments.Closed-end management of experimental teaching,experimental report-based performance evaluation methods and single model of teaching methods reduced the students' study interesting,lack of problem-solving abilities.Therefore,it is necessary to optimize the experimental curriculum,adopt an open experimental teaching,build a reasonable experimental evaluation system and promote the diversification of teaching model.
9.Study on the application value of percutaneous dilation tracheostomy on the neurological ;intensive patients

Objective To explore the application value of percutaneous dilational tracheostomy in the neurological intensive patients. Methods The clinical data of 298 neurological intensive patients treated with percutaneous dilational tracheotomy were analyzed retrospectively from June 2010 to July 2016. Results Percutaneous dilational tracheostomy was successfully performed in all of the 298 cases. The mean operation duration was (6.8 ± 2.2) minutes with a rang of 2 to 15 min. There were 1 case with tracheal tube placement difficulties, 1 case in whom the trachea spatium was strayed into, 1 case with arrhythmia, 10 cases with mild bleeding and 5 cases with moderate bleeding. No severe bleeding, subcutaneous emphysema, pneumothorax, tracheoesophageal fistula and other complications caused by the operation were found. And no death case was found. Conclusions Percutaneous dilational tracheostomy is a rapid and minimally invasive technique, which is a better choice for patients with severe neurological diseases.
