2.Application of Laparoscopy to Repair of Congenital Esophageal Hiatal Hernia

Objective To investigate the efficacy and safety of laparoscopic repair in children with congenital esophageal hiatal hernia. Methods From September 2001 to December 2008,seven children with congenital esophageal hiatal hernia,including 4 boys and 3 girls,were treated by laparoscopic repair and Nissen's fundoplication in our hospital. One of them received laparoscopic pyloroplasty in the meanwhile because of delayed gastric emptying (confirmed by preoperative gastrointestinal angiography); one child underwent laparoscopic high ligation due to right oblique inguinal hernia; spleneolus was found in one patient,no further treatment was carried out. Results The procedures were completed in all of the patients without conversion to open surgery. The mean operation time was 94.2 min (75-150 min) in this series,and the average blood loss was 5 ml (2-10 ml). No patient received blood transfusion after the surgery. The children began to intake milk or regular diet in 24-48 hours after the operation. They were discharged from hospital in a mean of 4.5 days (ranged from 3 to 7 days). Then,the children were followed up for 6 to 20 months (mean,12.6 months). One of the patients suffered from vomiting on the 10th day after the operation,1 patient showed recurrence in 1 year postoperation and then was cured by a second operation. The other five patients were healthy during the follow-up. Conclusions Laparoscopic repair of congenital esophageal hiatal hernia is a safe,effective,and minimally invasive procedure. It is feasible for patients complicated with other diseases. The indications for the surgery must be strictly selected.
3.Laparoscopically assisted anorectoplasty for intermediate imperforate anus: A report of 11 cases

Objective To explore the feasibility of laparoscopically assisted one-stage anorectoplasty for the treatment of intermediate imperforate anus. Methods Under laparoscopic visualization, the rectum was mobilized. Then the rectourethral (or rectovaginal) fistula was repaired laparoscopically (6 cases) or through perineal approach (4 cases). Afterwards a tunnel was made through the center of pelvic floor musculature by combined use of laparoscopic and perineal approach. The rectum was pull-through the perineum and anastomosed with skin. At Last the colostomy was closed. Results Laparoscopically assisted anorectal pull-through was successfully performed in all cases. The mean duration of laparoscopic dissection was 32.8 minutes (range, 25~78 minutes). The mean blood loss was 5.3 ml (range, 2~10 ml). No blood transfusion was needed. The intraurethral catheter or cystostomy tube was removed on the 11th postoperative day. No intraoperative complication was encountered and all infants had uneventful postoperative recovery without wound infection. During a follow-up for 3~66 months (mean, 45 months), the fecal continence evaluation showed excellent in 8 cases and good in 3 cases. Conclusions Laparoscopically assisted anorectoplasty is an effective and minimally invasive technique for the treatment of intermediate imperforate anus. This technique has advantages of simple performance, accurate identification of the center of the pelvic floor musculature, and low infection rate.
4.Miniaturized Corona Discharge-Atomic Emission Spectrometer for Determination of Trace Mercury

A miniaturized atomic emission spectrometer composed of a corona discharge source and a handheld charge-coupled device detector was constructed for the sensitive determination of trace mercury, with photochemical vapor generation for sample introduction. By using formic acid, mercury vapor was generated under the irradiation of UV light, and then the vapor was introduced into the corona discharge area with the carrier gas flow of argon. The optical signals were captured and analyzed by the charge-coupled device detector at 253. 7 nm. Under the optimized conditions including 4% formic acid, discharge voltage of 90 V, and carrier gas flow rate of 200 mL/min, the stable detection signals were obtained ( RSD=2. 5%). The calibration curves were plotted with the concentration of mercury ranging from 0. 5 to 1000 ng/mL, with a correlation coefficient >0. 99 and the limit of detection of 0. 03 ng/mL. Two certified reference materials were analyzed, and a t-test demonstrated that the determined results by the proposed method had no significant difference with the certified values at the confidence level of 95%. Water samples collected from both Funan River in Chengdu and the Lotus Pond on campus of Sichuan University were analyzed as well, with the recoveries of standard addition between 93 . 8% and 103%. The miniaturized instrumentation demonstrated several advantages including easy construction, low power consumption, high sensitivity, and portability for field analysis.
5.Expressions of cyclooxygenase-2,matrix metalloproteinases-9 and Ki67 in tissues of gastric cancer and their clinical significance

Objective To investigate the association of expressions of cyclooxygenase-2(COX-2),matrix metanoproteinases-9(MMp-9)and Ki67 with occurrence,infiltration and metastasis of gastric cancer.Methods Fifty-eight surgically obtained and pathologically performed cancerous tissues were collected between Jan.2003 and Dec.2005.Among them,37 were males and 21 were females with mean age of 58.2 years(ranged from 31 to 76 years).Fifty-eight paracancerous tissues(5-6 cm apart form the cancer)were taken as controls.The expressions of COX-2,MMP-9 and Ki67 in cancerous or paracancerous tissues were examined by immunohistochemistry.Results The expressions of COX-2 and MMP-9 in cancerous tissues(82.76%and 68.9%,respectively)were higher than those in controls(37.93%and 24.14%,respectively,P<0.01).The expressions of COX-2 and MMP-9 were not associated with sex,age,location and size of gastric cancer(P>0.05),but were correlated with the depth of lymph node infiltration.metastasis and TNM stage(P<0.05).The expression of MMP-9 was also associated with the degree of differentiation of gastric cancer(P<0.05).There was a correlation between COX-2 and MMp-9 expressions in gastric cancer tissues(P<0.05,C=0.359).The expression of Ki67 in tissues with positive expressions of COX-2 and MMp-9 was higher than that in negative ones(P<0.O 1).Conclusion The COX-2,MMP-9 and Ki67 play important roles in metastasis and infiltration,which promote occurrence and development of gastric cancer.
6.Clinical significance of hypokalemia in early stage of ST-elevation myocardial infarction

Objective To investigate clinical characteristics of patients with ST-elevation myocardial infarction (STEMI) and hypokalemia and the effects of hypokalemia on prognosis. Methods Consecutive 216 cases with STEMI who underwent emergency PCI were divided into group A (serum potassium < 3.5 mmol/L) and group B (serum potassium>3.5 mmol/L). Infarct site, infarct interrelated artery, peak level of CK-MB and cTnT were compared between two groups. Post-infarctional angina pectoris, arrhythmia, heart failure and cardiac death were compared. Results (1 )The percentage of anterior wall myocardial infarction , left anterior descending artery (LAD) lesions in group A were significantly higher than those in group B [61.2%(41/67) vs. 44.3%(66/149),55.2%(37/67)vs. 38.9%(58/149),P = 0.022,0.026]. The peak levels of CK-MB and cTnT in group A were significantly higher than those in group B [(194.39 ± 101.27) μg/L vs. (115.35 ±78.62)μg/L,(19.16 ±11.48)μg/L vs. (9.07 ±7.65) μg/L,P = 0.004,0.002].(2)Left ventricular ejection fraction in group A was significant lower than that in group B (P - 0.003). The incidence rates of post-infarctional angina pectoris, ventricular tachycardia, ventricular fibrillation and heart failure were significantly higher in group A [43.3%(29/67),32.8%(22/67), 11.9%(8/67),37.3%(25/67)] than those in group B [24.8%(37/149),18.1%(27/149),4.0%(6/149),20.8%(31/149)](P = 0.006, 0.017, 0.029, 0.010). Conclusions Hypokalemia is associated with infarct site and infarct interrelated artery. Hypokalemia has bad effect on prognosis of STEMI.
7.Th1/Th2 shift in gut of mouse model of postinfectious irritable bowel syndrome

Objective To observe the expressions of Th1 cytokin [interleukin (IL)-12] and Th2 cytokin (IL-4) in gut of the mouse model of postinfcctious irritable bowel syndrome (PI-IBS). Methods Thirty mice were divided into control group (n=8) and model group (n=22,infected with Trichinella spiralis). The weight of the mouse was measured weekly after infection. Visceral sensitivity of colorectal distention in mouse was accessed by abdominal withdrawal reflex (AWR) at 0 and 8 weeks after infection. All mice were sacrificed at the 8th week, and tissues of jejunum, terminal ileum, proximal colon and distal colon were observed for inflammation with HE staining. The mRNA and protein expressions of IL-12 and IL-4 were examined by RT-PCR and Western blotting, respectively. Results Two weeks after infection, body weight was decreased in model group compared with control group ( -1.08%±1.08 % vs 3.09%±1.85%, P<0.05), but there was no significant difference at the 8th week between two groups (P>0.05). Severe inflammation was observed in the gut of mouse 2 weeks after infection, however it was recovered at the 8th week. The score of AWR was higher in model group at 30,45 and 60 mmHg in comparison with control group (P<0.05), whereas the perceptual threshold was lower in model group than in the control group (P<0.05). In comparison with control group, increased expressions of IL-12 mRNA and protein in ileocecum and proximal colon as well as decreased expressions of IL-4 mRNA and protein in all parts of gut were found in model group (P<0.05). Conclusion Th1/Th2 shift may involve in gut of PI-IBS mouse, which provides a new idea for treatment of PI-IBS.
9.Stability analysis of allelopathic effects of Panax notoginseng on main crops by AMMI model.

This paper is aimed to study the differences of allelopathic effects of Panax notoginseng under different allelopathic chemicals resources and selection of appropriate rotation crops. The additive main effects and multiplicative interaction ( AMMI) model had been used to evaluate the stability of allelopathic effects of P. notoginseng on the varieties of corn, wheat and rice properly. The model could use not only to evaluate the stability of non-regional trial data but also explore the interaction between the rotation crop genotypes and donor substances more efficiently. Meanwhile, correspondence analysis can be used in the AMMI to evaluate genotype stability and donor substances. Ejingza No. 1 (g6) had stronger allelopathic effects with high stability, but Yunrui No. 1 (g9) which was appropriate rotation crop genotype, had weaker allelopathic effects with high stability. These findings will aid in choosing appropriate rotation crops and establishing proper rotation system.

10.Escherichia coli Drug Sensitivity Analysis in Hospital Infection

OBJECTIVE To survey the Escherichia coli infection in our hospital in the field of flora distribution,structure and status quo resistance in order to guide the rational use of antibiotics.METHODS From Nov 2005 to Dec 2006 247 E.coli strains were isolated from 2940 samples with conventional method identification,K-B method was used for bacterial susceptibility,and resistance test.RESULTS The top six susceptible antibiotics were cefoperazone/sulbactam(86.84%),nitrofurantoin(55.56%),chloramphenicol(43.25%),amikacin(40.43%),ceftriaxone(27.41%),and cefoperazone(27.16%).The top six resistant antibiotics were sulfamethoxazole/trimethoprim Co-trimoxazole(93.33%),ampicillin(93.18%),amoxicillin(92.00%),cefalotin(86.67%),cefradine(85.03%),and cefuroxime(81.25%).CONCLUSIONS Strengthening antibacterial drug sensitivity test,and conduct ing timely the effective treatment and corresponding measures could guide the clinician to provide a reasonable use of antibiotics.
