1.The Clinical Application Status and Development Trends of Hydrogen Peroxide Low Temperature Plasma Sterilizers.

The hydrogen peroxide low temperature plasma sterilization technology solved the problems of thermo-sensitive materials' disinfection and sterilization based on its development and unique characteristics. This paper introduced the researches of clinical application quality control, and showed the hydrogen peroxide low temperature plasma sterilizers were being widely used in hospitals and highly recognized. According to the clinical data and the literatures of the domestic equipment in preliminary application, it could be concluded that the technology maturity of domestic hydrogen peroxide low temperature plasma sterilizers was in a high level. The advantages of using domestic hydrogen peroxide low temperature plasma sterilizers to do disinfection and sterilization included lower cost, safer, faster and non-toxic, etc. Also the management system should be improved and the clinical staff should master the technical essentials, obey the procedures strictly, verify periodically and offer full monitoring to upgrade the quality of sterilization.

2.Image and pathology features of primary adrenal lymphoma

Objective To evaluate the CT features of primary adrenal lymphoma and its relationship with pathology.Methods Pathologically proven primary adrenal lymphoma 6 cases were reviewed.There were 4 males and 2 females.The age was 18-62 years,average age was 51 years.5 cases was single tomur in unilateral,1 case was bilateral disease.The main clinical manifestations were abdominal pain,abdominal discomfort.All patients had CT scan and were treated surgically.Results CT scan showed the characters of adrenal little low-density soft tissue mass:the maximum diameter of 3-11 cm,irregularly shaped or ovalshaped,multi-state a clear,homogeneous or slightly inhomogeneous density,CT value was 30-40 HU.Lesions could be embedded or close to blood vessels and the ipsilateral kidney.The lesions were not enhanced arterial phase enhancement,CT value was 39-50 HU.The lesions showed vein phase of mild to moderate enhancement,CT value was 47-66 HU.At the delay of continuing to strengthen phase,CT value was 60-78 HU.The pathology charaters showed that:Diffuse of tumor cells under light microscope dense,more uniform size,and a large,granular chromatin,tumor stromal components was relatively small.There was no significant bleeding,necrosis and calcification.one lesion showed sheet,tumor cell necrosis.Conclusions CT scan characteristics and pathological features of primary adrenal lymphoma might have a certain correlation.
3.Clinical features of 35 patients with myasthenia gravis associated with other autoimmune diseases

Objective To analyze the clinical features of 35 patients with myasthenia gravis (MG) associated with other autoimmune diseases (ADMG). Methods One hundred seventy MG patients were divided into 2 groups. One group included MG patients with autoimmune diseases ( n=35), and the other included MG patients without auto?immune diseases (NADMG, n=135). Clinical features such as gender, age of onset, types of MG, myasthenic crisis, thy?mus abnormalities, different therapies, ocular MG progressing to general MG, and relapse of MG within one year were compared between ADMG and NADMG. Results 85.7%ADMG patients were female which was higher compared with NADMG (P< 0.001); median age of onset was 40 years old (23~45 years old) in ADMG group which was younger than NADMG group. Ocular MG and thymic hyperplasia were more frequent in ADMG group than in NADMG group (45.7%vs. 24.4%, P=0.013;14.3%vs. 1.5%,P=0.001,respectively). Other clinical features such as myasthenic crisis, thymoma, different therapies, ocular MG progressing to general MG, and relapse of MG within one year were not signifi?cantly different between the two groups. In multivariate analysis, female (OR=4.76, 95% CI:1.64-13.77; P= 0.004), age of onset (OR=0.96, 95%CI:0.92-0.99;P=0.019), ocular MG (OR=3.10, 95%CI:1.30-7.41;P=0.011) and thy? mic hyperplasia(OR=16.26, 95%CI:2.22-119.11;P=0.006) were significantly different between the two groups. Con?clusions ADMG is more common in female patients with ocular MG and thymic hyperplasia.
4.Clinical value of preoperative serum carcinoembryonic antigen detection in the prediction of esophageal cancer lymph node metastasis

Objective To investigate the clinical value of preoperative serum carcinoembryonic antigen (CEA) detection in the prediction of esophageal cancer lymph node metastasis.Methods The clinical data of 111 patients with esophageal cancer who were admitted to the Chaoyang Hospital of Capital Medical University between December 2010 and January 2014 were retrospectively analyzed.Patients received preoperative serum CEA examination and enhanced CT of the chest.The surgical procedures were selected according to the condition of patients, including radical resection of esophageal cancer via left thoracic approach, transabdominal right thoracic approach (open and laparoscopic surgeries), cervico-thoracic-abdominal triple incision (open and laparoscopic surgeries) and transabdominal incision.The international standard was used for tumor location and TNM stage of esophageal cancer.The count data and comparison of ordinal data in the univariate analysis were analyzed using the chi-square test, Fisher exact probability and rank-sum test, respectively.The multivariate analysis was done using the stepwise logistic regression.The ROC curve was used for evaluating diagnostic value of serum CEA examination and enhanced CT of the chest.All the 111 patients were divided into 4 groups according to the interquartile range results of the CEA examination, and the lymph node metastasis rates of 4 groups were compared by the chi-square test.Results All the 111 patients underwent successful radical resection of esophageal cancer after preoperative serum CEA detection and enhanced CT of the chest, including 40 via left thoracic approach, 56 via transabdominal right thoracic approach, 8 via cervico-thoracic-abdominal triple incision and 7 via transabdominal incision.There were 3 patients with upper thoracic esophageal cancer, 52 with middle thoracic esophageal cancer, 36 with lower thoracic esophageal cancer and 20 with cancer of gastro-esophageal junction.The postoperative pathological type included 84 squamous cell carcinomas, 23 adenocarcinomas and 4 other carcinomas.There were 44 patients with negative lymph node metastases and 67 with positive lymph node metastases.The positive rate of elevated serum CEA in the 111 patients was 36.04% (40/111).Tumor location, pathological type and N stage of tumor were clinical pathological factors affecting the positive rate of serum CEA of patients (Z =6.815, 6.608, 16.928, P <0.05).N stage of tumor was an independent risk factor affecting the positive rate of serum CEA of patients by multivariate analysis [OR =2.206, 95% confidence interval (CI) :1.370-3.552, P < 0.05].The T stage of tumor and serum CEA level were risk factors affecting lymph node metastasis of esophageal cancer by univariate analysis (Z =18.971, x2=10.081, P <0.05), and those were also independent risk factors affecting lymph node metastasis of esophageal cancer by multivariate analysis (OR =3.558, 3.936, 95% CI: 1.798-7.041, 1.480-10.469, P <0.05).The lymph node metastasis rates of esophageal cancer were 46.43%, 48.28% , 55.56% and 92.59% when CEA level≤ 1.75 μg/L, 1.75 μg/L < CEA level ≤ 2.68 μg/L, 2.68 μg/L < CEA level ≤4.21 μg/L and CEA level > 4.21 μg/L by the stratified analysis, respectively, with a significant difference among the 4 groups (x2=16.026, P < 0.05).The areas under the curve of CEA level and enhanced CT of the chest for lymph node metastasis were 0.687 (95% CI: 0.590-0.785) and 0.689 (95% CI: 0.591-0.788) by ROC curve, which were significantly different from the area under the guides (P <0.05).The areas under the curve of CEA level and enhanced CT of the chest for lymph node metastasis were 0.785 (95% CI: 0.697-0.873, P < 0.05).Conclusions Serum CEA detection not only has certain predictive value for lymph node metastasis of esophageal cancer, but has a higher predictive value combined with enhanced CT of the chest.There is a risk of lymph node metastasis for patients with deep tumor invasion and elevated CEA level, and the range of lymph node dissection should be expanded.
5.Quantitative determination of aqueous flare and cells in healthy eyes

Objective To quantify aqueous flare and cells in the eyes of healthy subjects and to evaluate the effect of age and sex on the blood aqueous barrier. Design Prospective case series. Participants Four hundred and forty-two eyes of 221 healthy sub- jects. Methods Aqueous flare and cells of 442 eyes were evaluated with FC-2000 laser flare cell meter (LFCM). Main Outcome Mea- sures Aqueous flare and cells. Results The mean flare values of all of eyes was 4.7?2.9 pc/ms, it was 3.1 pc/ms in the age group of less than 10 years, 3.8 pc/ms in the age group of 40-49 years and 11.0 pc/ms in the age group of 80 years or over. 6.Extraction of the 7 Loci of the donor murine MHC gene and construction of plasmids

Objective To extract the loci of murine MHC gene and construct plasmids.Methods The RNA of mice was extracted and reversely transcribed into cDNA.By using nested PCR,the products were connected with T vector,cloned,and sequenced.Subsequently,the genes were digested by endonucleases,connected with expression vector,and sequenced again to choose the correct clones.Results After the nested PCR,the products were approved by sequencing.After being connected with the vectors,they were approved again by sequencing and the correct clones were chosen.Conclusion All of the loci of the MHC gene can be obtained by nested PCR.The plasmids from the correct clone can be used in the further experiments of transferring the gene to mitigate the transplantation rejection.
7.Analysis on Funded and Completed Projects of Young Scientist Funds of National Natural Science Foundation of China in TCM Field from 1989 to 2015

Objective To discuss the present situation and existing problems of the funded projects, especially in the gap between western region and eastern central region by analyzing the funding situation of the projects funded by the National Natural Science Foundation of China (NSFC) and the completed projects in the TCM field from 1989 to 2015. Methods The website was searched thoroughly, and data of Division Ⅹ of Health Science Department of National Natural Science Foundation of China from 1989 to 2015 (ie TCM, Chinese materia medica and integrated traditional Chinese and Western medicine) funded by the National Science Foundation for Young Scientists in China were obtained, and the projects of the National Science Foundation for Young Scientists funded by Division Ⅹ of Health Science Department of National Natural Science Foundation of China from 1989 to 2015 were included, and incomplete or inavailable items were eliminated. Relevant information according to the extracted forms was extracted. The situation of different regions and the situations of the projects have been concluded and the titles of different titles have been analyzed. Results By 2015, the number of funded projects and the amount of financing in the TCM field were increasing year by year. In the western regions, the number of funded projects and the amount of financing accounted for only 11.29% of the total number and 11.28% of the total financing. Compared with the less supporting western regions, much more were mainly in eastern and central regions. The number of SCI papers published and single cited times of the finished projects were 988 and 20.85. Compared with the developed areas in eastern and central regions, there was no statistical significance in the average number of SCI papers (P=0.493), average SCI papers cited article number (P=0.449), SCI papers in average cited frequency (P=0.589), the average CSCD papers number (P=0.579), average CSCD papers cited article number (P=0.883) and average single CSCD papers cited frequency (P=0.753) in western regions (P>0.05). Conclusion Under the stable supports of the Young Scientists Fund, scientific researches in the TCM field have achieved remarkable achievements. In addition, the ability of scientific research and innovation of the young scientific workers have been improved. The funded projects in western regions are far less than those in eastern and central regions, with great differences. However, there is no statistical significance in the research results of the items in western regions and eastern and central regions.
8.FasT-Fix technique for suturing degree Ⅲ horizontal meniscus tears

Objective To investigate the results of FasT-Fix technique in repair of degree Ⅲ horizontal meniscus tears.Methods From June 2010 through June 2013,12 patients with degree Ⅲ horizontal meniscus tears underwent arthroscopic vertical mattress suture using the FasT-Fix technique.There were 7 males and 5 females at mean age of 34.7 years.All the patients were diagnosed with combined anterior cruciate ligament (ACL) rupture with preoperative Lysholm score of (64.9 ± 9.4)points.Arthroscopic meniscus repair was performed prior to ACL reconstruction.A second-look arthroscopy examined the healing status of the meniscus at 6 to 18 months postoperatively (mean,12 months).Lysholm score was evaluated during outpatient follow-up.Results All the patients were available to the follow-up of 26 months (range,12-36 months).Second-look arthroscopy showed complete healing in 9 patients and incomplete healing in 3 patients.At final follow-up,Lysholm score was (93.4 ± 5.0) points.Conclusion FasT-Fix technique can repair degree Ⅲ horizontal meniscus tears with vertical mattress sutures and reconstruct ACL,which has advantages of allowing maximal preservation of the horizontally torn meniscus,minor trauma,easy operation and definite efficacy.
9.Treatment of posterior and anterior ankle impingement syndrome with combined anterior, tarsal sinus and posterolateral approaches under arthroscopy

Objective To discuss the methods and effects of arthroscopic treatment of posterior and anterior ankle impingement syndrome through anterior approach,tarsal sinus approach plus posterolateral approach.Methods The study enrolled 12 patients with posterior and anterior ankle impingement syndrome treated from January 2012 to March 2014.There were 8 males and 4 females,agedl7-65 years (mean,45 years).Left ankle injury was noted in 9 patients and right ankle injury in 3 patients.Eight patients had a history of obvious ankle injury (5 acute and 3 chronic sprains).Arthroscopic surgery was performed to manage the posterior and anterior ankle impingement syndrome through the combined anterior,tarsal sinus and posterolateral approaches,and prone position and posteromedial incision were not used during operation.Postoperative outcome was evaluated using the American Orthopedic Foot and Ankle Society (AOFAS) score.Results Operation time was 45-80 min (mean,62 min).Arthroscopy confirmed anterior bone impingement in all patients.Three patients suffered from posterior bony impingement of the talus,while nine patients os trigonum injury.Duration of follow-up was 10-18 months (mean,14 months).AOFAS score was (91.4-± 6.5) points after operation,significantly higher (34.4 ± 12.6) points before operation (t =14.607,P <0.01).All patients had normal ankle range of motion after operation.There were no complications such as neurovascular injury.All the surgical incisions achieved primary healing.Conclusion The procedure avoids body position shift and posteromedial incision during operation,and attains a good vision to the anterior and posterior portions and satisfactory short-term outcome.
10.Mechanisms of tigecycline resistance in carbapenems-resistant Acinetobacter baumannii strains

Objective To investigate the mechanisms of tigecycline nonsusceptibility in carbapen-ems-resistant Acinetobacter baumannii ( CRAB) strains in order to provide a theoretical basis for a reasonable use of antibiotics and the control of nosocomial infection .Methods Susceptibility testing of 120 non-dupli-cate CRAB strains to tigecycline was performed by using the broth microdilution method .Minimal inhibitory concentrations ( MIC) of tigecycline against the A.baumannii strains were determined by using the broth mi-crodilution method before and after exposing the strains to Carbonylcyanide-m-chlorophenylhydrazone (CCCP), which was the efflux pump inhibitor .Polymerase chain reaction (PCR) was used to amply the ef-flux pumps genes including adeB, adeJ, adeG, abeM, adeE, adeRS, tetX and tetX1.The real-time PCR was performed to measure the expression of efflux pumps genes including adeB, adeJ, adeG, abeM and adeE.Results A total of 120 CRAB strains were collected including 13 (10.8%) tigecycline non-suscep-tible A.baumannii (TNAB) strains and 107 (89.2%) tigecycline susceptible A.baumannii (TSAB) strains.The MIC values of tigecline to the 120 CRAB strains were in a range of 0.25 μg/ml to 8 μg/ml. The adeR and adeJ genes were detected in 90.0%and 92.5%of the 120 CRAB strains, respectively.The positive rates of adeB, adeS, adeG and abeM genes among the 120 CRAB strains were all 94.2%.None of the three genes including adeE, tetX and tetX1 were detected .The mean expression levels of adeB and adeJ in TNAB strains were respectively increased by 18.69 folds and 5.46 folds as compared with those in sensi-tive strains.No significant increase in the expression of adeG and abeM genes was observed in TNAB strains . A 4-fold decrease in the MIC was observed in 8 out of 13 TNAB isolates treated with 10 μg/ml of CCCP .The CCCP could partially reverse the resistance pattern of tigecycline .Conclusion The efflux pump sys-tems of adeABC and adeIJK rather than the abeFGH and abeM systems might play an important role in reduc-ing the tigecycline susceptibility in carbapenems-resistant A.baumannii strains.
