2.Antimicrobial stewardship and bacterial resistance in the intensive care unit

Objective To investigate the changes of antibiotic resistance and its correlations with antibiotics use density (AUD) in intensive care unit (ICU) after implementation of antimicrobial stewardship. Methods Data on AUDs and synchronous antibiotic resistance in ICU of the Affiliated Hospital of Taishan Medical University during 2010 and 2014 were retrospectively reviewed.Autoregressive integrated moving average ( ARIMA) model was used to identify the correlation between AUDs and antibiotic resistance.Results The total AUD which stood at 163.46 DDDs· 100 person-1 · d-1 in the year 2010 decreased to 68.20 DDDs · 100 person-1 · d-1 in 2014, and the most significant decreases were found in quinolones, imidazole, carbapenems, aminoglycosides, other beta lactam and macrolides. Synchronously, a wide range of improvements in antibiotic resistance of major pathogens were also observed from 2010 to 2014, and the improvements were positively correlated with the decrease of AUDs.However, antibiotic resistance in 2014 was not significantly improved compared with that in 2013. The rate of ciprofloxacin resistance in Pseudomonas aeruginosa and the resistant rates of amikacin, imipenem, and meropenem in Acinetobacter baumannii even increased in 2014.Conclusions With the implementation of antimicrobial stewardship, antibiotic consumption in ICU was decreasing, and antibiotic resistance rate in major pathogenic bacteria was decreasing.But the rebound in antibiotic resistance in some types of bacteria was also observed and should be put on enough caution.

4.Research on Shanghai resident satisfaction of primary healthcare after new health reform

Objective The research was to achieve Shanghai resident satisfaction of primary health service after new health reform.Methods During self-compiled questionnaire survey,the research adapted stratified random sampling to select 9 community health centers in Shanghai urban,rural and suburb areas.Results Shanghai resident satisfaction rate of new health reform was 85.2%.Satisfaction rate of basic medical security system was 85.0%.Satisfaction rate of basic medical service was 96.5%.Satisfaction rate of basic public health service was 95.1%.Age,region,job,education and income were main factors to satisfaction of primary health service.Conclusion From an overall point of view,Shanghai resident satisfaction of primary health service after new health reform was high.However,the level of basic medical security and convenience of reimbursement were not high.Medical expenses of primary health care institution had not been effectively controlled.Basic medicine could not fully meet the medication needs of primary health care institutions.

Remaining organic and functional damage of ischemia cardio-cerebrovascular disease is always a main trouble puzzling the clinicians. After the discovery of endothelial progenitor cells (EPCs), researchers realize that postnatal angiogenesis is an important biological process, which play a key role to repair the ischemia tissue and improve the function. So a new concept which names therapeutic angiogenesis supply a new treament way for the ischemia cardio-cerebrovascular disease. Traditional Chinese medicine (TCM) has accumulated rich experience on treating the ischemia disease, studies found that many Chinese medicine prescriptions and effective ingredients can increase the therapeutic angiogenesis, howerer the mechanisms were not the same, they mainly manifest in regular the secretion of angiogenic factors, increase the proliferation and differentiation etc. In this paper, we review recent studies, summary the Chinese medicine prescriptions and effective ingredients which can increase the therapeutic angiogenesis, and analyze the differernt pathway. We view to provide reference for the later researchers.

6.Logistic regression analysis of risk factors of postpartum hemorrhage of cesarean section

Objective To investigate the risk actors of postpartum hemorrhage of cesarean section,in order to provide the basis for the prevention of hemorrhage after cesarean section.Methods A case-control study was conducted in 100 cases with postpartum hemorrhage of cesarean section and 200 cases without bleeding of cesarean section.The clinical data were compared and analyzed.The single factor and multiple factors of the Logistic analysis was used to analyze the risk factors of postpartum hemorrhage of cesarean section.Results Fetal macrosomia (OR =3.678,95% CI:1.456-5.345),placenta adhesion (OR =4.264,95% CI:1.556-5.646),placenta praevia (OR =2.754,95% CI:1.754-5.456),multifetal pregnancy (OR =3.964,95% CI:1.274-4.453) were the risk factors of postpartum hemorrhage after cesarean section.Conclusion Evaluation of the above risk factors before cesarean section was important for prevention of postpartum hemorrhage.
7.Research on diet balance by the enteral nutritional support for patients after hematopoietic stem cell transplantation

Objective To discuss the diet balance by the enteral nutritional support for patients after hematopoietic stem cell transplantation. Methods We divided 47 patients after hematopoietic stem cell transplantation into the balanced-diet group and the ordinary-diet group. The balanced-diet group received diet according to "diet exchange share" method and took food by the proportional share of 5 kinds of food. The ordinary-diet group took food according to the habits of patients.The effect of nutritional support was compared between the two groups. Results The body weight,white blood cell,hemoglobulin,platelet,total protein and proprotein in the balanced-diet group at the 90th day were different from those at the 30th day after transplantation(t value were -8.5,-4.7,-4.2,-10.2,-3.9,-14.1 and -80.9),P < 0.01.In the ordinary-diet group only body weight,platelet and proprotein at the 90th were statistically different from those at the 30th day after transplantation (t value were -4.5,-4.6 and -14.9), P < 0.01. Conclusion Balanced diet contributed to the rehabilitation and nutritional support after hematopoietic stem cell transplantation.

9.Expression of Axin, β-catenin and p53 and their significance in pleural fluid of lung adeno-carinoma patients

Purpose To investigate the expression of Axin, β-catenin and p53 in the pleural fluid and to discuss their relationships and significances. Methods The expression of Axin, β-catenin and p53 was detected using immunocytochemistry in adenocarinoma cells of the pleural fluid from 40 patients with primary lung adenocarcinomas or reactive mesothelial cells of the pleural fluid from 40 pa-tients with benign lung diseases. Results Axin positive expression rate was 30% (12/40) in cases with primary lung adenocarcino-mas. The positive expression rate of β-catenin was 15% (6/40) on cell membrane and 60%(24/40) in cytoplasm. The expression rate of p53 was 57. 5% (23/40) in cases of primary lung adenocarcinomas. The expression of Axin was positively correlated with the membranous expression of β-catenin, and negatively correlated with p53 in cancer cells of the pleural fluids. The positive expression rate of Axin was 77. 5% (31/40), but the expression rate of p53 was 1. 25% (5/40). There was no expression ofβ-catenin in reac-tive mesothelial cells from benign lung diseases. Conclusions The expression of Axin is significantly reduced in adenocarcinoma cells. The expression rate ofβ-catenin in cytoplasm is higher than that in membrane of cancer cells. It has a certain value to detect the expression of Axin,β-catenin and p53 for diagnosis and differential diagnosis of lung adenocaicinomas from benign lung diseases.
10.Effects of 17?-estradiol on the gene expression of BMPR-ⅠA and Cbf?1 in bone marrow stromal cells

Objective To investigate the effects of 17?-estradiol (E2) on the gene expression of typeⅠA bone morphogenetic protein receptor (BMPR-ⅠA) and core-binding factor alpha 1 (Cbf?1) in rat bone marrow stromal cells exposed to the differentiation medium and to elucidate the effects of E2 on osteoblastogenesis. Methods Adherent bone marrow stromal cells were cultured in differentiation medium containing DEX (10 -7 mol/L), 1,25-(OH)2D3 (10 -9 mol/L) and different concentrations of E2. Effects of different concentrations of E2 on the gene expression of BMPR-ⅠA and Cbf?1 was quantified by RT-PCR based on the comparison with an internal reference, ?-actin expression, and identified by Northern blotting. Alkaline phosphatase (ALP) activity of cells was detected. Contents of type Ⅰ collagen were determined by Van Gieson staining. Results E2 could evidently inhibit the expression of BMPR-ⅠA and Cbf?1 mRNA during the differentiation process of bone marrow stromal cells into osteoblasts in a dose-dependent manner. These were confirmed by Northern blotting. The ALP activity increased in a concentration-dependent manner, but the amount of type Ⅰ collagen decreased in a concentration-dependent manner. Conclusion E2 can significantly inhibit the gene expression of BMPR-ⅠA and Cbf?1 in bone marrow stromal cells and inhibit osteoblastogenesis in vitro.
