3.Skin Tissue Changes Following Thermal Injury in the Paralysed Lower Limb of Spinal Injured Rats.

OBJECTIVE: To prove that the skin of paralysed limb of spinal injured rat is more susceptible to a thermal injury than control, and to find out that the possible relating factors for explaining the increased susceptibility of skin. METHOD: Of total 69 male Sprague-Dawley rats, 50 were randomly divided into two groups, the spinal injured of which cords were transected at T10-13 level and the control. They were subdivided into 5 subgroups according to the duration of thermal injury. Infrared ray was used for thermal injury. Arterial cannulation was done in the femoral artery for blood gas analysis. Temperature was measured with a digital thermometer. Biopsy samples were stained with HE, and also immunohistochemical staining for heat shock protein 70 (HSP-70) was done. RESULTS: After thermal injury, the spinal injured group showed more severe tissue damage and a higher temperature elevation than the control. There was a tendency of decreased blood pH and pO2, and increased pCO2. Contrary to the control, the immunoreactivity of HSP-70 was very tiny or rarely present in the spinal injured group. CONCLUSION: This study suggest that the increased susceptibility of skin to the thermal injury in spinal injured rats may be related to the vasomotor instability. And, the poor expression of HSP-70 from the skin of spinal injured rat can be a factor for the explanation of the defective cellular protective response in spinal cord injury.

4.Emerging Roles of Human Prostatic Acid Phosphatase.

Prostate cancer is one of the most prevalent non-skin related cancers. It is the second leading cause of cancer deaths among males in most Western countries. If prostate cancer is diagnosed in its early stages, there is a higher probability that it will be completely cured. Prostatic acid phosphatase (PAP) is a non-specific phosphomonoesterase synthesized in prostate epithelial cells and its level proportionally increases with prostate cancer progression. PAP was the biochemical diagnostic mainstay for prostate cancer until the introduction of prostate-specific antigen (PSA) which improved the detection of early-stage prostate cancer and largely displaced PAP. Recently, however, there is a renewed interest in PAP because of its usefulness in prognosticating intermediate to high-risk prostate cancers and its success in the immunotherapy of prostate cancer. Although PAP is believed to be a key regulator of prostate cell growth, its exact role in normal prostate as well as detailed molecular mechanism of PAP regulation is still unclear. Here, many different aspects of PAP in prostate cancer are revisited and its emerging roles in other environment are discussed.

5.The Effect of Desflurane on Myocardial Contractility and Coronary Flow in Isolated Rat Hearts.

Background: Desflurane, a fluorinated methyl-ethyl ether, has some advantageous properties including low blood solubility, stability in soda lime, and resistance to biodegradation. Desflurane in vivo has demonstrated myocardial depressant property. The purpose of this study was to test the direct effects of desflurane on myocardial contractile function and coronary flow in the isolated heart. Methods: Twelve isolated rat hearts were continuously perfused with modified Krebs solution containing 6, 9 and 12 vol% of desflurane for 10 min at each concentration. Systolic left ventricular pressure and rate of change of ventricular pressure (dp/dt) were measured. Heart rate and coronary flow were also measured. To differentiate direct vasodilatory effect of desflurane from an indirect metabolic effect due to autoregulation of coronary flow, oxygen delivery, myocardial oxygen consumption and percent oxygen extraction were calculated. Results: Heart rate (control 266+/-22 beats/min) decreased to 250+/-23 beats/min at 6 vol%, 236+/-26 beats/min at 9 vol% and 223+/-22 beats/min at 12 vol% of desflurane. Systolic left ventricular pressure and dp/dt decreased in a concentration-dependent manner. In spite of decrement of myocardial oxygen consumption, coronary flow (control 12.0+/-1.2 ml/min) increased to 12.8+/-1.6 ml/min at 6 vol%, 12.9+/-1.6 ml/min at 9 vol% and 13.7+/-1.4 ml/min at 12 vol% of desflurane. Oxygen delivery increased proportionally with coronary flow. Percent oxygen extraction decreased in a concentration-dependent manner. Conclusion: These results suggest that desflurane has a direct myocardial depressing and coronary vasodilating effect in a concentration-dependent manner.

6.The Results of Survey for Hansen's Disease of the Migrant Worker.

We already achieved the goal of the elimination of leprosy that was recommended by WHO. But the amazing change of our country's situation of the economical & social conditions makes the need of the migrant worker. Especially, the portion of migrant workers form India, Nepal, Indonesia, Philippines, Bangladesh, & Vietnam, the countries of relatively high prevalence rate of Hansen's disease are increasing among them recently. It means the attention about the influx of Hansen's disease form the foreign countries. We perform the survey for Hansen's disease of the migrant worker. Total 603 workers were surveyed. We found only 4 persons, that had the high titer(above 500) of the result of PGL-1 ELISA, and only 3 persons, that had some findings, the suggestive finding of Hansen's disease, of cutaneous & neurological symptoms. In spite of the special study, we did not conform the diagnosis of Hansen's disease among them. During this survey, we did not find the conformed patients of Hansen's disease. But we think that we will be concerned about the influx of Hansen's disease from foreign countries, because of the recently significance change of the epidemiological situations of Hansen's disease in Korea.

7.The Effect of Laryngeal Mask Airway on Postoperative Sore Throat in Prone Position.

Background: Postoperative sore throat is a complaint after general anesthesia of multifactorial etiology. The laryngeal mask airway (LMA) reduces sore throat and discomfort during maintenance of the airway and make patients more comfortable. The purpose of this study was to compare effect of patient's position during operation on postoperative sore throat after the use of LMA. Methods: The fifty three patients were randomly divided into two groups. Group 1 (n=30) was underwent general anesthesia with supine position and group 2 (n=23) with prone position. After the LMA was positioned in the hypopharynx and the cuff inflated, fiberoptic laryngoscope was immediately passed down through the LMA. Number of attempts, degree of postoperative sore throat and other complications were also noted. Results: The incidence of postoperative sore throat after the use of LMA was 10% in supine position and 4% in prone position. But the difference between the groups was not statistically significant. All of the reported sore throats were rated as mild. Conclusions: Postoperative sore throat after the use of LMA is mild and the incidence is not affected by the prone position during the operation.

8.Web-based Factor Analysis System for Prevention and Management of Habitual Substance Disorder in Adolescents.

OBJECTIVE: The purpose of this study is to design the web-based dialogue expert system for preventing and managing of habitual substance disorders in adolescents. METHODS: Research process includes the requirement analysis, making of questionnaires, actual survey, and its analysis. Based on the analysis, we designed the web-based dialogue expert system. RESULTS: The habitual substance disorders in adolescents are variable. Our system for managing habitual substance disorders contains the diagnosis of alcoholism, nicotinism, and narcotism. And it analyzes the facts of them based the questionnaires and answer. As the results of the analysis, main reasons in order of the power of influence are belief, will of refusal, personal characteristics, family environment, and psychological personality. Our system suggests the customized solution to conquer that for each adolescent. Moreover, our system also includes the history tracking mechanism to manage the status of each teenager constantly and to make the survey and analysis information. CONCLUSION: With our system, young boys and girls can know their seriousness of alcoholism, nicotinism, and narcotism. And they can get all possible helps through our system. This system will be the interactive and familiar way that anyone all over the world can use for overcoming problems.

9.Nucleic Acid Aptamers: New Methods for Selection, Stabilization, and Application in Biomedical Science.

The adoption of oligonucleotide aptamer is well on the rise, serving an ever increasing demand for versatility in biomedical field. Through the SELEX (Systematic Evolution of Ligands by EXponential enrichment), aptamer that can bind to specific target with high affinity and specificity can be obtained. Aptamers are single-stranded nucleic acid molecules that can fold into complex threedimensional structures, forming binding pockets and clefts for the specific recognition and tight binding of any given molecular target. Recently, aptamers have attracted much attention because they not only have all of the advantages of antibodies, but also have unique merits such as thermal stability, ease of synthesis, reversibility, and little immunogenicity. The advent of novel technologies is revolutionizing aptamer applications. Aptamers can be easily modified by various chemical reactions to introduce functional groups and/or nucleotide extensions. They can also be conjugated to therapeutic molecules such as drugs, drug containing carriers, toxins, or photosensitizers. Here, we discuss new SELEX strategies and stabilization methods as well as applications in drug delivery and molecular imaging.

10.Baha Attract Implantation Using a Small Incision: Initial Report of Surgical Technique and Surveillance

Objectives:
. To determine the appropriate anatomical borders of implantation on the temporal bone in a cadaver study, and to develop a simplified surgical technique for Baha Attract implantation through a small incision along the hairline using anatomical evidence and a navigation system.
Methods:
. In a cadaver study, 20 human adult dry skulls were used to find flat areas of the temporal bone for Baha Attract magnet implantation. Four borders of the “optimal surgical site” were defined: Asterion line, occipitomastoid suture line, sigmoid sinus line, and digastric groove line. In three patients, we implanted the Baha Attract according to the newly developed surgical procedure and validated the feasibility of this technique with a navigation system.
Results:
. We identified the appropriate position of the implant on the temporal bone, suggesting a simplified surgical technique for Baha Attract with a small incision. We determined the spot of implantation, and the implants were inserted through a small surgical incision (<2.5 cm) under local anesthesia; the procedure lasted approximately 30 minutes.
Conclusion
. The optimal surgical site of the temporal bone is a safe and easily accessible location for implantation of the Baha Attract.
