The bony spur formation without degenerative change in the lumbar spines in the young age group is quite interesting and not uncommon. This bony spur is different from the degenerative spur in the old age group in shape and mechanism of formation. The authors analyzed 111 patients with such posterior lumbar bony spur for recent 3 years and 9 months. The authors concluded that posterioly located Schmorl's node is a main cause of the bony spur without degenerative change at lumbar region. The authors advocated 'Juvenile spur' as a new disease entity.

We report a case of Ael in a 44-year old woman. The patient s red cells were typed as 0 and her serum had both anti-A and anti-B, but the agglutination strength with Al cell was weaker (2+) than with B cell (4+) in her serum. Additional tests showed that the red cells were not agglutinated by anti-A,B and A antigen on patient s RBC was demonstrated by adsorption-elution test. Her saliva contained H but no A substance, and the ABO genotyping test identified her blood type as AO. We concluded that this was a case of blood type Ael with anti-A. PURPOSE: We have examined the utility of 99mTc-MAG3 perfusion indices for assessing renal graft function in early post-transplantation period. MATERIALS AND METHODS: Our study included 80 renal transplant recipients (48 men and 32 women, mean age: 40.3 years). Diagnosis was based on biopsy, laboratory data and clinical course. Renal scintigraphy (RS) was obtained using 100 MBq of 99mTc-MAG3 from 11 days to 23 days of kidney transplantation. We measured 5 indices in whole-kidney (WK) and cortical (C) renograms; Hilson's perfusion index (PI), transplant perfusion index (TP) and transplant function index (TF) as perfusion parameter, and the time to peak activity (Tmax) and the ratio of renal counts at 20 min to that at 3 min (K20/3) as functional parameter. RESULTS: The diagnoses at the day of RS were normal graft (NG) in 44, acute rejection (AR) in 14, acute tubular necrosis (ATN) in 10, and Cyclosporine A nephrotoxicity (CsA) in 12. TP and TF were significantly decreased in AR, ATN and CsA, compared to those in NG. K20/3 of AR and ATN were significantly greater than that of NG. WK-Tmax of AR was significantly longer than that of NG. K20/3 of AR and C-K20/3 of ATN were significantly prolonged relative to those of CsA. There were no statistically significant perfusion indices among complication groups. CONCLUSION: TP and TF reflecting microperfusion and initial tubular extraction are reliable in assessing graft function. However, it is required to correlate perfusion indices with functional indices and clinical course in differentiating from one another among complication groups.

Wegener's granulomatosis is a systemic necrotizing vasculitis of unknown cause. The disease is characterized by the involvement of the upper airway, the lung, and the kidney. Skin lesions are frequent and the most common lesion is purpura distributed on the limbs and trunk. A 34-year-old female showed recurrent purpuric macules on the both lower extremities and buttocks. The patient. showed nasal septal perforation with saddle nose deformity and C-ANCA positivity. Histopathologic findings of purpuric lesion revealed the features of necrotizing vasculitis. The histologic specimen from the nasal cavity showed chronic inflammation with granuloma formation and kidney showed focal necrotizing glomerulonephritis. Therefore, we treated the patient with prednisolone and cyclophosphamide having diagnosed Wegener's granulomatosis.

We report a case of tuberculosis verrucosa cutis in a 34-year-old male patient who presented with asymptomatic verrucous plaque over the right buttock, right inner thigh and right inguinal area. The Mantoux test revealed a strong positive reaction. The histologic picture consists of hyperkeratosis, acanthosis and papillomatosis overlying an inflammatory infiltrate in the upper dermis. Tuberculoid granulomas without caseation necrosis are seen in the dermis. Tubercle hacilli were not demonstrated. The patient was treated with INAH, Rifampicin and Ethambutol for 6 months. To date, the skin lesions have markedly improved.

Twenty five cases of fractures of the femur & tibia on the same leg (floating knee) in 24 patients were treated in St. Mary's Hospital, Catholic Medical College and Center during the period 1977-1985. We studied all of these patients, divided by four groups according to the methods of treatment, retrospectively with analysis of treatment and end result. Our policy of treatment for these multiple fractures in single extremity was directed toward early weight bearing, active and passive knee exercise by early open reduction and rigid internal fixation for both fractures. The results obtained were as follows: l. Of 25 cases, 20 patients were male and 5 were female. 2. The right lower extremity has constituted 56% of the cases. 3. Average age of patients were 37 years. 4. Most common level of fractures was on middle one third of femur (71.4% ) and proximal one third of tibia (46.2%). Of these 25 cases, three cases of femur and one case of tibia were segmental fractures, and one case of femur and five cases of tibia were open fractures initially. 5. Of 28 cases of femur fracture, 23 were given operative treatment and five conservative method. But out of 26 cases of tibia fracture, each half cases of fracture were treated operatively and conservatively. 6. We analysed the final results of treatment by the time of fracture union roentgenologically and five lower limb functions clinically at the termination of treatment. The average time for fracture union was 20.5 weeks for femur and 21.9 weeks for tibia roentgenologically. The acceptable clinical results could be achieved in 18 cases (72%) of early open reduction and rigid internal fixation for both tibia and femur as early as possible after accident for early knee exercises. 7. The status of fractured tibia was the most important factor to treat these multiple fractures in single extremity because of troublesome to start knee exercise and weight bearing.

The Pumping technique generally has been known as a diagnostic method that predicts intracapsular status and selects required evaluation and treatment. But, in addition to this indication, when it is followed by Lavage and Manipulation technique, this may be used as therapeutic method that is effective on Closed Lock with limited mouth opening and pain. Therefore, we examined results of this technique abut Closed Lock and would like to report therapeutic effects of Lavage and Manipulation technique. We selected 40 patients of 200 patients diagnosed as Closed Lock over 53 months from SEP. 1991 to PEB. 1996, who were possible follow-up and performed by Lavage and Manipulation technique. Appraisal between pre-procedure and post-procedure was performed just after procedure, in 1 month and in 3 months, and therapeutic effects were divided by marked, useful, a little useful and ineffective. As a result, Lavage and Manipulation technique had remarkable increase of maximum mouth opening(>5mm) in 39 persons(98%) just after proceure, 36 persons(90%) in 1 month, and 39 persons(98%) in 3 months, and subsided pain completely just after procedure and had remarkable decrease of pain in 30 persons(75%) in 1 month, 32 persons(80%) in 3 months. So we concluded that the Lavage and Manipulation technique is effective therapy of Closed Lock which can be used with ease.

Thermography is a sensitive and reliable method for diagnosis of radiculopathy. Skin temperature alterations of the involved dermatome named thermatone are diagnostic for sensory root involvement in radiculopathy which can be demonstrated by thermography. Digital infrared thermographic imaging system using computer is development and could measure thermal difference more exactly without hazards or discomforts to patient. Authors present 186 cases of thermographic evaluation in herniated lumbar disc disease by digital infrared thermographic imging system and the results are evaluated with literature review.
