3.Malignant Lymphoma of Mucosa-associated Lymphoid Tissue Arising in the Conjunctiva.

We report two cases of malignant lymphoma of mucosa-associated lymphoid tissue arising in the conjunctiva. Case 1, an 18-year-old girl, showed her left lower conjunctival swelling for four months and Case 2, a 29-year-old man, showed similar lesions on both his eyes for one year. Histologically, both cases revealed monomorphous dense lymphoid infiltrates invading the overlying conjunctival epithelium with diffuse patterns. The infiltrates were composed of small lymphocytes admixed with centrocyte-like cells within the conjunctival epithelium and stroma in association with B-cell lineage. No relapse was noted during the period of about 1½ years follow-up in case 1 treated by local excision only and in the follow-up period of 2 months in case 2 treated by local excision and subsequent chemotherapy.

4.The Structural Properties of Israeli Carp ( Cyprinus carpio L. ) Immunoglobulin.

Serum immunoglobulins from Israeli carp (I. carp) were purified using affinity chromatography. Fish were immunized with purified mouse IgG, and the specific fish antibodies purified from the immune serum on a mouse IgG-immobilized agarose gel. Rabbit anti-carp Ig (Raclg) antibodies were produced following hyperimmunization with mlgG specific I. Carp Ab. SDS-PAGE analysis under reducing condition showed that I. carp Ig (clg) were composed of two u-like heavy chains with about 82 and 50 kD, respectively and one light chain with about 25 kD. On immunoblotting analysis, however, Raclg failed to react with light chain. When both protein A and protein G purified normal clg were compared with mlgG specific clg, no significant structural differences among them were observed. To investigate if there is any homology between other fish Ig molecules, cross-reactivity of Raclg against Ig molecules from 6 different fish sera and mouse control serum was checked on immunoblotting analysis. As a result, Raclg responded to only carp and tilapia Ig molecules, indicating that both tilapia and carp are very closely associated, especially, in the genetic basis of immunoglobulin structure. In flow cytometry study, Raclg appeared to recognize 45.8% of carp Ig+, 14.5% of catfish Ig+ and <5% of tilapia Ig+ cells. The result suggest the heterogeniety between receptor immunoglobulins on B-like lymphocytes and soluble immunoglobulins in serum. It is crucial to obtain pure fish immunoglobulins to produce reagent antibodies as tools for the study of their specific immune response.

5.Phospholipase A2 Contributes to Hemorrhage-induced Acute Lung Injury Through Neutrophilic Respiratory Burst.

BACKGROUND: The present study was carried out in association with neutrophilic respiratory burst in the lung in order to clarify the pathogenesis of acute respiratory distress syndrome(ARDS) following acute severe hemorrhage. Because oxidative stress has been suggested as one of the principal factors causing tissue injury, the role of free radicals from neutrophils was assessed in acute hemorrhage-induced lung injury. METHOD: In Sprague-Dawley rats, hemorrhagic shock was induced by withdrawing blood(20 ml/kg of B.W) for 5 min and the hypotensive state was sustained for 60 min. To determine the mechanism and role of oxidative stress associated with phospholipase A2(PLA2) by neutrophils, the level of lung leakage, pulmonary myeloperoxidase(MPO), and the pulmonary PLA2 were measured. In addition, the production of free radicals was assessed in isolated neutrophils by cytochemical electron microscopy in the lung. RESULTS: In hypotensive shock-induced acute lung injury, the pulmonary MPO, the level of lung leakage and the production of free radicals were higher. The inhibition of PLA2 with mepacrine decreased the pulmonary MPO, level of lung leakage and the production of free radicals from neutrophils. CONCLUSION: A. neutrophilic respiratory burst is responsible for the oxidative stress causing acute lung injury followed by acute, severe hemorrhage. PLA2 activation is the principal cause of this oxidative stress.

6.Modified Trajectory of C2 Laminar Screw - Double Bicortical Purchase of the Inferiorly Crossing Screw.

The crossing laminar screw fixation might be the most recently developed approach among various fixation techniques for C2. The new construct has stability comparable to transarticular or transpedicular screw fixation without risk of vertebral artery injury. Quantitative anatomical studies about C2 vertebra suggest significant variation in the thickness of C2 lamina as well as cross sectional area of junction of lamina and spinous process. We present an elderly patient who underwent an occipito-cervical stabilization incorporating crossed C2 laminar screw fixation. We preoperatively recognized that she had low profiles of C2 lamina, and thus made a modification of trajectory for the inferiorly crossing screw. We introduce a simple modification of crossing C2 laminar screw technique to improve stability in patients with low laminar profiles.

8.Multifocal Electroretinography in Retinitis Pigmentosa With Non-recordable Full-field Electroretinography.

PURPOSE: To investigate the clinical potential of multifocal electroretinography (mf-ERG) for the evaluation of residual retinal function in retinitis pigmentosa (RP). METHODS: From a group of 41 patients with RP examined for full-field and mf-ERGs, 24 patients were selected in whom the full-field ERGs were non-detectable. Data from the mf-ERGs of this group were reviewed retrospectively. Patients were divided into two groups based on best corrected visual acuity (BCVA); Group A, with a BCVA of less than 0.5, and group B with a BCVA greater than 0.5. The amplitudes and implicit times of Ring 1 and Ring 2 among six concentric rings were measured for comparison between mf-ERG groups. RESULTS: In the mf-ERGs, the amplitudes of Ring 1 and Ring 2 were reduced and implicit time was delayed in all 48 eyes of 24 patients in whom scotopic and photopic responses were non-detectable in full-field ERGs, whereas, there were no detectable responses in Rings 3 thru 6. There was a greater reduction in amplitude and an increased delay in implicit time in group A. CONCLUSIONS: The mf-ERG can estimate the residual macular function in a group of patients with RP in whom the full-field ERG is non-recordable.

9.Forensic Pathological Review of Acute Myocarditis.

Cardiovascular disease is the most important cause of unexpected sudden death. Although ischemic heart disease by coronary atherosclerosis is the most common cause of sudden cardiac death, other diseases such as cardiomyopathy, congenital heart disease, valvular heart disease and rarely myocarditis can produce sudden cardiac death. Myocarditis is an inflammatory process of the myocardium characterized by an interstitial inflammatory infiltrate and an injury to myocytes adjacent to the inflammatory cells. The clinical expression of myocarditis ranges from the asymptomatic state to fulminant fatal congestive heart failure. Because myocardial involvement is subclinical in most acute infectious disease, the majority of patients have no specific complaints referable to the cardiovascular system. It is possible that it causes a medicolegal problems in case of sudden death. Therefore, author reviewed the clinical and pathological findings of 18 myocarditis autopsy cases. The victims 'age was varied 4-month-59-year olds. 5 (28%) were male and 13 (72%) were female. 15 cases received medical treatment due to clinical symptoms(URI symptoms: 2 cases, tonsillitis and tonsillar abcess: 3 cases, GI symptoms: 5 cases, URI & GI symptoms: 3 cases, pneumonia: 1 case, operation due to ovarian tumor: 1 case) and were associated with medicolegal problem. By the results of this study, important gross findings were pericardial effusions, characteristic cardiomegaly, and pulmonary edema.
