1.A case of acral persisitent papula mucinosis.

We report a case of acral persistent papular mucinosis in a 5 year-old female with multiple papules on both dorsa of the hands, wrists and forearms. Examination of a biopsy specimen showed a domeshaped lesion in the dermis composed of spindle-shaped fibrobla,sts and widely spaced collagen fibers. These spaces were filled with mucinous material, as demonstrated by positive staining with aleian blue at pH 2.5 and pH 0.5.

2.Three cases of benign symmetric lipomatosis.

Benign symmetric lipomat,osis is a rare disease of undetermined cause characterized by symmetric deposits of adipose tissue o:n the neck, upper trunk and abdomen. We report three cases of kienign symmetric lipomatosis, one of which is associated with alcoholic liver disease and the patient complained of a diffieulties in neck motion, All of them are about sixty year old men with history of frequent alcohol consumption. In histopathologic findings, the reticular dermis was completcly replaced by mature fat cells and there was a slight increase in vascular and fibrous elements.

3.Histologic Changes of Normal Human Skin in Organ Culture.

Since adult human skin can be grown in chernically defined medium without serum, the skin organ culture has gained a great interest as a method for studies concerning skin biology, pharmacology and toxicology. however, serum supplementation has extensively been used to improve the viahility of tissue culture. This study was undertaken to evaluate the effect of serum on the histologic changes ohserved during the organ culture of the normal human skin. The general architecture of the skin was well maintained for 6 days with or without seru. After then, fetal calf serum or autologous human serum was found to enhance the viability of the epidermis. A confluent layer of necrotic spinous ceils was ovserved earlier and more widespread without serum. The addition of serum had an impressive effect on epibolization. In the absenee of serum, the formation of the epibolus was not only minimal, but also, susceptible to degeneration, and no epibolus remained at 10 days rif incubation. No difference can be found between fetal calf serm and autologous human serum in the formation of the epibolus. There was no favorable effect of serum on the formation of new stratum corneum. The thickness of new straturn corneum increased in parallel with the number of parakeratatic cells, increasing most rapidly between 6 and 8 days of incubation. Parakeratosis was more prominent in the presence of serurn.

4.A Mycologic Study from the Fourth Toe Web of the High School Studyent.

Exogenoue exposurea to pathogenic fungi have been suggested as causes of acute atta.eks of tinea pedis. Psthogenic fungi is rarely present on the skin aurfaces without cauaing definite and characteristic reactions. However, it was suspected by eome observers that pathogenic fungi might be more or liesa normal inhabitant of the human skin. Alterations in host susceptibility and loss, of local immunity are more to blame than new exposure to exogenous micro-ganisms. To determine this point, fungal culture was done from the fourth toe web of 824 high school students. The results of the study were summarized as follows,' l. Among 824 cases, 780 cases(94.7%) had grossly normal appearing toe web and 44 cases(5,3%) were suspected superficial fungal infection. 2. Positive rate of KOH examination was 4.6%(36,'780) in normal toe web and 45.4%(20/44) in dermatophytic lesion. 3, The positive rate of fungal culture from normal and infected toe web are 7.9 %(62/780) a.nd 40.9%(18/44), respectively. In the normal toe web, Trichophyton rubrum(66.1%), T, entagrophytea(include T. interdigitaLe) and Epidermophyton, floccosum(3.2%) were isolated. In the dermatophytic lesion, T. rubrurn(88.8%) and T. me ntagrophytes(11.2% ) w ere isolated. Although toe webs do not present clinical symptoms and signs, pathogenic fungi as a normal inhabitant can be isolated from clinically normal toe webs. Some of these cases, therefore, will develope to tinea pedis in time.

6.Research of Malpractice in Skin Diseases.

Among the patients visting Department of Dermatology, Chungman National University, 58 cases had various side effects due to the malpractice of skin disorders. We analysed these 58 cases and the results can be summerized as follows ; 1. 37 cases(64%) were in the second and third decades and 11 cases(19%) were under 19 years-old. 2. Drug medication from pharmacy was the most common(79%), and then local clinics(12%), herb clinics(9%) in decreasing orders. 3. In the route of administration, oral ingestion(38%) was the most common, and 47 cases (81%) were medicated with adrenocorticoids. 4. In motivation to the visit, 31 cases(53%)were by advice, 15 cases by advertizement and 12 cases (21%) by themselves. 5. Acne and psoriasis were about a half of pre-existing skin diseases. 6. Side effects due to malpractice may be divided into 5 items : systemic side effects due to adrenocorticoids (59%), aggravation of pre-existing skin diseases (22%), topical side effects due to adrenocorticoids (10%), chemical irritation and burn (7%), and mercury intoxication (2%) in decreasing orders.

7.The Histologic Study of the Neoendothelialization of Polytetrafluoroethylene as an Arterial Substitute in Rabbit.

The purpose of this study was to investigate the patency rates and the histologic findings of neoen- dothelialization according to the length of implanted polytetrafluoroethylene (PTFE) with an internal diameter of 3 mm. Under the operating microscope, grafts of 8 and 24 mm in length were implanted in the right carotid arteries of thirty rabbits by interrupted end-to-end microanastomosis. They were divided into two groups according to the length of implanted PTFE. Each group compised fifteen rabbits. All implanted grafts were 25 pm in fibril length and 0.39 mm in wall thickness. Three grafts per group were harvested at 1, 2, 4, 8, and 12 weeks after implantation respectively and all grafts were observed for patency rates and the histologic findings with light microscope and scanning electron microscope. In conclusion, there was no difference in patency rates according to the length of implanted PTFE and histologically the formation of neointima and subintimal tissue was delayed and incomplete in longer implanted PTFE.

9.Leg Length Equalization by Correction of Pelvic Obliquity and Acetabular Dysplasia

Fixed pelvic obliquity refers to a composite deformity induced by contractures both above and below the pelvis and the elements of this deformity are frequently interrelated during the period of growth. From the functional standpoint, leg length discrepancy is caused by deformities of the pelvis and lower extremities, such as pelvic obliquity and acetabular dysplasia, as well as by inequality of true limb bone length, and these deformities either aggravate or compensate functional discrepancy. During fhe fourteen years period, from August 1968 to August 1982, at the Department of Orthopaedic Surgery, Seoul National University Hospital, we treated 35 cases of fixed pelvic obliquity and acetabular dysplasia associated with true or functional limb length discrepancy by means of lumbodorsal fasciotomy or pelvic osteotomies such as Salters innominate osteotomy or Steels triple osteotomy, combined, if necessary, with contralateral abductor fasciotomy to gain functional limb length as well as to improve posture and balance. In many cases of residual poliomyelitis, epiphysiodesis was also performed when indicated. These cases were reviewed and following observations were made: 1. Of the 35 cases, residual poliomyelitis with 29 cases (83%) was by far the main cause of leg length discrepancy. Cerebral palsy (2cases), Legg-Perthes disease (2 cases), and fibrous ankylosis secondary to septic hip (2 cases) comprised the remainder. 2. The male-to-female ratio was about equal, being 17 to 18. 3. The average age at the time of operation was 17.9 years, the youngest being 7 years and the oldest being 30 years. The average age at the time of current follow-up was 18.8 years. 67% of those followed was skeletally mature. 4. An average of 1.35cm of bone length was gained radiographically by pelvic osteotomies. Steels triple osteotomy was more effective in gain than Salters innominate osteotomy. 5. An average of 2.43cm of functional length when standing was gained radiographically by lumbodorsal fasciotomy alone. 6. An average of 2.61cm of functional length when standing was gained radiographically by lumbodorsal fasciotomy and combined contralateral Soutters or Campbells fasciotomy. 7. An average of 3.57cm of functional length when standing was gained radiographically by lumbodorsal fasciotomy and combined ipsilateral Steel's triple osteotomy. 8. An average of 1.73cm of functional lengthening when standing was corrected radiographically by ipsilateral Soutter's fasciotomy. 9. When lumbodorsal fasciotomy and Steel's triple osteotomy were combined with contralateral Soutter's or Campbell's fasciotomy, the average radiographic gain in standing length was 3.77cm. 10. Leg length discrepancy in terms of true bone length is conventionally corrected either by epiphysiodesis or bone shortening on the longer limb, or by bone lengthening on the shorter limb. We believe that when leg length discrepancy is associated with fixed pelvic obliquity, frequently aggravating the disability functionally, lumbodorsal fasciotomy and/or pelvic osteotomies on the shorter side and, combined if necessary, Soutters or Campbells fasciotomy on the longer side, can, in many instances, successfully correct or reduce functional limb discrepancy and improve balance, posture and function. Any residual discrepancy, true or functional, may then be corrected by conventional methods.

10.Changes in Blood Superoxide Dismutase Activities after Alcohol Withdrawal.

OBJECTIVES: The aims of this study were to evaluate changes in plasma superoxide dismutase(SOD) activities in alcohol depedence, to fine out variables to influence on the SOD activities, and finally to identify the correlation of SOD activities with the alcohol-associated cognitive disorders. METHODS: For 24 male alcoholics and 21 healthy male controls, plasma SOD activities were measured by spectrophotometry on 1-2 wks after alcohol withdrawal. Structured interviews and laboratory tests were also performed. RESULTS: 1) Upon comparing SOD activities between controls and alcoholics, the SOD activities were significantly(p<0.01) lower in alcoholics(0.308+/-0.140 units/mL) than in healthy controls(0.313+/-0.086 units/mL). 2) Upon comparing SOD activities according to the presence of alcohol-related cognitive disorders, the SOD activities were significantly(p<0.05) lower in alcoholics with cognitive disorders(0.247+/-0.049 units/mL) than in alcoholics without cognitive disorders(0.317+/-0.148 units/mL). 3) Upon comparing SOD activities according to the presence of alcoholic polyneuropathy or alcohol withdrawal seizure, the SOD activities showed no significant differences between alcoholics with polyneuropathy or epilepsy and those without. 4) Upon analyzing variables influencing on the SOD activities in alcoholics, the SOD activities had the negative correlation with hemoglobin(gamma=-0.433) and severity of alcohol withdrawal symptoms(gamma=-0.375). 5) Upon comparing variables according to the presence of alcohol-related cognitive disorders, the occurrence of alcoholic polyneuropathy(p<0.05) and blood phosphorus concentrations(p<0.01) were significantly higher in alcoholics with cognitive disorders than those without. 6) Upon analyzing an association between SOD activities and variables in alcoholics with cognitive disorders, the SOD activities were positively correlated with the onset age(gamma=0.995), and negatively correlated with the severity of alcohol withdrawal symptoms(gamma=-0.996). CONCLUSIONS: Lower SOD activities in alcohol dependence suggested alcohol-associated cognitive disorders and alcohol withdrawal symptoms might be caused by oxidative stress.
