Splints

Reactive perforating collagenosis, first described by Mehregan et al in ]967, is a skin disorder characterized by transepidermal eliniination of altered collagen fibers. This rare disease appears as recurrent, umbilicated, crusted papules in response to minor trauma. We report here a case of reactive perforating collagenosis in a 21-year-old man. Examination of the skin revealed a number of centrally umbilicated, asymptomatic papules filled with keratotic plugs in various development and regression. The lesions were distributed on the dorsa of the hands, face, and neck. The biopsy spccimen showed a few of thin bundles of collagen which v ere extruded from the dermis through the atrophic epidermis and the turinels within the epidermis, reaching the buttom of the plug.

Treatment modalities of gastric variceal bleeding are endoscopic injection sclerotherapy, endoscopic variceal ligation, combined above two method, operation, TIPS, and percutaneous transhepatic embolization. Recently, Histoacryl was introduced as a new sclerosant of acute variceal bleeding failed by others. So, we studied 18 cases of acute gastric variceal bleeding uncontrolled by EIS using Ethanolamine oleate. Intravariceal injection of Histoacryl is considered as a safe and effective method for hemostasis of intractable acute gastric variceal bleeding uncontrolled by other sclerosants. But, it is important that endoscopist should be in harmony with assistant at injection of Histoacryl. And its shooting speed should be fast because of clogging within the catheter. It is necessary to do long term study for evaluation of eradication effect in acute gastric variceal bleeding.

We report a case of Ael in a 44-year old woman. The patient s red cells were typed as 0 and her serum had both anti-A and anti-B, but the agglutination strength with Al cell was weaker (2+) than with B cell (4+) in her serum. Additional tests showed that the red cells were not agglutinated by anti-A,B and A antigen on patient s RBC was demonstrated by adsorption-elution test. Her saliva contained H but no A substance, and the ABO genotyping test identified her blood type as AO. We concluded that this was a case of blood type Ael with anti-A. BACKGROUND: Respiratory syncytial virus (RSV) is the most common cause of lower respiratory infection in infants and young children, but the pathogenesis of RSV-induced inflammation is not well defined. MATERIAL AND METHOD: In order to examine the potential interactions between virus-infected airway epithelial cells and neutrophils, we studied the ability of neutrophils to adhere to yirus-infected airway epithelial cell monolayers by myeloperoxidase assay. Also we measured the ability of airway epithelial cells to secrete interleukin-8(IL-8) and inter-cellular adhesion molecule-1(ICAM-1) in virus-infected airway epithelial cell cultures by enzyme-linked immunosorbent assay(ELISA). The degree of IL-8 and ICAM-1 gene expression in the RSV-infected BEAS-2B cell cultures were analyzed by reverse transcription-polymerase chain reaction(RT-PCR). RESULTS: The RSV-infected BEAS-2B cell resulted in significantly enhanced level of neutrophil adherence compared to the uninfected control(p (0.001). IL-8 and ICAM-1 production significantly increased by RSV infection(p<0.05). There was a significant positive correlation between neutrophil adherence and IL-8 level(r=0.73, p=0.002), and ICAM-1 level (r=0.843, p=0.001) in RSV-infected cells. The degree of both IL-8 and ICAM-1 mRNA expression increased in the RSV-infected cells compared with the uninfected ones. CONCLUSION: These results suggest that RSV infection significantly enhances the production of IL-8 and ICAM-1 in airway epithelial cells which then results in increased neutrophil adherence.

A 26-year-old man presented with a bluish speckled patch on the dorsum of the left hand. It had apperaed at birth as a bluish spot. Since adolescence the lesion extended and at the time of visiting hospital it covered the most part of the dorsum of the left hand and even some palmar aspect. The histopathological and ultrastructural examination revealed numerous dermal melanocytes, mainly in the upper dermis. Clinically and pathologically, the patient was diagnosed as having dermal melanocyte hamartoma, a distinct type of dermal melanocytosis.
