1.Therapeutic result of lavage and manipulation technique for closed lock cases of the TMJ.

The Pumping technique generally has been known as a diagnostic method that predicts intracapsular status and selects required evaluation and treatment. But, in addition to this indication, when it is followed by Lavage and Manipulation technique, this may be used as therapeutic method that is effective on Closed Lock with limited mouth opening and pain. Therefore, we examined results of this technique abut Closed Lock and would like to report therapeutic effects of Lavage and Manipulation technique. We selected 40 patients of 200 patients diagnosed as Closed Lock over 53 months from SEP. 1991 to PEB. 1996, who were possible follow-up and performed by Lavage and Manipulation technique. Appraisal between pre-procedure and post-procedure was performed just after procedure, in 1 month and in 3 months, and therapeutic effects were divided by marked, useful, a little useful and ineffective. As a result, Lavage and Manipulation technique had remarkable increase of maximum mouth opening(>5mm) in 39 persons(98%) just after proceure, 36 persons(90%) in 1 month, and 39 persons(98%) in 3 months, and subsided pain completely just after procedure and had remarkable decrease of pain in 30 persons(75%) in 1 month, 32 persons(80%) in 3 months. So we concluded that the Lavage and Manipulation technique is effective therapy of Closed Lock which can be used with ease.

4.Reliability of Singh Index with the Dual Photon Absorptiometry

Osteoporosis is a metabolic bone disease characterized by decrease of bone matrix, resulting in generally reduced bone mass, and the diagnosis and grading of progression are assessed by many methods. There are Singh Index as a simplest method, for grading the trabecular pattern of the femur in pelvis anteroposterior X-ray, and the recently developed Dual Photon Absorptiometry which measures the bone mineral contents in the lumbar spine(L2-L4), femur neck, Ward's triangle and trochanteric area. Study for correlation between the Singh index and bone mineral contents measurements by Dual Photon Absorptiometry was performed in 36 normal Korean persons and 35 persons with osteoporosis. The results and conclusion were as follows; 1. A significant difference in bone mineral density at each area was noted between the control group and osteoporosis group. 2. A significant difference in Singh Index at each area was noted between the control group and osteoporosis group. 3. Positive correlation was noted between the mineral density and Singh Index. 4. We think Singh Index is a useful method in the diagnosis and grading of progression of osteoporosis.

6.Esophageal Cancer Metastasis to the Kidney.

We report a case of 54-year-old man who presented with a right renal tumor secondary to an esophageal cancer, which was detected during a periodic follow-up computerized tomographic examination. In December 2000, the patient underwent a transhiatal esophagectomy with a cervical esophagogastrostomy and a histological examination showed an infiltrative squamous carcinoma (T3N1M0, stage III). The patient was treated with 6 cycles of adjuvant chemotherapy with 5-FU (500 mg/m2) and cisplatinum (40 mg/m2). Subsequently, a follow-up study revealed no evidence of recurrence until December 2001. In June 2002, he attended a follow-up examination and complained of general weakness and a 3 kg weight loss over a month. CT scan detected a hypodense space occupying lesion in the inferior pole of the right kidney and enlarged aortocaval lymph nodes below the right renal vein. Intravenous pyelography showed a phantom calyx in the inferior pole after a contrast infusion. The cystoscopic examination was negative. He underwent a right nephrectomy and a periaortic lymph node dissection under the impression of a metachronous transitional cell carcinoma. However, a histological examination revealed that the tumor was a metastatic squamous carcinoma with lymph nodes involvement from the previous esophageal squamous carcinoma.

7.Effects of 5-fluorouracil on culture of Bovine Retinal Pigment Epithelial cells.

Inspite of technical advances, long term retinal reattachment of proliferative vitreoretinopathy following the successful surgery is often disturbed by the occurrence of cell proliferation and traction. A variety of antimetabolite or anticancer drugs have been tried to use for testing their antiproliferative effects on proliferative vitreoretinopathy. To evaluate the antiproliferative property of 5-FU, we treated with the different concentration of 5-FU(0.5, 1, 5, 10 mg/L) on cultured bovine retinal pigment epithelial cells and performed dose inhibition studies. We also examined the 5-FU treated bovine retinal pigment epithelial cells by inverted microscope daily to evaluate the morphologic change for 5 days. The results were as follows; 1. 5-FU inhibited proliferation of bovine pigment epithelial cells in a dose dependent and a time dependent manner. 2. The drug concentration required for 50% inhibition of cell growth(ID50) was found to be 1.87 mg/L. 3. As the drug concentration increased, the cell density gradually decreased and the degenerative changes such as irregular shape of cells, cytoplasmic vaculoes and cell debris were aggravated. In 10 mg/L of 5-FU treated well, disruption of cell membrane and cell death were noted from the first day of incubation. These results show that 5-FU has a potent antiprolifertive effects in retinal pigment epithelial cell and toxic effet in high dose.

8.Total Excision of an Arterio-Venous Malformation of the Corpus Callosum: Case Report.

The arteriovenous malformations(AVMs) of the corpus callosum are distinct anatomical and surgical entities. The AVMs of the corpus callosum are supplied by the pericallosal arteries if the malformations are located in the anterior and middle portion of the callosum and the AVMs of the splenium are usually fed by the pericallosal and posterior cerebral arteries. Radical excision of the callosal AVMs became successful with microsurgical techniques since a few years ago. The authors report a case of the callosal AVM, who had subarachnoid hemorrhage twice prior to this admission, and the studies with angiography and computed tomography revealed an AVM involing the middle portions of the corpus callosum. Radical excision of the AVM was accomplished by microsurgical techniques without neurologic sequela.

10.Seroconversion in Nonresponders to Hepatitis B vaccination after double dose vaccination only once.

BACKGROUND: Korea is, known to be an endemic area of viral hepatitis B, and 5-6% of population are carriers. Immunization can reduce the spread of hepatitis B infection. Hower, not all people respond to hepatitis B immunsation. 5-15% of primary vaccinees fail to be immunized. After revaccination, only 37.5 44% of the primary failures elicit antibody response. Recent studies have reported that it is effective to inject a double dose vaccination in immunocompromsed patients. In some country, they recommend that neonate whode mother is HBV carrier shoud receive double dose vaccination. This study documented the results of double dose vaccintion only once to nonrsponders toward the HBV immunzation. METHODS: Healthy nonresponders to Hepatitis B immunization were recuited from Sep. 1995 to Aug. 1996. Subjects with their AST/ALT level over 40, or over 65 years old were excluded. They were received 2ml of plasma-derived vaccine(Hepavax-B) intramuscularly in the deltoid muscle. Anti-HBs antibody testing were performed at 1 to 3 months after vaccintion. RESULTS: 17 healthy nanreponders to Hepatitis B immunization(male 10, female 7) were included. Their mean ages were 47(32 to64), AST/ALT level 20/19(16 to 28/11 to 35), smoking 4.3pys(1 to 7). Seroconversion occured in 13 of them(76.5%). CONCLUSIONS: Nonresponders to Hepatitis B vaccintion respond well to double dose vaccination once.
