1.The Genetic and Clinical Studies of Ichthyoses.

This study was attempted mainly to estimate the frequency of ichthyoses and its genetic characteristics among general population in Chonnam Province. One hundredand thirtyone subjects heing comprised of 121 with ichthyosis vulgaris, 6 with X-linked recessive ichthyosis and 4 with lamellar ichthyosis visited the department of Dermatology, Chonnam National University Hospital from 1971 through 1980. All except 7 patients had visited the hospital with chief complaints of a skin problem other than ichthyoses itself. The frequency of ichthyosis vulgaris seems to be somewhere between, 1/300 and 1/400. 86% were found to either be sporadic cases or come from families from which not enough information was obtained to define a mode of inheritance. The fact that 43% of the sibling of affected patients developed ichthyosis vulgaris points out that it is a highly penetrant autosomal dominant, disease. Skipped generation was observed in 2 families, and gonadal mosaicism in 1 family. Atopic diseaaes were found only in 12.0% of cnses of ichthyosis vulgaris. The disease appeared in 81% of cases before 1 year af age. The skin lesions of ichthyoses were developed on the lower limhs, back, upper limbs, abdomen and scalp in the order of frequency.

2.A Study on Serum Zinc Concentration in some Methamphetamine Abusers.

The author investigated the zinc concentrations in sera of 110 methamphetaimine abusers (male 87, female 23) and of 41 healthy official workers(male 29, female 12) using inductively coupled plasma emission spectrometer. The serum zinc concentrations were analysed according to hepatitis C virus antibody(Anti HCV), hepatitis B surface antigen(HBsAg) and alanine aminotransferase (ALT) value in both groups. The analytical results were obtained as follows; 1. The mean zinc concentration of MAP abuser group(298.5+/-246.9 microgram/100 microliter) was 2.5 times higher than that of control group(118.8+/-25.4 microgram/100 microliter). 2. The distribution of zinc concentration in both groups were skewed pattern. 3. The zinc concentration were higher in Anti HCV-positive group and abnormal ALT value group than Anti HCV-negative group and normal ALT value group. But HBs-Ag positive group had lower zinc concentration than HBs-Ag negative group. 4. The number of anti HCV-positive subjects was increased and the zinc concentration were decreased in proportion to the abusing duration of MAP.

3.Socio-Psychological Analysis and Rate of Syphilis Infection in Prostitutes, Surrounding the U. S. Air Force in Kunsan.

Following the Korean War, the presence of foreign troops in Korea and lowered. sexual morality caused an increase in the rate of prostitution and related venereal diseases. Though the health authorities made efforts to handle these problems, they didnt obtain any noticeable results, because they neglected the scientific prevention and treatment based on expert knowledge, and the sociopsychological problem, which was the basic cause of the increment in the rate of prostitution. They only took administrative action against the prostitutes. Therefore, in July, 1976, we went to a town(named Silver Town) located on the outskirts of a U.S. Air Force base, and, examined and guestioned a number of prostitutes. Each of the 288 subjects residing in the town was given a questionaire that dealt with individual living attitudes, family and social background, and sociopsychological aspects of tbeir lifestyle. Questionaires were completed anonymously. Serological tests for syphilis, the UDRL and TPHA, were also administered. The results of our atudy are as follows. 1) Most of the prostitutes had grown up in large families, in which females were predominant. Because of economic difficulties they dropped out of school a.t an early age and became prostitutes. The average age of a prostitute at the time of her first sexual experience was 19. 9 years, the expriences usually taking place with her first boyfriend. Forty six psrcent of tbe subjects had intercourse for the first time between the ages of 15 and 19. Fortynine & two tenths percent of them because prostitutes within one year of their initial sexual act, and all of them at an average interval of 2.5 years following the first sexual experience, the average age being 22. 4 years. 2) The longer the women had worked as prostitutes, the more they felt dissatisfied with their work, and the more they desired to change to an occupation which was healthier and entailed more self-respect. 3) Out of 283 subjects, 39% had a history of venereal disease, predominantly of gonorrhea. Tbe results of the STS administered to all subjects were,; VDRL 7.7% positive and TPHA 6% positive. It was also concluded that the subjects had no educational or working knowledge of venereal diseases.

6.Role of local government in responding to environmental health challenges: a case study of Chungnam

The purpose of this study is to introduce the establishment process, policy target, and projects for “Chungnam’s master plan on environmental health policy (2017-2020)” as the local government’s role in addressing local environmental health challenges. We first analyzed existing studies and social issues on the media related to “Chungnam’s master plan” to understand Chungnam’s environmental health status and discussed domestic and international policy trends and related plans. An environmental health perception questionnaire survey and a Delphi expert questionnaire survey were conducted among provincial residents to collect various actors’ opinions on Chungnam’s environmental health issues and policy. An expert advisory panel was launched, and a residents’ voice workshop and cities-and-guns-policy-suggestion workshop were held. The vision of Chungnam’s environmental health policy is minimizing environmental hazards. We finally selected “Pleasant environment, healthy people, happy Chungnam” to represent the will to shape a pleasant environment and prevent and manage health damages for a happy Chungnam. We selected five strategies based on status analysis and a review of domestic and international policy trends and related plans and identified 2 targets (policy objectives) to accomplish the strategies. The strategies to achieve the first target, “Leader in environmental health policy: Chungnam,” include ‘Empowering active provincial capabilities,’ ‘Setting up province-specific systems for environmental health surveys and research,’ and ‘Preventing and managing newly emerging pollutants.’ The strategies for the second target, “Everyone is healthy: Chungnam,” include ‘Relieving health inequalities among vulnerable regions and residents’ and ‘Enlarging the resident-friendly environmental health policy.’ We developed 29 projects in total, according to these strategies. The establishment of “Chungnam’s master plan” is highly valuable; we developed it through discussion involving diverse actors to address environmental health challenges together. It is necessary to continue to strengthen participation, communication, and cooperation among actors to develop an environmental health policy model for the future.

7.Is β-Lactam Plus Macrolide More Effective than β-Lactam Plus Fluoroquinolone among Patients with Severe Community-Acquired Pneumonia?: a Systemic Review and Meta-Analysis.

Adding either macrolide or fluoroquinolone (FQ) to β-lactam has been recommended for patients with severe community-acquired pneumonia (CAP). However, due to the limited evidence available, there is a question as to the superiority of the two combination therapies. The MEDLINE, EMBASE, Cochrane Central Register, Scopus, and Web of Science databases were searched for systematic review and meta-analysis. A total of eight trials were analyzed. The total number of patients in the β-lactam plus macrolide (BL-M) and β-lactam plus fluoroquinolone (BL-F) groups was 2,273 and 1,600, respectively. Overall mortality of the BL-M group was lower than that of the BL-F group (19.4% vs. 26.8%), which showed statistical significance (odds ratio [OR], 0.68; 95% confidence interval [CI], 0.49 to 0.94; P = 0.02). Length of hospital stay was reduced in the BL-M group compared to the BL-F group (mean difference, −3.05 days; 95% CI, −6.01 to −0.09; P = 0.04). However, there was no significant difference in length of intensive care unit (ICU) stay between the two groups. Among patients with severe CAP, BL-M therapy may better reduce overall mortality and length of hospital stay than BL-F therapy. However, we could not elicit strong conclusions from the available trials due to high risk of bias and methodological limitations.

8.Effect of Hydrogen Peroxide-induced Oxidative Stress on the Senescence of Trabecular Meshwork Cells.

PURPOSE: To investigate the effects of oxidative stress on the senescence of trabecular meshwork (TM) cells and the effect of L-ascorbic acid (LAA) against oxidative stress-induced senescence. METHODS: Primary cultured human TM cells were exposed to 0.05 or 0.1 mM hydrogen peroxide for 30 minutes and incubated for 1 week with or without co-exposure of LAA. Cellular survival, nitrite production, and senescence were assessed with MTT, Griess, and SA-beta-gal assays, respectively. RESULTS: Hydrogen peroxide decreased cellular survival and NO production accompanied increased cellular senescence. LAA did not prevent hydrogen peroxide-induced senescence. CONCLUSIONS: Oxidative stress-induced senescence of TM cells may be related to the dysfunction of trabecular meshwork in glaucoma.

9.The Comparison of Analgesic and Hemodynamic effects in Different Nitrous Oxide Concentration.

BACKGROUND: Nitrous oxide is a safe analgesics which has analgesic and sedative effects without loss of consciousness. The advantage of Nitrous oxide is that it has a rapid onset time and that we can modulate duration of its action. The purpose of this experiment is to obtain the safe and effective concentration of Nitrous oxide in Emergency Department as use of analgesics and sedative drugs. METHODS: Twenty volunteers were investigated according to double blind method. Nitrous oxide was administrated to volunteer subjects at different concentrations ; 33%, 50%, 67% each. Then we evaluated the Pain score, Blood pressure, Heart rate, Respiratory rate, Oxygen saturation and its side effects. RESULTS: Pain Score were 8.4+/-2.1 at 33%, 6.1+/-1.5 at 50%, and 3.65+/-2.2 at 67% of Nitrous oxide. Pain Score is decreased significantly following administration of Nitrous oxide at concentration of 50%, 67%(P<0.05). There was no difference between preadministration group and postadministration group. While no remarkable change in systolic blood pressure, heart rate, and oxygen saturation. Respiratory rate showed significant increase when 679o of Nitrous oxide was administrated. There was no side effect in administration of 33% of Nitrous oxide. When 50% of Nitrous oxide was administrated, there were dizziness 5(25%), paresthesia 2(10%), nausea 1(5%), palpitation 1(5%) and hyperethesia 1(5%). At 679o of Nitrous oxide, there were nausea 7(35%), dizziness 6(30%), paresthesia 5(25%), headache 5(25%), diplopia 2(10%), dysphoria 2(10%), salivation 1(5%), dyspnea 1(5%), nystagmus 1(5%), tinnitus 1(5%) and euphoria1 (5%). Incidence of side effect increased as the concentration of Nitrous oxide was increased. CONCLUSION: We can conclude that safe and effective concentration of Nitrous oxide is 50% as analgesics and sedative drugs in the emergency department.

10.Intermittent Parathyroid Hormone Treatment for Stimulation of Callus Formation in Elderly Patients.

PURPOSE: The purpose of this study was to evaluate the effect of parathyroid hormone (PTH) on fracture healing in elderly patients. MATERIALS AND METHODS: We analyzed the radiologic results in 14 patients. Group I (n=7) was administrated intermittent PTH after surgical treatment and group II (n=7) was treated only with surgery. We checked the time of initial callus formation, bridging callus formation, and bone union through periodic follow-up radiographs by a radiologist who did not know the patient's information. RESULTS: The mean time to initial callus formation was 6 weeks for group I, compared with 6.7 weeks for group II. The mean time to bridging callus formation was 15.9 weeks for group I, compared with 23.0 weeks for group II. The mean time to bone union was 28.7 weeks for group I, compared with 41.9 weeks for group II. The difference in the cumulative detection rate (CDR) of the initial callus formation of group I and II was not statistically significant (p=0.793). However, the CDR of the bridging callus formation and bone union for group I were higher than those of group II (p=0.008, p=0.001, respectively). CONCLUSION: The intermittent PTH administration after surgical treatment and maximum possible preservation of the periosteum in elderly patients accelerates fracture healing.
