1.Effect of fitness Qigong (Yi Jinjing) on blood lipid and free radical metabolism of the elder women

Objective:To explore the effect of Fitness Qigong(Yi Jinjing)exercise on blood lipid and free radical metabolism of the elder women.Methods:The subjects of this experiment were 64 elder women who were divided into 2 groups randomly:exercise group(34 cases)and control group(30 cases).The exercise group practiced 40-50min pre time,six times pre week for six months,and the control group had no exercise with collective or individual scheme.The indexes of TC,TG,HDL,LDL,SOD,GSH-Px and MDA of the elder women were measured before and after 6-month exercise.Results:After six months Yi Jinjing exercise,the exercise group showed increase in HDL(P0.05),increase in SOD and GSH-Px(P0.05).Conclusion:Fitness Qigong(Yi Jinjing)exercise had the positive effects on blood lipid metabolism,and can suppress lipid peroxidation and maintain high antioxidative capacity of elder female individuals.

3.Clinical analysis of patients with hepatitis B virus-related liver failure complicated with invasive pulmonary aspergillosis

Objective To investigate the clinical features, risk factors, diagnose, treatments and prognosis of patients with hepatitis B virus (HBV)-related liver failure complicated with invasive pulmonary aspergillosis (IPA). Methods A case-control study was performed to retrospectively analyze the clinical data of 43 patients with HBV-related liver failure complicated with IPA. A cohort of 43 patients with HBV-related liver failure complicated with bacterial infection only was analyzed at the same time and another cohort of 43 patients with HBV-related liver failure without any infections served as the control group. Results Compared with the control group, patients with infection had lower platelet counts and prothrombin activity, higher scores of model for end-stage liver disease (MELD), especially the patients with IPA (F=42.43,13.69,14, 22, all P<0.01). And the similar results were observed in patients with IPA group before and after Aspergillus infection (F= 12.09,14.52,-16.74, all P<0.01). Furthermore, the annual mortality was 100. 0% in patients with IPA,which was higher than that of patients complicated with bacterial infection only; and both groups were higher than control group of patients without infection. The statistic results showed that patients with longer length of stay, more antibiotics usage, and more invasive procedures were all risk factors of HBV-related liver failure complicated with IPA. Conclusions Conditions of patients with HBV-related liver failure deteriorate rapidly after complicated with IPA and patients have higher mortality and poorer prognosis.Therefore, it will be effective to enhance the recognition of the early clinical manifestations, and make diagnosis timely and treat with potent anti-fungal therapy to improve their survival.
4.Metabolic pathway and metabolites of pseudolaric acid B.

The metabolic profile of pseudolaric acid B (PB) was investigated by using in vivo and in vitro tests. Pseudolaric acid C2 (PC2) was identified as the specific metabolite of PB in plasma, urine, bile and feces using HPLC and HPLC-ESI/MS(n) after both oral and intravenous administration to rats, and almost no prototype was detected in all kinds of samples. The metabolic behaviors of PB orally administered in rats treated with antibiotics to eliminate intestinal microflora were identical with those in untreated rats, demonstrating that the metabolism of PB is independent of intestinal microflora. PB was stable in 48 h respective incubation with artificial gastric juice and artificial intestinal juice, suggesting that neither pepsin nor trypsin is in charge of metabolism of PB, and also demonstrating that PB is stable in both pH environments of gastric tract and intestinal tract. In vitro research on metabolism of PB in rat liver microsomes incubation revealed that little PB was metabolized and that the proposed metabolites were the demethoxy and demethoxydecarboxy products of the prototype. The amount of metabolites was extremely low compared with the prototype, indicating that liver microsomes are not responsible for the metabolism of PB either. PB was gradually metabolized into PC2 during 1 h in whole blood incubation in vitro, and the metabolic process showed dynamically dependent manner with incubation time. Once absorbed into blood, PB was quickly metabolized into PC2, accordingly, little prototype was detected in all kinds of samples. The metabolism was attributed to the rapid hydrolysis of C-19 ester bond by plasma esterase. These results clarified the metabolic pathway of PB for the first time, which was of great significance to identify the in vivo active form and interpret acting mechanism of the active compounds of P. kaempferi.
5.Metabolic pathway and metabolites of total diterpene acid isolated from Pseudolarix kaempferi.

The preliminary metabolic profile of total diterpene acid (TDA) isolated from Pseudolarix kaempferi was investigated by using in vivo and in vitro tests. Pseudolaric acid C2 (PC2) was identified as the predominant metabolite in plasma, urine, bile and feces after both oral and intravenous administrations to rats using HPLC-UV and HPLC-ESI/MS(n), and demethoxydeacetoxypseudolaric acid B (DDPB), a metabolite proposed to be the glucoside of PC2 (PC2G), as well as pseudolaric acid C (PC), pseudolaric acid A (PA), pseudolaric acid A O-beta-D glucopyranoside (PAG), pseudolaric acid B O-beta-D glucopyranoside (PBG) and deacetylpseudolaric acid A (DPA) originated from TDA could also be detected. It was demonstrated by tests that the metabolism of TDA is independent of intestinal microflora, and neither of pepsin and trypsin is in charge of metabolism of TDA, TDA is also stable in both pH environments of gastric tract and intestinal tract. The metabolites of TDA in whole blood in vitro incubation were found to be PC2, DDPB and PC2G, which demonstrated that the metabolic reaction of TDA in vivo is mainly occurred in blood and contributed to be the hydrolysis of plasma esterase to ester bond, as well as the glucosylation reaction. These results clarified the metabolic pathway of TDA for the first time, which is of great significance to the in vivo active form and acting mechanism research of P. kaempferi.
6.Compariron of the therapeutic effect between sustained low-efficiency dialysis (SLED) and continuous blood purification (CBP) in critically ill patients

ObjectiveTo compare the therapeutic effec between sustained low-efficiency dialysis (SLED) and continuous blood purification (CBP) in critically ill patients.MethodsAccording to the treatment ways,96 critically ill patients were divided into SLED group and CBP group.A comparison was made on the biochemical indicators,in-hospital duration,hemodynamic parameters,acute physiology and chronic health evaluation (APACHE-Ⅱ ),the survival and the mortality rates.ResultsAfter treatment,the levels of serum creatine kinase isozyme MB (CK-MB),creatine kinase (CK),creatinine (Cr),glutamic-oxalacetic transaminase (AST),glutamate-pyruvate transaminase (ALT),APACHE Ⅱ score on the 1st,2nd and 7th day were lower than those prior to the treatment in both groups ( P <0.05).There were no statistical differences in in-hospital duration, biochemical indicators, APACHEⅡscore,hemodynamic parameters,the survival rate and the mortality rate between the two groups (P > 0.05 ).ConclusionsSLED has similar hemodynamic stability with CBP,and the two methods have similar treatment effects in critically ill patients.However,SLED can be relatively economical and convenient for critically ill patients in clinical.
8.Characterization of diterpenoids in the bark of Pseudolarix kaempferi by HPLC-ESI/MSn.

Fragmentation behavior of diterpenoids was investigated by ESI/MSn and the qualitative analysis of diterpenoids in the bark of Pseudolarix kaempferi was performed using high-performance liquid chromatography/ multi-stage mass spectrometry (HPLC-ESI/MSn). The characteristic fragmentation behaviors of the diterpenoids are the cleavages of the lactone ring and C4-O bond. Furthermore, the eliminations of substituent groups at C-18, C-7 and C-8 can also be observed in the MS" (n = 3-4) spectra. For C-4 acetoxy subsititued diterpenoids, [M+Na-60]+ and [M-H-104] are the base peaks of MS2 spectra in the positive and negative ionization modes, respectively. For C-4 hydroxyl subsititued diterpenoids, [M+Na-44]+ and [M-H-62] are the base peaks of MS2 in the positive and negative ionization modes, respectively. For C-18 glucosylated or esterized diterpenoids, [M+Na-44]+ is the base peak of MS2 spectra in positive ionization mode. These fragmentation rules were successfully exploited in the identification of diterpenoids in methanol/water (6:4) extract of P. kaempferi by LC-MS in positive ionization mode. A total of 9 diterpenoids were identified or tentatively characterized, and one of them is reported here for the first time. The described method could be utilized for the sensitive and rapid qualitative analysis of P. kaempferi.

10.Characteristics of blood donors under COVID-19 epidemic in Tianjin

【Objective】 To study and analyze the basic characteristics of blood donors under the COVID-19 epidemic in Tianjin area, so as to provide data support for improving the blood collection for emergency incidents. 【Methods】 The blood collection data of Tianjin Blood Center under the COVID-19 epidemic in 2020 were collected in terms of the number of blood donations, changes in blood donation units over time, group blood donation, donations given by regular blood donors and donations inside fixed blood donation sites, registered residence, gender, age, education, occupation, individual donation, group donation, and the annual daily blood collection and delivery data in Tianjin. 【Results】 The number of blood donations and donation units in Feb 2020 decreased by 56.96%(3 226/7 496-1)and 62.40%(5 382/14 314.5-1)compared with the same period in 2019, and decreased by 56.87%(3 226/7 480-1)and 60.70%(5 382/13 695.7-1)compared with the average value in the previous five years. The number of blood donations, donation units and rate of 400 mL donation in 2020 decreased by 13.25%(119 107/137 293-1), 17.7%(205 196.5/249 405-1)and 12.83%(66.0/75.8-1)respectively. Meanwhile the units of group donation increased by 22.59%(44 118.5/35 989.0-1)as compared with those in 2019. The proportion of regular blood donors raised up to 17.31%(20 623/119 107, which was the highest in recent 6 years. During the epidemic period, the number of blood donations of local registered residence, women, over 36 years old, with bachelor and above degree increased by varied extent compared with previous years. Workers, staff members and others were the main groups of voluntary blood donation, and civil servant, medical workers, farmers and teachers also played a certain role during the epidemic. 【Conclusion】 The clinical blood use in Tianjin was basically guaranteed during the COVID-19 epidemic. It is suggested to further optimize the structure of voluntary blood donors, establish a diversified and multi-level group blood donation team, and increase the number of regular blood donors.
