1.The Correlation Analysis of Endolymphatic Hydrops Degree in Vestibule and the Symptoms and Audiovestibular Functions in Patients with Meniere’s Disease

Objective To evaluate the correlation between the endolymphatic hydrops in vestibule and symp‐toms ,audiovestibular functions in patients with Meniere's disease .Methods Twenty -four hours after bilateral intra-tympanic 8 times diluted gadolinium administration , three - dimensional fluid - attenuated inversion recovery MRI ,using a three-Tesla unit ,was performed in 34 patients .The extent of endolymphatic hydrops in the MRI images was assessed and compared the hydrops with age ,duration ,stage of the disease ,frequency of attacks ,pure tone audiometry ,caloric test ,electrocochleography and vestibular evoked myogenic potentials .Results Thirty -four patients showed different levels of enhancement of perilymth in the inner ears .In these patients ,obvious signs of endolymphatic hydrops were visualized ,including 8 slight hydrops ,11 mild hydrops and 15 significant hydrops . In all 34 patients ,the results of PTA were 9 dB HL to 90 dB HL ,with average of 43 .56 dB HL and there were 21 abnormal and 13 normal patients in caloric test with a diagnostic rate 61 .76% ;17 normal ,7 abnormal and 10 not elicited patients in VEMP with a diagnostic rate 50% ;22 abnormal and 12 normal patients in electrocochleography with a diagnostic rate 64 .71% .There were almost no complications in all 34 patients after bilateral intra-tympanic injec‐tion except short vertigo among 5 of them .A statistically significant correlation was found between degrees of en‐dolymphatic hydrops in vestibule and age ,stage of the disease and VEMP (P<0 .05) ,whereas no significant corre‐lation was found between the extent of endolymphatic hydrops and duration ,frequency of attacks ,pure tone audi‐ometry ,caloric test ,electrocochleography(P> 0 .05) and the correlation coefficient were 0 .494 ,0 .568 ,0 .590 , 0 .495 ,respectively .Conclusion The degree of endolymphatic hydrops in Ménière's disease has significant correlation with age ,VEMP ,stage of the disease and pure tone audiometry .No significant correlation was found between hy‐drops and duration ,frequency of attacks ,functional level scale ,caloric test and electrocochleography .
2.Detection and grading of endolymphatic hydrops in Ménière's disease using 3D FLAIR MRI.

OBJECTIVE:
To evaluate the extent of endolymphatic hydrops as shown by three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging (3D FLAIR MRI) performed 24 hours after bilateral intratympanic gadolinium administration and discuss the positive rate of endolymphatic hydrops in vestibule and cochlea.
METHOD:
Twenty-four hours after bilateral intra-tympanic 8 times diluted gadolinium administration, three-dimensional fluid-attenuated inversion recovery MRI, using a three-Tesla unit, was performed in 48 patients, and then assessed the extent of endolymphatic hydrops in the MRI images.
RESULT:
Forty-eight patients showed different levels of enhancement of perilymth in the inner ear. In these patients, obvious signs of endolymphatic hydrops were visualized in vestibule,including 8 slight hydrops, 17 mild hydrops and 23 severe hydrops with a diagnostic rate of 83.3%. There were almost no complications in all 48 patients after bilateral intra-tympanic injection except short vertigo in some of them.
CONCLUSION
3D FLAIR MRI resonance imaging has a high positive diagnostic rate in assessing endolymphatic hydrops of Ménière's disease and provides direct imaging evidence for diagnosing Ménière's disease.

3.The inhibitory effect of FA-MNP-MMP-9-ASODN nanocomposite on nasopharyngeal carcinoma in vitro.

OBJECTIVE:
To observe the inhibitory effect of FA-MNP-MMP-9-ASODN nanocomposite on nasopharyngeal carcinoma (NPC) HNE-1 cell in vitro.
METHOD:
To observe the MMP-9 mRNA and protein expression levels, proliferation ability, cell apoptosis and invasion ability of HNE-1 cell 48 hours after FA-MNP-MMP-9-ASODN transfection by RT-PCR, Western-blot, MTT assay, flow cytometry and Matrigel invasion test.
RESULT:
MMP-9 mRNA and protein expression in HNE-1 cell of FA-MNP-MMP-9-ASODN group was significantly decreased compared to control group and nonsense sequence group of FA-MNP-MMP-9-NSODN. At the same time, for the HNE-1 cell in FA-MNP-MMP-9-ASODN group, growth inhibition rate was about 35.66%, proliferation activity significantly decreased compared to the control group and the nonsense sequence group, cell cycle was also inhibited, cell apoptosis rate was about 12.60%, the apoptosis rate was significantly higher than that in the control group and the nonsense sequence group. Invasion assay showed that the transmembrane cells in FA-MNP-MMP-9-ASODN group were about 21.00, significantly lower than that in the control group and the nonsense sequence group.
CONCLUSION
By inhibiting the expression of MMP-9, FA-MNP-MMP-9-ASODN nanocomposites could reduce NPC cell proliferation and invasion ability, and promote apoptosis, it had a good inhibitory effect in vitro.

4.Factors accounting for different response of pulmonary and cerebral vessels to hypoxia

Roles of sympathicus, sensory neuropeptides (SNP), metabolites of cyclooxygenase, metabolites of lipoxygenase, endothelium derived relaxing factor (EDRF), reactive oxygen (ROS) and potassium channels (PC) in the hypoxic pulmonary vasoconstriction (HPV) and hypoxic cerebral vasodilation (HCVD) were studied in intact rats, rabbits and dogs. Results were as follows: during hypoxia, the excitation of sympathicus results in a constriction of both pulmonary and cerebral vessels; SNP, EDRF and the opening of 4-AP sensitive PC caused the dilation of both of them; metabolites of lipoxygenase mediated HPV and HCVD, whereas metabolites of cyclooxygenase were their modulators; hypoxia induced blockade of the ATP sensitive PC mediated HPV, but had no effect on HCVD; reduction of O_2~+ in the lung might potentiate HPV, but had no effect on HCVD. It is suggested that the alteration of lipoxygenase metabolites, ROS and ATP sensitive PC are factors accounting for the difference in response of pulmonary and cerebral vassels to hypoxia.
5.Risk factors for retained common bile duct stones of laparoscopic cholecystectomy

Objective To investigate the related risk factors caused the retained common bile duct (CBD)stones after laparoscopic cholecystectomy,to provide the evidence for preventing from retained common bile duct stones of laparoscopic cholecystectomy.Methods Selected 654 cases of laparoscopic cholecystectomy patients as the objects whom hospitalized in Beijing Chuiyongliu Hospital from January 2002 to June 2013.All cases were divided into the group of retained CBD stones (27cases) and the group of non-retained CBD stones (627 cases).Collected the potentially relevant factors of two groups with the retained CBD stones,including medical history of jaundice and pancreatitis,common bile duct internal diameter (B Ultrasound) ≥ 8 mm,acute cholecystitis,emergency surgery,gallbladder removal order,calot triangle adhesion,sludge calculus,stones neck incarcerated,fulltype stones,cystic duct thickening,cystic duct reserved≥ 1 cm,the minimum diameter stones ≤5 mm,the number of gallbladder stones ≥ 5,partial cholecystectomy,purulent bile.Statistics analyses was proceeded using the IBM SPSS 20.0.Result Through dichotomy logistic regression analysis to the univariate analysis results with statistical significance,sorted the results according to the influence degree,found the independent risk factors:common bile duct internal diameter (B Ultrasound) ≥8 mm,gallbladder removal order (retrograde removal),medical history of jaundice pancreatitis,cystic duct reserved ≥ 1 cm,sludge calculus,in total of 5 indexes,which caused the retained CBD stones after laparoscopic cholecystectomy.Conclusion There are several independent risk factors for retained CBD stoes after LC such as medical history of jaundice and pancreatitis,common bile duct internal diameter (B Ultrasound) ≥ 8 mm,the order of cholecystectomy (retrograde remoral),shudeg calculus and the remaining length of bile duct ≥ 1 cm.The surgeons should pay close attention to them and take appropriate measures in the preoperation and intraoperation of LC,which contribute to preventing the acurence of postoperative retaimed CBD stone.
6.FA-MNP-MMP-9-ASODN complex construction and the study of its FA molecular targeting ability.

OBJECTIVE:
To construct FA targeted magnetic nanocomplex (FA-MNP-MMP-9-ASODN) loading matrix metalloproteinase 9 (MMP-9) antisense oligonucleotide (ASODN) and evaluate its targeting capacity and efficiency of gene transfection to folate receptor (FR) positive NPC.
METHOD:
FA-MNP-MMP-9-ASODN was constructed by MMP-9-ASODN coupling with FA-MNP prepared by our research team through the aldehyde-ammonia condensation reaction. To analyze the feasibility of ASODN coupling with nanocarrier agarose gel electrophoresis. Two kinds of HNE-1 and CNE-2 cells and implanted tumors phagocytosis of FA-MNP-MMP-9-ASODN were observed by MRI on tumor-bearing nude mice, iron staining and TEM. To analyze gene transfection of the vector by observing FITC in the cell.
RESULT:
The electrophoresis results revealed ASODN successfully coupling with FA-MNP. HNE-1 cell can effectively ingest the nanocomposite,with more FITC in the cell, but CNE-2 cell had not uptake for the nanocomposite, with no FITC in the cell. By comparing with CNE-2 tumor, HNE-1 tumor also can efficiently swallow the nanocomposite.
CONCLUSION
FA-MNP-MMP-9-ASODN nanocomplex is constructed successfully with good FA targeting ability and gene transfection.

7.Therapeutic effect of modified surgery for cystic lymphangioma.

OBJECTIVE:
To discuss the surgical treatments of cystic lymphangioma and their curative effect.
METHOD:
A retrospective review of 12 patients (9 males, 3 females) of cystic lymphangioma was performed, with the age ranged from 1 to 35 (median age was 11). Six patients were born with cervical cyst, three patients had symptoms in childhood, and 3 in adolescence. Ten patients underwent modified operation, that is the extracapsular dissection performed under direct vision after open cystic, from July 1998 to March 2008, a patients who had operative procedure in other hospital with recurrence and a 1 year child received local injection of bleomycin A 5.
RESULT:
Cystic lymphangioma were complete excised in 10 surgical patients, without any complication like nerve injury. One of the two patients who received injection was lost in follow-up, and the other was still in follow-up with the tumor basically eliminated.
CONCLUSION
Surgery is the primary treatment of cystic lymphangioma. Modified operative method is helpful to complete resection of lymphangioma and the protection of surrounding tissues.

OBJECTIVETo evaluate the consistency between the clinical diagnostic criteria and the ascertained diagnostic criteria for diabetic peripheral neuropathy (DPN) in the Preventive and Treatment Guidelines of Diabetes in China (2013) and explore an economic, convenient, and accurate approach to DPN diagnosis.
METHODSThe patients with type 2 diabetes admitted in our department from April to June, 2014 were examined for nerve conduction velocity, 10 g nylon silk, vibration threshold value, sense of temperature and pain, and ankle reflex. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, and Kappa value were calculated to assess the diagnostic power of the two diagnostic criteria.
RESULTSOf the 151 patients enrolled, 106 (70.2%) had a diagnosis of DPN consistent with the ascertained diagnostic criteria, as compared to 86 (56.95%) who were diagnosed according to the clinical diagnostic criteria; the latter patients accounted for 81.13% of former cases. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, and Kappa value of the clinical diagnostic criteria were 80.19%, 97.78%, 98.84%, 67.69%, 77.97%, and 0.69, respectively, which were highly consistent with those of the ascertained diagnostic criteria; the sensitivity to compression showed a poor consistency between the two diagnostic criteria. In the 5 screening tests, the combined test of temperature sensation, vibration perception, and ankle reflex showed the highest AUC value among their different combinations.
CONCLUSIONThe clinical diagnostic criteria for DPN show good consistency with the ascertained diagnostic criteria, and for patients with clinical symptoms or with only one positive sign, combination of the two diagnostic criteria can achieve the maximum diagnostic power.
