1.Clinical analysis of infectious complications in 189 patients with lupus nephritis

Objective To investigate the clinical and epidemiologic features of infection in patients with lupus nephritis (LN). Methods The incidence, sites and pathogen species of infection 1 year after diagnosis in 189 patients with LN were retrospectively analyzed, and the relationships between the clinical, histological types, treatment programs and infection were analyzed. Results Among 189 LN patients (16 males and 173 females), 97 patients had infection (infection group), and 92 patients had not infection (non-infection group). The incidence of infection was 51.3% (97/189), while the incidence of nosocomial infection was 39.7%(75/189). In infection group, the patients had 133 infection episodes, and the principal infection sites were lung and urinary tract system, accounting for 67.7% (90/133); the principal pathogen was bacteria, accounting for 72.9%(97/133), and the second was virus, accounting for 17.3% (23/133). Different clinical or pathological types and treatment options of LN had different infection rates, but there were no statistical differences (P>0.05). The incidence of nosocomial infection in patients with glucocorticoid + cyclophosphamide double impact was significantly higher than that in patients treated with glucocorticoid + mycophenolate mofetil: 52.9% (45/85) vs. 7.7% (2/26), and there was statistical difference (P<0.01). One year after diagnosis, the mortality in infection group was significantly higher than that in non-infection group:8.2%(8/97) vs. 1.1%(1/92), and there was statistical difference (P<0.05). Conclusions The more serious the clinical and pathological types, the higher the incidence of infection. The incidence of infection is associated with the treatment options. Infection is still the most important reason for LN patients′death.
2.Establishment of APP and PS-1 double gene stably transfected cell lines

AIM To study pathogenesis of Alzheimers disease and screen ?- secretase and ?- secretase inhibiting drug, PS-1(M 146L)and APP 751 gene double stably transfected CHO cell lines have been established. METHODS A? cDNA encoding human PS-1 was obtained by polymerase chain reaction from a human placental library. Mutant PS-1 (M 146L) cDNA was also generated by polymerase chain reaction from wild type PS-1 cDNA. Wild type and mutant PS1 were subcloned into CMV-based mammalian expression vectors PCI-neo,then recombined plasmid were co-transfected into APP expressing CHO cells at 1∶10 ratio using LipofectAmine. Stable expressing cell lines were screened and selected by using selection media. RESULTS Several CHO cell lines stably transfected with wide type or mutant PS-1 (M 146L) as well as APP 751 genes were established. Overexpression of PS-1 in CHO cell accumulated full length 45 kDa PS-1 protein. A? released to conditioned media were not changed in wild type PS-1 transfected APP expression CHO cells. A? 1~42 level in conditioned media of M 146L mutant PS-1 stably transfected of APP expression CHO cells were elevated about 1.6 fold. CONCLUSION Expression of M 146L mutant PS-1 in stably transfected APP expression CHO cells increased a secretion of A? 1~42. The PS-1 and APP double stably transfected CHO cell lines we generated can be used for either ?-or ?-secretase inhibitor study and related drug screening.
3.Effects of tirofiban, aspirin and clopidogrel on the incidence of major adverse cardiac events after percutaneous coronary intervention in patients with acute myocardial infarction

Objective To observe the effect of tirofiban,aspirin and Clopidogrel Hydrogen triple antiplatelet in patients with acute myocardial infarction undergoing primary percutaneous coronary intervention and the effect on the incidence of major adverse cardiac events.Methods a total of 60 patients with acute myocardial infarction(AMI)in our hospital from January 2015 to-2016 in December were enrolled in this study.All patients were treated with percutaneous coronary intervention(PCI).The patients were randomly divided into control group(n=30)and observation group(n=30).The control group using clopidogrel hydrogen treatment,observation group were treated with tirofiban,aspirin and Clopidogrel Hydrogen triple antiplatelet therapy,the incidence and clinical effects of the two groups of major adverse cardiac events.Results the two groups before treatment,the platelet aggregation rate and platelet count had no statistically significant difference; no significant observation group after treatment compared 30d platelet count difference; observation group 30d after treatment,the platelet aggregation rate was lower than the control group(P<0.05); the observation group after the treatment of 30d MA-ADP on platelet drug reaction the inhibition rate is lower than the control group(P<0.05); the observation group after treatment of AA drugs 30d platelet inhibition rate,the inhibition rate of ADP,higher than that of the control group(P<0.05); the observation group after treatment,the incidence of major adverse cardiac events was 13.33%,33.33%in the control group,two groups of major adverse cardiac events was statistically significant the difference(P<0.05).Conclusion The patients with acute myocardial infarction undergoing primary percutaneous coronary intervention after treatment with tirofiban,aspirin and chlorine Pigre triple antiplatelet ideal treatment effect,can reduce the incidence of major adverse cardiac events,it is worthy of popularization and application.
4.One stage total hip arthroplasty for acetabular fractures

Objective To explore operative indications,methods and effect of one stage total hip arthroplasty(THA)in treatment of acetabular fractures.Methods One stage total hip arthroplasty (THA)was carried out in 11 patients with acetabular fractures including 10 males and one female(at mean age of 42.4 years)8-37 days(mean24 days)after injury.There were anterior column fractures in three patients,posterior wall fracture in one,posterior column fracture in two,posterior column fracture combined with posterior wall fracture in two,transverse fracture in one,transverse fracture combined with posterior wall fracture in one and two-column fracture in one.Fresh acetabular fractures were first fixed with reconstruction plate or screws to restore shape of acestbular wall;then,granule or bulk of resected femoral head were implanted into acetabulum posterior to placement of acetabular cup prosthesis.While for old acetabular fractures,the resected femoral head was first implanted on acetabular superior wall and posterior wall and then acetabular cup prosthesis was placed.Results Follow-up for 6-45 months (mean 28 months)showed obdous improvement of hip joint function,with average Harris score of 78 points and average active degree of 95°.X-ray film presented loosening of acetabulum and osteolysis in one patient.All patients began weight bearing three months after operation,with no dislocation of joint prosthesis.Conclusions Under strict control of indications,one stage THA can be done for patients who miss operative reduction due to obvious acetabular fracture dislocation and severe articular cartilage injury.Stable aeetabular cup may beget satisfactory short-term clinical results.
5.Effects of curcumin preconditioning on activity of xanthine oxidase during intestinal ischemia-reperfusion in rats

Objective To evaluate the effects of curcumin preconditioning on the activity of xanthine oxidase (XOD) during intestinal ischemia-reperfusion (I/R) in rats.Methods Thirty female Sprague-Dawley rats,aged 180-220 g,were randomly divided into 3 groups (n =10 each) using a random number table:sham operation group (S group),intestinal I/R group (I/R group),and curcumin preconditioning group (Cur group).Intestinal I/R was induced by clamping the superior mesenteric artery for 75 min followed by reperfusion.Curcumin 200 mg/kg was given everyday for 5 days before intestinal I/R in Cur group and the equal volume of normal saline was given instead of curcumin in S and I/R groups.The rats were sacrificed at 4 h of reperfusion and the intestinal specimens were obtained for microscopic examination of pathologic changes which were graded using Chiu scoring system and for determination of XOD activity,content of malondialdehyde (MDA),and superoxide dismutase (SOD) activity.Results Compared with S group,the Chiu score,activity of XOD and content of MDA were significantly increased,while the activity of SOD was decreased in I/R and Cur groups (P < 0.05).Compared with I/R group,the Chiu score,activity of XOD and content of MDA were significantly decreased,and the activity of SOD was increased in Cur group (P < 0.05).Conclusion Curcumin preconditioning can attenuate intestinal I/R injury in rats,which may be due to inhibition of XOD activity and decreased oxidative stress in intestinal tissues.
6.Role of nitric oxide in spinal cord in maintenance of diabetic neuropathic pain in rats

Objective To evaluate the role of nitric oxide (NO) in the spinal cord in the maintenance of diabetic neuropathic pain in rats.Methods Male Sprague-Dawley rats,aged 2 months,weighing 180-200 g,were used in the study.Diabetes mellitus was induced by intraperitoneal streptozotocin (STZ) 60 mg/kg and confirmed by blood glucose > 16.7 mmol/L on day 2 after STZ injection.Twenty diabetic rats were randomly allocated into diabetes mellitus group (DM group,n =10) and L-NAME (non-selective NOS inhibitor) group (LN group,n =10).Another 10 age-matched normal rats served as control group (C group).On 21 days after STZ injection,L-NAME 10 mg/kg was injected intraperitoneally once a day for 7 consecutive days in LN group,whereas the equal volume of normal saline 5 ml/kg was given instead of L-NAME in DM group.Paw withdrawal threshold to yon Frey filament stimulation (PWT) was measured before STZ infection and on 7,14,21 and28 days after STZ injection.The rats were sacrificed after the last measurement of PWT and the lumbar segments of spinal cord were removed for determination of NO content and neuronal nitric oxide synthase (nNOS) expression (by Western blot analysis) in spinal cord tissues.Results Compared with C group,PWT was significantly decreased on 14,21 and 28 days after STZ injection,and the NO content and nNOS expression in spinal cord tissues were increased in DM and LN groups (P < 0.05).Compared with DM group,PWT was significantly increased on 28 days after STZ injection,and the NO content and nNOS expression in spinal cord tissues were decreased in LN group (P < 0.05).Conclusion NO in the spinal cord is involved in the maintenance of diabetic neuropathic pain in rats and the mechanism is related to the enhanced function of nNOS.
7.Characteristics of Beijing Psychological Crisis Hotline callers with the diagnosis of mental disorders

Objective:To understand the characteristics of the callers with the diagnosis of mental disorders called Beijing Psychological Crisis Hotline for help,in order to provide specific psychological intervention services for them in future.Methods:From December 2002 to December 2008,24217 different callers'data collected from a computer-based operating system of the Beijing Psychological Crisis Hotline were analyzed.According to their self-reported of having the diagnoses of mental disorder or not,the callers were divided into the ones with diagnosis (n=6516)and the ones without diagnosis (n =17701).The callers'demographic characteristics,their main counseling problems and the suicide-related factors in the two groups were compared.Results:The top five main counseling problems in the group with diagnosis were mental problems,the mental illness related knowledge,the problems of family relationships,working problems and interpersonal problems.Mental problem ranked first in both groups.Compared to the group without diagnosis,the diagnosed ones were more likely to be found in such group of people as women,aged between 20 -29 and 30 -44,having 10 or more years of education and the unem-ployed.The results of the assessments of 11 suicide-related risk factors showed that callers with diagnoses had higher prevalence of most suicide-related factors than those in the other group(P <0.001 or 0.05),which included suicidal intention and behavior while calling,history of attempted suicide,severe depressive symptoms,severe physi-cal illness,history of being abused,afraid of being attacked,suicidal history of relatives or associates,severe of hopeless.Conclusion:The hotline counselors should be aware of the necessity of assessing the severity of depres-sion and other suicide-related factors for the callers with mental disorder diagnoses before providing any psychologi-cal intervention.Moreover,except for providing interventions on the callers'psychiatric problems,the hotline coun-selors should also identify and help to deal with the stress events in their lives.
8.Effect of curcumin preconditioning on activation of mast cells during intestinal ischemia-reperfusion in rats

Objective To evaluate the effect of curcumin preconditioning on the activation of mast cells during intestinal ischemia/reperfusion (I/R) in rats.Methods Twenty-four female Sprague-Dawley rats,weighing 180-220 g,were randomly divided into 3 groups (n =8 each) using a random number table:sham operation group (S group),intestinal I/R group (I/R group),and curcumin preconditioning group (Cur group).Intestinal I/R was produced by occlusion of superior mesenteric artery (SMA) for 75 min followed by reperfusion.At 5 days before I/ R,curcumin 200 mg/kg (in 20 mg/ml normal saline) was given through a gastric tube in Cur group,while the equal volume of normal saline was given instead in S and I/R groups.All the rats were sacrificed at 4 h of reperfusion,and the intestinal specimens were obtained for microscopic examination and for determination of tryptase expression and β-hexosaminidase content.Intestinal damage was assessed and scored according to Chiu.Results Compared with S group,Chiu's score and β-hexosaminidase content were significantly increased,and the expression of tryptase was up-regulated in I/R and Cur groups.Compared with I/R group,Chiu' s score and β-hexosaminidase content were significantly decreased,and the expression of tryptase was down-regulated in Cur group.Conclusion The mechanism by which curcumin preconditioning attenuates intestinal I/R injury is related to inhibited activation of mast cells of rats.
9.The effect of different combinations between irradiation and cisplatin or lobaplatin on tumor growth in mice

Objective To learn the effect of different combination model between irradiation and cisplatin or lobaplatin on the radiosensitization of xenographt tumor in mice.Methods Seventy C57BL/6 mice with Lewis lung carcinoma were randomly divided into fourteen groups.Then a single intravenous bolus injection of 10 mg/kg either cisplatin or lobaplatin was given.Tumor tissues were collected at the indicated times of 0.5 h, 2.0 h, 4.0 h, 24.0 h, 48.0 h, 72.0 h, and 96.0 h.The platinum levels were determined by inductively coupled plasma-mass spectrometry.Eighty tumor-bearing mice were randomly divided into 10 groups, including a blank control group, a irradiation group, two drug treatment groups and 6 combined treatment groups.The tumors were irradiated at 1 h, 24 h or 72 h after either cisplatin or lobaplatin injection.The tumor size of the groups was compared.Results The concentrations of cisplatin and lobaplatin in tumors rapidly reached 4.78 μg/g and 2.79 μg/g (t=3.82,P=0.005), respectively, then declined rapidly to 3.39 μg/g and 0.99 μg/g (t=9.10,P=0.000) at 4 h, 1.41 μg/g and 0.23 μg/g (t=3.70,P=0.006) at 96 h, respectively.The tumor growth among the three groups of irradiation at 1 h, 24 h or 72 h after cisplatin was similar, which was slower than the blank control group, the irradiation group and the cisplatin treatment group.At the 15th day, the relative volume of tumor in the three combined treatment groups were 4.73, 5.52 and 2.15(F=0.84,P=0.451), While was 16.63(F=10.50,P=0.000) in the blank control group, 10.34(F=3.12,P=0.046) in the irradiation group, and 12.80(F=8.06,P=0.001) in the cisplatin treatment group, respectively.The tumor growth among the three groups of irradiation at 1 h, 24 h or 72 h after lobaplatin was also similar, which was slower than the blank control group, the irradiation group and the lobaplatin treatment group.At the 15th day, the relative volume of tumor in the three combined treatment groups were 3.49, 4.90 and 3.86(F=0.32,P=0.727), While was 16.63(F=15.21,P=0.000) in the blank control group, 10.34(F=4.12,P=0.016) in the irradiation group, and 14.28(F=10.67,P=0.000) in the lobaplatin treatment group, respectively.The sensitizing enhancement ratio (SER) at 1 h, 24 h and 72 h after the injection were 2.13, 2.03 and 3.45 of cisplatin, and 2.53, 2.00 and 2.50 of lobaplatin, respectively.Conclusions After intravenous bolus injection, the cisplatin concentration in the tumor can be kept at least 96 hours, which results in a persistent radiosensitizing effect.Lobaplatin and cisplatin have similar anti-tumor and radiosensitizing effect.
10.Finite element analysis on point contact locking plate fixating femoral shaft fractures

Objective To set up a three-dimensional finite element model (FEM) to investigate biomechanics of point contact locking plate (PC-LP) fixating femoral shaft fractures. Methods One intact fresh adult cadaveric femur was scanned by CT at 1 mm interval. Then, the data of CT were utilized to establish three-dimensional FEM by using software Mimics and PRO/E and simulate the different clini-cal loading conditions. The changes of theoretical stress of femur and PC-LP were analyzed under flexion, axial compression and torsion loads. Results (1) Under four-point bending load, the distribution of femur stress was in uniformity, with the largest stress of the PC-LP focused on the edge. (2) Under axial compression load of 250 N, the largest stress of the femur was focused on the screw holes on beth distal ends, with the largest stress of the PC-LP focused on the middle screw holes. (3) Under the torsion load cused on the middlepart and the middle screw holes. Conclusions Under the four-point bending, ax-ial compression and torsion loads, the distribution of femur stress is in uniformity, when the largest stress of the PC-LP focuses on edge or the middle screw holes, while that of the PC-LP on two screw holes of proximal or distal ends.
