1.Role of whey protein hydrolyzed formula as an adjunctive therapy for treating patients with infantile eczema

Objective To study the clinical value of extensively whey protein hydrolyzed formula and partially whey protein hydrolyzed formula as an adjunctive therapy in treating infantile eczema.Methods Totally 59 bottle-feeding babies with infantile eczema were divided into three groups depending on different formula feeding:extensively hydrolyzed formula feeding group (eHF group,n =18),partially hydrolyzed formula feeding group (pHF group,n =22),and conventional cow's milk formula feeding group (CMF group,n =19).Meanwhile,all patients received the same drug treatment.The effective rate and remission rate were evaluated using the SCORing Atopic Dermatitis (SCORAD) index.Results After 2 weeks of treatment,the effective rate was 38.9% and the remission rate was 50.0% in the eHF group,which were significantly different from those in the CMF group (5.3% and 31.6%) (x2 =12.225,P =0.002).The effective rate was 9.1% and the remission rate was 40.9% in the pHF group,showing no significant difference when compared with the CMF group (x2 =0.761,P =0.683).After 8 weeks of treatment,the effective rate was 55.6% and the remission rate was 38.9% in the eHF group,comparing to 15.8% and 47.4% in the CMF group,the difference was statistically significant (x2 =8.498,P =0.014).The effective rate was 31.8 % and the remission rate was 59.1% in the pHF group,a trend towards higher than that of the CMF group,but the difference was not statistically significant (x2 =4.912,P =0.086).Before (F =0.773,P =0.466) and after (F =1.313,P =0.277) the treatment,the growth and development data in different groups showed no significance differences.Conclusion Both the extensively and partially whey protein hydrolyzed formula play an adjunctive role in treating infantile eczema,although the treatment effectiveness of the extensively hydrolyzed formula appears earlier than that of the partially hydrolyzed formula.

By analyzing 94 questionnaires of majors in medical imaging technology on medical ethics teaching,we found that some teaching models will help students realize the necessity of studying medical ethics and case teaching,including increasing the expansion of case teaching,selecting certain videos and short films into case teaching,expanding case-study model and case-exploration model,strengthening teachers' after-case-teaching summary to advance and further ethical themes.These models will also improve students' ability in ethical analysis and their sense of ethics,and will surely apply the theoretical principles into practice once they are interested in studying medical ethics.

Breast cancer and thyroid diseases are common diseases in women.It is reported that breast cancer and thyroid diseases have some relevance.Breast cancer patients have relatively high incidence of thyroid dysfunction,thyroid node and thyroid cancer,which are easily misdiagnosed for lacking of obvious clinical manifestations and will affect the treatment and prognosis of breast cancer.In breast cancer patients,screening and diagnosis of thyroid function and disease through thyroid function detection and ultrasound examination should be strengthened.
5.Expression of COX-2 and VEGF and their relation with cervical lymph metastases of papillary thyroid carcinoma

OBJECTIVE To investigate the relation of cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF) expression with cervical lymph metastases in papillary thyroid carcinoma. METHODS The expression of COX-2 and VEGF in 79 specimens of papillary thyroid carcinoma were evaluated with SP immunohistochemical methods. In all the 79 cases, there were 46 cases with cervical lymph node metastases and 33 cases without cervical lymph node metastasis. RESULTS The positive expression rates of COX-2 and VEGF in the cases with cervical metastases were 81.6 % and 86.8 % respectively, and in the cases without cervical lymph metastases were 54.5 % and 66.7 % respectively. Postoperative adjuvant therapy for primary invasive breast cancer patients with synchronous ax-illary metastases is mainly based on the characteristics of biomarker of the primary tumor. Recently, some studies have showed the discordance and clinical significance of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2) status between primary breast cancer and synchronous axillary lymph node metastases. As local metastasis, the synchronous axillary metastases may represent the potentially metastatic breast cancer cells much better than the primary tumor. Hence, determination of biomarkers status should be performed in synchronous axillary metastasis, together with primary tumor, to guide therapy management and evaluate the prognosis of primary invasive breast cancer patients with synchronous axillary metastases.
8.The significance of axillary sentinel lymph node biopsy after neoadjuvant chemotherapy

Objective Comparing the influence of neoadjuvant chemotherapy on sentinel lymph node biopsy and axillary lymph nodal staging. Methods From June 2005 to June 2007 one hundred and sixty five breast cancer patients of stage Ⅰ,Ⅱa,Ⅱb and Ⅲ underwent sentinel lymph node biopsy(SLNB) using 99mTc combining methylene blue dye as tracer to help axillary lymph node dissection(AND) were retrospectively reviewed. Sixty five patients staged Ⅱb or Ⅲ were treated with three circles neoadjuvant chemotherapy prior to SLNB and AND , another one hundred patients staged ⅠorⅡa had SLNB and AND directly without neoadjuvant chemotherapy.Results Average 14.60 axillary lymph nodes were retrieved in patients had neoadjuvant chemotherapy and 14.74 lymph nodes in patients did not have neoadjuvant chemotherapy(P>0.05),a mean number of 1.46 sentinel lymph nodes in neoadjuvant chemotherapy group and 1.5 sentinel lymph nodes in non-neoadjuvant chemotherapy(P>0.05), sentinel lymph node identification rate 96.9% in neoadjuvant chemotherapy group and 97% in non-neoadjuvant chemotherapy(P>0.05), the false negative rate was 4.6% in neoadjuvant chemotherapy group and 4% in non-neoadjuvant chemotherapy group(P>0.05). Conclusions The mean number of lymph nodes,sentinel lymph nodes,SLN identification rate and false-negative rates after neoadjuvant chemotherapy are similar to those seen in patients without neoadjuvant chemotherapy.The SLNB can accurately predict lymph node status of axillary lymph node following neoadjuvant chemotherapy.
10.The status of glucose intolerance in breast cancer patients without DM history after systemic treatment with surgery and/or chemo-therapy

Objective:To determine the status of glucose intolerance in breast cancer patients without DM history after combined treatment with surgery and/or chemotherapy through an oral glucose tolerance test (OGTT). Methods:All 121 breast cancer patients more than 3 months after combined treatments with surgery and/or chemotherapy and without the diagnosis of diabetes underwent OGTT and fasting. Then, 2 h glucose levels were measured to identify glucose tolerance and diabetes. Meanwhile, six patients with a history of diagnosed diabetes did not undergo OGTT. Results:The median ages of all breast cancer patients and the mean duration after combined treatments with surgery and/or chemotherapy were 50.4 years and 19 months, respectively. Among the 121 breast cancer pa-tients without the history of diabetes, the incidences of diabetes, prediabetes, and normal glucose intolerance were 19.8%(24 cases), 45.5%(55 cases) and 34.7%(42 cases), respectively. Among all breast cancer patients, the incidences of previously diagnosed diabetes, undiagnosed diabetes, and prediabetes were 4.72%, 18.9%, and 43.3%, respectively. The ratio of previously undiagnosed diabetes was about 80%. About 80.0% of undiagnosed diabetes and 74.5% of prediabetes met the criteria for elevated 2 h plasma glucose levels through OGTT instead of elevated fasting glucose levels. Conclusion: Breast cancer patients during follow-up after combined treat-ments with surgery and/or chemotherapy highly suffer from glucose intolerance, with high incidences of undiagnosed diabetes and pre-diabetes. OGTT should be made for breast cancer patients after combined treatments for early diagnosis, prevention, and treatment of di-abetes.
