1.The study on lumbar disc herniation complicated with serious cauda equina syndrome

Objective:The goal of this study was to investigate the mechanisms of secondary cauda equina syndrome(CES) ,a serious complication of lumbar disc herniation(LDH) ,and to provide a clue of the therapeutic methods and prevention measures. Methods : In the retrospective study,24 cases, who suffered from LDH complicated with CES,were evaluated in regards to etiology, surgical and post - operative results. All the cases were operated with total - laminectomy together with discoidectomy. Results : 24 cases in the follow-up, post-operative period, were analyzed for full,partial and no recovery. Among them, the motor function recovered fully, the disturbance of sphincter recovered partially, the sensory disturbance of sella area recovered partially, and sexual function recovered poorly. Conclusion :CES is a serious complication of LDH,and early operation is the key of the treatment. There are some factors, including the degree of the primary lesion of cauda equina, the choice of the operative chance, the application of the operative method and the manipulative technique of the operation et al,that are closely related with the recoverable degree of CES.
2.Effects of edaravone on expression of caspase-3 and caspase-12 in juvenile rat hippocampus after status convulsion

[ ABSTRACT] AIM:To investigate the effect of edaravone ( ED) on the expression of caspase-3 and caspase-12 in the juvenile rat hippocampus after status convulsion ( SC) .METHODS: Juvenile male Sprague-Dawley rats were ran-domly divided into normal saline ( NS) control group, SC group and ED treatment group.The rats in each group were fur-ther divided into 5 subgroups according to different time points.The rats in SC group were kindled into epilepsy by lithium-pilocarpine chemical method.The protein expression of caspase-3 and caspase-12 was determined by immunohistochemistry methods.The mRNA expression of caspase-3 and caspase-12 was detected by RT-PCR.RESULTS:(1) The IA value of caspase-3 positive cells in 24~72 h SC group increased compared with NS group.With ED intervention, the IA value of caspase-3 positive cells decreased as compared with 48~72 h SC group.The results of RT-PCR showed that the mRNA ex-pression of caspase-3 was similar to the changes of protein.( 2 ) The results of immunohistochemistry showed that the IA value of caspase-12 positive cells in 12~72 h SC group increased compared with NS group.With ED intervention, the IA value of caspase-12 positive cells decreased as compared with 24~72 h SC group.The results of RT-PCR showed that the mRNA expression of caspase-12 was similar to the changes of protein.( 3 ) In ED group, Ⅴ grade convulsion was lower than that in SC group, and the latent period of seizures in ED group was significantly longer than that in SC group.CON-CLUSION:Edaravone inhibits the expression of caspase-3 and caspase-12 in pilocarpine-induced seizures in rat hippo-campus, suggesting that edaravone has protective effect against the damage caused by status convulsion.
3.A multi-factor analysis on the prevalence of lesion of hip joint in patients with ankylosing spondylitis

Objective To analyze the multiple factors for ankylosing spondylitis(AS)patients developing hip joint disease. Methods One hundred and two patients with AS complicated with hip joint damage (group A)were compared with 54 patients with AS without hip joint disease(group B). A univariate and multivariate unconditional-Logistic regression analysis was carried. Results The mean age at the time of disease onset was(17±8)years old in group A and(24±7)years in group B(P<0.05). The course of disease onset was(5±4)years old in group A and(11±5)years in group B(P<0.05). The childhood of disease onset was 37.3% in group A and 20.4% in group B(P<0.05). The patients who had hip pain at the disease onset was 38.2% in group A and 25.9% in group B(P<0.05).The incidence of peripheral arthritis was 39.2% and 20.4%(P<0.05)in patients of group A and group B respectively. Laboratory and X-ray findings showed that ESR, CRP, IgG and IgM levels were higher in group A than those in group B. SASP and thalidomide dosage taken in group A was lower than that in group B(P<0.01), the dosage of prednisone taken was higher in group A than in group B. A multivariate unconditional logistic regression analysis showed 5 factors, including the younger age of the time of disease onset, the short disease duration, the childhood of disease onset and hip joint involvement at the onset were associated with the occurrence of hip joint involvement. Conclusion The younger age, childhood and hip joint involvement at the time of disease onset, short disease duration may be the risk factors and SASP may be the protecting factor for patients developing hip joint lesion. More cases and factors analysis may be helpful to predict hip joint lesion in AS and to reduce the prevalence of disability.
4.Effect of palmatine sequential combination of exercise therapy on serum inflammatory markers in pelvic inflammation and its clinical effect

Objective To explore the effect of palmatine sequential therapy combined with exercise therapy on serum inflammatory markers in the treatment of patients with pelvic inflammatory disease.Methods 120 cases of pelvic inflammatory disease from January 2015 to January 2016 admitted to our hospital and randomly divided into experimental group and control group (60 cases of each group).The two groups of patients were given basic treatment of ciprofloxacin and tinidazole, while experimental group were given fibrauretine combined with exercise therapy, with a course of 4 weeks. Results Before treatment, there was no significant difference in serum IL-1β, IL-6, TNF-αand CRP levels between two groups.After treatment, the above indicators in experimental group were lower than those in control group(P<0.05).After treatment, the scores of abdominal pain, lumbosacral pain, vaginal discharge volume, white and thin leucorrhea, dizziness and tinnitus, menometrorrhagia or hypomenorrhea were significantly lower in experimental group than those in control group(P<0.05).The total effective rate of experimental group was 96.67%, which was significantly higher than 83.33%in control group, the difference was statistically significant(P<0.05).Conclusion Palmatine sequential therapy combined with exercise therapy could further improve the patient's inflammatory response syndromes, thereby improving clinical outcomes.
5.Processing of realgar and re-evaluation on medicinal efficacy and toxicity

Objective: To prepare the processing solution of realgar by bacteria and to explore the change of pharmacodynamic action and toxicity. Methods: The processing solutions of realgar were prepared using the strain thiobacillus ferroxidans, the realgar powder processed by normal method was selected as control. The effects on RPMI8226 cell proliferation of them were investigated by methyl thiazolyl terazolium (MTT),cell cycle distribution was determined by FCM in RPMI8226 cell. The distribution of arsenic concentration in blood and tissues including heart, liver, spleen, lung, and kidney in the samples were determined by Inductively Coupled Plasma-atomic Emission Spectrometry (ICP-AES). Results: Compared with control group, the solubility rate and the induced apoptotic rate increased, and dosage significantly reduced, while arsenic was hardly found in the tissue in processing group by bacteria. Conclusions: The processing group by bacteria may be enhanced medicinal efficacy and inhibited the toxicity in realgar. It’s intending to provide new approaches for scientific processing realgar.
6.Synergistic effects of Honghua injection combined with oxaliplatin in treatment of liver metastasis from colon cancer

Objective The liver is the main target of colon cancer metastasis, and liver metastasis is important and difficult in colon cancer therapy.This study was to investigate the synergistic effects of Honghua injection combined with oxaliplatin in treatment of liver metastasis from colon cancer. Methods Spleen-preserving method was used to establish the model of liver metastasis from colon cancer.The model mice were randomly divided into 5 groups:control group, oxaliplatin group, low dose ( low dose of Honghua injection combined with oxaliplatin) group, medium dose ( medium dose of Honghua injection combined with oxaliplatin) group, high dose (high dose of Honghua injection combined with oxaliplatin) group (n=10).Oxaliplatin were given twice a week at a dose of 5 mg/kg.Honghua injection was given at doses of 2.5, 1.25 and 0.625 g/kg for consecutive 14 days, and the control group received sa-line.After 14 d, all the mice were killed, and the tumor inhibiting rates and liver metastasis rates were calculated.The levels of serum vascular endothelial growth factor ( VEGF) and hepatocyte growth factor ( HGF) were measured by ELISA, and the mRNA expressions of VEGF and HGF in liver were determined by PCR. Results The inhibition rate of low, middle and high dose group achieved 63.2%, 69.2%and 71.4%respectively, which was significantly higher than that of control group (P<0.01).Compared with oxaliplatin group,middle and high dose groups showed a significant higher inhibition rate (P<0.05).Compared with control group, the liver metastasis rates in middle and high dose groups significantly decreased from 90%to 60%and 50%respectively (P<0.05).The serum VEGF and HGF levels showed significant decrease in all treatment groups([83.4 ±25.6],[81.5 ±23.7],[75.8 ±19.4],[72.7 ±20.5] pg/mL),([56.2 ±13.8],[55.4 ±15.7],[48.9 ±13.6],[42.3 ±11.8]pg/mL) compared with control group ([125.3 ±36.2], [98.4 ±26.1]pg/mL) (P<0.01).The mRNA expressions of VEGF and HGF in liver were significantly decreased in all the Hong-hua injection combined with oxaliplatin groups compared with control group ( P<0.05) .Compared with oxaliplatin group, the serum VEGF level in high dose group showed a significant reduced mRNA expression of VEGF (P<0.05). Conclusion Honghua injec-tion can significantly enhance the inhibitory effects of Oxaliplatin on liver metastasis from colon cancer, and the mechanism may con-tribute to the inhibition of VEGF expression and the prevention of angiogenesis in liver.

8.Research progress of the influence of alcohol on the local microenvironment of femoral head

BACKGROUND: The pathogenesis of alcoholic avascular necrosis remains unclear. OBJECTIVE: To introduce the physiological structure and pathologic changes induced by alcohol of the microenvironment of femoral head, and to review the research status of the influence of alcohol on the microenvironment of femoral head. METHODS: A computer-based online research of PubMed database for clinical and basic studyies concerning the effect alcohol on the microenvironment of femoral head published from 1990 to 2016. The pathological changes of alcoholic osteonecrosis of the femoral head were overviewed. RESULTS AND CONCLUSION: There are various theories about the pathogenesis of alcoholic osteonecrosis of thefemoral head, but the essence is the disturbance of blood circulation in femoral head, and long-term decrease in the nutrients and oxygen supplying of bone leading to bone degeneration and necrosis. Large-dose intake of alcohol will induce the imbalance of oxidation and anti-oxidation system, which dose harm to the microenvironment of femoral head. The change of oxygen supplying accelerates the necrosis of femoral head, the weakened differentiation of bone marrow mesenchymal stem cells into osteoblasts and the disturbance of bone resorption-formation imbalance finally result in the necrosis and collapse of femoral head. It is expected to provide a new therapeutic strategy for delaying or preventing the early femoral head necrosis by controlling the microenvironment of femoral head.

10.Relationship Between Syndromes and Prescriptions of Damp Disease: a Neural Network-based Study on Cases from Lin Zheng Zhi Nan Yi An Shi

Objective Taking medical cases in Lin Zheng Zhi Nan Yi An Shi as examples to analyze the network relationship between the syndromes and prescriptions through building a medical case model forecasting medication via artificial neural networks for the syndromes and prescriptions in medical cases. Methods The study screened medical cases in Lin Zheng Zhi Nan Yi An Shi, and standardized and entered the data with Python language programming. PyBrain module was used to build and train a network model. The MatPlotLib module drew the error curve and the predicted fit curve, and evaluated the sensitivity and specificity. NetworkX module realized the visual expression of the network relationship between the syndromes and prescriptions, and analyzed the medicine within the prescriptions and compatibility relationship and the relationship between the pathogenesis and pathology. Results The sensitivity of the constructed medical case network model was 96.15% and the specificity was 75.00%. The visual mapping of the network relationship between the syndromes and prescriptions and the analysis on single, single group, and multi-angle were realized. Conclusion Neural network is capable to simulate the relationship between syndromes and prescriptions of medical knowledge. The visual combination and manifestation of network can provide a feasible solution for the knowledge discovery in medical literature.
