1.Effects of Radix Paeoniae Rubra Extracts on the Endoplasmic Reticulum Stress and Macrophage Recruitment of Diabetic Cardiomyopathy Rats

Objective To investigate the protective effects of Radix Paeoniae Rubra(RPR)extracts on reticulum stress and macrophage infiltration on diabetic cardiomyopathy rats and the mechanism. Methods Sprague ̄Dawley rats were randomly divided into 5 groups: normol control, model control (induced by streptozotocin),RPR extracts at low dose (i.p.15 g.kg-1 ), middle dose (i.p.30 g.kg-1 ), and high dose (i.p.60 g.kg-1 ).The artery intubation was carried out in all rats for detecting cardiac function 10 weeks later.The myocardial structure was detected by masson staining.The level of myocardial tissue glucose regulatory proteins GRP78,caspase ̄12 and caspase ̄3 expression were measured by Western blot.The ED ̄1 protein was tested by immunohistochemical staining. Results Compared with the normal controls, the model rats presented decreased LVSP, +dp/dtmax ,-dp/ dtmax , LVEF, increased expressions of GRP78, caspase ̄12,caspase ̄3, ED ̄1 and the synthesis of collagen fiber(P<0.05 or P<0.01).RPR extract inhibited the above indexes of model control rats in a concentration ̄dependent manner(P<0.05 or P<0.01). Conclusion The RPR obviously ameliorates the cardiac function and myocardial fibrosis of diabetic cardiomyopathy rats,which may be associated with suppressing the expression of GRP78,caspase ̄12,caspase ̄3 and macrophage infiltration.

3.Correlations of ambulatory pulse pressure to large artery elasticity and vascular endothelial function in patients with primary hypertension

BACKGROUND: Ambulatory blood pressure monitoring can sensitively and objectively reflect blood pressure level, which is closely related to target organ damage and disease prognosis. In hypertension, vascular endothelial damage is the most common lesion to target organs. There is little known about how ambulatory pulse pressure correlates to large artery elasticity and vascular endothelial function. OBJECTIVE: To investigate changes of large artery elasticity and of vascular endothelial function in patients with primary hypertension using an automatic pulse wave velocity determinator and ultrasound techniques, and to analyze the correlation of ambulatory pulse pressure to large artery elasticity and vascular endothelial function.DESIGN: A non-randomized concurrent control clinical observation. SETTING: Diagnosis and Treatment Center for Coronary Heart Disease, the 305 Hospital of Chinese PLA. PARTICIPANTS: A total of 156 inpatients and/or outpatients, who were recently confirmed with primary hypertension, were recruited for this study between June 2005 and April 2007. Patients consisted of 114 males and 42 females. All patients averaged 56 ± 4 years of age (range: 40-75). Inclusive criteria: Corresponding to diagnostic standards for preventing and treating hypertension instituted in 2004 by Chinese scholars. Confirmed as primary hypertension within 1 month. Not receiving any blood pressure lowering, hypolipidemic or nitrate-like drug treatments. Written informed consents for laboratory measurements were obtained from all subjects. The study was approved by the hospital's Ethics Committee. METHODS: According to the mean pulse pressure over 24 hours, all patients were assigned into 3 groups: Group A (mean pulse pressure < 40 mm Hg, n=92), group B (40 mm Hg ≤ mean pulse pressure < 60 mm Hg, n=39) and group C (mean pulse pressure > 60 mm Hg, n=25). In each group, daytime pulse pressure and night-time pulse pressure, as well as 24-hour mean pulse pressure were measured using a non-invasive portable ambulatory blood pressure monitor (ABPM-04, Meditech Inc, USA). Carotid-femoral and carotid-radial arterial pulse wave velocities were measured using an automatic pulse wave velocity determinator to evaluate large artery dilation. Blood flow mediated and nitroglycerin-dependent dilatation of the brachial artery was determined using a high-resolution ultrasound technique to evaluate vascular endothelial function. MAIN OUTCOME MEASURES: Correlations of ambulatory pulse pressure to large artery dilation and arterial endothelial function. RESULTS: All 156 patients were included in the final analysis. Correlation of ambulatory pulse pressure to large artery dilation: Carotid-femoral arterial pulse wave velocity was significantly positively correlated to daytime pulse pressure, night-time pulse pressure and 24-hour mean pulse pressure, with coefficient of partial correlation being 0.310, 0.281 and 0.303, respectively, P < 0.01). There were no significant correlations of carotid-radial arterial pulse wave velocity to daytime pulse pressure, night-time pulse pressure or 24-hour pulse pressure (P > 0.05). Correlation of ambulatory pulse pressure to arterial endothelial function: There was a linear relationship between ambulatory pulse pressure and blood flow-mediated blood vessel dilatation values. Linear correlation analysis was performed, taking ambulatory pulse pressure as an independent variable, and endothelial-dependent dilatation as a dependent variable. Results demonstrated that blood flow-mediated blood vessel dilatation was significantly negatively correlated to daytime pulse pressure, night-time pulse pressure and 24- hour mean pulse pressure (r = -0.684, -0.597, -0.668, P < 0.01). There was no correlation of ambulatory pulse pressure to non-endothelial-dependent blood vessel dilatation. CONCLUSION: Ambulatory pulse pressure increase is closely related to large artery elasticity decrease and injury to endothelial function in patients with primary hypertension.
4.DRGs-based medical performance evaluation of tertiary hospitals in Yunnan

Objective To identify objective methods for medical service performance evaluation,and means to improve the efficiency of medical services.Methods By means of collection of medical records from 10 tertiary hospitals in Yunnan in 2012,and Diagnosis-related groups of Beijing version(BJDRGs)as a risk-adjustment tool,medical service performance was made to such hospitals.Results The capacity,efficiency and medical safety of hospital of the hospitals were subject to a comprehensive evaluation of their medical service capabilities.In terms of discharged inpatients and total weight,hospital A were found to be the highest(77 458 cases,104 227.5 respectively),and hospital I the lowest(25 450 cases,38 162.28)among these hospitals in 2012; compared with other hospitals,DRG groups of hospitals A,b and D were found higher,while the case mix index(CMI)of hospitals e and I were higher.The expenditure index of hospital C is the lowest(0.9359,0.8514); the mortality in cases of low risk of hospitals B,c and D were the lowest,while hospital F was the highest.Conclusion DRGs as a risk adjustment tool,can significantly improve the reliability,science and continuity of medical service performance evaluation,providing reliable basic data for hospitals accreditation in Yunnan.
5.Effect of total flavonoids of Astragalus on apoptosis of cultured bovine retinal capillary pericytes under high glucose

Objective: To study the effects of total flavonoids of Astragalus on apoptosis of bovine retinal capillary pericytes(BRPs) under high glucose.Methods: The third generation of nearly symphysic bovine retinal vessel pericytes cultivated in vitro were divided into normal control group,high glucose group,and Astragalus total flavonoids groups(0.25,0.5,1.0,2.0 mg/ml) at random.After being incubated for 6 days,apoptosis of BRPs were detected by TUNEL method.TBA method was used to detect the contents of MDA.Xanthine oxidase method was used to detect the SOD activities.Results: Compared with high glucose group,the apoptosis,MDA contents,SOD activity,MDA content/SOD activity of BRPs reduced markedly in total flavonoids of Astragalus groups(0.5,1.0,2.0mg/ml)(P
6.Quantitative analysis of different files in full-text papers published in biomedical journals

The variability and number of appendixes, publication year and publication journals of full-text files conforming to the JATS standards, selected from 891334 full-text papers published in biomedical journals, were analyzed, which showed that the number of non-text files increased sharply with their appendixes reached 45, and jpg, png, tif accounted for 81%.
7.Evaluation of arterial function in Chinese Miao tribe population and analysis of its influential factors

Objective To explore the arterial function of the Miao nationality people who live in natural circumstance.Methods 244 Miaos were mcmited in our studv.Right blood pressure and biochemistry examination were measured.PWV and CAVI were measured using validated automatic devices and used as indexes of arterial stiffness.Ultrasonic indexes such as carotid diameter,carotid plaque,IMT、flow-mediated dilation(FMD) were measured as well.Results Body mass index,waist-to-hip ratio,heart rates,blood lipid level and glucose had no significant difierence between male and female groups.Arterial function indexes such as C-F PWV(9.50±2.14 vs.8.81±1.69,t=2.074,P=0.006),C-R PWV(11.39±1.74 vs.10.54±1.77,t=3.506,P<0.001),R-CAVI(8.25±1.35 vs.7.41±1.20,t=4.768,P<0.001),L-CAVI(8.21±1.30 vs.7.41±1.15,t=4.616,P<0.001),R-IMT(0.67±O.12 vs.0.63±0.13,t=2.503,P=0.012) and L-IMT(0.67±0.11 vs.0.63±0.14,t=2.337,P=O.018) had significant difference between the two groups.Conclusions The medical condition of the Mians needs to be impmved.It is useful to diagnose patients and hish risk people by systemic arterial function examination.
8.The role of mitochondrial apoptotic pathway in the apoptosis of cultured bovine retinal capillary pericytes induced by constant and intermittent high glucose

Objective To study the effects of constant and intermittent high glucose on the apoptosis of cultured bovine retinal capillary pericytes (BRPs), and to investigate the role of mitochondrial apoptotic pathway in the apoptosis of BRPs. Methods After being cultured under glucose with different concentrations tot 6 days, the change of uhrastructure of BRPs was observed under electronmieroscope, the apoptosis of pericytes was detected by TUNEL method, the mitochondrial inner transmembrane potential (△Ψm) was detected by laser scanning confocal microscopy, the change of cytochrome c (cyt-c) was assayed by spectrophotometer and the expression of apoptotic genes was detected by immunohistochemical method and RT-PCR. Results (1) BRPs showed typical changes of apoptosis in constant and intermittent high glucose concentrations. The apoptosis induced by constant high glucose concentration was more obvious than that by intermittent high glucose. (2) Constant and intermittent high glucose concentrations obviously decreased △Ψm compared with control group. The △Ψm of BRPs was correlated negatively with the apoptotie rate of BRPs (r = - 0.89, P < 0.01) ; (3) Constant and intermittent high glucose concentrations increased the release of cyt-c from mitochondria to cytoplasm, and the concentration of cyt-c in the cytoplasm was correlated positively with the apoptotic rate of BRPs (P < 0.01) ; (4) Constant and intermittent high glucose concentrations increased the expression of proapoptotie gene Bax and decreased the expression of prosurvival gene Bcl-2, resulting in increased Bax/Bcl-2 ratio. Bax/Bcl-2 ratio was negative correlatied with the △Ψm of BRPs, and positively eorrclaticd with the concentration of cyt-c in cytoplasm and apoptotic rate (both P < 0.01).Conclusion Constant and intermittent high glucose concentrations could decrease △Ψm, increase the release of cyt-c and induce the apoptosis of BRPs, the effects being stronger with constant high glucose concentration. The mitochondrial apoptotic pathway plays an important role in the apoptosis of BRPs, in which Bax and Bcl-2 are involved.
9.Effects of maternal position in the second stage of labor on delivery outcomes

Objective To evaluate the effects of the non-supine position and the supine position during the second stage of labor on delivery outcomes.Methods In total,642 singleton term primiparas in the Fourth Hospital of Hebei Medical University between January 2013 and March 2014 were divided into the non-supine position group (n=320),which included the lateral (n=204) and hands-and-knees position (n=116),and the supine position group (n=322) according to primipara's choice.Delivery outcomes in the two groups were compared using the Student's t test,Mann-Whitney rank sum test and the x2 test.Results The incidence of caesarean section was similar in the non-supine position and supine position groups [0.9%(3/320) vs 1.9% (6/322),x2=0.438,P=0.508].Compared with the supine position group,women in the non-supine position group had a higher incidence of perineal lacerations grede Ⅰ [85.2% (270/317) vs 65.2% (206/316),x2=33.884],longer duration of the second stage of labor [48.0 min (31.0-78.0 min) vs 41.0 min (25.0 67.3 min),Z=-3.066] and greater blood loss after 2 hours of labor [240.0 ml (200.0-287.5 ml) vs 210.0 ml (160.0-260.0 ml),Z=-3.736],but a significant reduction in posterolateral episiotomies [5.3%(17/317) vs 23.4% (74/316),x2=41.908],perinealedema [13.6% (43/317) vs 21.2% (67/316),x2=6.430] and meconiumstained liquor [20.8% (66/317) vs 33.2% (105/316),x2=12.356] (all P<0.05).In the non supine position group,fetal heart rate showed fewer early decelerations,variable decelerations and late decelerations than in the supine position group [10.1% (32/317) vs 17.1% (54/316),x2=6.593; 2.8% (9/317) vs 6.6% (21/316),x2=5.079; 3.2% (10/317) vs 7.6% (24/316),x2 6.139; all P<0.05,respectively].Conclusions The non supine position during the second stage of labor may improve delivery outcomes.
10. Evaluation of the reliability and validity of Chinese-version Good Death Inventory among oncology nurse specialist

Objective:
To evaluate the reliability and validity of Chinese-version Good Death Inventory among oncology nurse specialists.
Methods:
A further revision has been made in the Chinese-version Good Death Inventory from Taiwan experts through expert review and preliminary experiment. The reliability and validity of the Chinese-version GDI was evaluated among 180 oncology nurse specialists.
Results:
The Chinese-version of GDI was easy to understand. Most respondents finished the whole scale in 25 minutes and the valid response rate of the scale was100%. The I-CVI value of each item was 0.67-1.00, S-CVI/Ave was 0.938. KMO was 0.832, and the revised Chinese-version GDI consisted of 54 items in 18 dimensions (Core 10 Domains and Optional 8 Domains). The total scale′ s Cronbach′ s alpha coefficient was 0.950, split-half reliability coefficient was 0.832. Cronbach′ s alpha of Core 10 Domains was 0.938 and that of Optional 8 Domains was 0.876.
Conclusions
The Chinese-version GDI is a valid scale to measure end-of-life care comprehensive outcomes from Chinese oncology nurse specialists′ perspective.
