1.Relationship among contract nurses′ perceptions of organizational climate,job burnout and turnover tendency

Objective To explore the influence of organizational climate and job burnout on turnover tendency among contract nurses, and establish a model of this influence. Methods Totally 450 nurses were selected and investigated with Organizational Climate Questionnaire(OCQ), Maslach job burnout inventory(MBI) and Turnover Intention Scale(TIS). Results The total score of OCQ was 2.71 ± 0.58, the emotional exhaustion was 25.99 ± 13.89, the depersonalization was 10.21 ± 5.37,the personal accomplishment was 21.92 ± 9.27, and the score of TIS was 2.66 ± 0.94. Emotional exhaustion and depersonalization were significantly and negatively correlated to OCQ(r=-0.26,-0.22 ,P<0.01),and positively correlated to TIS(r=0.27,0.25,P<0.01). The personal accomplishment was positively correlated to OCQ(r=0.26,P<0.01), and negatively correlated to TIS(r=- 0.26,P<0.01).OCQ was negatively correlated to TIS(r=-0.37,P<0.01).Multiple linear regression analysis showed that OCQ and personal accomplishment could negatively predict nurses′ turnover tendency(β=-0.43,-0.14,P<0.01), and emotional exhaustion could negatively predict nurses′ turnover tendency(β=-0.10,P<0.01). The path analysis showed organizational climate negatively influenced nurses′ turnover tendency via job burnout, and the value of mediating effect was 23.8%(-0.10/-0.42). Conclusions Turnover tendency is significantly correlated with organizational climate and job burnout among contract nurses. The relationship between organizational climate and nurses′ turnover tendency among contract nurses is mediated in part by job burnout.
2.Preparation of Domiphen Bromide Osmotic Tablets

OBJECTIVE:To prepare domiphen bromide osmotic tablets,and to observe,analyze the mechanism of drug release.METHODS:Coating prescription was optimized by uniform design and the cumulative rates of drug release of different formulated preparations were measured,pharmaceutical features of domiphen bromide osmotic tablet and mechanism of drug release were studied.RESULTS:The dosage of PEG-400 and the coating membrane thickness all had an effect on drug release.The optimal coating technique was analyzed as 12%of PEG-400 and 10mg of coating.Spray speed,temperature and rotation speed did not affect releasing profiles.CONCLUSION:Improvement in formulation of coat of domiphen bromide osmotic tablet can result in drug release for 12h,whose mechanism includes diffusion and osmotic pump principle.
3.Formulation optimization of Domiphen osmotic pump tablets with Uniform design

Objective To prepare Domiphen osmotic tablets of zero release above 80% in vitro. Methods Coating prescription was optimized by uniform design and the drug release characteristic was tested in vitro. Results The lemma thickness was (shown as lemma weight) 10.75 mg and the content of the porosity-making agent was 12%. The three batchs of drug tablets release pattern were in accord with zero order kinetics during 12 h. Conclusion This method is easy to produce and reaches initial devise request.
4.Risk factors for healthcare-associated infection in patients with aneurysmal subarachnoid hemorrhage

Objective To investigate risk factors for healthcare-associated infection (HAI) in patients with aneurysmal subarachnoid hemorrhage(aSAH)in the department of neurology of a hospital.Methods Clinical data of 47 patients with aSAH who were admitted to a hospital from January 2014 to January 2015 were retrospectively analyzed.Results Of 47 patients with aSAH,17 developed 19 times of HAI,HAI rate was 36.17%,case infection rate was 40.43%.The main infection site was lower respiratory tract (n = 17,89.47%),followed by urinary tract (n=2,10.53%);2 patients had both lower respiratory tract infection and urinary tract infection.17 patients were performed pathogenic detection,13 pathogenic isolates were isolated from 9 patients.Multivariate non-conditional logistic regression analysis revealed that risk factors for HAI in patients with aSAH were disturbance of consciousness (OR,24.878 [95%CI ,3.996 - 156.040])and intensive care unit admission(OR, 8.645 [95%CI ,1.419-52.670]).The prognosis of patients with HAI was poorer than those without HAI(Z =4.108,P < 0.001 ).Conclusion Patients with aSAH are at high risk of HAI,it is necessary to take targeted prevention and control measures for reducing the occurrence of HAI and promoting good prognosis.
5.Allelic dropout of amelogenin gene on X chromosome

Objective To investigate the mechanism that cause allelic dropout of amelogenin gene on X chromosome(Amel-X)when using routine Sullivan106/112 bp primer set in sex identification and discuss its influence on the forensic sex identification and the clinical diagnosis.Methods Amel-X dropout was validated with Sullivan212/218 bp and Haas-Rochholz80/83 bp primer sets.Amplification of amelogenin gene was used to analyze dropout of the Amex-X followed by sequencin.Results Sullivan212/218 bp and Hgas-Rochholz80/83 bp primer sets could be used to identify gender correctly.Three types of point mntation were observed in the forward primer binding region of the Sullivan106/112 bp primer set by sequencing in the lost Amel-X,including single point mutation at 2nd and 13th sites,respectively,and heterozygous multiple point mutations at 2nd and 13th sites.Conclusions Point mutation in the primer binding region may result in a failure to amplify amelogenin allele and thus lead to a null allele.This finding should be mid attention to because it may interfere with the sex identification.
6.Clinical features and genetic analysis of 17 cases of glucose-6-phosphate dehydrogenase deficiency in Guizhou province

Objective To explore the clinical and genetic characteristics of 17 cases with glucose-6-phosphate dehydrogenase(G6PD) deficiency in Guizhou,China.Methods The clinical features of 17 patients with G6PD deficiency were analyzed,DNA samples were obtained from the patients and some mothers,and the exons and flanking intronic sequences of the G6PD gene were analyzed by the polymerase chain reaction and sequencing.Results The cases had diverse phenotypes,these patients had acute haemolytic anaemia triggered by eating broad beans,infections,ingestion of specific drugs or the neonatal period and chronic nonspherocytic haemolytic anaemia.Three cases of the patients had concomitant diseases for α Mediterranean anemia,acute myeloid leukemia M2 type and neonatal anal membrane stenosis,respectively.G1376T,G1388A and A95G were the commonest G6PD variants in patients in Guizhou,China.G1376T,G1388A and A95G mutations were observed in 82.4% cases.Two patients had only compound variants(c.1311 C > T,IVS11 nt 93 T > C).One case in the Rongjiang County,Guizhou Province had novel compound variants (c.G1388A,IVS10-10 T > G) in the world.A patient's mother in the Guiyang City,Guizhou Province,China had compound variants (c.1376 G > T,1311 C > T,IVS11 nt 93 T > C) as a carrier.Conclusions G6PD deficiency has a wide range of clinical heterogeneity.A novel G6PD compound variant haplotype c.G1388A,IVS10-10 T > G was first found in the world,and the SNP spectrum of G6PD was enlarged.There may be a G6PD compound variant haplotype c.1376 G > T,1311 C > T,IVS11 nt 93 T > C in Guizhou.
7.Comparative study in pain and the success rate of puncture between venipunctnre methods from various angles

Objective To explore a venipuncture method that can reduce the pain, improve the suc-cess rate of puncture as well as reduce vascular injury. Methods Self-controlled method was applied in 200 patients, needle tips with 20~25°,31~40°,41~50°, 51~60°, > 60° angles on the top of the blood ves-sels,the inclined plane of the needle parallel to the longitudinal axis of the blood vessel (that was,the in-clined plane to the left or right) or traditional method with upward of needle tips were adopted, the observa-tion group and the control group were divided according to different methods, pain and success rate of puncture were compared. Results There were differences between the two groups,results of the observa-tion group were better than those of the control group (except when > 60°, success rate showed no differ-ence). Conclusions Venipuncture with needle tip parallel to the longitudinal axis of the blood vessels can significantly reduce pain in patients, improve the success rate of puncture and reduce the vascular in-jury, which were superior to the traditional venipuncture method. Whatever method is chosen, 51~60° is the best angie into the vessel.
8.On hemolysis of extracts of Lonicera Macranthoides flower bud and two saponins

Aim To figure out whether Lonicera mac-ranthoides could induce hemolysis. Methods In vitro, macroscopic observation and spectrophotometry were used to observe whether the solutions of extracts from Lonicera macranthoides, MacranthoidinB and Dipsa-cosideB could induce hemolysis in 2% red cell suspen-sion of New Zealand white rabbits. And the three test-ed materials were prepared in concentration gradient of 5, 10, 20, 30, 40, 50, 60, 70, 80, 90, 100 mg· L - 1; and in vivo, mice were respectively treated with MacranthoidinB (0. 110 g·kg - 1 , 0. 055 g·kg - 1 ), DipsacosideB(0. 020 g·kg - 1 , 0. 010 g·kg - 1 ), ex-tracts (2. 275 g·kg - 1 , 1. 137 g·kg - 1 , crude drugs) once per day for 7 days, and all of the tested doses de-pended on the clinical doses. Then, RBC, RET and MCHC before and after administration were tested. Re-sults The hemolytic ratio in each treated group was below 5% in vitro. And in vivo, the three materials did not induce hemolysis and had no significant influence on RBC,RET and MCHC(P > 0. 05). Conclusions Extracts from flower bud of Lonicera macranthoides, MacranthoidinB and DipsacosideB, have not caused hemolysis in vivo and in vitro in this research.
9.Therapeutic effect of autologous stem cell transplantation of patients with cerebral hemorrhage sequelae under the stereotactic

Objective To investigate the therapeutic effect of autologous stem cell transplantation research for patients with cerebral hemorrhage sequelae under the stereotactic.Methods One hundred patients with cerebral hemorrhage from Jan.2011 to Sep.2013 in our hospital were selected and randomly divided into the experimental group (n =50) and the control group (n =50).The patients of experimental group were given autologous stem cell transplantation under the stereotactic in 6 months after cerebral hemorrhage,while the patients in control group were just given traditional treatment.At 6,7 and 12 months after cerebral hemorrhage,rate with neural function defect scale and functional independence measure(FIM) scores of the two groups were compared.Results FIM scores in the experimental group was 102.08 ± 8.28,significant higher than that in control group(95.28±8.75,P<0.05).Functional independence measure scores in the experimental group at 7 months after cerebral hemorrhage was 13.12±4.00,significant lower than that in control group(20.40±4.33,P <0.05).While,there was no statistical difference at 6 months and 12 months after cerebral hemorrhage between the two groups(P>0.05).Conclusion The therapeutic of autologous stem cell transplantation on patients with cerebral hemorrhage sequelae under the stereotactic is benefit at short term,but the long term therapy effective still needs further study.
10.Correlation between diabetic distress and coping styles among patients with type 2 diabetes

Objective To investigate the correlation between diabetic distress and coping styles among patients with type 2 diabetes. Method One hundred inpatients with type 2 diabetes were engaged in the investigation of their diabetes distress and coping styles by the diabetes distress scale (DDS) and medical coping modes questionnaire (MCMQ) and the correlation between them. Results The average score of diabetic distress was (2.17 ± 0.71), which was at mild level. 57%of them were at the higher level of diabetic distress. Confrontation and avoidance dimensions were lower than the norm in coping styles, and the acceptance dimension was higher than the norm (all P<0.01). The average scores of diabetic distress, regimen-related distress and physician-related distress subscales were negatively correlated with confrontation (all P<0.05). The average score on diabetic distress and the subscales were positively correlated with avoidance and acceptance dimension (all P<0.01). Conclusions Diabetic distress is prevalent in type 2 diabetic patients and the degree of positive coping is still not insufficient. The diabetic distress is negatively correlated to the confrontation, and positively correlated to the avoidance and the acceptance dimension. The medical staff should instruct the patients to adopt positive coping styles, and avoid negative coping styles to reduce diabetic distress. It will be conductive to the physical and mental rehabilitation of the patients as well.
