1.Effect of palmatine sequential combination of exercise therapy on serum inflammatory markers in pelvic inflammation and its clinical effect

Objective To explore the effect of palmatine sequential therapy combined with exercise therapy on serum inflammatory markers in the treatment of patients with pelvic inflammatory disease.Methods 120 cases of pelvic inflammatory disease from January 2015 to January 2016 admitted to our hospital and randomly divided into experimental group and control group (60 cases of each group).The two groups of patients were given basic treatment of ciprofloxacin and tinidazole, while experimental group were given fibrauretine combined with exercise therapy, with a course of 4 weeks. Results Before treatment, there was no significant difference in serum IL-1β, IL-6, TNF-αand CRP levels between two groups.After treatment, the above indicators in experimental group were lower than those in control group(P<0.05).After treatment, the scores of abdominal pain, lumbosacral pain, vaginal discharge volume, white and thin leucorrhea, dizziness and tinnitus, menometrorrhagia or hypomenorrhea were significantly lower in experimental group than those in control group(P<0.05).The total effective rate of experimental group was 96.67%, which was significantly higher than 83.33%in control group, the difference was statistically significant(P<0.05).Conclusion Palmatine sequential therapy combined with exercise therapy could further improve the patient's inflammatory response syndromes, thereby improving clinical outcomes.
2.Study of the surgical technique for tetralogy of Fallot with complete atrioventricular septal defect

Objective To discuss the surgical technique in correction of tetralogy of Fallot with complete atrioventricular septal defect (TOF-AVSD). Methods 16 patients aged 2-16 years underwent correction of TOF-AVSD. The atrioventricular septal defect was closed through a right atriotomy and longitudinal right ventriculotomy in each case. The three-patch technique was used for the first 7 cases and two-patch technique for the later 9 cases. The commissure between the superior and inferior bridging leaflets of the left portion of the common atrioventricular valve was closed in each patient. 3.Double switch operation for congenital corrected transposition of great arteries with heart anomaly

Objective To report the outcomes of double switch operation as the primary approach for congenital corrected transposition of great arteries (cTGA) with heart anomaly. Methods From April 2002 to June 2004, seven patients ranged 4 to 15 years with cTGA underwent double switch operation. Six patients were situs solitus (SLL segmental anatomy) while one patient was situs inversus (IDD segmental anatomy). The heart defects included ventricular septal defect in 6 cases, secondary atrial septal defect in, double outlet of right ventricle in 1, pulmonary stenosis in 6, dextrocardia in 3 and levocardia in 1. The operative procedures comprised of 4 modified Senning+Rastelli, Mustard +Rastelli +bidirectional Glenn, 1 Senning +Rastelli, 1 modified Senning+switch. Results There was one early operative death after modified Senning+switch operation. The cause of death was left ventricular failure. The postoperative complications included severe low cardiac output syndrome in 1, temporary atrioventricular block in 1, pleurisy and low plasma protein in 2. The survivors were followed up from 2 to 24 months, all of them were in sinus cardiac rhythm except one case with junction cardiac rhythm. All were in NYHA I class. Conclusion Anatomic correction of cGTA by double switch operation can be performed with lower operative mortality and good medial-term outcome. In the SLL cases, modified Senning operation may yield better outcomes than Mustard operation.
4.Surgical treatment of tetralogy of Fallot with pulmonary atresia

Objective To evaluate the experiences of complete repair of tetralogy of Fallot with pulmonary atresia (TOF-PA). Methods From June1984 to December 2003, 24 patients with TOF-PA underwent complete surgical repair. There were ten males and fourteen females. The age ranged from 6 months to 9 years. 14 patients were in type I TOF-PA, type II 8, type III and type IV 1 each. Through a standard median sternotomy incision, all patients were operated on under moderate hypothermia and cardiopulmonary bypass. An artificial vessel patch with a monocuspid valve was applied to enlarged the outlet of right ventricle in typeIpatients and a conduit with valves from right ventricle to pulmonary artery was applied in typeII patients. Midline one-stage complete unifocalization and repair of ventricular septal defect and major aortopulmonary collaterals were used in patients with type III and IV. Results Early mortality was 13.3% (4 patients). The causes of death were severe low cardiac output (2 patients), respiratory function failure (1 patient) and multiorgan function failure (1 patient). There was no late death. 18 patients were followed up from one month to 15.5 years. Postoperative heart function (NYHA) was class I or II in 16 patients and class III or IV in 2. Conclusion Surgical repair of patients with TOF-PA can be achieved with acceptable mortality and good results. In selected patients one stage surgical correction can be done through a midline sternotomy approach.
5.The study of pulmonary arterial development in patients after 5 years of extracardiac tota cavopulmonary connection

ObjectiveTo study the pulmonary arterial development over five years in patients underwent extracardiac total cavopulmonary connection (ETCPC).Methods43 survived patients,who had undergone ETCPC were examined with pulmonary perfusion at one month and five year following the operation.Central venous pressure (CVP) and arterial oxygenation saturation (SatO2 % ) were measured by right cardiac catheter,pulmonary arterial index (PAI) and pulmonary vascular resistance (PVR) were calculated.Pulmonary blood distribution were measured and calculated by 99m Tc-MAA perfusion imaging.ResultsThe PAI and PVR of the follow-up group reduced significantly ( t =2.41,P < 0.05 ; t =2.08,P < 0.05 ),CVP also reduced significantly ( t =2.69,P < 0.05 ),but SatO2 % did not changed significantly.Total radionuclide counts and the ratio of rightorleft pulmonary perfusion did not change significantly.( t =0.38,P > 0.05 ;t =1.12,P > 0.05 ),but the ratio of the superior and inferior lobe decreased significantly( t =2.54,P < 0.05 ).ConclusionThe weak pulsation and low dynamic of Fontan circulation also can promote pulmonary vascular development.However,the improvement of hemodynamic in pulmonary circulation at mid-term follow will not lead to an increased amount of pulmonary perfusion or oxygen supply,which is probably due to the massive opening of the arteriovenous shunt and increased futile circulation.
6.Clinical features and genetic analysis of 17 cases of glucose-6-phosphate dehydrogenase deficiency in Guizhou province

Objective To explore the clinical and genetic characteristics of 17 cases with glucose-6-phosphate dehydrogenase(G6PD) deficiency in Guizhou,China.Methods The clinical features of 17 patients with G6PD deficiency were analyzed,DNA samples were obtained from the patients and some mothers,and the exons and flanking intronic sequences of the G6PD gene were analyzed by the polymerase chain reaction and sequencing.Results The cases had diverse phenotypes,these patients had acute haemolytic anaemia triggered by eating broad beans,infections,ingestion of specific drugs or the neonatal period and chronic nonspherocytic haemolytic anaemia.Three cases of the patients had concomitant diseases for α Mediterranean anemia,acute myeloid leukemia M2 type and neonatal anal membrane stenosis,respectively.G1376T,G1388A and A95G were the commonest G6PD variants in patients in Guizhou,China.G1376T,G1388A and A95G mutations were observed in 82.4% cases.Two patients had only compound variants(c.1311 C > T,IVS11 nt 93 T > C).One case in the Rongjiang County,Guizhou Province had novel compound variants (c.G1388A,IVS10-10 T > G) in the world.A patient's mother in the Guiyang City,Guizhou Province,China had compound variants (c.1376 G > T,1311 C > T,IVS11 nt 93 T > C) as a carrier.Conclusions G6PD deficiency has a wide range of clinical heterogeneity.A novel G6PD compound variant haplotype c.G1388A,IVS10-10 T > G was first found in the world,and the SNP spectrum of G6PD was enlarged.There may be a G6PD compound variant haplotype c.1376 G > T,1311 C > T,IVS11 nt 93 T > C in Guizhou.
7.Risk Factors for Arterial Stiffness in Elderly Patients with Essential Hypertension

Results After adjustment for body mass index (BMI),sex,stroke and fasting blood glucose,logistic multivariable analysis showed that cfPWV associated with age,systolic blood pressure(SBP),pulse pressure,coronary artery disease,diabetes,carotid atheromatous plaque and hemoglobin A1c. Conclusion Arterial stiffness in elderly patients with essential hypertension was strongly positively associated with age,SBP,pulse pressure,coronary artery disease,diabetes,carotid atheromatous plaque and hemoglobin A1c.

Object To investigate the effects of ultra-fine powder technique and granularity of pellets on dissolution rate in vitro. Methods The dissolution rate of ultra-fine ERMIAO PILL with different granule diameters in vitro was measured and compared with the index of berberine by UV spectro-photometry. Results The dissolution parameters T 50 and T d of four kinds of ultra-fine ERMIAO PILL are 61.60, 19.48, 17.84, 8.97 min and 102.3, 33.29, 26.98, 14.77 min, respectively. Those of general powder ERMIAO PILL with granule diameter of 2.4 mm are 89.61 and 155.68 min. Conclusion The dissolution rate of ultra-fine powder is quicker than that of general powder, and the rate increases with the granularity of PILL decreasing.
10.Compound radix angelicae sinensis injection improves oocyte and segmentation sphere injury resulted from subacute exposure to heavy diesel exhaust particles in female mice

Objective To investigate the effects of compound radix angelicae sinensis injection on oocyte and segmentation sphere injured by subacute exposure to diesel exhaust particles (DEP) in female mice. Methods Two hundred and ten 21-day-old ICR female mice were randomly divided into 5 groups: the control group (group A), DEP group (group B), DEP+low-dose group (group C),DEP±middle-dose group (group D) and DEP+high-dose group (group E). There were 42 mice in each group. The mice were inoculated with 30 μ1 DEP suspension at 12.0 μg/μl (group B-E) or the same volume of vehicle (PBS, group A) on pharynx posterior wall by sample pipettor beginning at day 21 and repeated every 3 days for 4 times. The mice were sacrificed three days after the last exposure.Compound radix angelicae sinensis injection containing 75 (group C), 150 (group D) and 300 (group E)grams of crude drug, respectively, which was intraperitoneally administered for each mouse daily from the day of the first DEP inoculation till the day before sacrifice, consecutively for 12 days. The general conditions were observed, and the body weight and ovary/body weight ratio were tested. Superoxide dismutase (SOD) activity, malondialdehyde (MDA) and reduced glutathione (GSH) contents in ovarian tissues were assayed. Rates of survival, germinal vesicle breakdown, extrusion of the first polar body and in-vitro fertilization, and quantity of mitochondrial DNA for oocytes were investigated.Ultrastructural changes of oocytes were observed. Results ( 1 ) No significant difference of the body weight was found among all the groups (P>0. 05). The ovary weight, ovary/body weight ratio, ovary SOD and GSH content were significantly decreased in groups B [( 1.5 ± 0. 6) mg, ( 7.2 ± 2. 5) × 10-5 ,(192. 10±23.67) nU/mg prot and (262.40 ± 31.60) nmol/mg prot], and C [( 1.7 ± 0. 2) mg,(8.9±0.6)× 10-5, (198.92±24.27) nU/mg prot and (271.66±14.58) nmol/mg prot] and D [(2. 1±0. 2) mg, (9. 8±1. 1)×10-5, (214. 37±27. 19) nU/mg prot and (285. 93±9. 55) nmol/mg prot] as comparing to groupA [(3. 3±1. 5) mg, (15.4±7.3)×10-5, (292. 30 ± 40. 03) nU/mg prot and (367.98±24.59) nmol/mg prot (P< 0. 05 or P<0.01); and significantly increased in group E [(3. 7±1.1) mg, (18. 7±5. 4)× 10-5, (279. 10±12. 63) nU/mg prot ]and (353. 59±10. 61) nmol/mg prot]comparing to group B (P<0. 01). MDA content was signi-ficantly increased for groups B, C and D [(3. 88±0.35) nmol/mg prot, (3. 62 ± 0. 19) nmol/mg prot and (2. 63 ± 0. 34) nmol/mg prot] comparing to group A [(2. 18±0. 44) nmol/mg prot](P<0. 05 or <0. 01, respectively); and significantly decreased for group D and E (2. 35±0. 37 nmol/mg prot) comparing to group B (P<0. 01). (2) In all observed time points, oocyte survival rate in group B and C, extrusion rate of the first polar body and in-vitro fertilization rate in group B, C and D were significantly lower than in group A (P < 0. 05 or <0. 01), and all those in group E were significantly higher than in group B (P<0. 05). Rates of germinal vesicle breakdown were 100% in all five groups. (3) Logarithmic values of mitochondrial DNA copy numbers in group C, D and E were significantly lower than in group A; whereas significantly higher in group C and D than in group B (P<0.01). (4) Normal appearance for oocytes in group A was seen. In groups B and C, a number of cytoplasmic organelles were dramatically degenerated in part of the oocytes and some necrotic oocytes were seen. Large body of mitochondria in the oocytes swelled and vacuolized in group D, while such changes dincished to a lesser extent and scope in group E. Conclusions Compound radix angelicae sinensis injection exerts a favorable curative and protective effect on oocyte and segmentation sphere injured by subacute DEP exposure in female mice.
