In order to improve nursing informationization,and to promote the development of nursing informatics, our nursing department set up a new position called informatics nurse and a series of working systems and man-agement methods were developed. After implementation,the informatics nurse had achieved various goals,including:playing a role in developing 60 nursing information system programs,improving 11 medical and nursing process modification,collecting 399 items of nursing information system problems with 338 items solved,completing 22 nurs-ing information system using instructions,and organizing training meetings and lectures in the hospital for 12 times. The informatics nurse has been appreciated by medical staff and IT engineers who was believed to have promoted the development of nursing informatics in our hospital.

WHO released the International Classification of Functioning, Disability and Health (ICF) in 2001. ICF is a kind of classification of multi-purpose used for different disciplines and fields, it belongs to by WHO for different applications in the field of health and to establish the international classification of family, it is a tool for internationally accepted, its overall goal is to provide a unified, standard language and framework to describe the condition of health and health-related. This article will review the application status of ICF at home and abroad, to explore the nursing of ICF application value, in order to explore the application of ICF in nursing standardized terminology space.

Objective To investigate the effect of depression on the incidence of osteoporosis in patients with maintenance peritoneal dialysis (MPD).Methods We enrolled 80 MPD patients,who underwent peritoneal dialysis in our dialysis center.The clinical data and biochemical parameters of all patients were collected,bone mineral density was detected and the Self-rating depression scale was used to evaluate depression state.Results Among the 80 enrolled MPD patients,34 patients had osteoporosis (42.5％),48 patients had the presence of depression (60％).Logistic regression analysis showed that age,dialysis age,gender,diabetes history and depression state were the risk factors for osteoporosis in MPD patients,the depression state was negatively correlated with BMD of lumbar and femoral neck (r=-0.347,r=0.426,P＜0.05).Conclusion Depression state may be a risk factor for osteoporosis in clinic,it is of great significance to focus on the psychological state of MPD patients in the prevention and treatment of osteoporosis.

Objective To evaluate the value of bile duct drainage using ureter catheter through cystic duct remnant with primary closure of the common bile duct(CBD) incision after CBD exploration.Methods (Retrospective) analysis of the clinical data of 218 cases using this method. Results All cases were operated on successfully.There were no complications such as bile leakage,bile duct stricture in this series.The (catheter) were removed 5-7 days after operation in 215 cases,10 days after operation in 3.None of the 218 cases had complications after removal of the catheter.The rate of stone clearance was 99.7%(217/218).Average hospitalization time after operation was(6.5?3.0)days.All the 218 cases were followed up for 1-5 years.On ultrosounography no bile duct stricture was found in the followed up cases.Conclusions (Using) ureteral catheter through cystic duct remnant for drainage of bile duct after CBD exploration with primary closure of the CBD is an effctive,reliable and safe method in preventing postoperative bile leakage,bile duct stricture and diagnosis of the postoperative retention of bile duct stones.

Objective To investigate whether there is a correlation of serum interleukin 18 (IL 18) and nitric oxide (NO) with Fulminant Hepatic Failure(FHF) and the effect of N acetylcysteine (NAC) on IL 18 and NO levels. Methods The level expression and dynamic changes of serum IL 18 and NO in 65 FHF patients serum were measured by ELISA. IL 18 and NO and their expression levels were observed comparatively in NAC treatment group (29 cases) and composite treatment group (36 cases). In order to improve the clinical outcomes and promote the development of enhanced recovery after surgery(ERAS),a full-time specialized role of nurse navigator(NN) was established in Department of Surgery in February,2016. Clinical pathway for ERAS was designed,and role functions,core competencies and responsibilities of NN were formulated. According to the functions and the working frame,NN connected and coordinated teamwork activities,monitored the implementation of ERAS during the overall process. After implementation,the NN had dealt with 10 unusual incidents,the execution efficiency of the 14(82.4%) measurements were 100%,1 measurement was 97%,2 measurements were 94.1%,and 33 patients and 42 medical staff thought highly of the NN. This practice effectively promoted the development of ERAS team and specialist nurse role in our hospital.

Objective To evaluate the impact of vagal denervation (VD) that is derived from circumferential ablation of pulmonary vein ostium for paroxysmal atrial fibrillation (AF) on the therapeutic results. Methods A total of 50 patients with paroxysmal atrial fibrillation were enrolled in this study. Circumferential ablation of pulmonary vein ostium was carried out in all the patients. The end point of ablation was pulmonary vein electricity isolation. The patients in whom VD occurred during the performance of ablation were regarded as VD- positive group (n = 19), and the remaining patients were used as VD- negative group (n = 31). The recurrence rate of AF six months after the treatment was recorded, and the results were compared between the two groups. Results The end point of ablation was successfully achieved in all the fifty cases. Six months after the ablation, the therapeutic effect of VD- positive group was significantly better than that of VD- negative group (84.21% vs 64.51%, P ≤ 0.05). Conclusion The vagal denervation effect that is derived from circumferential ablation of pulmonary vein ostium in treating AF can significantly increase the success rate of radiofrequency ablation for AF.

Objective To observe the effects of hydrogen sulfide on hepatic and renal injury induced by thioacetamide (TAA).Methods Twenty-four male SD rats were equally and randomly divided into TAA induced model group ,control group,TAA +sodium hydrogen sulfide group , and TAA +propargylglycine group .TAA was given by intraperitoneal injection to the model group , sodium hydrogen sulfide and propargylglycine groups at the dose of 600 mg/kg on the first day, and at the dose of 300 mg/kg 24 hours later.Hydrogen sulfide sodium at the dose of 0.15 mmol/kg and propargylg-lycine at the dose of 30 mg/kg were injected abdominally 1 h before TAA injection.The dose of these two agents was halved in the next 24 h.All the rats were sacrificed and specimens were collected to test hydrogen sulfide , and hepatic and renal function.Urine after the first 24 hours was collected after the administration of TAA .Results The structure of the liver in TAA group was disordered , especially in sodium hydrogen sulfide group .Liver function of TAA group was severely damaged (ALT:980.0 ±32.0 vs 38.3 ±10.6 U/L),and deteriorated with the application of sodium hydrogen sulfide (ALT:1095.6 ± 684.2 U/L), but recovered (ALT:66.3 ±8.32 U/L) with propargylglycine application.Compared with the normal group, the renal function in TAA group was significantly injured ,but the urea nitrogen and creatinine in sodium hydrosulfide group were reduced more remarkably than in TAA group (BUN:8.4 ±1.9 vs 11.62 ±6.0 mmol/L,Cr:32.6 ±8.2 vs 42.8 ±4.4 μmol/L, P<0.05).Nevertheless, creatinine was increased in propargylglycine group ( Cr 56.7 ±14.9 vs 30.8 ±4.4μmol/L, P<0.05).The urine content within 24 hours was more significantly decreased in TAA group than in the normal group (9.2 ±2.5 vs 20.5 ±6.7 ml, P<0.01), which was mildly recovered in sodium hydrogen sulfide group (11.7 ± 1.5 vs 9.2 ±2.5 , P<0.05) but further decreased (4.2 ±1.3 vs 9.2 ±2.5, P<0.01) in the propargylglycine group . Conclusion Hydrogen sulfide can alleviates renal injury while aggravating liver injury induced by thioacetamide .

Objective To compare the efficacy of three solutions used in the patients who suffered chronic obstructive pulmonary diseases (COPD ) when combined with oral fungal infection,and to find the most suitable one. Methods Using randomized controlled trial method, 98 patients who suffered COPD complicated with fungal infection were randomly divided into three groups. Method 1, using 0.5% povidone for the oral care. Method 2, making 200 million U of nystatin powder dissolved in 2.5% NaHCO3 for the oral care.Method 3, gave oral care with 2.5% NaHCO3 first, then put 200 million U of nystatin powder into 20 ml glycerol and covered the oral spots with them. Three groups were observed in the effect of oral fungal infection and the feedback from patients and nurses. Results In the analysis of the white spots sharply disappeared or pharyngeal swab smear turned negative, method 1 and method 3 had no statistical difference, P>0.05, but method 2 and method 3 had statistical significance, t=3.892, P <0.01; the three groups had no significant difference of feedback from the patients’ oral comfort, P >0.05. The time made solution of method 1 occupied 41.7% of method 3, and the time made solution of method 2 occupied 54.4%of method 3, while there was no statistical significance in total time of three groups (P>0.05). Conclusions The oral care with 0.5% povidone had good effect on the patients who suffered COPD complicated with oral fungal infection,with good comfort,and most nurses saving time. Nystatin powder dissolved in 2.5% NaHCO3 worked later than nystatin powder dissolved in glycerol.

AIM:To investigate the expressions of methylthioadenosine phosphorylase(MTAP)and ornithine decarboxylase(ODC)in human ovarian cancer.METHODS:60 fresh samples of ovarian cancer were collected.The expressions of MTAP mRNA and protein were analyzed by using RT-PCR and Western blotting,respectively.ODC activity was measured by high performance liquid chromatography.RESULTS:The expression levels of MTAP mRNA and protein in ovarian cancer were lower than those of control.In 9 of the 60 samples(15%)there were absence of detectable MTAP mRNA and protein.No significant relevance was found between the expression of MTAP and clinical pathologic features.ODC activity in ovarian cancer was(3.82?1.03)U,which was higher than that of normal ovarian tissues(1.38?0.59)U.ODC activity was related with tumor grade.In MTAP-deficiency ovarian cancer tissues ODC activity was significantly increased when compared with that of MTAP-expressing ovarian cancer samples.CONCLUSION:Down-regulated MTAP expression and up-regulated ODC activity really exist in ovarian cancer.Activation of ODC resulting from MTAP deletion may be one of the pathogenetic factors of ovarian cancer.
